【目的】慢性痛では副作用をできるだけ少なくしながら痛みの管理を行うことが重要である.今回,プレガバリン(pregabalin:PGB)が効果不十分な症例に対してミロガバリン(mirogabalin:MGB)へ変更し,添付文書の用法・用量よりも少量から開始して副作用について調査した.【方法】神経障害性疼痛の要素を含む慢性痛患者に対して,PGBの一日投与量を150 mg以下に減量後,MGBへ切り替えた.MGBは添付文書の用法・用量よりも少量から開始・漸増し,2,4,8,12,24週間後に副作用を質問票で,痛みを数値評価スケールで調査した.MGB以外の内服薬は固定量で継続投与し,神経ブロック治療は同内容を継続した.【結果】対象は257名,副作用によりMGBの投与を中止となった症例は87例(33.9%),おもな中止理由は傾眠,浮動性めまいであり,MGB承認時臨床試験の副作用発現率およびおもな副作用と同様であった.副作用中止例のうち74例はMGB投与開始から12週間後までの増量期間中に出現していた.痛みを理由にPGBへ再変更した症例はなく,PGBの減量および中止による退薬症状を認めなかった.【結論】PGBからMGBへ変更する際,添付文書の用法・用量より低用量から開始・漸増しても,副作用は添付文書と同等に出現する可能性がある.MGBはより低用量から開始・漸増する方が安全と考えられる.

テフラを用いて北海道東部の釧路地域に分布する第四系の編年を試みた.釧路地域の海成層(上位から,大楽毛層,釧路層)の各模式地において複数のテフラを記載・採取し,火山ガラスの主成分化学組成などの岩石学的特徴や層位関係から,阿寒・屈斜路火山地域のテフラと対比を行った(阿寒および屈斜路テフラはそれぞれ上位から,Ak1~17およびKpI~VIII).大楽毛層の上位にはKpVIが見いだされ,また大楽毛層上部と下部に挟まる2層の火山灰層は,それぞれAk5および給源不明の広域テフラ(LowK-1)に対比される.一方,釧路層中に含まれる軽石礫・溶結凝灰岩岩塊は,すべてAk13~Ak17由来である.以上の対比結果と既報のテフラ年代から,大楽毛層の堆積は,少なくとも,0.8 Maに開始し0.1 Maより前に終了したと推測できる.釧路層の堆積時期は,1.5 Ma以降,1.0 Maより前と考えられる.

大転子骨折単独は稀とされている.初診時の単純X線で大転子のみの骨折であった20例を対象とし評価した.20例のうちMRIを16例に施行し,2例は大転子骨折のみ.14例が大腿骨転子部骨折を認めた.大転子骨折単独の報告はいくつかあるが,MRIを行っていないものが多く,実際は不顕性の大腿骨転子部骨折であった可能性もあるのではないか?

3 次元点群間の位置合わせは,点群内の点数が増加するほど計算コストが増加するため,3 次元点群を均一リサンプリングやランダムリサンプリングによって間引くことにより,位置合わせの高速化が行われてきた.しかし,リサンプリングによって点を削減した場合,疎な 3 次元点群となるため,位置合わせの精度が低下する.本稿では,3 次元形状を保ちつつ点を削減するために,3 次元点群の局所特徴に基づいてリサンプリングする手法を提案する.リサンプリング処理を高速化するために,3 次元点群から推定される法線と 2 次元距離画像の勾配の関係に着目した.距離画像の勾配は 2 次元画像処理によって高速に算出されるため,各点の法線を距離画像の勾配によって表現することで,距離画像から高速な法線推定を行うことが可能である.その後,3 次元形状が特徴的な領域であるほど点を残すように 3 次元点群を間引いてリサンプリングを行う.実験では,位置合わせの精度と処理時間に関する比較評価を行い,提案手法の有効性を示した.

瘙痒性皮膚疾患の治療効果判定においては,検証された基準に基づいてかゆみが評価されるべきである.かゆみの評価尺度としてVisual Analogue Scale(以下VAS)などの信頼性,妥当性が検証されている中,5D itch scale(以下5D)が開発された.5Dはかゆみの持続時間,強さ,経過,悪影響,身体分布を評価して点数化する自記式質問票で,著者らは5Dの日本語版を,順/逆翻訳,プレテストを経て作成した.5D日本語版により,かゆみの複数の側面を簡便に評価し定量化できることが期待される.

オブジェクト指向分散環境OZ++は、広域ネットワークに接続された計算機群においても、オブジェクトの共有と交換の機構を利用することで分散したソフトウェア資源を共用し、柔軟な分散アプリケーションの開発を可能にすることを目指したシステムである。OZ++では、分散アプリケーションの開発を支援するツール群としてデバッガ、ブラウザ、ランチャ、コンパイラフロントエンドなどを提供している。本諭文では、OZ++で提供する開発支援ツールを統合的にまとめあげ、プログラマ個々人のプログラム環境を提供する枠組みとしてのワークベンチの概要に関して述べるとともに、このワークベンチを利用した複数プログラマによる協調作業支援への応用に関して述べる。

We examined the relationship between muscle injection of drugs into the quadriceps and its contracture.Subjects were children who had received medication at a clinic during the period from January 1967 to December 1970. Among the patients of the clinic, a high incidence of quadriceps contracture had been observed during the period.As basic data, we took the results of interviews with the subjects, the results of clinical examinations for quadriceps contracture, and medical records of the subjects which had been kept in the clinic.The following results were obtained.1) There were no abnormalities among the subjects who had never been injected in the quadriceps muscle. The incidence and the severity of quadriceps contracture were closely related to the total amount of the drugs injected into the muscles.2) The total numbers of injections of the following drugs were significantly larger in subjects with symptoms of contracture than in subjects without any symptoms.The drugs were Terramycin (Ox-tetracycline), Obelon (Sulpyrin, Aminopropyrin, Theoclate diphenyl pyralin), Chloromycetinsol (Chloramphenicol), Atarax-P (Hydroxyzin hydrochloride), Gammavenin (immuno-Globlin), Panvitan (Vitamin A-D), 10%-Pantocin (Pantethine) and Phenobarbital (Phenobarbital natrium).3) It was revealed by means of quantification theory type II analysis that the main attributable factors of muscle contracture might be the age when the subjects had been injected for the first time, the total number of Terramycin+Obelon injections, and the experience of injection of 10% Pantocin or Phenobarbital, but sex and the year of the beginning of injection might be ignored.Discrimination based on these results between the two groups, a normal group and a moderate or more severe symptomatic group showed reasonable sensitivity and specificity.

死後の個人データの扱いは個人の尊厳の維持,あるいは生前の意思にできるだけ沿うことが重要である.この観点から,本報告では故人に永遠の生命をディジタルな形で与える不死のディジタル人格に関して,そのビジネス化あるは商業利用の実情を述べる.故人の顔画像の福笑いのような行き過ぎた,あるいはエンターテイメント化した故人の個人データのビジネス化などにみられる問題点を指摘して,故人の個人データのビジネス化のあるべき姿を模索する.

本稿の課題は消費者が食品の安心をどのように捉えているかを明らかにすることである。課題は、第1に、社会心理学の概念、理論を援用し、山岸が規定する安心、信頼に関するキーワードを基に安心の概念整理を行い、食品における安心の構成要素を検討し構成要素がどう安心に繋がるのかを明らかにすること、第2に、食品に対する安心の評価と構成を実証的に検討することである。安心に概念を整理し、その要素を検討すると、安心は、「信頼」と「漠然とした安心感」の2つの要素から形成され、その下位要素として、「信頼」は「表示」、「情報」、「コミットメント」。「漠然とした安心感」は「経験」、「イメージ」の5要素から構成される。消費者アンケートの結果より、安心と評価する食品を安心だと思う理由として圧倒的なのは「国産品」であること、次いで、「食品表示」であった。クラスター分析の結果から、「表示」、「情報」、「コミットメント」は同じクラスターを形成し、「経験」、「イメージ」は別のクラスターを形成しており、仮説の安心の構造図は妥当である。また、野菜における安心の構造は性別によって異なっていることが明らかとなった。

この取り組みは,「十勝"夢"プロジェクト真冬のマンゴーづくり大作戦」と題し,平成22年より地域おこしの為に経済人11名で始まった事業です。何もない北海道十勝の冬に亜熱帯果実を産地化できたらとの思いで始まり,当初から栽培にあたっては地産地消型の再生可能エネルギーの有効活用を重視しています。特に,十勝地域は太平洋型の気候なので冬も好天が続き,広大な雪原に真っ青な空に輝く太陽の光が燦々と降り注ぐという冬だからこその明るく綺麗な景色が楽しめます。一般的に北国の冬は,演歌の「津軽海峡冬景色」や,映画の「北の零年」等に代表されるように曇天で雪が毎日降っている暗く辛いイメージがあるが,十勝の冬の好天を正確に伝える事で,真冬の十勝のイメージを180度変えさせ,今は閑散期となっている冬期間の観光客増につなげていくことを考えました。同時に,農業において収穫及び収入の無い時期に新たな収入をもたらす事業を展開することによって,冬期間の地域の新たな雇用の創出につなげていくことを考えています。さらに,マンゴーの収穫時期を真冬にシフトさせながらも,化石エネルギーに頼らず,自然エネルギーで栽培する仕組みを確立することで農業生産の新たな可能性を見出しています。

The previous paper of the authors made clear the mechanism of top-spin performance in tennis and its improvement by lubrication of strings according to the high speed video analysis. As the main strings stretch and slide side ways more, the ball is given more spin due to restoring force parallel to the string face when the main strings spring back and the ball is released from the strings. Since the notches of strings decrease spin rate, the lubricant materials are effective to the notched strings and the spin rate of newly strung tennis racket without notches is much larger than that of used racket with notches. Furthermore, it showed that the more spin results in the reduction of shock vibrations of the wrist joint during impact. There are several strings now on the market that are designed explicitly to take advantage of the slippery string mechanism for top spin improvement. This paper showed the spin behavior of the ball and the revolutionary flat strings during oblique impact in the laboratory by the high speed video analysis and recognized that flat strings are more slippery and generate more spin than the conventional strings.

The purpose of this study was to evaluate the maintenance of fitness with aging. The subjects were 59 men of 49-55 years of age, who entered to Hokkaido University and once had been examined for their fitness between the years of 1950-1952. They were reexamined to determine the maintenance of fitness with age in 1983. After 31-33 years, declination of the mean values in fitness shows from 50.9 to 42.1cm in vertical jump, from 154 to 143kg in back strength, from 4.35 to 3.73L in vital capacity and from 8.4 to 3.2 times in pull-ups. Grip strength was increased from 44.0 to 47.1kg. The correlation coefficients of the values between the time they were freshmen and the present time, were relatively higher in vertical jump (r=0.61, p<0.01), moderatory higher in back strength (r=0.50, p<0.01), grip strength (r=0.48, p<0.05) and vital capacity (r=0.46, p<0.05), but lower in pull-ups (r = 0.17, not significant). Back strenght and grip strength were maintained better in a group who had been engaged in daily physical activity than those who had been inactive. But the differences in the rate of decline were not significant in vertical jump; pull-ups and vital capacity. It was noticed that 14 (23.7%) of the 59 subjects indicated that walking was felt to be the most effective factor to maintain fitness.

Upon his arrival in Japan in early 1895, Dr. B. C. Northrop (1817-1898) received an unforeseen warm welcome. The Japanese had not forgotten the old man who had never stopped working for newly-opened Japan as an introducer of Dr. David Murray and Dr. William Clark, a proposer for the return of the Shimonoseki indemnity fund and a friend of Japanese youths in the U. S.<BR>Thanks to the late Mr. Shunichi Kuga's pursuits made with worldwide collaborators, we can now look on Northrop as a man with another title of the founder of School Arbor Day here. Believing that about Northrop, like other'unemployed' foreigners, there are still more stories to tell, I tried to reassess him through newly-found materials, chiefly local papers and magazines in both languages, and was lucky enough to pick up some data, which, I hope, will cast a little more light on Northrop studies.<BR>This paper chiefly concerns : <BR>1. The content and background of his lectures both in Tokyo and in Kyoto<BR>2. His concerns over our new educational system<BR>3. His life on both sides of the Pacific and especially his friends (American and Japanese) who helped him introduce the true pictures of School Arbor Day Movement.

In the construction of tunnel in squeezing rock, where the creep is taking a part, there is a need to consider about the displacement occur on the primary support. It is necessary to place the lining because the existence of long-time convergence. At the present, the design of the lining, the handling of the primary support, the design load in such ground condition above, have not established yet. In this paper, authors proposed the new consideration of total load acting on the lining from viewpoint of life cycle cost minimum in consideration of long-term degradation of the primary support strength and the ground.
