良性対称性脂肪腫症(Madelung病)は頸部や躯幹,四肢などに対称性,多発性に脂肪が蓄積する疾患である。Madelung病の3例を経験したので考察を加えて報告する。症例1は76歳男性で,嗄声や嚥下障害,喉頭周囲の脂肪沈着を認めた。手術後の嚥下機能の悪化を懸念して経過観察の方針とした。症例2は61歳男性で,咽頭後間隙に脂肪組織を認め腫瘍摘出術を行った。症例3は72歳男性で,咽頭後および傍咽頭間隙に脂肪沈着を認め,腫瘍摘出術を行い整容的に高い満足度が得られた。本疾患では患者に整容的な改善の希望がある場合や気道閉塞,嚥下障害などの機能異常を認めた際には積極的に外科的治療を検討することが重要である。

Exercise necessitates a large supply of O2 and nutrients and rapid removal of CO2 and waste products. Histamine is a regulator of the microcirculation (which performs these exchanges), suggesting a possible involvement of histamine in exercise. Histamine is released from either mast cells or non-mast cells. In the latter, histamine is newly formed via the induction of histidine decarboxylase (HDC) in response to an appropriate stimulus, and it is released without being stored. Here, in mice, we examined the role of histamine or HDC induction in exercise. Prolonged walking (PW) (in a cylindrical cage turned electrically) increased HDC mRNA and HDC activity in quadriceps femoris muscles. Mice given a histamine H1-receptor antagonist [fexofenadine (peripherally acting) or pyrilamine (peripherally and centrally acting)] or an irreversible HDC inhibitor (α-fluoromethylhistidine) displayed less PW endurance than control mice. Ranitidine (H2-receptor antagonist) tended to reduce endurance. Other histamine-receptor (H3 and H4) antagonists had no significant effects on endurance. Mice deficient in HDC or histamine H1-receptors displayed markedly less endurance than control mice, and HDC activity in the quadriceps femoris of H1-deficient mice was rapidly elevated by PW. Fexofenadine significantly reduced the muscle levels of nitric oxide (NO) metabolites and glycogen after PW. The results support the ideas that (i) histamine is involved in protecting against exercise-induced fatigue or exhaustion, (ii) histamine exerts its protective effect via H1 receptors and the ensuing production of NO in skeletal muscle, and (iii) histamine is provided, at least in part, by HDC induction in skeletal muscles during prolonged exercise.

ソーシャルスキルは,対人関係の形成・維持・発展に役立つ技能を指す心理学の概念であり,内容や機能に関する調査や実験が蓄積され,教育訓練の方法が発達している。異文化圏では,挨拶,主張,遠慮,社交辞令などの対人行動が未知であると,困難や誤解が生じる。そこで,文化的行動の背景や異文化交流の要領として,異文化圏での人付き合いに役立つ認知や行動を抽出し,社会的行動の学習を試みるのが,異文化間ソーシャルスキル学習である。本稿では,在日留学生を対象とした研究をもとに異文化適応とソーシャルスキルの関わりを論じ,この心理教育を異文化間教育として使う場合の概念枠組みと学習セッションの概略を紹介する。そして,将来的な研究課題を述べる。ソーシャルスキルが適応にどのように関わるのか,ホストとの対人関係形成,およびホスト文化への関わり方の観点から考察し,日本語教育との接点を探る。

授乳期間中の母親の魚介類摂取及びn-3系脂肪酸代謝と、出生児の成長と発達の関連性を検証した。その結果、n-3系脂肪酸であるドコサヘキサエン酸(DHA)の母乳組成及び濃度は、魚介類摂取量と正相関した。⊿5不飽和化酵素遺伝子型(rs174547)C/C群(対象者の約15%)では、T/T群に対して母乳中DHA組成が有意に低値であった。出生から7か月までに児が摂取したDHA濃度を推測し、その濃度別に児の発育について検討したところ、児の体重増加との間には関連はみられなかった。今後、DHA摂取と発達との関連においてさらなる検討が必要と考えている。

日本は世界的に見ても不眠大国である。社会構造が常に複雑になり,人手不足や長時間労働などの問題により,睡眠時間を確保することが難しくなっている。このため,何らかの方法で睡眠の質を向上させることが重要になっており,我々はヒトの食事に着目した。特にオメガ3系脂肪酸,リン脂質やコリン化合物などの脂質は中枢機能に関わりがある。本総説において,筆者の研究を交えて脂質摂取と睡眠の関係について紹介する。

Fabry disease is an X-linked lysosomal storage disorder caused by a deficiency of α-galactosidase A and is classified into two types: classical and variant. The classical type exhibits classic manifestations, but the variant type does not and is therefore difficult to identify sometimes. A 73-year-old woman with a first episode of heart failure was admitted to our hospital. Her left ventricular wall motion was mildly reduced without hypertrophy. Urine sediment revealed mulberry cells, leading to the diagnosis of Fabry disease. In cases without typical clinical findings, urinary mulberry cells may help diagnose Fabry disease.

Background: Failed back surgery syndrome (FBSS) is a common and incapacitating condition affecting patients with previous spine surgery in whom treatment approach can be challenging. This study aimed to summarize existing secondary studies and up-to-date randomized clinical trials (RCTs) that assess the effectiveness of available treatment options for FBSS.Methods: Systematic searches were carried out in five databases (PubMed, Cochrane, Scielo, Epistemonikos, and Google scholar) for all systematic reviews on the effectiveness of treatment options for FBSS published after 2012. Outcomes of interest were pain levels measured through visual analog scale or numeric rating scale, Oswestry Disability Index, and quality of life. Methodological and risk of bias assessments were performed with the AMSTAR-2 tool for systematic reviews and the Joanna Briggs Institute checklist for RCT. Prospective PROSPERO registration: CRD42022307609.Results: Fifteen studies, seven systematic reviews, and eight RCTs met the inclusion criteria and fulfilled the methodological quality assessment. Of the 15 included studies, 8 were on neurostimulation, 4 on adhesiolysis, 4 on epidural or intrathecal injections, and 3 on other treatment modalities. The risk of bias was low in seven studies, moderate in five, and high in three.Conclusions: Based on this systematic overview and the considerable heterogeneity among studies, the FBSS therapeutic approach must be individualized. FBSS treatment should start with conservative management, considering the implementation of neurostimulation, a technique with the most robust evidence of effective results, in cases of refractory axial or neuropathic pain. As the last resource, in light of the evidence found, more invasive procedures or new surgical interventions are indicated.

インスリン製剤(以下,製剤)中のインスリン結晶の存在,ならびに懸濁製剤におけるインスリン結晶の濁度変化を測定し,高温環境下でのインスリン製剤の性状変化について検討を行った.方法は,2種の非懸濁製剤と2種の懸濁製剤を,38, 50, 70°Cの恒温恒湿度器内に3, 6, 12時間放置したときの濁度を吸光度計にて測定し,その変化を求めた.また,生物顕微鏡を用いて,70°C·12時間環境下のインスリン結晶を観察した.結果は,非懸濁製剤はいずれも変化がなかったが,懸濁製剤ではいずれも,38°C以上の保管で濁度が有意に変化した.また,顕微鏡観察では,懸濁製剤で70°C·12時間保管結晶の形状に明らかな変化がみられた.結果から,懸濁製剤は室温以上の環境下に長時間放置しないことが求められ,患者の日常生活におけるインスリン製剤保管時の温度管理について,十分注意する必要があると思われる.

The brain deals with rationality-related processes in multi-dimensional and diverse ways. It is supposed that the brain starts processing such complicated complex cognitive processes immediately after the stimulus appears. Components in event-related potentials can reflect those processes that are underlying them. For example, mismatch negativity (MMN) reflects the violation of a rule established by a sequence of sensory stimuli. N2 components are supposed to be related to cognitive control. P300 components are elicited when stimulus detection engages memory operations. N400 is related to meaning processing. P600 is associated with syntactic and semantic reanalysis processes. Early left anterior negativity (ELAN) is supposed to be a marker of syntactic first-pass parsing. Error-related negativity (ERN) is a negative waveform that arises after a participant makes a detectable error. These components can reflect various rationality-related processes in decision making. In dual process theory, there are two distinctive types of processes: fast and intuitive process, that is System 1 (Type 1), and deliberative and reflective process, that is System 2 (Type 2). It is supposed that the two processes deal with rationality based on their respective characteristics. However, the detailed processes are not clear. For an analysis using ERP components, it is expected that the rational-related processes in the course of time reveal up to about 1000ms.

圧縮性流れ場には、衝撃波などの不連続面が存在するという興味深い現象がある。このような不連続性をもつ現象の数値解析に、高精度解析スキームを適用すると、非物理的な現象である数値振動が現れるということが知られている。その擬似振動を抑えるために、圧縮性流れの数理モデルであるEuler方程式の解析には、TVD (Total Variation Diminishing)の概念に基づいた高解像度スキームが多用されている。本研究では、圧縮性Euler方程式を対象に、高解像度TVDスキームの概念をペトロフ・ガラーキン有限要素法の定式化に導入することを目的として、TVD有限要素法の構築を行う。数値計算例として、Riemann (shock-tube)問題を通して解析手法の適用性及び妥当性を検討する。
