暑熱による脱水症が誘因と考えられた高齢者脳梗塞例を通して暑熱, 脱水症と脳梗塞との関係を検討した. 対象は最高気温が連日概ね30度を越えていた2週間に当病院を受診した65歳以上の急性期脳卒中 (高齢猛暑群n=5) である. 対照にはその前後4週間に脳卒中で受診した高齢者の前群 (n=5), 後群 (n=3) を, また65歳未満の若年猛暑群 (n=1), 若年前群 (n=5), 若年後群 (n=2) を各々高齢対照群, 若年群として用い, 臨床所見, 画像所見を後方視的に検討した. 高齢猛暑群は全て脳梗塞で, その頻度は高く, いずれも活動した日の正午までに発症したのが特徴的であった. 1例 (78歳) は橋梗塞例で, 既往に多発性ラクナ梗塞があり, 嚥下障害がみられていた. 2例 (73,89歳) はラクナ梗塞例で, このうち1例は前立腺肥大症による頻尿を恐れて飲水制限をしていた. 他の2例 (76,83歳) は心原塞栓性梗塞例 (1例は再発例) であった. 高齢猛暑群では皮膚緊張度の低下, 舌の乾燥が全例に, BUN/Cr比≧25も透析患者を除く4例中3例に, フィブリノゲン上昇も3例中2例にみられ, 特にBUN/Cr比は若年群より有意に高かった. ヘマトクリット値の上昇はなかったが, 発症時の状況や臨床所見から脱水症が疑われ, 補液したところ, 皮膚緊張度は改善し, 3例の非塞栓性脳梗塞には抗血栓療法を施行して2例は軽快した. 以上より, 猛暑下での過剰発汗が高齢者の脱水症を助長し, 脳梗塞を惹起したものと考えられた. 高齢者は暑熱で脱水症をきたし易く, また脳梗塞が午前中に多かったことから, その予防には起床時の十分な飲水が重要であることが示唆された.

背景:IgA腎症は慢性糸球体腎炎のうち,糸球体メサンギウム細胞・基質の増殖性変化と,メサンギウム領域へのIgAを主体とする沈着物を認める疾患である.近年IgA腎症患者に口蓋扁桃摘出術とステロイドパルス療法を行う扁摘パルス療法の有効性が注目されている.またIgA腎症患者の口蓋扁桃では,慢性扁桃炎患者の口蓋扁桃と比較して病理組織学的に異なった特徴を示すとされている.対象,方法:昭和大学藤が丘病院腎臓内科で2007年から2008年にIgA腎症と診断され,口蓋扁桃摘出術を施行した49例の口蓋扁桃の病理組織学的特徴について検討した.結果:IgA腎症患者の口蓋扁桃では,リンパ濾胞の大きさが大小様々で境界が不明瞭となる,濾胞間領域も不規則に拡大,上皮では上皮間に形質細胞系細胞が増加・充満するといった特徴を各項目で半数以上の症例で認められた.結論:IgA腎症患者の口蓋扁桃には特徴的所見を高率に認めた.これらの特徴とIgA腎症の病態との関連は明らかではなく,今後の検討課題と考えられる.

In recent broadband photonic network system, high speed optical data transmission is required. Electrooptic (EO) polymers are attractive because of their large nonlinearity, fast response time and ease of device fabrication. Therefore the EO polymers are expected for the application of ultra-high speed EO switch and/or wide bandwidth EO modulator, both of which will be used in future photonic network. In this report, recent progress of EO polymers and their application to waveguide type devices are described.

The life history of Hucho perryi (BREVOORT) was studied in the Faren River in northwestern Nemuro, Hokkaido, from late April to early June, 1960. H. perryi is a salmonid fish, which has flattened head, wide gape with stout teeth, many dark dots on the body and larger scales than those of other salmonid fishes (Pl. 4, A). This fish is distributed in the streams and lakes of Hokkaido, southern Kurile Islands and Sahalin.From April to May after thawing of ice in the river, this fish is observed in deep pools with rocks or many sunken timbers (Fig. 2), and in June when the water temperature rises about 10°C the fish is found also in the shallow and swift waters as well as deep pools. The stomach contents of 22 specimens, 2.9 to 56.0cm in total length, were examined. The larvae of aquatic insects, especially Plecoptera and Tricoptera, were eaten by the fry from 2.9 to 3.2cm. These diets were occurred commonly in the stomach of juveniles from 14 to 18 cm, and loarch, Barbatura toni oreas (JORDAN and FOWLER), were occasionally found together with the insect larvae. The fish larger than 30cm did not eat the insect larvae but took mainly the loarch. Although chum salmon fry, Oncorhynchus keta (WALBAUM), were found as the same time in the river where the examined fish were captured, they were not eaten by this piscivorous fish.The spawning season of this fish seems to extend from mid-March after breaking of ice to late April in Nemuro district. The knowledge about maturation was a little. The author obtained three mature males larger than 55cm, and also observed one mature female measuring 93 cm from the Shibetsu River. A spawning redd was found in the tributary pouring into the Furen River at Mimakka (Fig. 3), and 1, 358 eyed eggs in all were taken from this redd on May 22. These eggs were salmon pink in color and measured 5.8 to 6.4mm in diameter. They were brought into the laboratory of Nemuro Branch of Hokkaido Salmon Hatchery, and began to hatch on May 28. The alevins were reared until the yolk sac were completely absorbed.The newly hatched alevins preserving in 70% alcohol were measured 15.1 to 16.6mm in total length (Pl. 4, D). About 40 days after hatching, the alevins attained to 24.0 to 25.6mm. Two-thirds of the yolk sac were consumed and parr marks began to appear vaguely on the body side (Pl. 5, A). It took 52 days for the alevins to absorb completely the yolk sac at the water temperature of 6.0°to 11.3°C. The fry just after consumed the yolk were measured 28.1 to 28.5mm (Pl. 5, B), and they grew up about 38mm by August (Pl. 5, C).

インスリンの構造異常によりそのインスリン作用が低下するため,代償的にインスリン分泌が亢進し,このため高インスリン血症をきたす異常症は世界で12家系発見されているが,糖尿病は軽症であることが多い.最近の遺伝子解析の手法の進歩により簡単に構造決定されるようになり,インスリンの構造と作用, NIDDM発症の機序などこれらの症例から学ぶことが多い.

Several methods for quantifying the purine content in food and drink have been described using high-performance liquid chromatography (HPLC). We have developed an improved HPLC method that is based on a method reported by Kaneko et al. and that is more sensitive yet simple, and suitable for determining the purine content of beer and beer-like alcoholic beverages. Quantitative HPLC separation was performed on a Shodex Asahi Pak GS-320HQ column with an isocratic elution of 150 mmol/L sodium phosphate buffer (H3PO4/NaH2PO4 = 20:100 (v/v)). The retention times for the four analytes, namely, adenine, guanine, hypoxanthine and xanthine, were 19.9, 25.0, 29.3 and 43.0 min, respectively. The resolution was good, and there was no excessive interference from the other compounds in the beverages at these retention times. Furthermore, the detection limit for all the analytes was improved to less than 0.0075 mg/L, and all the calibration curves showed good linearity (r2 > 0.999) between 0.013 and 10 mg/L for adenine and guanine, and between 0.025 and 10 mg/L for hypoxanthine and xanthine. The pretreatment was simplified by removing some procedures and optimizing the perchloric acid hydrolysis and the enzymatic peak-shift assay. We reduced the sample dilution rate by almost 50%, and the time spent on pretreatment from 4 days to only 180 min. The recovery of the analytes from spiked samples was 94.8 – 103.8%. This method may be useful for evaluating quantitative and qualitative differences in the purine content of beer and beer-like alcoholic beverages.

The purpose of this study was to clarify the geographic distribution of public sports center users with regard to the distance decay model, and to verify the difference of the parameters of the use frequency level. The findings of the analyses are summarized as follows ;     A logarithmic relationship was observed between the distance from sports center and number of users (R2=.572) . This suggests that the distribution of public sports center users is affected by the distance.    And the distance decay parameter of low frequency users is much smaller than that of high and middle frequency users. This suggests that there is a possibility that users who live a short distance from the sports center would be higher frequency users than those who live a long distance from the sports center.

Javaアプレットで動作するフリーの分子ビューアJmolを利用し,ブラウザで3D構造を参照できる生体高分子のWeb教材集を作成して公開した.Protein Data Bank(PDB)に収録されている構造データを必要に応じて加工し,Jmolスクリプトを用いて各データに適した様々な表示変更が可能になるようにしたもので,生命科学の教育・学習に活用可能である.これまで本研究室では以前広くChimeを用いて公開してきた教材を順次変換しているもので,膜タンパク質,シトクロムP450, 糖タンパク質など,種類別にしてそれぞれ構造の特徴を強調可能にすることで使いやすくしている.

ホタテ貝中腸腺中金属元素の多元素同時定量法の開発を行った.試料の溶液化はマイクロウェーブ密閉容器を用いる酸分解法で行い,金属元素測定には誘導結合プラズマ質量分析法(ICP-MS)及び誘導結合プラズマ発光分析法(ICP-AES)を用いた.分析値の評価にはNIST頒布の標準試料SRM 1566b Oyster tissueを用いた.試料0.5 gを採取し,硝酸,フッ化水素酸,過塩素酸の混酸によるマイクロ波酸分解を行った.分解後の試料をICP-MS及びICP-AESで測定した結果,K,Na,Zn,Mg,Ca,Fe,Al,Cu,Mn,Ba,Sr,Cd,Ni,Co,Pb,Y,希土類元素の定量が可能であった.得られた定量値はPbを除き認証値及び参考値とよく一致した.測定精度はPb,Ni,希土類元素を除きすべて相対標準偏差4% 以内であった.本法を青森産ホタテ貝中腸腺試料の分析に適用したところ,30元素の定量が可能であった.特にカドミウムは中腸腺中に69.0 mg kg-1含まれており,中腸腺中元素濃度と頁岩中元素濃度及び海水中元素濃度の比に基づき算出した生体濃縮係数は他の金属元素と比較して非常に高い結果が得られた.

チャもち病菌(Exobasidium vexans)の自然状態での侵入,感染は,若い葉の裏面に付着した担子胞子が発芽して,主に細胞縫合部で付着器と侵入菌糸を形成して行われた。付着器からの侵入菌糸の形成は,低率であった。

心筋細胞のミトコンドリア内膜に存在するATP感受性K+ (mitoKATP) チャネルやCa2+活性化K+ (mitoKCa) チャネルは, プレコンディショニングの成立に重要な役割を担っている. MitoKATPチャネルの活性化によりK+が細胞質からマトリックスへ流入すると, ミトコンドリア内膜が脱分極し, Ca2+のdriving forceが減少するのでミトコンドリアCa2+過負荷が抑制される. MitoKCaチャネルも同様のメカニズムでミトコンドリアCa2+過負荷を抑制する. ミトコンドリアCa2+過負荷の軽減は膜透過性遷移孔の開口を遅らせるためアポトーシスも抑制するので, 強力な心筋保護作用を発揮すると考えられる.

食物誤嚥は気道異物の原因の中でも高い割合を占める.他の気道異物と比較して,①X線検査での確定診断が困難,②構造が脆弱で細片化しやすく,摘出が困難,といった点に特徴がある.気道内食物異物の診断・治療について我々の症例をまとめ考察した.2000年1月~2011年10月までに経験した気道異物9例のうち,食物異物6例.内訳は小児4例,成人2例で,原因となる食物は豆類が5例,肉片が1例であった.あらかじめ標準化された手順のもと,全例が全身麻酔下に異物摘出処置を受けた.3例が軟性気管支鏡下,3例が硬性気管支鏡下で施行され,摘出処置に起因する重篤な合併症の発生はみられなかった.気道インターベンションは複雑で多様性に富み,かつ危険を伴う外科処置であるが,他科の協力を含む手順の標準化(プロトコール化)により安全な施行が可能となると考えられた.

Apical leakage continues to be a topic of great interest, because in spite of the advances in Endodontics, clinical failures still occur. Most failures are probably attributed to the proliferation of bacteria that remain viable after chemical-mechanical preparation and cause periapical tissue irritation. Coronal leakage has aroused researchers’ interest over the last few years, since canals may be re-contaminated after loss of coronal sealing or fracture of the remaining tooth. In this review, the various methodologies used for assessing root canal sealing capacity are critically analyzed, as they are not standardized, which makes it difficult to compare the results obtained when different methodologies are used, even though the same substrate is being assessed. This paper describes topographic and geologic features of the northern part of the Sagami Trough and surrounding areas, off central Japan, and discusses the role of the Philippine Sea plate in the development of the trough's topography and fault systems. In the study area, the Okinoyama Bank Chain, Miura Basin Chain, and several spurs are aligned NW-SE parallel to the axis of the Sagami Trough. We examine the lithological compositions of rock and piston-core samples collected from the R/V Hyper Dolphin during Japan Agency for Marine-Earth Science and Technology dives 906 and 907, and rock samples dredged during Ocean Research Institute of the University of Tokyo cruise KT88. These samples represent mainly volcaniclastic rocks derived from the volcanoes of the Izu arc and subordinate amounts of terrigenous sediment interpreted as derived from the Sagami and Sakawa Rivers. The topographic and geologic data define five fault systems in the eastern part of the northern Sagami Trough. We interpret the Okinoyama Bank Chain as an elevated outer-ridge belt developed at the landward slope of the plate boundary, which is represented by NW-SE trending right-lateral reverse faults (F1). At the northeast side of the Okinoyama Bank Chain, NW-SE trending normal faults (F2) formed and the Miura Basin Chain developed. The cutting relationship between these fault sets suggests that the collision of the Izu arc with the Honshu arc imposed a right-lateral shear stress field on the Okinoyama Bank and Miura Basin Chains. This shear stress may have led to clockwise rotation of these tectonic blocks formation of a set of NE-SW trending left-lateral reverse faults (F3). The initiation of the Izu block collision is interpreted as a turning point after which the northward motion of the Philippine Sea plate abruptly changed to NW and the Sagami Bay area came under a NW-SE compressional stress field, which resulted in the development of E-W trending right-lateral faults (F4) in the narrow shelf off the Miura Peninsula. A number of spurs subsequently developed along these faults. Finally, NNE-SSW trending right-lateral normal faults (F5) developed under an E-W extensional stress field, which caused the formation of N-S trending topographic depressions such as Tokyo Canyon and submarine highs such as Okinoyama and Oiso Spur.

Coral reef terraces are one of the best recorders of biological response to environmental change events (e.g., sea-level changes). Kikai Island provides a rare opportunity to show biological and ecological frameworks (e.g., competition, coexistence, and succession) during a recent geological period. The island is fringed by raised Holocene raised reef terraces, which formed as a result of periodic tectonic uplifts. This study aims to characterize the spatial and temporal changes of corals at this island during the Holocene. The analysis is based on topographical and biological data obtained for the three sites (Shidooke, Kadon, and Nakugama reefs). Three raised reef terraces (Terrace II, III, and IV) grew from 7300 to 4500 years ago (during 2800 years), from 4500 to 2900 years ago (during 1600 years), and from 2900 to 1800 years ago (during 1100 years), respectively. Terrace II and III were uplifted 1-2 m around 4500 years ago and around 2900 years ago. Terrace IV was uplifted 1-2 m around 1800 years ago. The modern reef has been composed of corals for 1800 years. Sixteen coral genera and 53 species were recorded from the reef terraces. Terrace III and IV were dominated by four coral species (A. digitifera, A. robusta, G. retiformis, and F. stelligera), but Terrace II was predominantly composed of A. digitifera and A. robusta. These biological and ecological variations between the terraces represent a growth strategy responding to differences of reef growth time and/or insolation.
