An 18-year-old Jehovah's Witness injured in a traffic accident developed hypovolemic shock. Total blood loss due to a crushed pelvic fracture and degloving injury of the lower limb was estimated at more than 3, 000ml and the patient's hematocrit fell to 10.8%. The patient refused blood transfusion on religious grounds, virtually ensuring death. Failing to provide blood transfusion involved the following potential problems: i) the truck driver hitting the patient could charged with homicide rather than unintentional infliction of injury, and ii) the hospital could be sued by the patient's family or the truck driver for not providing reasonable treatment. Blood on these considerations, we transfused the patient with 30 units of blood before obtaining consent. The patient recovered uneventfully thanks to the blood transfusion followed by embolization of the iliac artery and surgery. Fortunately, neither the patient nor his family complained about treatment, and even thanked us.

With respect to collaborative physical tasks, gaze and gestures play significant roles when referring to physical objects. In video-mediated communication, however, such nonverbal cues become "ineffectual" when they are presented via a 2D monitor, making video-mediated collaborative physical tasks inefficient. This study focuses on gaze cues to support remote collaborative physical tasks using a mobile terminal and uses an eye-shaped display, "ThirdEye," a simple add-on display that represents a remote participant's gaze direction. ThirdEye is expected to be especially effective when used with mobile terminals. We investigated whether the ThirdEye’s gaze shift is effective in leading a local observer's attention toward objects in the local environment, even when ThirdEye is presented with the actual face image of a remote person. Experimental results show that ThirdEye can lead the local participant’s attention to intended objects faster than without ThirdEye.

Self-distancing is the method of adjusting the psychological distance from one’s own experience. Keeping a psychological distance from the issue affects how we address it. For example, people often devise more creative ideas for others’ problems than for their own problems. In this study, we employed virtual reality for supporting self-distancing. We developed a system that enables the user to operate his/her avatar from the third-person perspective by changing the user’s perspective out of the body. We conducted an experiment in which participants were asked to solve problems that require insight and creative thinking from either the first-person or third-person perspective (3PP). The results indicate that the 3PP increases users’ psychological distance from their experiences and brings greater insight. However, there was no significant difference in the number of ideas that users could devise. Based on these findings, we discuss the interface design required for incorporating self-distancing, regardless of users’ abilities and circumstances.

Episodic memory is known to be environmental context-dependent. The most famous effect is the reinstatement effect: retrieval works better in the same context (SC) as encoding than in a different context (DC). While various environmental context manipulation methods have been examined, we propose a new method using 360-degree video and self-avatar. Comparing the number of words recalled between the SC condition and the DC condition in a within-participant design, there was no reinstatement effect. The result was consistent with the previous works using immersive virtual environments as context. In addition, we found a negative correlation between the presence and the number of words recalled, but it didn’t reach statistical significance. Also, we revealed a time-compression effect of VR only in the condition using self-avatars relevant to virtual environments, not in the condition using a neutral avatar. These findings would be important for the various VR applications.

Recently, virtual reality technology has been used in numerous practical applications. In this paper, we developed and evaluated a proto-type system of educational VR system by using networked VR technology. To build proto-type systems, we constructed high quality historical VR contents, Copan ruins of Mayan civilization, and integrate two different types of VR systems, theater-type and home-type VR systems. By using VR Theater, a teacher can give lecture on Mayan civilization to his students as if they are in the real Copan ruins. Also, we divide students into four groups and inspect the effect of mutual observation to group stydy. In proto-type system, each group walk through Copan ruins by using home-type VR systems set up in front of the VR theater, and solve historical questions about Mayan civilization. Throughout the experiments, almost all students are interested in Mayan civilization and communicate each other in both real and VR environments.

症例は73歳,男性.7年前に糖尿病性腎症で血液透析を導入,冠動脈ステント留置後で抗血小板薬を内服中であった.接触者検診で新型コロナウイルス感染症(COVID‒19)と診断され当院に入院した.肺炎像は軽微であったが,D‒dimerが陽性でありヘパリンの予防投与を開始した.第2病日より38℃台の熱が続くため第4病日にデキサメタゾンを開始した.第6病日に腰痛が出現し,翌日には腹痛に変化した.同日の透析中にショックを呈し,貧血も進行しており透析を中止した.造影CTで左後腹膜出血と造影剤の漏出を認め,輸血を開始し感染対策を行った上で血管造影を行った.腰動脈出血を同定しコイル塞栓術で止血した.以後は貧血の進行を認めず,第60病日に転院した.COVID‒19では血栓性合併症が多くしばしば予防的ヘパリン投与が行われる.一方で抗血小板薬内服例や透析例は出血合併症のリスクが高く,抗血栓療法に関して慎重な判断が求められる.

本論文では,2つの調査に基づいて,うつ病患者の家族介護者を支援するための知見を述べる.1つ目の調査では,患者の気分の上下や予期せぬ振舞いなどに対処する家族介護者の介護活動の現状とニーズを把握する.その調査結果をふまえて介護記録Webアプリケーション「みまもメイト」を開発する.2つ目の調査では,家族介護者がみまもメイトを6週間にわたって利用することによって,家族介護者のうつ病患者に対する関わり方や患者との人間関係がどのような影響を受けたかを調べる.利用後のインタビュー調査から,家族介護者がみまもメイトを利用することによって,自身の介護活動を客観的に見つめ直す効果がある(第三者視点の導入)ことが分かった.さらに興味深いことに,みまもメイトは患者,病気,家族介護者の間の関係を変化させ,これによって,家族介護者とうつ病患者間のコミュニケーションを改善する効果があることも分かった.具体的には,みまもメイトを用いることによって,家族介護者単独で病気をかかえる患者に対処するという構図(家族介護者vs.患者+病気)から,患者と家族介護者が協調しながら病気に立ち向かうという構図(家族介護者+患者vs.病気)へと変化した.

平成22年度はまず、柏崎中学校において授業を実施した。生徒を、提案システムを利用した実験群と、提案システムからCG映像を削除した統制群に分け、それぞれの群にプレテストとポストテストを実施した。その結果、地球の自転の理解に関して、実験群の点数の上昇が、統制群の点数の上昇よりも有意に高い傾向があることがわかった。しかし、学習の様子を観察した結果、システムには、俯瞰視点(学習者がタンジブル地球儀を見る視点)と地上視点(地上から空を見上げた状況をCGによって合成した映像)を結びつけることが困難であるという問題点があるという知見を得た。そこで、俯瞰視点と地上視点を結びつける補助として、天球映像を提示することを考案した。これは、半球をスクリーンとして利用し、上部からプロジェクタで太陽の動きを投影する装置である。この装置の有効性を確認するために、被験者を、装置を利用した実験群と利用しない統制群の2群に分けて比較実験をおこなった。プレテストとポストテストによって評価をおこなったが、提案した装置の有効性を示すことはできなかった。被験者の感想や実験の様子を観察した結果、天球映像をあまり参照しない学習者が多いことがわかった。この問題を改善するためには、学習課題や学習のためのインストラクションを見直して、それぞれの装置の機能や目的を意識して学習できるようにする必要がある。また、学習の様子をより詳細に分析し、天文学習において何が問題となっているのかということに対する理解を深める必要がある。

ビデオ会議システムにおいて,遠隔参加者の視線方向を正確に伝達することが 1 つの課題となっている.この問題を解決するために,我々は,ビデオ会議システムの簡便なアドオンとして利用できる眼球型立体ディスプレイ 「ThirdEye」 を開発した.ThirdEye は人工ウレキサイト (テレビ石) を半球状に加工して作成し,底面の画像を表面に投影することができる.すなわち,LCD 上に瞳を描画し,その上に ThirdEye を置くことで,人の眼球を模倣することができる.我々は,ビデオ会議システムにおいて,ThirdEye によって対話者の視線誘導が可能かどうかを評価する実験を行った.その結果,ビデオ会議の映像のみの場合よりも,ThirdEye によって視線を併せて提示した時の方が,対話者の視線がより誘導されることを示した.

(1)2003年中に当該テーマの基礎的文献を検討した後、2004年2月に、フランス司法省行刑局および青少年司法保護局を尋ね、本研究の進行についての協力依頼を行った。その後、同年7月および2005年1月の2度にわたって、フランスの官民共同運営刑事施設2施設、民間委託型の犯罪少年収容施設1施設を参観し、官・民双方の職員に対する調査を行った。その内容は、上記施設における職員組織、職員と被収容者との関係、刑務作業、医療、外部交通、社会復帰プログラムなどについて、官・民の協力形態の実情とその具体的課題に関するものであった。(2)この過程で、フランスの官民共同運営施設は、厳罰政策によって惹起される過剰収容への対策という側面と同時に、民営化論と並行して進行していた1980年代以降の被収容者に対する政府の一貫した社会化推進政策(被収容者に市民として可能なかぎりの公役務を提供しようとする政策)によってその構想が規定されているとの認識を得た。そこで、日本の行刑改革会議(03〜04年)においても議論となったフランスの施設医療を例に、フランスの近年の政策変化とその意義について検討を行った。(3)また、この社会化推進政策が、拘禁施設をめぐるフランス犯罪社会学の理論動向(施設の閉鎖的性格を否定し、被収容者の継続的な生活時間からその役割を捉える)から強い影響を受けている点を確認し、その原点となった哲学者フーコーの監獄観とその行刑実務への影響を理論的に整理した。(4)最後に、利益追求を目的とする民間企業による刑事施設民営化は、NPO団体を中心とする施設外の人的資源による行刑への介入によって条件付けられ、この二種の外部世界との接触によって規定される点を理論的に検討した。そして、民営化による刑罰機能の変化と人的民間資源の活用に関する継続的研究を、新たな共同研究プロジェクトとして策定した。

65歳,男性.左上腹部痛・腫瘤触知を主訴に来院.入院精査の結果胃平滑筋肉腫と診断され,1988年6月29日にリンパ節郭清を伴った胃全摘・膵尾部脾合併切除を施行した.同年10月左側腹壁に境界不明瞭な腫瘤を認め,再発治療目的でAdriamycin,Cisplatin,Etoposide併用療法(EAP療法)を2クール施行した.その治療効果は著明で,触診上および腹部computed tomography(CT)検査上complete response(CR)の状態となり,現在再発の兆候なく健在である.EAP療法は,切除不能の進行胃癌患者に対して高い奏効率を認めると報告された強力なcombination chemotherapyである.一般に胃平滑筋肉腫に化学療法が奏効したという報告は少ないので,われわれの経験した有効例を今後への展望を期待し報告した.副作用としては高度の骨髄抑制,悪心・嘔吐,脱毛が認められたが,いずれも回復可能であった.

When designing systems that support remote instruction on physical tasks, one must consider requirements : 1) participants should be able to use non-verbal expressions, 2) they must be able to take an appropriate body arrangement to see and show gestures, 3) the instructor should be able to monitor operators and objects, 4) they must be able to organize the arrangement of bodies and tools and gestural expression sequentially and interactively. GestureMan was developed to satisfy these four requirements by using a mobile robot that embodies a remote instructor's actions. The mobile robot mounts a camera and a remote control laser pointer on it. Based on the experiments with the system we discuss the advantage and disadvantage of the current implementation.

Previous research has shown that human walking speed is influenced by visual stimuli in the environment. For example, the speed of optical flow in one’s peripheral vision negatively correlates with his/her walking speed. However, the effect of seeing the other walkers in the peripheral on his/her walking speed is unclear. In this study, we investigate how seeing avatars walking alongside a person affects his/her walking speed. In our experiments, participants wore a head-mounted display and saw three types of virtual avatars walking alongside him/her: whole-body silhouettes of walking persons, point-light biological motions, and spatially scrambled point-light motions. Results show that the walking whole-body silhouettes and the point-light biological motion significantly increased the participants’ walking speed, and a significant interaction between their global translational speed and local motion speed was found to change the participants’ walking speed. These results indicate that we can apply a similar technique to control multiple persons’ walking speed in the real environment.

この論文では,リモートコラボレーション空間において,遠隔地にいる作業者がどのようにして時間と身体的空間を組織化しているかを,エスノメソドロジー的相互行為分析の手法で明らかにする.さらにそうしたリモートコラボレーション空間での共同作業に対するエスノメソドロジー的・社会学的分析に基づき,社会学者と工学者からなる筆者らの共同研究グループがデザインした,いくつかのリモートコラボレーションシステムについて紹介したい.

高齢者の手術増加に伴い, 高齢者にみられる術後認知機能障害 (postoperative cognitive dysfunction : POCD) や術後せん妄 (Postoperative delirium: POD) が問題となり, 周術期の予防と管理が重要である.  抑肝散と抑肝散加陳皮半夏は神経症, 不眠症, 小児夜泣き, 小児疳症 (神経過敏) が適応で, 消化器症状 (悪心, 嘔吐) を有する場合は抑肝散加陳皮半夏を選択する. 作用機序は, ①セロトニン神経系と②グルタミン酸神経系が関与している. 通常, 成人1日7.5gを2‐3回に分割し, 食前または食間に経口投与する. 術前5~7日前より服用し, 術後早期より再開し退院日まで服用させる. 重篤な副作用に低カリウム血症 (偽アルドステロン症) がある. POCDやPODを改善し, また入院中の睡眠導入剤や抗不安薬などの併用薬の使用を減少させる可能性がある.  本稿では, 抑肝散と抑肝散加陳皮半夏によるPOCDとPODの予防と管理について述べる.
