Pharmacists are expected to be active members of the healthcare team in emergency medicine, because many pharmaceuticals are administered to patients with life-threatening conditions. However, adequate education for pharmacists and pharmacy students is not provided. The “Emergency Pharmaceutical Sciences” course was introduced for the first time in Japan by the Department of Pharmacy, Okayama University, to offer advanced education in emergency medicine and research related to critical care. We offer an emergency pharmaceutical training program with high-performance simulators and have succeeded in improving the clinical skills and confidence of pharmacy students. In this review, we introduce our activities intended to mold pharmacy students into emergency pharmacists who can contribute to emergency medicine.

アズキ熱水抽出物からヒト胃ガン細胞KATO III細胞の増殖を抑制する物質を単離した. 本物質をFT-IR, ¹H-NMR, ¹³C-NMR, DEPT, COSY, NOE, HSQC, HMBC, ESI-MS, APCI-MSにより構造解析したところ, 新規セスキテルペノイド配糖体であったことから, Vignosideと命名した. Vignosideは低濃度ではヒト胃ガン細胞KATO III細胞の増殖を抑制しないが, 750μMでは約60%の増殖抑制効果を示した. その増殖抑制機構はアポトーシスによるものではないことが判った.

これまでに我々は, アズキ熱水抽出物がヒト胃がん細胞にアポトーシス誘導を誘発させることやベンゾピレンにより化学発がんさせたマウスに本抽出物を摂取させることで, がんの増殖を抑制することを報告した。本研究では, さらにこの抽出物を水-メタノール系ODSカラムクロマトグラフィーに供し, その後, アポトーシス誘導物質を分取HPLCにより分離・精製した。アポトーシス誘導物質は質量分析, ¹H-, ¹³C-NMRなどから, カテキン-<i>O</i>-7-β-グルコピラノシド (C7G) と同定された。C7Gは培養ヒト胃がんKATO III細胞だけでなく, ヒト白血病細胞HL-60にもアポトーシス誘導を誘発したが, 正常細胞には影響がなかった。このC7GによるDNAの断片化は, <i>N</i>-Acetyl-L-cysteine により抑えられた。以上の結果から, C7Gによるアポトーシス誘導には活性酸素が関与しているのではないかと推測される。

昭和59年度より運輸省第二港湾建設局が実施している「東京国際空港沖合展開事業」の中核プロジェクトの一つである「新C滑走路」は, 計画どおり平成9年3月に供用を開始する予定である.<br>新C滑走路は, 東京湾内の浚渫工事から発生したヘドロや東京都内から発生した建設残土が捨て込まれて形成された人工の超軟弱地盤上に建設されている. 当該地区は埋立地盤の透水性の低さに起因して地下水位が高く, さらに残留沈下量も供用開始後50年で最大1.5m程度が予想される劣悪な環境下に置かれている. このため, 設計段階から, 特に高地下水位対策および残留沈下を見込んだ検討を行ってきたものであり, 本論文はこれら検討の概要と, 設計, 施工の成果について報告するものである.

Guanine and xanthine nucleoside derivatives (3, 4 and 6) bearing nitrosourea functional groups were synthesized from guanine nucleoside ureas (2) obtained by the reaction of 2'-deoxy-2'-aminoguanosine (1) with isocyanates and their antitumor activity against sarcoma-180 solid tumor and leukemia L-1210 were determined. Among the compounds tested, 2'-deoxy-2'-[3-(2-chloroethyl)-3-nitrosoureido]-xanthosine (4b) found to have the most potent activity. Moreover, very slight decrease in white blood cells of mice bearing sarcoma-180 solid tumor was observed after administration of 4b.

暗号の基本的な機能は守秘と認証であり,それに基づいて,ネットワークにおける情報セキュリティの基盤が構築される.このため,ネットワーク社会において暗号技術の果たす役割は極めて大きい.本講演では,このように重要な暗号技術について,その概要を述べ,ついで,今後の安心できるネットワーク社会形成のために最も重要な課題である暗号評価,長期間安全な暗号,人と暗号の調和に対するソリューションとして,暗号技術評価プロジェクトCRYPTREC,情報量的に安全な暗号システム,およびヒューマンクリプトの三つの話題をとりあげて論じる.The functions of cryptography are confidentiality and authenticity. The information security infrastructure of the network society is constructed on the basis of these functions. Thus cryptographic techniques play very important roles in the modern society. In this talk, after introducing fundamental concepts of cryptology, we discuss three topics, CRYPTREC (national initiative of evaluating cryptographic techniques available for the electronic government), cryptosystems of information theoretic security, and humancrypt, as solutions to important problems of security evaluation, long-term security, and harmonization of cryptography and human beings.

Direct photographs of the solar granulation near the limb were obtained with a 30cm balloon-borne telescope. The observed position on the solar disk was at cosθ=0.7,where θ is the angle between the observer and the normal to the solar surface and the wavelength used was 545nm with a pass band of 40nm. Total half-width (FWHM) of the autocorrelation function of the contrast of the granulation was found to be 780km and 660km in radial and tangential direction to the solar limb, respectively. Although the latter value agrees with the older determination at the disk center, the distance, 2000km, to the minimum value of the auto-correlation function was much larger than those obtained by Leighton at the disk center. Two dimensional power spectrum was obtained and after corrections due to noise spectrum and finite resolution of the optical system, the root-mean-square brightness fluctuation of the granulation was found to be 7.0-7.9%. It is to be noted that the modulation transfer function of the optical system was determined at the partial eclipse of July, 30,1981. Two dimensional power spectrum per unit frequency area (arcsec^2) was fonund to be proportional to exp (-1.9f) where f is the frequency (cycles/arcsec). Further the autocorrelation function as determined from the earlier balloon flight with the use of a 10cm telescope showed a positive maximum around 24&acd;45Mm, which roughly coincides with the size of the supergranulation.

The possibility of West Nile virus (WNV) activity in Japan is discussed in reference to WNV ecology and epidemics in the USA, where the virus appeared in 1999 and rapidly expanded its distribution area with increasing numbers of patients. The arrival of WNV in Japan would occur through three transporting systems: WNV-infected mosquitoes carried mostly by plane; import of infected pet birds; and migration of infected wild birds. Once in Japan, the spread and symptoms of WNV disease among human beings and horses would be more serious and drastic in Japan than in the USA. The reasoning for this is that the level of cross-immunity of Japanese against WNV induced by the smaller wave of recent epidemics of the closely related Japanese encephalitis virus must be lower than that of Americans due to the larger wave of recent epidemics of the closely related Saint Louis encephalitis virus. Public participation as well as governmental efforts would be a vital necessity to prevent the arrival of WNV and to control epidemics after arrival. As countermeasures, surveys of three subjects should be conducted first: vector mosquitoes inside planes from WNV epidemic regions, including the USA; dead birds infected with WNV; and vector mosquito breeding sites. Second, reduction of breeding sites and/or application of insect growth regulator (or insecticides) should be carried out in and around dwellings and workplaces. Third, citizens should be encouraged to protect themselves from mosquito bites.

急性期・回復期のリハビリテーション(以下リハ)は、理学療法士、作業療法士、言語聴覚士の専門職が関わり、多い場合には1日に3時間ものリハを受ける機会がある。しかし退院後、生活期に移行すると、当施設をはじめ多くの老健施設では専門職によるマンツーマンで実施する個別リハの時間は大幅に減少する。そのため、入所される利用者の中にはリハに対し不安を抱かれる方も少なくない。武原は、急性期・回復期のリハが機能障害の回復を目的とした「治療モデル」であるのに対して、老健施設のリハは残された心身機能でいかに快適な生活を実現するかをテーマにした「生活支援モデル」であると位置づけている。そこで今回、病院リハを終了後に当施設へ入所された利用者の身体機能、精神機能、ADLを評価し、当施設のリハにおける現状と課題を検証した結果、精神機能・ADLにおいて有意差を認め改善傾向を示すことが示唆された。

Aim/background: Prognosis of hepatocellular carcinoma with extrahepatic metastasis (IVb) was reported less than 6 months. Sorafenib (NEX) is an only established therapy for IVb. We evaluated the clinical factors of IVb treated with NEX for prolonging prognosis. Method/Patients: Clinical backgrounds of 23 IVb were investigated (Average age; 66.4 years old), retrospectively. Results: All were Child-Pugh A. In 8 cases, NEX was continued after progressive disease (PD). One year survival rate of them were 58% and average period of treating with NEX beyond PD was 327 days. There was a tendency that NEX was abandoned in IVb with portal vein tumor thrombosis (PVTT) at early timing. Conclusion: Continuing NEX after becoming beyond PD can be a therapeutic option to improve the prognosis in IVb without PVTT.

Numerous attempts have been conducted in order to recover emergent plants and submerged plants in various lakes. However, floating-leaved plants, which make water quality worse, have been rarely focused. The purpose of this paper is to evaluate the effect of mowing down the floating-leaved plants area partially in Lake Suwa, where floating-leaved plants-dominated state is observed in shallow zone. We measure water temperature, turbidity and water velocity inside and outside of the floating-leaved plants area. It is confirmed that 1) The variation range of water temperature inside of floating-leaved plants area is smaller than that of outside. 2) Floating-leaved plants lessen the vertical water circulation. 3) Outside of floating-leaved plants area is easier to be influenced by cold water than inside when cold water intrude into water grass area. 4) Turbidity inside of floating-leaved plants area is higher than outside. 5) The leaves of floating-leaved plants become the resistance of surface current.
