症例は56歳,男性。単独で登山中,富士山8合目(海抜3,100m)で突然昏倒した。居合わせた外国人医師が救助に当たるとともに同行者が8合目救護所に通報した。自動体外式除細動装置(automated external defibrillator; AED)を持って出発し,昏倒から30分後胸骨圧迫を受けている傷病者と接触した。AEDを装着したところ適応があり,除細動を施行した。まもなく呼吸開始,脈を触知した。呼吸循環が安定したところでキャタピラ付搬送車(クローラー)で下山を開始。5合目で救急車とドッキングし,約2時間後山梨赤十字病院に到着した。第1病日に施行した心臓カテーテル検査で前下行枝の完全閉塞,右冠動脈からの側副血行路による灌流を認めた。低体温療法を施行せずに第1病日に意識レベルJCS I-1まで回復し,とくに神経学的後遺症なく4日後に退院となった。富士山吉田口登山道では7合目,8合目に救護所があるが,2007年から全山小屋にAEDを装備し,山小屋従業員に対してBLS講習会を施行している。今回の事例はこれらの取り組みの成果であり,healthcare providerに対するBLS・AED教育の重要性が再確認された。

本研究は、メタリック/パール色の粒子感を定量評価するためのハードウェアとアルコリズムを確立することを目的とした。粒子感の定量化は、(1)CCDを用いて試料面を撮像し、(2)画像データを人間が感じるイメージに合わせて加工し、(2)光輝材によってキラキラとした高輝度部分の特徴を抽出して粒子感を求めるという手順で行った。(2)の画像データの加工については、解像度や明るさのスケールを人間の感じるイメージに合わせることで、高い目視相関が得られると考えた。(2)の粒子感の計算については、まず、光輝材によってキラキラとした明るい部分を高輝度部と定義し、粒子怪を3つの物理量、高輝度部明るさ、高輝度部面積、高輝度部の数で表した。これらのパラメータを説明変数とした重回帰式によって目視相関のある粒子感を定量化できると考えた。39種のメタリック/パール塗板を用いて目視相関を検証した結果、高輝度郡明るさと目視粒子感の相関が非常に強く、それ以外のパラメータは相関があまり強くないことが分かった。

毎年授業開始時に, 5味識別テストを行って来たが, 最近正解率に低下傾向がみられるため, '93年から'96年の4年間における結果を比較し, 学生の味の識別能力を検討したところ, 以下のことが明らかとなった。1) 6試料を識別出来た者, 即ち6点正解率は'93年では, 40%であったが, '96年には, 26%と低下し, '93年を100とすると'96年は65%にすぎなかった。4点正解率は, 6点正解率に次いで高く, 年度による差はなかった。しかし, 3点以下は'93年では全体の30%以下と少ないのに対し, '96年では47%と高く, 年度とともに増加する傾向であった。6試料の識別能力の低下傾向が認められた。2) 6点満点の正解率と有意な正の相関を示した単一味正解率は, 甘味, 無味, 酸味であり, うま味も有意ではないが, 同様の傾向であると考えられた。3) 6試料のうち, 甘味, 塩味, 苦味では正解率が高く, 年度間の差は少なかった。一方, 酸味, うま味, 無味の正解率は低く, 年度と共に低下した。うま味正解率低下に対しては, 酸味誤答率が, 有意な上昇を示し, 次いで酸味正解率低下に対しては, 無味誤答率が, 無味正解率低下に対しては, うま味誤答率が, それぞれ上昇傾向を示した。以上の結果より, 酸味, うま味及び無味, の3者間相互の, 識別が困難なために誤答率が高くなり, 成績低下に結びつくものと判断された。

The aging population has resulted in the percentage of GDP allocated to healthcare costs reaching about 10%, and further increases are expected. To manage this issue, the Ministry of Health, Labor, and Welfare is attempting to improve the population's lifestyle and to moderate healthcare cost increases by conducting medical checkups and providing health guidance for lifestyle improvements. The study data comprise medical prescriptions and medical examinations of 24,725 people who have developed diabetes and 13,453 people who have developed high blood pressure (data from the Japan Medical Data Center). Based on a multiple regression analysis, we compare people with good lifestyle habits and people with bad lifestyle habits to examine how the increases in their healthcare costs changed as a result of their diagnoses. For people who developed diabetes, those who properly controlled their BMI and LDL cholesterol levels, refrained from smoking and drinking, and exercised regularly (i.e., practiced good lifestyle habits) were able to maintain satisfactory medical examination results (diabetes). Based on these results, we prove statistically that the increase in healthcare costs is less for people who keep good lifestyle habits, even if they develop diabetes. Similarly, people who developed high blood pressure and practiced good lifestyle habits were able to maintain satisfactory medical examination results (high blood pressure). Again, the increase in the healthcare costs for this group was less, even if they developed high blood pressure, although, in this case, exercising regularly was not a relevant factor.

The anticipation of Japan's decreased birthrate and a shift to an aged society has led to the field of medical care to review the country's social security system. The review calls for a reform of the medical insurance system under the universal health coverage system to fit the changing demographic. The target year of 2025 has the government advocating for an “integrated community care system”, with the ‘baby-boom generation’ turning 75. This is a sustainable regional system that can cope with the aging society and the declining birthrate simultaneously. Within this new system, I would like to discuss The Strategic Plan of the Japan Pharmaceutical Association, released with the aim of realizing pharmacists and pharmacy services that can immediately respond to the expectations of society; from the perspective of the theme of this symposium, “Communicating the image of pharmacists in a community-based society”.

In 1530s, Church of England was separated from Roman Catholic Christendom through so-called King Henry VIII’s divorce, while Wales was united to the Kingdom of England by so-called Acts of Union. This article considers what was the relation between the English Reformation and the Union, or what was the influence of the English Reformation on the church and society of Wales. To investigate the subject above, this article takes up the following topics. First, it considers the installation of Bishops who propelled the Reformation policy in their dioceses in Wales. Second, it discusses the role of the Council of the Marches of Wales on the Reformation in Wales. These two topics aim to clarify the promoting force of the protestant religious policy in Wales. In the last section, it considers the importance of the Welsh language on the process of the Reformation in Wales. As Church of England promoted vernacularisation of the service of the church, it needed translation of the Bible and the Book of Common Prayer to Welsh. It was the most important task for the established church propelling the Reformation policy in Wales. On the process of translation, the role of William Morgan( later Bishop of Llandaff, and then of St. Asaph) as the Bible translator was so crucial that deserves further investigation.

<p>熊本県阿蘇地域の山地部は主に草原や人工林として利用されているが、景観や防災、水源かん養等の面からみた最適な土地利用のあり方について模索する動きが地域社会で進められている。草原は放牧・野焼きの有無や優占種の違い(ススキ,ネザサ)によって違いが見られ、人工林も管理(間伐)の程度や樹種(スギ,ヒノキ)によって林相に大きな違いが見られる。しかし、これらの植生の諸特性がその土地の多面的機能をどのように特徴づけているのかについては定量的な知見が限られている。そこで、本研究では熊本県阿蘇市・南阿蘇村内の複数の草原と人工林において土壌の透水性および保水性を調査し、植生の違いが阿蘇地域における山地斜面の水源かん養機能や防災機能に及ぼす影響について検討した。</p>

昭和20年から現在に至るまでの性教育に関する書物を調査し,教育のなかで性教育はどのようにとりあげられ,位置づけされてきたかを検討した。その結果,性教育は,1)道徳面を重視した純潔教育の時代 2)純潔教育批判から生じた性の生理的側面が強調された教育の時代 3)生理的,心理的,社会的側面を含む人間としての総合的な教育の時代の三つの歴史的段階が認められた。また,学校教育のなかでの位置づけが不明瞭であることが教育現場での性教育の定着を妨げていることが示唆された。

The decrease of nitrite in ham during cooking process was investigated.Hams containing 64.1mg/kg of nitrite were used to estimate the decrease of nitrite.60 percent of nitrite was decreased after boiling ham in water for 20 seconds. But after boiling in water containing one weight percent of vinegar or sake, only 30 percent of nitrite was decreased.After soaking ham in water at room temperature for 1 minute,60 percent of nitrite was decreased. Fifty percent of nitrite was decreased by sauteeing with butter or margarin for 30 seconds, and only 30 percent of nitrite by sauteeing with corn oil.Consequently, fair amount of nitrite was removed from hams during the simple cooking process.
