小児気管支喘息を持つ親又は保護者が記入する,身体,社会,家族,感情,個人の発展の5つの項目からなるQOL (quality of life)調査票を作成した.回答は最大の支障を1点,支障なしの5点の5段階評価とし,4歳未満は23問,4歳以上は31問とした.本報告は初診時のデータに基づき,反復再現性等の信頼性,因子的妥当性を検討した.2週間発作発現の回数が安定していた喘息患児,4歳未満8例,4歳以上26例の級内相関係数は0.879,0.793を示し,再現性を示した,喘息患児102例,健常児40例,感冒等の罹病中の非喘息患児12例のQOL総合点のCronbachαは0.6329〜0.8829で内的一貫性を示した、喘息患児の個人の発展項目以外の身体,社会,家族,感情との関連質問の回答は健常児と比べ有意(p<0.05)に低下した.因子分析により,QOLを構成する5つの項目および項目内の各機能が累積寄与率60%以上で確認され,本QOL調査票は再現性,構成上の信頼性,因子的妥当性を有し,経時的な治療法のなかで調査票の適用が可能と考えられ,喘息管理と治療目標のあり方を検討する上で有用な調査票と考えられた.

終末期についての話し合いは患者家族のquality of lifeを規定する重要な要因である.本研究はがん治療医を対象とした質問紙調査の自由記述の質的分析から,終末期の話し合いにおける課題に関するがん治療医の意見を収集した.質問紙は864名に送付し490名から回答を得た.自由記述から合計420意味単位を分析対象とした.がん治療医が終末期の話し合いを行う際の問題として(1)患者家族の課題(【患者家族の個別性に対応することの難しさ】【病状理解の難しさ】)(2)医療者に起因する課題(【患者家族・医療者双方への精神的サポートの不足】【医療者間の考え方の相違】など)(3)システムと体制に関する問題(【時間・人的リソースの不足】【教育・研究の不足】など)が抽出された.本研究の知見は,今後緩和ケア医とがん治療医が共同してがん患者との終末期の話し合いを行う際の相互理解に役立つと考えられる.

Eyeblink conditioning is an associative learning paradigm in which an association is formed by the paired presentation of a neutral conditioned stimulus (CS), such as a tone, and an unconditioned stimulus (US), such as a corneal air puff or a periorbital shock. In the rabbit, cerebellum-brainstem circuitry plays an essential role in delay eyeblink conditioning, in which the US is delayed and terminates simultaneously with the CS. However, the neural mechanisms underlying delay eyeblink conditioning in mice and rats are unclear. In addition to cerebellum-brainstem circuitry, the amygdala is reportedly important for delay eyeblink conditioning in the mouse and rat. Here, we review the neural mechanisms underlying delay eyeblink conditioning in the rabbit, mouse, and rat, discussing relevant neural circuitry specific to mice and rats versus rabbits. We also review the neural circuitry underlying trace eyeblink conditioning, in which the CS and US are separated by a stimulus-free trace interval. The forebrain and cerebellar-brainstem circuitries are important for trace eyeblink conditioning in the rabbit, mouse, and rat. This review provides useful information for future research on eyeblink conditioning and for understanding neural substrates subserving learning and memory.

小腸アニサキス症は強い腹痛および炎症所見を呈し,診断が困難で急性腹症として開腹される例が多く報告されている。今回我々は,①発症前数日以内の鮮魚の生食,②腹部CT上腸閉塞など特徴的所見,によりER受診当日に小腸アニサキスと診断し,保存的加療にて改善した6例を経験した。[症例1]54歳の男性。生秋刀魚を摂食した3日後,上腹部痛が出現した。[症例2]63歳の男性。生鯖を摂食した2日後に臍周囲痛・嘔吐が出現した。[症例3]57歳の男性。ほぼ毎日刺身を摂食していたが,受診前夜よりの腹部全体の間欠痛が出現した。[症例4]36歳の男性。生鰹を摂食した翌日に心窩部痛・嘔吐が出現した。[症例5]63歳の女性。生サンマを摂食した翌日,下腹部痛・嘔気が出現した。[症例6]55歳の男性。しめ鯖を摂食した2日後,心窩部から臍周囲にかけての腹痛出現が出現した。全例において来院時の腹部造影CT上,造影効果を伴う限局性・全周性の小腸壁の肥厚と内腔の狭小化,および口側の小腸の拡張と液面形成,腹水貯留を認めた。小腸アニサキスと診断し,保存的加療にて改善した。後日来院時と発症4-5週とのペア血清で特異的抗アニサキス抗体価の上昇を確認した。発症前数日以内に鮮魚を生食後,強い腹痛を主訴に来院し,特徴的な腹部造影CT所見を呈した患者は,小腸アニサキス症を常に念頭に置き,早期診断,治療をする必要があると考えられた。

肝細胞癌切除後の肺門リンパ節転移巣を切除し,良好な経過を得た症例を経験したので報告する.症例は63歳男性.平成5年10月,肝S7の肝細胞癌に肝後区域切除を施行し,約2年後S8の残肝再発と右肺上葉の肺転移に対し肝部分切除と肺部分切除を施行した.平成8年8月の胸部CTにて約2cmの肺門リンパ節腫大を認め, 3カ月後には腫瘤は約3cmに増大した.肺門リンパ節転移と診断し,平成9年1月よりミトキサントロンによる化学療法を施行し経過観察した. 5月の胸部CTで腫瘤は約4cmと増大したが,画像上で新たな他病変はなく手術適応と考えた.血管造影で大血管への浸潤はなく, 6月25日右開胸下右肺上葉切除とリンパ節郭清を行い術後15日目に退院した.術後約1年経過した現在,外来にて厳重に経過観察をしているが血液検査および画像診断で再発の徴候はない.

アスペルガー症候群(Asperger syndrome 以下AS と表記)を持つ3人の若い女性において見られた、恋愛と性の課題の特徴をまとめ、援助の仕方について検討した。AS を持つ女性にとって、恋愛や性の課題のみがとりわけ特別な障害というわけではない。しかしこれらの課題は、健常人でも各自の認知バイアスの歪みを受けやすいもので、AS の方にとっては、とりわけその個性の独自性に修飾されやすいものであった。また、危険な対処行動に至る場合もありうることがわかった。このため、3 例のそれぞれが持つ、AS 独自の認知特徴に焦点を当てた治療とカウンセリングの仕方について検討し、考察した。Romance and sexual behavior are two of many behavioral problems in people suffering from Asperger's Syndrome (AS). The problems are easily affected by distorted cognitive bias in both AS individuals and normal subjects, which may lead to unsafe coping behaviors. Problematic behaviors related to romance and sexuality of three women with AS were investigated, and methods of psychological support are suggested.Based on the results of the study, the authors recommend methods of treatment and counseling utilizing supportive psychotherapy focusing on their own cognition..

Design of a very precise space structural system requires an accurate estimation of uncertainties on structural and environmental parameters. For the purpose, high accurate estimations of uncertainties of the structural parameters and the effect of the paremeter uncertainties on the structural response uncertainty are required. Then, the robust design optimization based on the accurate estaimation is required. This study focuses on the former part, a high accurate estimation of the uncertainties. The applicability of a bootstrap method known as a nonparametric technique is investigated to estimate the confidence intervals of the uncertain parameters related to vibration responses of simple structures. The target structure is a cantileverd bow-shaped structure composed of beam and tether that is a breadboard model of a lightweight space reflector. The confidence intervals identified from several numbers of vibration tests is compared by the nonlinear finite-element analysis.

The purpose of this study was to examine how university social capital and subjective social capital could predict undergraduate students' subjective well-being including depression, school satisfaction, and life satisfaction. In this cross-sectional study, we conducted multilevel structural equation modeling on the data of 2,021 students at 38 universities in Japan. At the university level, we found the associations between social capital (fellows) and depression, social capital (classmates) and life satisfaction, and social capital (faculty) and school satisfaction. At the student level, all subjective social capital (fellows, classmates, and faculty) were associated with all the factors of subjective well-being. These results suggest the influence of university social capital and that of subjective social capital are associated with students' subjective well-being.

We Reported two cases of pyogenic arthritis of the knee which were obliged to undergo arthrodesis. Both cases had past histories of repeated steroid injection into the knee joints. Initially they were treated with closed irrigation-suction therapy, but did not improve at all, and finally underwent arthrodesis.Second case was a deep sepsis after total knee arthroplasty. In both cases bacterial isolation from specimens revealed pseudomonas pseudomonas aeruginosa, but bone union was obtained favorably by external fixation.
