Do Not Attempt Resuscitation(DNAR)の概念形成から約半世紀を経たが,いまだにその誤解と誤用が大きな問題になっている。日本集中治療医学会倫理委員会は,世界と本邦におけるDNAR指示の歴史と経緯を学び,その考え方を理解・把握して日本集中治療医学会会員諸氏に正しく伝えることを企画した。DNARは心停止時に心肺蘇生を行わない指示であり,ICU入室を含めて酸素投与,栄養・輸液,鎮痛・鎮静薬,抗不整脈薬,昇圧薬,人工呼吸器,血液浄化法など,通常の医療・看護内容に影響を与えてはいけない。倫理委員会は,本報告に基づきDNAR指示が正しい対象に正しい方法で運用されることを期待する。

目的:海外旅行に参加した夫婦にインタビュー調査を行い,重度失語症者にとっての本旅行の意味付けと旅行後の生活への影響を考察することで,作業療法における旅行の活用方法について示唆を得ることである.対象と方法:失語症のある人の中から,配偶者が調査時に同席可能であり,旅行後の生活において事故などの旅行とは無関係なイベントのない4 名を対象とし,「発障前の生活」「発症後の生活」「旅行後の生活」「本旅行への関心」「本旅行に対する満足・不満足」「また旅行に行きたいか」について聴取した。結果・考察:本旅行への意味付けと,参加態度,満足度,旅行前後の生活変容の状況を事例毎に整理した.結果より,主体的な旅行参加はその後の生活にも肯定的影響を与える可能性がある,旅行への参加態度は本人の抱く旅行への意味付けの能動性が影響する,旅行に対する満足度の高低と旅行後の生活活性化の有無は関係しない傾向がある,の3 点が考察された.

Cystic tumors, such as craniopharyngiomas and Rathke’s cleft cysts, as well as arachnoid cysts have been reported to rupture occasionally. Approximately 8–10% of glioblastomas (GBMs) are known to have a significant cystic component; however, to the best of our knowledge, no studies have reported cystic rupturing of GBMs. Here, we describe a unique case of cystic GBM rupturing and penetrating into the cerebral ventricle. A 77-year-old man with a right frontal lobe lesion suspected as GBM with a large cyst was referred to our hospital. At admission, disorientation and left facial weakness were detected. Consciousness disturbance worsened on the 8th day of hospitalization. Computed tomography (CT) revealed prominent shrinkage of the tumor and intratumoral cyst. Signs of meningeal irritation were observed, and chemical meningitis due to cystic tumor rupture and leakage of necrotic components into the ventricle was highly suspected. Surgical resection of the right frontal lobe tumor was performed on the 10th day of hospitalization. During the surgery, clear and colorless cerebrospinal fluid was obtained upon penetration of the tumoral cyst, suggesting traffic of tumor cysts and cerebral ventricle. Adjuvant chemoradiation therapy was initiated postoperatively. Local recurrence was noted at the corpus callosum 7 months postoperatively and was treated with a gamma knife. Further therapy was performed after this recurrence. However, his condition gradually deteriorated 15 months postoperatively, and he was subjected to terminal care. To the best of our knowledge, this is the first report on a cystic GBM rupture.

This paper aims to clarify and to seek solutions to the problem of regional Japan where the population and the economies are shrinking. After the collapse of Japan’s bubble economy hit the finances of local governments, new leaders elnerged with the broad support of nonaligned voters in local politics. HASHIMOTO Daijiro, who became the Kochi governor, having beaten the candidate of established political parties in 1991, was first new leader. They were called “independent reform-minded governors” because they opposed the conventional growth policy dependent on allocations for public works projects and attempted to break away from the 1955 regime dominated by the Liberal Democratic Party. Under the 1955 regime the Japanese government tried to reduce the economic gap between urban and rural areas by concentrating public investment in the rural areas and local governments targeted economic affluence though the public work projects that linked government subsides. This regime deeply relied on constant econolnic growth, especially on the growth of urban areas but the regime collapsed after the bubble economy. Then, how can the regional society be sustained without the fiscal support of Japanese government? This paper will explore the possibilities of the regions to survive in the time of shrinking society by analyzing the policies of TANAKA Yasuo who was the Nagano Prefectural Governor from 2000 to 2006. Although he could not realize these new policies and he was finally defeated in the election, his slogans such as “No Dam Proclamation”, “Post-Materialism” and “Commons” introduced the new values of the shrinking society. One of his unique achievements was to try to avoid the gap between urban and rural societies. This is the reason why Tanaka was supported by people in the urban as well as the rural areas. It contrasted with other independent reform-minded governors who enjoyed more support in the urban areas than rural areas. Tanaka’s policies consisted of two points. Firstly, he promoted agriculture and tourism to reactivate agricultural villages by commodifying agricultural villages. Secondly, he attempted to bridge different interest groups and create “Commons” by dissolving traditional communities and providing a system of citizens' participation.

本研究は、3名の引っ込み思案幼児の社会的スキルの長期的維持を出現させるために、(1)社会的スキル知識を促進する訓練室でのコーチング、(2)社会的スキル実行を促す自由遊び場面でのコーチング、(3)トレーナーによる構造化された遊び場面の設定、(4)訓練場面への仲間の参加とを組み合わせた社会的スキル訓練(SST)を構成した。15セッションからなるSSTを受けた訓練対象児は、訓練終了後、仲間に対する働きかけ、仲間からの働きかけ、協調的行動を増加させ、社会的孤立行動を減少させた。さらに、一年後のフォローアップ査定では、3名中2名の訓練対象児が、訓練効果を維持していることが分かった。これら2名の訓練対象児のポジティブな行動変容は、担任教師による社会的行動評定得点にも反映されており、本研究で実施されたSSTが長期的維持を効果的に促進していたことが実証された。

筋肉および神経への電気刺激による筋収縮誘導の知見は,すでに臨床の現場で活用されている技術となっている.健常者に対する筋力増強の効果はすでに確立しているが,下肢を中心とする運動器疾患での臨床成績への効果については報告にばらつきがあり,高いエビデンスとはなっていない.こうしたばらつきは,対象とする疾患の重症度や廃用の状態に影響されていると考えられる.最大随意収縮との比較(%MVIC)など刺激強度を確認する方法と,介入する筋肉の状態を把握したうえでの介入研究の蓄積が求められる.

慢性副鼻腔炎患者の約半数に鼻粘膜粘液繊毛輸送機能の低下が認められることが知られている。本研究では生理的食塩水エアロゾルが慢性副鼻腔炎患者の低下した鼻粘膜粘液繊毛機能にどのような影響を及ぼすかを検討した。生理的食塩水0.7mlをジェットネビュライザーで鼻腔に投与し, 投与前後のサッカリン時間 (ST) を粘液繊毛機能の指標として測定した。23名の正常人では生理的食塩水投与前後のSTに有意の変化は認められなかったが, 慢性副鼻腔炎患者19名では生理的食塩水投与後に投与前に比し有意にSTが改善した。慢性副鼻腔炎患者の低下した鼻粘膜粘液繊毛機能に及ぼす生理的食塩水のこのような効果を本症における鼻粘膜粘液繊毛機能低下のメカニズムと併せて考察した。

Krautら(1998)が最初のインターネット・パラドクスに関する論文を発表した当時と比べ,インターネット利用環境は大きく変化した。特に近年の社会的な関係に基づくソーシャルメディアの普及は,実社会での社会的な関係に大きな影響を及ぼす可能性がある。その一方で,ソーシャルメディア上での人間関係に負担を感じるいわゆる「ソーシャルメディア疲れ」などの議論もある。すなわちソーシャルメディアの利用によって友人関係や精神的健康にネガティブな影響を及ぼすソーシャルメディア・パラドクスの可能性が考えられる。そこで本稿では,2011〜2012年にかけて実施したインターネットを用いたパネル調査のデータを用いて,このソーシャルメディア・パラドクスの可能性について分析を行った。ソーシャルメディアの新規利用者では,よく投稿をする人が一時的に友人関係満足度にポジティブな影響を及ぼしているが,全体でみるとソーシャルメディアをよく投稿するほど友人関係満足度や孤独感に対してネガティブな影響を及ぼす傾向が示された。つまり社会的な関係を基礎とするソーシャルメディアの利用において,友人関係や精神的健康にネガティブな影響を及ぼすというソーシャルメディア・パラドクスの存在の可能性を示した。

本稿は, 性虐待の定義, 性虐待をめぐる多職種・多機関ネットワーク, 専門職が知っておくべき最低限の知見, そしてグローバルスタンダードとなった性虐待の多職種・多機関連携を行うためのChild Advocacy Center(CAC)モデルを検討する。介入と支援を一箇所で展開するCACが機能するには10個の認定基準があり, ①医療-福祉-司法の多職種・多機関連携(Multi-Disciplinary Team), ②文化的多様性と問題への対応力, ③司法面接, ④被害者支援とアドボケイト, ⑤全身医学的評価, ⑥メンタルヘルス支援, ⑦チーム全員での事例検討会, ⑧ケースの追跡調査, ⑨組織的な能力(研修など), ⑩子どもを最優先にした設定がある。今後日本においても性虐待をめぐるネットワークを進化させるために, 医療-福祉-司法のMDT構築を促進する仕組みが必要である。

走行中の水上オートバイから転落した場合,船体の推進力であるジェット水流により直腸・膣に特徴的な損傷(以下,本外傷)を生じる.水上オートバイ関連外傷の中で稀な外傷ではあるが,損傷が複雑で治療に難渋し,時に致命的となる.症例1:25歳の女性,走行中の水上オートバイから後方に転落し,会陰部から多量の出血を認め当院へ搬送された.肛門裂傷と腹膜翻転部上に及ぶ直腸損傷を認め,人工肛門造設術を行った.患者は術後12日目に退院したが人工肛門閉鎖には至っていない.症例2:27歳の女性,走行中の水上オートバイから後方に転落し,直腸・膣を複数箇所で損傷し活動性の出血を認めた.直腸損傷に対しては人工肛門造設術にて対応しえたが,膣損傷は円蓋部にまで及んでおり止血が困難であった.一時は重篤な出血性ショックに至ったものの,経カテーテル的動脈塞栓術により止血・救命した.患者は術後35日目に転院となった.

[Synopsis] We found large numbers of the exotic millipede, Chamberlinius hualienensis, which is originally distributed in Hualien, Taiwan, from two areas in Wakayama Prefecture. Because there was no trace of a large amount of soil or plants brought into the study sites, and the millipede was univoltine, these populations suggested their invasion was not recent in Wakayama Prefecture. The density of the millipede was 9.5±8.1 ind./m2 in the forest in Tanabe City. Its density was lower than observed when outbreaks occurred in Kanagawa and Okinawa Prefectures (1/6- 1/54 times). Therefore, it was considered that there was no outbreak of millipede at this site at this time. This is the first report showing the possibility of the millipede has invaded in the Kinki region in Central Japan.
