降圧利尿薬は特にレニン―アンジオテンシン系抑制薬との併用による降圧作用の増強はあきらかで,厳格な降圧にはむしろ必須であり,多くの心血管イベントを抑制するというエビデンスがあり,副作用も適切な用量と併用薬により避けられる可能性が高い.現在安全性に関する臨床試験が進行中であるが,糖尿病合併高血圧患者などでも臨床試験を実施し,心血管リスクを減少させるための適切な使用を推進していく必要がある.

This study aims to determine the cutoff values for the compound muscle action potential (CMAP) stimulus in anatomically identified anterior (motor nerve) and posterior roots (sensory nerve) during cervical intradural extramedullary tumor surgery. The connection between CMAP data from nerve roots and postoperative neurological symptoms in thoracolumbar tumors was compared with data from cervical lesions. The participants of the study included 22 patients with intradural extramedullary spinal tumors (116 nerve roots). The lowest stimulation intensity to the nerve root at which muscle contraction occurs was defined as the minimal activation intensity (MAI) in the CMAP. In cervical tumors, the MAI was measured after differentiating between the anterior and posterior roots based on the anatomical placement of the dentate ligament and nerve roots. The MAIs for 20 anterior roots in eight cervical tumors were between 0.1 and 0.3 mA, whereas those for 19 posterior roots were between 0.4 and 2.0 mA. The cutoff was <0.4 mA for both the anterior and posterior roots, and sensitivity and specificity were both 100%. In thoracolumbar tumors, the nerve root was severed in 12 of 14 cases. All MAIs were determined to be at the dorsal roots as their scores were higher than the cutoff and did not indicate motor deficits. The MAIs of the anatomically identified anterior and posterior root CMAPs were found to have a cutoff value of <0.4 mA in the cervical lesions. Similar MAI cutoffs were also applicable to thoracolumbar lesions. Thus, CMAP may be useful in detecting anterior and posterior roots in spinal tumor surgery.

Thirty years have passed since the establishment of Japan Association for Urban Sociology in 1982. Until that time, Japan had experienced huge migration from rural area to metropolitan region, as well as rapid economic growth. Urbanization and its impacts on social, economic, and political life caused many tensions and problems in expanding urban areas, which certainly required a new type of knowledge for understanding and solution from a structural point of view. Urban sociology as a way of thinking was one of such attempts to answer these questions. From its beginning, a variety of thoughts, such as Chicago School. Marxian tradition, and New Urban Sociology, went into this emerging discipline in Japan. As its result, urban sociology, as an intellectual arena, was always filled with controversies and tensions among different schools and scholars. In addition, since its institutional establishment, urban sociology has faced with a kind of “identity crisis,” due to coming of post-urbanization situation. What is a research question specific for “urban” sociology in this highly-urbanized country? For instance, globalization was one of major factors which brought distinctive features to Japanese cities, but its impacts were actually mediated and transformed by more complex sets of global, national, regional, and local factors. This article tries to describe and evaluate historical development of this discipline in Japan since 1960s, which has always sought for key questions under ongoing changes in cities.

The purpose of this paper is to investigate the quantitative changes of autonomic nerve activity using AC electromagnetic fields. In previous research, we attempted to measure autonomic nervous activity using a Heart Rate Variability (HRV). However, due to influence of the magnetism, a correct result could not be obtained. In this research, we measured the autonomic nerve activity not affected by magnetism using measuring equipment of skin potential activity. The experiment was repeated for one subject. As a result, it was found that the skin potential level tends to rise and skin potential reflex fluctuates when magnetic exposure and after the exposure. It is thought that the blood flow was accelerated by magnetism, and the sympathetic nerve activity became dominant. However, this experiment haven’t had examined enough data regarding the researched subjects. For this reason, there is a need of gathering more data for the classification of autonomic nerve activity.

近年、無意識の認知プロセスを探究する手段として、無意識的思考理論が注目を集めている。本研究の目的は無意識的思考研究を消費者行動論に応用するため、2つのレビューを通して、研究の深化と課題を示すことである。定量的手法を用いたレビューでは、無意識的思考研究の領域を概観し、既存研究が2つのテーマに類型化できることを明らかにした。そのなかでも、判断・意思決定が重要な研究テーマであることを確認し、それらの研究がマーケティングや消費者行動の分野において展開されていることを示した。主要論文のレビューでは、既存研究を系統的に分類したうえで、思考モードの情報処理メカニズムに注目した研究と思考モードと判断・意思決定の調整変数に注目した研究を紹介した。最後に、無意識的思考研究の学術的、実務的意義を検討し、今後の研究課題を指摘する。

コニカミノルタにおいて開発してきたSIDMは,デジタルカメラの手振れ補正用,携帯電話カメラのAF用のアクチュエータとして既に実用化されている.ただし,これらはSIDMの粗動(ノコギリ変位で動かす)のみを用いる応用であった.これに対し今回,SIDMの粗動,微動(圧電素子そのものの変位で動かす)を用いた位置決め用ステージをナノコントロールと共同で開発したので報告する.

If a person's eyes are greater attracted to the target objects by matching the smell to an important scene of a movie or commercial image, the value of the image contents will rise. In this paper, we attempt to describe the image system that can also present smells, and the reason behind the improvement, from gaze point analysis, of the presence of smell when it is matched to the image. The relationship between the eye catching property and the position of the sight object was examined using the image with the scene where someone eats three kinds of fruits. These objects were gazed at for a long time once releasing their smells. When the smell was not released, the gaze moved actively to try and receive a lot of information from the entire screen. On the other hand, when the smell was inserted, the subject was interested in the object and there was a tendency for their gaze to stay within the narrow area surrounding the image. Moreover, we investigated the effect on the memory by putting the smell on the flowers in the virtual flower shop using immersive virtual reality system (HoloStageTM). It was memorized more easily compared with a scentless case. It seems that the viewer obtains the information actively by reacting to its smell.

青森ベイブリッジは, 青森港の東西の港湾施設の一体化を目的とした臨港道路であり, その主橋部は, 中央径間240m, 橋長498rnの日本最大級の3径間連続PC斜張橋である。本斜張橋は, 逆Y形主塔による広幅員一面吊り構造であり, 斜材には, 規格引張強度1942tの大容量現場製作斜材を採用し, その外套管には, FRP管を使用した。本文では, PC斜張橋の上部工の設計, 施工のうち, 主げた, 斜材の施工, および張出し架設中の施工管理について報告する。

はじめに 重症心身障害児(者)では不眠など睡眠リズムの不規則さが見られることがある。乳児に対してホワイトノイズは胎児のときに聞いていた母親の心臓の鼓動や、胎盤の血流の音に似ているため、安眠に導く効果があるという報告がある。そこで夜間覚醒している睡眠リズム障害のある利用者に対してもホワイトノイズが睡眠誘導に有効であるか検討した。 研究方法 睡眠リズム障害を有し、家族の同意が得られた2名を対象とした。A氏 (56歳男性) 脳性麻痺 B氏(41歳女性) 脳性麻痺・レノックス症候群 周波数が異なるホワイトノイズ音源を7種類用意した。両名共に不眠時にブラウンノイズが穏やかになる様子が見られたため、これを使用するとした。 実施期間 1期:2013年8月〜9月、不眠時にブラウンノイズを30分、胎児が体内で聞いている音量と同じぐらいの音量の60〜70dbで流した。 2期:2013年9月〜11月、客観的測定のため、不眠時にブラウンノイズ使用の前後に唾液アミラーゼ値を側定し、ストレス値の変化を調査した。ブラウンノイズ使用後、入眠していれば測定せずとした。 結果 A氏1期では12回中10回(83.3%)入眠した。2期では15回中6回(40%)入眠した。アミラーゼ値は9回中6回の低下認めた。B氏1期では4回中4回(100%)入眠した。2期では2回中1回(50%)入眠した。アミラーゼ値は1回中1回の低下認めた。 考察 今回は2名での検証であったがホワイトノイズの効果はあった。1期では、両名共に高い比率で入眠を認めた。2期では両名共にアミラーゼ値の低下は確認出来たが、唾液アミラーゼ測定自体が利用者に不快を及ぼし、入眠を妨げる要因となり、1期と2期の結果に差が出たのではないかと考える。重症心身障害児(者)でのホワイトノイズ使用例がないため、今後の取り組みの結果によっては、睡眠薬を使わず入眠を促す1つの手段になる可能性がある。対象者を増やして検討したい。

症例はアルコール多飲暦を有する53歳男性で,咳嗽と呼吸困難が強くなり入院となった.胸部X線写真にて左側に多量の胸水貯留を認め,CTにて膵体部から縦隔に達する膵嚢胞を認めた.MRIにて縦隔に向かう内瘻が明瞭に描出された.膵尾側切除と脾摘を施行し,術後経過は良好で膵炎や胸水の再発は認められていない.MRIは非侵襲性で縦隔内仮性膵嚢胞の診断に有用である.

Pneumonia caused by Legionella longbeachae, transmitted through contaminated soil, is very rare in Japan. A 70-year-old man with severe respiratory failure was admitted to our hospital and underwent multidisciplinary procedures. Although a urinary antigen test was negative for Legionella, he was clinically diagnosed with legionellosis and administered levofloxacin. His condition subsequently improved. Thereafter, sputum culture detected L. longbeachae. Because the DNA of L. longbeachae was detected in the gardening soil, it is suspected source of infection. Therefore, it is important to suspect legionellosis based on clinical information, even if a urine antigen test is negative.

Fossil winged ants (family Formicidae) are sometimes found in lacustrine deposits. In the Shiobara Group, however, only a fossil of a large wingless queen ant has been described. Two types of fossil winged ants belonging to the subfamily Formicinae are newly described here and identified as Lasius sp. and Camponotus sp.
