近年、病態などを考慮し特定の栄養素を強化した経腸栄養剤が数多く開発され、活用されるようになった。今回、われわれは高齢の難治性褥瘡患者に対し、アルギニン配合の高濃度液状栄養食を投与し、若干の知見を得たので報告する。 症例は、入院時に仙骨部と腸骨部左右に褥瘡あり。仙骨部は黒色壊死と内部にポケット、感染兆候を認めた。局所ケア、半消化態栄養剤による栄養補給を行うとともに、体圧分散を図ったが、褥瘡改善を認めないため、栄養剤をアイソカルR プラスEX に変更した。その結果、褥瘡、生化学データともに改善したが、投与開始後9 日目よりBUN が上昇し、36~53 mg/dl で推移した。 BUN 上昇は、栄養剤の投与量をエネルギーに主眼をおいて設定したため、アルギニンが増加し尿素産生に影響を与えることになったためと考える。高エネルギーを必要とする症例に対し、アルギニン配合の栄養剤を投与する場合には、単独投与を避け、他の栄養剤と組み合わせるなどの工夫が必要であると考える。

To level the fluctuations in electric power sourced from renewable energy, the transmission network can be spread over a wide area, but this is expected to dramatically increase the renewable energy rate. Therefore, this paper proposes an algorithm that analyzes the maximum amount of renewable energy in the network, and hence optimizes the type of electric power source connected to the transmission network, and the arrangement and capacity of each power source. The proposed algorithm is based on a genetic algorithm, which effectively processes many nonlinear variables concurrently. Accounting for the power interchange in the transmission network and the energy storage in electric heat pumps and heat storage tanks, the objective function plans the arrangement of the electric power sources that maximizes the economic efficiency of the system. The developed algorithm is applied to a renewable-energy network in Hokkaido, Japan. In this area, the introductory rate of renewable energy was 39.5% of the total electricity production. Moreover, the cost of a distributed power-supply network was 9.99 × 1010 USD. The proposed system is equivalent to 1.88 years of Hokkaido's energy consumption.

The forest shredder is a down-cutting type working machine equipped with many flail-type stirrup-shaped cutting blades around a horizontal shaft parallel to the brush-cutting plane. This type generally is called a flail mower. In cutting small-diameter trees of Quercus serrata THUNB. and in brush-cutting Peioblastus chino MAKINO var. using this machine, we clarified the features of both down-cutting and up-cutting types by comparatively studying the power requirements for both types. That required for cutting small-diameter trees of Q. serrata using the stirrup-shaped cutting blades of the down-cutting type is 40 to 50% greater than that by the up-cutting type. In cutting small-diameter standing trees, therefore, the power required can be reduced by using the up-cutting type. However, in the power required for brush-cutting of P. chino, no difference was seen between down-cutting and up cutting types. But the power required was 20 to 30% (Brush cutting is based on the same condition) greater than a power required for brush-cutting Sasa kurilensis MAKINO et SHIBATA var. which had been conducted previously using the stirrup-shaped cutting blades. This seem to be because the inside dimension of the stirrup-shaped cutting blade had been clogged with finely cut pieces of bamboo grass to increasing the exhaust resistance.

牛乳由来のスフィンゴミエリン(以下,SPM)高含有素材を経口摂取した場合の皮膚への効果を検証した。健康な日常生活を営む男女25名を対象に,SPM 高含有素材(330 mg/日,SPM 22 mg/日)のプラセボを対照にした 6 週間の二重盲検摂取試験を実施した。摂取 3 週目,摂取 6 週目および摂取終了 2 週間後に皮膚の水分量,水分蒸散量および油分量を測定した。その結果,SPM摂取群においてプラセボ群に対して左眼下部の水分量が有意に高値を示し,油分量は高値を示す傾向であった。また,自覚症状のアンケート調査では,SPM 摂取群において皮膚の「つや」および「はり」に改善が認められた。以上のことから,SPM 高含有素材は皮膚の健康を維持する上で有効な食品であることが示唆された。

Effect of treatment condition on carbonation of calcium hydroxide compact was examined. The result indicated that 100% relative humidity was quite effective for carbonation of calcium hydroxide compact. When calcium hydroxide compact was prepared under high compaction pressure, the calcite block with high mechanical property could be obtained after the carbonation although it took longer time to transform to calcite. High temperature more than 200℃ could enhance the carbonation, however, the heating more than 600℃ facilitated calcium oxide formation which caused lower mechanical property.

The aim of this study was to apply the dynamic micro-indentation method as a simple method for determining the mechanical behavior of orthodontic wire alloys. Moreover, the effect of the indentation peak load on the mechanical behavior of orthodontic wire alloys was investigated. As a result, we confirmed that the dynamic hardness and elastic modulus of orthodontic wire alloys on micro indentation scales is dependent on the indentation size effect. In conclusion, the dynamic micro-indentation method is a useful technique for determining mechanical properties within very small areas on the surface of orthodontic wires.

介護保険制度が施行されて以来8年が経過している。介護保険制度については、制度施行当初から様々な問題点が指摘されている。特に介護現場で事故が生じた場合の対応である。介護事故に関する判例の件数が多いわけではなく、裁判に至るまで発展したものは事故事例のごく一部である。しかし、裁判所の判断も数々の問題点が指摘されている。本論文では日本における介護事故の裁判例に対して提起される問題点を検討し、近年強化されている品質保証の内容及びドイツでは介護事故の責任を負う者について裁判所がどのように判断しているか、及びわが国で指摘される問題点がドイツでも生じているかにつき検討した後、今後の日本における制度の方向性を考察する。
