Background:Because the early diagnosis of subclinical cardiac sarcoidosis (CS) remains difficult, we developed a deep learning algorithm to distinguish CS patients from healthy subjects using echocardiographic movies.Methods and Results:Among the patients who underwent echocardiography from January 2015 to December 2019, we chose 151 echocardiographic movies from 50 CS patients and 151 from 149 healthy subjects. We trained two 3D convolutional neural networks (3D-CNN) to identify CS patients using a dataset of 212 echocardiographic movies with and without a transfer learning method (Pretrained algorithm and Non-pretrained algorithm). On an independent set of 41 echocardiographic movies, the area under the receiver-operating characteristic curve (AUC) of the Pretrained algorithm was greater than that of Non-pretrained algorithm (0.842, 95% confidence interval (CI): 0.722–0.962 vs. 0.724, 95% CI: 0.566–0.882, P=0.253). The AUC from the interpretation of the same set of 41 echocardiographic movies by 5 cardiologists was not significantly different from that of the Pretrained algorithm (0.855, 95% CI: 0.735–0.975 vs. 0.842, 95% CI: 0.722–0.962, P=0.885). A sensitivity map demonstrated that the Pretrained algorithm focused on the area of the mitral valve.Conclusions:A 3D-CNN with a transfer learning method may be a promising tool for detecting CS using an echocardiographic movie.

Background: The efficacy of sodium-glucose cotransporter 2 inhibitors (SGLT2i) in patients with acute chronic heart failure (HF) is increasingly being reported. However, it is not clear when SGLT2i should be initiated in patients with acute decompensated HF (ADHF) after hospitalization. We retrospectively analyzed ADHF patients with newly prescribed SGLT2i.Methods and Results: Among the 694 patients hospitalized due to HF between May 2019 and May 2022, data were extracted for 168 patients with newly prescribed SGLT2i during the index hospitalization. These patients were divided into 2 groups: and early group (92 patients who started SGLT2i within 2 days of admission) and a late group (76 patients who started SGLT2i after 3 days). Clinical characteristics were comparable between the 2 groups. The date of cardiac rehabilitation initiation was significantly earlier in the early than late group (2.5±1.2 vs. 3.8±2.2 days; P<0.001). Hospital stay was significantly shorter in the early group (16.4±6.5 vs. 24.2±16.0 days; P<0.001). Although there were significantly fewer HF readmissions within 3 months in the early group (2.1% vs. 10.5%; P=0.044), the association disappeared in a multivariate analysis including clinical confounders.Conclusions: Early initiation of SGLT2i may shorten hospital stays.

We herein report the case of a 38-year-old man who developed parkinsonism 4 years after ingesting glyphosate. The patient presented with right-sided bradykinesia and cogwheel rigidity without autonomic symptoms. Magnetic resonance imaging of the brain and [123I]-metaiodobenzylguanidine myocardial scintigraphy were normal. A drastic response to levodopa and the presence of levodopa-induced dyskinesia without strong non-motor symptoms were seen in this patient. We considered that young-onset atypical parkinsonism was associated with a history of sublethal glyphosate ingestion. Epidemiologic investigations have shown that exposure to pesticides is a risk factor for Parkinson' s disease (PD). Our findings support the notion that glyphosate exposure might be related to the onset of PD.

A new species of the sand-burrowing dogielinotid amphipod genus Haustorioides Oldevig, 1958, Haustorioides furotai Ogawa, sp. nov., is described from Banzu tidal flat, east coast of Tokyo Bay, Japan. The new species is mainly characterized by a slender and triangular outer plate of the maxilliped, poorly setose antennae and pereopods, straightly acute posterodistal corner of epimeral plates 2 and 3, and an uncleft and ridged telson. Analyses of the mitochondrial cytochrome c oxidase subunit I sequences verified that H. furotai belongs to the monophyletic clade consisting of the other Haustorioides species. Additionally, Eohaustorioides Bousfield and Tzvetkova, 1982, and Parhaustorioides Ren, 2006 are synonymized with Haustorioides based on morphological analysis. An identification key to the species of the genus Haustorioides is also provided.

Endovascular therapy is strongly recommended for acute cerebral large vessel occlusion (LVO) with an Alberta stroke program early computed tomography score (ASPECTS) ≥6 due to occlusion of the internal carotid artery or M1 segment of the middle cerebral artery. However, the effect of endovascular therapy for patients with a large ischemic core with an ASPECTS ≤5 (0–5) was not established. A multicenter, randomized, open-label, parallel-group trial was conducted to investigate the superiority of endovascular therapy over medical therapy without endovascular therapy for a large ischemic core with ASPECTS (3–5). Patients were randomly assigned to receive endovascular therapy or without endovascular therapy at a ratio of 1:1. The primary outcome was a moderate functional outcome, defined as a modified Rankin scale (mRS; scores ranging from 0 [no symptoms] to 6 [death]) ≤3 after 90 days. The secondary outcomes were defined as ordinal mRS, good functional outcome (mRS ≤2), excellent functional outcome (mRS ≤1), mRS shift analysis after 90 days, and early improvement of neurological findings at 48 hours. A total sample size of 200 was estimated to provide a power of 0.9 with a two-sided alpha of 0.05, for the primary outcome, considering a 15% dropout rate. This randomized clinical trial reported the applicability of endovascular therapy in patients with acute cerebral LVO with a large ischemic core.

Atrial fibrillation (AF) is the most common cardiac rhythm disorder and a major risk factor for stroke. For more than 60 years, warfarin has been the only approved anticoagulant for prevention of stroke in patients with AF. Although highly effective, it has many limitations that make its use difficult. Therefore, several novel anticoagulants are under development to overcome the limitations of warfarin, and some of these have entered phase III clinical trials. Dabigatran is an oral, reversible direct thrombin inhibitor approved in Europe and in several other countries for the prevention of venous thromboembolism after elective knee and hip replacement surgery. It has also been approved in the United States and Japan for the prevention of stroke and systemic embolism in patients with nonvalvular AF. In this review, the mechanism of action and pharmacological properties of new anticoagulants are described in detail, and the correct use of dabigatran in clinical practice is discussed. This paper proposes two high-fidelity and computationally efficient neural voice conversion (VC) methods based on a direct waveform modification using spectral differentials. The conventional spectral-differential VC method with a minimum-phase filter achieves high-quality conversion for narrow-band (16 kHz-sampled) VC but requires heavy computational cost in filtering. This is because the minimum phase obtained using a fixed lifter of the Hilbert transform often results in a long-tap filter. Furthermore, when we extend the method to full-band (48 kHz-sampled) VC, the computational cost is heavy due to increased sampling points, and the converted-speech quality degrades due to large fluctuations in the high-frequency band. To construct a short-tap filter, we propose a lifter-training method for data-driven phase reconstruction that trains a lifter of the Hilbert transform by taking into account filter truncation. We also propose a frequency-band-wise modeling method based on sub-band multi-rate signal processing (sub-band modeling method) for full-band VC. It enhances the computational efficiency by reducing sampling points of signals converted with filtering and improves converted-speech quality by modeling only the low-frequency band. We conducted several objective and subjective evaluations to investigate the effectiveness of the proposed methods through implementation of the real-time, online, full-band VC system we developed, which is based on the proposed methods. The results indicate that 1) the proposed lifter-training method for narrow-band VC can shorten the tap length to 1/16 without degrading the converted-speech quality, and 2) the proposed sub-band modeling method for full-band VC can improve the converted-speech quality while reducing the computational cost, and 3) our real-time, online, full-band VC system can convert 48 kHz-sampled speech in real time attaining the converted speech with a 3.6 out of 5.0 mean opinion score of naturalness.

With a specific and strong molecular-recognition capability cultivated in humoral acquired immunity, an antibody has been extensively utilized in various applications, such as diagnostics and therapy. However, so far most of its uses have been limited to be in the liquid phase. In view of its potential uses, such as a gas-phase biosensor or a high-performance air filter, we have tried to verify a previously undescribed binding reaction between protein antigens and corresponding antibodies immobilized on a solid surface by using fluorescence resonance energy transfer between the two. Our data showed that the antibody on a solid surface specifically reacted with a protein antigen supplied from the gas phase under the normal ambient condition. Also discovered was that the reaction occurred even faster than that in the liquid phase under several assay conditions.

The cotylean polyclad flatworm Bulaceros porcellanus Newman & Cannon, 1996 has so far only been recorded in a few tropical locations in the Indo-Pacific Ocean, including the Lakshadweep Islands (India), the Great Barrier Reef (Australia), and the Marshall Islands (Republic of the Marshall Islands). In this study, we report B. porcellanus for the first time from Japanese waters based on specimens collected in Misaki (Kanagawa) along the Pacific coast of Honshu. This new record expands the species’ known geographic distribution northward to include the warm temperate region of Japan. Our molecular phylogenetic analysis revealed that B. porcellanus formed a clade with Pseudoceros harrisi Bolaños, Quiroga & Litvaitis, 2007, and was separated from the monophyletic group composed of 19 other species of Pseudoceros Lang, 1884, including its type species Proceros velutinus Blanchard, 1847 (now Pseudoceros velutinus). Based on the morphological and phylogenetic closeness between P. harrisi and B. porcellanus (type species of Bulaceros Newman & Cannon, 1996), we transfer P. harrisi into Bulaceros with a revision of the diagnosis of Bulaceros.

Objective: Low-carbohydrate diets have favorable short-term effects on body weight and risk factors for cardiovascular disease. However, they are potentially associated with an increased long-term risk of mortality. Our objective was to elucidate their effects on the incidence of diabetes.Methods: Several databases (MEDLINE, EMBASE, ISI Web of Science, Cochrane Library, and ClinicalTrials.gov) were searched for relevant articles that were published prior to May 2015. Cohort studies with a follow-up period of at least one year were included. Identified articles were systematically reviewed, and those with pertinent data were selected for inclusion in a meta-analysis. The pooled risk ratio (RR) with 95% confidence interval (CI) for the incidence of diabetes was calculated using the random-effects model with inverse-variance weighting.Results: We included 13 studies in a systematic review, followed by a meta-analysis using pertinent data. Among the 440,669 people that were included in 11 cohort studies, 27,887 (6.3%) cases of diabetes were documented. The risk of incident diabetes among individuals with a low-carbohydrate diet was not significantly different from that of individuals with a high-carbohydrate diet: the pooled RR was 1.03 (95% CI, 0.91–1.16).Conclusion: Low-carbohydrate diets did not show any benefit on the risk of diabetes. However, this analysis is based on limited observational studies, and large-scale trials examining the complex interactions between low-carbohydrate diets and long-term outcomes are needed.

The abundant clouded salamander from western Japan, Hynobius nebulosus, has long been considered a single widespread species, although some authors have suggested the inclusion of several cryptic species. This led to a molecular and morphological analysis of populations from all parts of the known range. Phylogenetic relationships were inferred from complete sequences of the mitochondrial cytochrome b gene, and nuclear genome differentiations were estimated by multiplexed inter simple sequence repeat genotyping by sequencing (MIG-seq). The results suggest that H. nebulosus hitherto recognized consists of at least nine species. We applied existing names to two of them, H. nebulosus (Temminck et Schlegel) and H. vandenburghi Dunn, and described seven others as new species.

Recent studies have demonstrated that BRAFV600E mutation is a common event in papillary thyroid carcinoma and a majority of these lesions have shown a direct relationship between BRAFV600E mutation and aggressive characteristics, including a worse patient prognosis. However, there are no studies from Japan regarding this issue in a large series with adequate postoperative follow-up periods. We investigated BRAFV600E mutation in 631 patients with papillary carcinoma having median follow-up periods of 83 months. The prevalence of BRAFV600E mutation was 38.4%, and the rate was higher in carcinoma larger than 1.0 cm but did not successively increase with tumor size. Furthermore, the prevalence did not significantly increase in cases demonstrating high-risk biological features such as clinically apparent lymph node metastasis, massive extrathyroid extension, advanced age, distant metastasis at surgery, and advanced Stage. The disease-free survival of patients with BRAFV600E mutation did not differ from that of those without BRAFV600E mutation. These findings indicate that, although BRAFV600E mutation may play some roles in local carcinoma development, there is no evidence that BRAFV600E mutation significantly reflects the aggressive characteristics and poor prognosis of patients with papillary carcinoma in Japan.
