Snails of the genus Pomacea in several regions of Japan showed genetic divergence. Phylogenetic analysis based on the mitochondrial cytochrome c oxidase subunit I gene segregated them into two major clades, one corresponding to P. insularum, the other to P. canaliculata. We developed a simple molecular method to distinguish between the two species to investigate their distribution in Japan. We identified P. canaliculata at all 16 sampling locations except Iriomote Isle, and P. insularum at four locations (Iwata, Fukuyama, Ishigaki Isle, and Iriomote Isle), supporting a previous report that P. canaliculata is the dominant species in Japan. Morphological comparison of the two species collected from the same habitat (Ishigaki Isle) revealed that most P. canaliculata had clear dark spiral bands on their shells, and that P. insularum had no or only faint spiral bands. However, it is difficult to distinguish the two species using only shell morphology.

Background: Gastric cancer, the most common cancer in the world, is affected by some foods or food groups. We examined the relationship between dietary intake and stomach cancer risk in the Korean Multi-Center Cancer Cohort (KMCC).Methods: The KMCC included 19 688 Korean men and women who were enrolled from 1993 to 2004. Of those subjects, 9724 completed a brief 14-food frequency questionnaire at baseline. Through record linkage with the Korean Central Cancer Registry and National Death Certificate databases, we documented 166 gastric cancer cases as of December 31, 2008. Cox proportional hazard models were used to estimate relative risks (RRs) and 95% CIs.Results: Frequent intake of soybean/tofu was significantly associated with reduced risk of gastric cancer, after adjustment for age, sex, cigarette smoking, body mass index, alcohol consumption, and area of residence (P for trend = 0.036). We found a significant inverse association between soybean/tofu intake and gastric cancer risk among women (RR = 0.41, 95% CI: 0.22–0.78). Men with a high soybean/tofu intake had a lower risk of gastric cancer, but the reduction was not statistically significant (RR = 0.77, 95% CI: 0.52–1.13). There was no interaction between soybean/tofu intake and cigarette smoking in relation to gastric cancer risk (P for interaction = 0.268).Conclusions: Frequent soybean/tofu intake was associated with lower risk of gastric cancer.

A 5-year-old Shih Tzu was presented with intermittent vomiting and anorexia. Microhepatica and reversed position of the abdominal organs were observed on radiography. Ultrasonographically, portosystemic shunt (PSS) was tentatively diagnosed. On computed tomography (CT), the distended portal vein drained into the left hepatic vein. The caudal vena cava (CdVC) splited postrenally and converged at the renal level. Cranial to this, the azygos continuation of the CdVC was confirmed. On thorax, persistent left cranial vena cava (CrVC) along with right CrVC was found. This is first report of dog with persistent left CrVC in accordance with multiple abdominal malformations. CT angiography was useful to evaluate the characteristics of each vascular anomaly and determine the surgical correction in this complex case.

症例は66歳,女性.2001年10月,上行結腸癌に対し回盲部切除,D3郭清を施行した.組織学的には粘液癌SE N0 Stage IIであった.2002年8月,腹部CTにて右腎下部およびS状結腸近傍に再発を指摘され,小腸部分切除,S状結腸切除,横行結腸切除および右卵巣切除を行った.さらに2003年1月,臍右側に腹壁に浸潤する腫瘤および胃幽門輪近傍に胃壁に浸潤する腫瘤を認め,腫瘤摘出および幽門側胃切除,Billroth I法再建を行った.その際腹腔内に小結節散見し,2カ所術中迅速病理検査に提出したところ播種と診断されたため,肉眼的に確認できる小結節を可及的に切除した.組織学的には,同様に粘液癌で播種病変と考えられたが,小結節に関しては,術中迅速検査に提出した検体以外は炎症性の結節であった.術後化学療法はいずれも副作用出現し中断した.その後,最終手術より7年経過した現在でも無再発生存中である.

腹腔内に投与された水溶性抗がん剤のほとんどは速やかに循環血液中に吸収される.そのため少量の水に溶解した抗がん剤溶液の腹腔内投与では,有効な腹腔内抗がん剤濃度の維持を得ることが出来ない.この問題を解決するため,さまざまなdrug delivery system(DDS)を用いた腹腔内がん化学療法が考案・開発されてきた.また,近年ようやく腹膜転移の成立機構や腹腔内からの薬物吸収動態などの解明が行われるようになってきた.本稿においては,それらに基づくDDSを用いた腹腔内がん化学療法について,最近のtopicsを交えながら概説する.

Since 1984, we have performed osteosynthesis for intertrochanteric fracture of the femur using CHS method.79 Compression Hip Screws and 12 Captured Hip Screws were used for 46 stable and 45 unstable intertrochanteric fractures of the femur.The mean age was 77.9, ranging from 61 to 96 years.Among 91 patients, 8 patients could not walk even before the injury. 62 patients out of the remaining 83 patients (74.7%) were discharged with one cane gait.The major symptoms of 62 patients who could walk postoperatively were knee pain (25.8%) and thigh pain (9.7%).The major radiographic findings of those 62 patients were shortening of the neck more than 5mm (25.8%). But the shortening of the neck and the knee pain don't seem to correlate to each other.21 patients out of 83 patients (25.3%) were not able to walk on one cane. But systemic complications seemed to be the main causes in more than half of those 21 patients.

当院で行われた耳鼻咽喉科手術症例で75歳以上の患者295例について術前併存症の有病率や術後合併症の発症について検討した。75歳以上の症例において,術前併存症では循環器疾患の割合が比較的多く,60~64歳の群と比べると,高血圧・冠血管疾患・呼吸器疾患の割合が有意に高かった。高齢者における術前検査の循環器異常は既往に循環器疾患がある例が多かったが,呼吸器異常は既往のない症例の方が多かった。重篤な術後合併症は,術前併存症の状態が悪い上に侵襲の大きい手術を行った2件であったが,年齢的な要因が関与しているとは考えにくいと思われた。

【目的】A府B市C校区では,2010年度より開始された国土交通省の「高齢者等居住安定化推進事業」の支援を受け,運営されている地域レストランがある。レストラン調製弁当の喫食ならびに食と運動の健康講座参加と会食を通じて,食生活状況および高齢者の精神的・身体的健康の状況を把握し,地域レストランと連携した高齢者の食生活支援プログラムにおける介護予防の有用性を検討した。【方法】事前に研究趣旨を説明し同意の得られた男性19名・女性30名の高齢者(65歳~92歳)49名が,会食群22名(3食喫食/週,講座への参加と会食),配食群14名(弁当の配食,3食/週),対照群13名に分かれて10週間のプログラムに参加した。各群とも参加前・参加後・終了1ヵ月後に,老研式活動能力指標,主観的幸福感などの自記式による質問紙調査を行い,食事バランス管理・運動の自己管理にはTAKE10!を用いた。【結果】会食群ではプログラム参加後,主観的幸福感が有意に高くなり,食品の多様性を心がけている者は,主観的幸福感が高いことおよび食生活満足度に正の相関がみられた。【結論】高齢者に対する10週間の地域レストランを活用した食と運動の健康講座は,他者との交流や会食によって主観的幸福感が高くなるという精神的健康の改善を促し,高齢者の介護予防の一助となることが示唆された。

Renewal of conditioned fear by changing contexts after extinction was explored in an experiment with rats' lick suppression preparation. After repeated pairing of a tone with an electric shock in one chamber (Context A), conditioned fear to the tone was extinguished in the other chamber (Context B) on the water licking baseline. Return to the original chamber renewed the conditioned fear (ABA renewal effect). It was also found that shifting to Context B after both conditioning and extinction in Context A resulted in a brief recovery of the conditioned fear (AAB renewal effect). Implications for relapse of phobia after behavioral therapeutic treatments, such as flooding and systematic desensitization, were discussed.

In a traditional environment Object VR virtual object is displayed on the display, the operation is performed with a mouse. Therefore, dealing with like a smooth interaction with real objects in the hand is difficult to say, the immersion fades. This research is no need to wear special equipment users, proposal and implementation of a spherical display the real-world oriented interfaces enables natural view point movement as well as dealing with object in hand for Objective.

Aim: Community Consultation Center was established in 2007 as a core facility of a project entitled "Community Support Network for Citizens with Mild Cognitive Impairment and Dementia" subsidized by the Ministry of Education, Culture, Sports, Science and Technology. This study reports the activity within the facility and users' outcome. Methods: At the facility, users consulted their memory problem, and a screening tool with a touch-panel type computer (TP) was used to check their memory loss. Dementia was suspected when the TP score was 12 or less points, and clinical psychotherapist implemented Mini-Mental state examination. All the results were summarized in reports, and we prompted users to see their primary doctors, or nearby medical institutes that we offered. In this study, we asked these medical institutes of their outcome. Informed consent was obtained from all users. Results: A total of 2,802 people visited the Center, and 1,565 people registered (male/female=519/1,046; mean age, 74 years). 561 people used the center twice or more. Among 1,354 who had TP, 722 users got a score under 12 (46.1%). A total of 409 responses from medical institutes were collected. The data revealed that Mild cognitive impairment (MCI) was 11.2%, Alzheimer's disease was 37.1%, and vascular dementia was 8.00%. Conclusion: These results indicate that approximately half of the users of the Center was suspected dementia, and a prevalence of both MCI and dementia reached to about 60%. This Center has proven to be useful for early detection and diagnosis.

本研究は,独自に考案した徒手筋力計を用い,多発筋炎8例,皮膚筋炎1例(平均44.8歳,男性3例,女性6例)の頸屈曲,肩関節外転,および下肢伸展挙上筋力の3動作筋力を経時的に測定した。その結果,3動作の総和した筋力は,全例においてクレアチンキナーゼと相関関係を認めた。このことから,3動作の総和筋力は,個人の全身筋力として反映することができ,さらに継時的な測定結果は多発筋炎・皮膚筋炎の病勢を反映するため,急性期のリハビリテーションにおいてその評価と治療効果判定を行う上で有用であることが示唆された。
