医療保険制度における薬剤服用歴管理指導料の算定は, 現在, 紙形式の手帳が従来より用いられている. 薬局を対象として, 紙形式および電子化形式のお薬手帳に含まれるデータの種類などを明らかにするため, アンケート調査を行った. また, 薬局に来局した患者を対象として, お薬手帳に対する患者の意識を明らかにするため, アンケート調査を行った. 1,000薬局のうち547薬局が回答を返送してきた. 対象としたすべての薬局が “紙媒体のお薬手帳” を利用していたが, “電子化お薬手帳” を “紙媒体のお薬手帳” とともに使用している薬局は, 回答した薬局の15.9%にすぎなかった. 307人の患者に回答を依頼し227人の患者が回答した. “電子化お薬手帳” を利用している患者は, 回答した患者の0.5%にすぎなかった. また, “電子化お薬手帳” に記載されている項目は, “紙媒体のお薬手帳” に記載されている項目に比べて少なく, ばらつきがあった. お薬手帳を利用している患者のうち, 20.9%はお薬手帳に自分自身で情報を記入することがあると答えた ( “血圧”, “体調の変化” など). お薬手帳に対する患者の意識としては, お薬手帳の情報を, “みせたい医療関係者だけにみせたい” とする患者と, “医療関係者ならだれでもみてもらってよい” とする患者に考え方が二分していた. したがって, “電子化お薬手帳” の運用にあたっては, “紙媒体のお薬手帳” の項目と同様の項目を網羅して記載すること, 患者の意識に応じて閲覧の制限ができることなどが必要と考えられた.

Sprinters are often coached to increase their sprint running velocity by minimizing the horizontal deceleration force during the first half of the foot contact phase. The present study was undertaken to clarify the relationship between changes in the horizontal velocity of the body's center of gravity (CG) during the foot contact phase and sprint running velocity. The subjects were 26 male sprinters (100 m personal best record : 10.27-11.50 s). The experiments were carried out on an allweather track into which a force platform was planted. Subjects performed sprint running at their maximal effort. The ground reaction forces were recorded and the subjects were videotaped from the side (100 fps). The horizontal distance from the toe to the CG at the moment of foot touch-down and foot release showed no significant correlation with the maximal sprint running velocity. The duration of deceleration decreased (r=-0.517, p<0.01) and that of acceleration showed a tendency to decrease (r=-0.385, p=0.0519) with the increase of the maximal sprint running velocity. The peak forces of deceleration and acceleration increased (r=0.542, p<0.01 ; r=0.442, p<0.05) with the maximal sprint running velocity. The rate of deceleration and acceleration of the CG during the foot contact phase calculated from the impulse of the horizontal ground reaction force showed almost a constant value despite the big difference in the maximal sprint running velocity. The present results suggest that reducing the rate of CG deceleration during the foot contact phase would not improve the maximal sprint running velocity. The ability to develop a higher acceleration force to the ground moving faster to the backward relative to the body's center of mass during the shorter foot contact phase is probably a major factor in determining the maximal sprint running velocity.

慢性的便秘で乾燥皮膚を有する20歳~39歳までの女性56名を2群に分け,LB81乳酸菌(Lactobacillus delbrueckii subsp. bulgaricus 2038株ならびにStreptococcus thermophilus 1131株)を使用したヨーグルトあるいはこれにコラーゲンペプチド(CP)ならびにセラミド(CE)を配合したヨーグルトをそれぞれ120 mlずつ1日2回,4週間摂取させた.摂取前後の比較で,両群ともに皮膚の弾力性,乾燥および鱗屑の程度が改善された.CP・CE含有ヨーグルト摂取群ではキメ密度が有意に改善された.肌状態に関するアンケート調査(19項目)では,両群とも多くの項目で改善がみられた.両群ともに便秘の症状が有意に改善されたことから,本結果にヨーグルトの整腸作用が関与している可能性が考えられた.

「聞こえない」状態を医学的障害としてみる場合は、「聞こえない人、聞こえにくい人、聾者、聴覚障害者、難聴者、補聴器装用者、人工内耳装用者」などの言葉で表される。そして、「手話を使う人」のみが聞こえないことを医学的障害として捉えるのではなく、手話を使う、聞こえないことを社会文化的な違いとして捉える言葉である。次に口話法優先教育か手話法優先教育かのどちらかを受けたかによって、「聞こえない人」はグループ分けできる。そして、これらのグループ分けは、社会での立場、すなわち、アイデンティティについてのグループ分けにつながる。「聞こえない人」は、異なる「社会での立場」によって分かれる。障害者として自助努力を強いる同化主義的社会、と公的支援を得られる手話言語をもった社会ではアイデンティティが変化する。人工内耳、補聴器、口話法訓練など自助努力をして「聞こえる人」の社会で生きるか、手話通訳支援を提供され、手話通訳者の養成や手話を公用語にしようとする法律などの公助努力が進められている社会に生きるかによってアイデンティティは異なる。本稿では異なる「聞こえない人」のアイデンティティがあることを検証することにする。

びまん性特発性骨増殖症(diffuse idiopathic hyperostosis:DISH)にともなう腰椎骨折が原因で腰動脈損傷を生じ,大量後腹膜出血より腹部コンパートメント症候群(abdominal compartment syndrome:ACS)をきたした1例を報告する. 症例は77歳男性,歩行中の転倒による腰痛で近医受診後転院となった.造影CTで第2腰椎椎体の水平骨折とDISHによる骨増殖部の直接損傷と思われる腰動脈損傷,大量後腹膜出血を認めた.直ちに経カテーテル動脈塞栓術(transcatheter arterial embolization:TAE)を施行したが,膀胱内圧の上昇(39mmHg)に加え呼吸循環障害も出現しACSを合併した.緊急減圧開腹術後にsilo+vacuum packing closureによるopen abdomen managementを施行しACSを解除した.その後,観血的腰椎後方固定術を行い,神経学的後遺症を残すことなく72病日に軽快転院となった.

Jurassic accretionary complex of the Chichibu belt is distributed in the western Akaishi Mountains, central Japan. The Miocene Wada Formation, composed mainly of mudstone with basal conglomerate, is exposed in Minami-Shinano, Iida City, Nagano Prefecture, central Japan. This article reports the occurrences of Permian, Triassic, and Jurassic radiolarians from the Jurassic accretionary complex and Triassic radiolarians from the chert pebbles within the basal conglomerate of the Wada Formation. The chert pebbles are most probably derived from accretionary complex of the Chichibu belt exposed near the distributional area of the Wada Formation. Meanwhile, clasts of Cretaceous-Paleogene chert derived from the Shimanto belt nor metamorphic rock from the Sambagawa belt have never been discovered from the Wada Formation so far. This implies that the Shimanto and Sambagawa belts have not largely exposed in the Miocene in the hinterland of the Wada Formation.

本研究の目的は、うつ病の治療において、うつ症状とともに評価が必要とされている社会的機能に着目し、うつ症状と社会的機能に関連する心理的要因を明らかにすることである。単極性のうつ病と診断された66名を対象に、自動思考尺度(ATQ-R)、対処方略尺度(TAC)、ソーシャルスキル尺度(KISS)、うつ症状尺度(BDHI)、社会適応状態尺度(SASS)を実施した。その結果、うつ症状に対しては、"自己に対する非難"が関連していたが、社会適応状態には"肯定的思考"と"肯定的解釈と気そらし""社会的スキル"が関連しており、うつ症状と社会適応状態では関連する要因が異なることが示唆された。これらのことから、うつ病患者への支援においては、うつ症状だけでなく社会適応状態に関連する要因を検討する必要性が議論され最後に本研究の限界について述べられた。

Coronavirus disease 2019 (COVID-19) has been spreading globally since 2019;however, comprehensive rehabilitation of elderly patients with COVID-19 pneumonia remains a challenge. A 76-year-old American woman with COVID-19 pneumonia was admitted to our hospital. Because her disease was complicated by acute respiratory distress syndrome (ARDS), she was treated with intensive care, including invasive ventilation and extracorporeal membrane oxygenation (ECMO). During and after intensive care, she exhibited physical symptoms such as weakness, pain, shortness of breath, and difficulty in movement and exercise. Furthermore, during approximately 3.5 months of hospitalization, she received swallowing and speech therapies along with physical therapy. These rehabilitation therapies enabled her to get home in the United States. Her rehabilitation schedule had to be carefully planned according to her symptoms and infectiousness of COVID-19. This paper highlights few important points regarding the difficulty in rehabilitation including that of physical function, mental health, and cognitive function of patients with COVID-19. Furthermore, this report provides a problem-solving approach for long-term rehabilitation in elderly patients with COVID-19 pneumonia.

近年,特に若年者においてうつ病が多様化し,医学教育の現場でも対応困難な場面を体験することは多い.本稿では,「ゆとり世代」の特徴も踏まえたうえで若者のうつ病について概説し,医学生・研修医のメンタル面での初期対応に資する情報を提供する.

This research investigated mental health of marreed people in relation to their marital relationship, occupational life, and household income. A questionnaire was administered to husbands and wives, either in middle age or child-rearing years, to measure their degree of satisfaction in marital relationship, occupational life, household income, and subjective well-being. Results showed that satisfaction in workplace for men, and additionally satisfaction in marital relationship of men in child-rearing years, strongly predicted their subjective well-being. As for women, however, the strong association with subjective well-being was found for satisfaction in marital relationship, for those who were unemployed or employed part-time. The association was strongest for those who were in child-rearing years. Satisfaction in workplace was as important as satisfaction in marital relationship for women who were employed full-time. These findings suggested that satisfaction gained from what a person concentrated most of his/her energy on, explained his/her subjective well-being very well.
