Chronic pain is a complex state that involves unpleasant emotion, autono­mic responses, helplessness against pain and movement disorder, as well as the sensation of pain itself. These memorized responses influence the present experience and behavior of the patient. A single treatment option is not enough to treat such a complex clinical state. Thus, our facility has several treatment programs to fit individual patients’ needs. We applied an individualized hypnotic approach to 161 patients who had not shown satisfactory improvement and were considered suitable subjects for psychotherapy.A hypnosis session consisted of the introduction stage, which prepared the therapeutic contexts accommodated to the change in chronic pain as well as each patient’s history, and the induction stage, which mainly targeted non–pain body sensations. Among the patients who tried hypnosis, 71.1% experienced in–session analgesia (ISA), and 46.3% experienced out–of–session analgesia (OSA). The most of the first ISA was experienced before approximately the 10th session, and the most of the first OSA was experienced by approximate­ly the 15th session after ISA.Based on the process and degree of analgesia, the number of sessions, the leaning rate of self–hypnosis, and the patients’ characteristics and experiences in the above process, we attempted to determine the conditions under which the patients were successfully engaged in hypnosis, the stage of changes in their chronic pain, and the inhibitory factors against analgesia. Despite clinical differences among the patients and their pain situations, their responses to the hypnosis implied the importance of achieving pain cessation through their own therapeutic efforts.

背景:COVID-19蔓延期の災害対応において,病院クラスター発生は病院機能停止を伴う大きなリスクである。感染者が含まれる多数傷病者受入方法を平時から検討しておく必要がある。目的:大規模災害発生時の多数傷病者受入に伴うCOVID-19クラスター発生の防止。 方法:防災訓練ワーキンググループにてCOVID-19蔓延期の災害対応を検討し,大規模災害発生時の多数傷病者受入方法とPCR検査を施行不能な場合に行う入院時のCOVID-19感染リスク分類のためのトリアージ法を策定する。結果:従来の二次トリアージに加えCOVID-19のリスク評価を行い,以下の4段階に分類するCOVIDトリアージを開発した。カテゴリーⅠ(紺)は発災前PCR陽性または抗原陽性,Ⅱ(紫):はCOVID-19の可能性が高い,Ⅲ(ピンク)はCOVID-19の可能性が低い,Ⅳ(白)は発災前PCR陰性とした。結語:災害拠点病院は潜在的なCOVID-19患者を受け入れるために従来のトリアージに加え,感染リスク別の入室カテゴリーを策定し,後日陽性者が判明した場合でも被害を最小限にする対策を準備しておく必要がある。

はじめに VC(verrucous carcinoma)は細胞異形成をほとんど示さないで増殖する扁平上皮の増殖病変であり,1948年にAckerman1)により提唱された.悪性であるのかどうかは長年議論があったが,現在では扁平上皮癌の一亜型と位置付けられている. 生検標本のみでは診断が不可能であり,よく良性疾患と誤診されることがある.間質浸潤は極めてまれとされ,臨床予後は摘出さえできれば極めて良好である.

症例の特徴 verrucous carcinomaは1948年にAckerman1)によって報告された白色調隆起性病変を形態学的特徴とする,予後のよい角化の強い高分化型扁平上皮癌である.発生部位は頭頸部,口腔,皮膚,生殖器,子宮などにみられるが,食道はまれである2).

「のど越し」という言葉はしばしば使われるがその実体はかなり複雑な感覚である。のど越しスッキリとかキレがいいと言う時とそうではない時とでは咽頭・喉頭からの神経情報にどのような違いがあるのであろうか。咽喉頭の味覚情報を担う上喉頭神経と舌咽神経咽頭枝は水の刺激に対して高い応答性を示すという特徴がある。この両神経が顕著なのど越しの感覚を誘起するビール、炭酸水、吟醸酒などに対して特徴的な応答パターンを示すという最近の知見を紹介し、「のど越し」との対応について述べた。

Historical documents show us that many Jinaichos were established around 1530's and '40s. So, I'd like to call them the second stage Jinaicho as well, the former ones of them, the first stage, the later ones, the third stage Jinaicho. I intend in this paper to clarify the process how the city plan has changed during these three stages.At first, on the topographical condition of location, places where Jinaichos are located can be classified into, (1). Plateau or hill, (2). river-terrace, (3). natural levee, (4), low swamp. The third is a good place not only for Jinaicho, but also for any other settlement, however, the first and the second must have been for the protection of the town. In respect of topography, the defence of the town has become weak for ages. And, when Jinaicho was constructed, Jori system was utilized as basic lines in many cases. This rule is the most notable in the second stage.Next, I consider the inside plan of the town from the following viewpoints. a, The scale of the town and its transformation…… The scale of Jinaicho tends to center especially upon some definite sizes, among them four square scale can be regarded as the “standard” size of Jinaicho. Moreover, we must not miss that the scale of the town is not necessarily original one, but can be found to expand or reduce in some cases. b. The position. of the principal temples…… The temple which dominates the town is almost situated at southern or western side of the town. c. How many moats enclose the town? d. Are the refraetions of roads in the town remarkable or not?…… I put up these two as the indicators which show the degree of the protective function of the district. As far as these two items are concerned, those protective functions remain even in Jinaichos established in modern ages. By the way, on the Japanese castle town, the district plan is considered at the level of “form of block” (machiwari), and “plotting of residental areas” (yashikiwari) (cf. Yamori “The Study of City Plan”).Here, I'd like to approach the characteristics of Jinaicho at one more micro-scopic level, i.e, from the viewpoint of “house type”. Then we notice there are two types of Jinaicho. We find that one is where the residental district is made up of urbanstyle houses, and the other is fundamentally, the same type as farmhouses. Now, I temporarily call the former, “Urban” Jinaicho, and the latter “Rural” Jinaicho.Such difference, is, I assume, caused upon whether agricultural factors have remained in the town or not. Considering not only above-mentioned a. to d. items, but also other factors, the pattern of Jinaicho's transformation can be summarized as follows; In the first stage, Jinaicho Plan have gradually been arranged, and completed in Yamashina, In the second stage, many Jinaichos are constructed according to its arranged plan. But in third stage, we can see the collapse of the plan, however, some Jinaichos, such as the inhabitants took initiative at the establishment of the town, succeed to the same plan as in the second stage. Moreover, we must pay attention to the geographical arrangement of Jinaicho, too. That is, the first stage is in the age of searching for the suitable place for construction, in the second stage, Jinaichos are constructed in the form of encircling Ishiyama.Finally, there remains the problem to explain the relations between Jinaicho and other kinds of settlements, but I have not had my distinct idea upon this problem yet. Therefore I will take up this problem as my next theme.

Sense of touch is fundamental perceptual modality that most people experience in their daily life. Although we tend to pay little attentions to touch modality, recent study showed that our cognitive behaviors such as the preference to warm materials and decision makings were affected by the haptic impressions obtained in our daily life. This article introduces several examples about human behaviors which are associated with haptic impressions, and provides basic knowledge on how we can elucidate such perceptual and cognitive phenomena based on multiple research aspects concerning sense of touch and bodily sensation.

触覚は,体表全体に備わっているが,特に手が果たす役割は大きい.人は触れることで,対象の形状や質感を知覚することができる.また,滑りの予知など巧みな操作に不可欠な知覚もある.触知覚は,対象と皮膚との力学的相互作用による皮膚の変形や熱の移動を機械受容器が取得することで行われている.皮膚は,機械受容器にとって一種の力学的フィルタであり,人の手や指,皮膚の構造には,触知覚のための巧妙な力学的メカニズムが仕組まれている.多くの触知覚メカニズムがまだ未解明で断片的ではあるが,本稿では,機械受容器に有益な触覚増強をもたらす皮膚構造や知覚対象に適切な指や手の構造について概観し,触覚の観点から人工の手を考察する.

23Na-MRI provides information on Na+ content, and its application in the medical field has been highly anticipated. However, for existing clinical 1H-MRI systems, its implementation requires an additional broadband RF transmitter, dedicated transceivers, and RF coils for Na+ imaging. However, a standard medical MRI system cannot often be modified to perform 23Na imaging. We have developed an add-on crossband RF repeater system that enables 23Na-MRI simply by inserting it into the magnet bore of an existing 1H MRI. The three axis gradient fields controlled by the 1H-MRI system were directly used for 23Na imaging without any deformation. A crossband repeater is a common technique used for amateur radio. This concept was proven by a saline solution phantom and in vivo mouse experiments. This add-on RF platform is applicable to medical 1H MRI systems and can enhance the application of 23Na-MRI in clinical usage.

Wild cyclamen (Cyclamen purpurascens) is considered as a precious breeding material for the development of new cultivars. Malvidin 3,5-diglucoside is the main anthocyanin in the petals of C. purpurascens, whereas the F1 progeny of the C. persicum × C. purpurascens cultivars cross contains 3,5-diglucoside-type anthocyanins as the main pigment. The anthocyanin 5-O-glucosyltransferase (A5GT) enzyme is responsible for the glycosylation of the A ring of anthocyanin at the 5-O-position, which implies that the expression of A5GT is dominant in the petals of C. purpurascens × C. persicum cultivars. Here, we isolated the complete open reading frame of the A5GT gene from C. purpurascens (Cpur5GT). Results of qRT-PCR revealed that Cpur5GT shows tissue-specific expression, with strong expression in fully opened petals and weak expression in young petals. In vitro enzyme assay showed that when uridine diphosphate glucose was used as the sugar donor, recombinant Cpur5GT could catalyze the glycosylation of 3-glucoside-type anthocyanidins at the 5-O-position, but when uridine diphosphate galactose was served as glycosyl donor, the reaction could not be performed. These results demonstrate that Cpur5GT exhibits valid anthocyanin glucosylation activity and could be used to analyze the mechanism of A5GT-mediated flower coloration in cyclamen in future studies.
