Dyslipidemia is classified into primary and secondary types. Primary dyslipidemia is basically inherited and caused by single or multiple gene mutations that result in either overproduction or defective clearance of triglycerides and cholesterol. Secondary dyslipidemia is caused by unhealthy lifestyle factors and acquired medical conditions, including underlying diseases and applied drugs. Secondary dyslipidemia accounts for approximately 30-40% of all dyslipidemia. Secondary dyslipidemia should be treated by finding and addressing its causative diseases or drugs. For example, treatment of secondary dyslipidemia, such as hyperlipidemia due to hypothyroidism, by using statin without controlling hypothyroidism, may lead to myopathy and serious adverse events such as rhabdomyolysis. Differential diagnosis of secondary dyslipidemia is very important for safe and effective treatment. Here, we describe an overview about diseases and drugs that interfere with lipid metabolism leading to secondary dyslipidemia. Further, we show the association of each secondary dyslipidemia with atherosclerosis and the treatments for such dyslipidemia.

Background: Quantitative measures of the burden of tobacco smoking in Asian countries are limited. We estimated the population attributable fraction (PAF) of mortality associated with smoking in Japan, using pooled data from three large-scale cohort studies.Methods: In total, 296,836 participants (140,026 males and 156,810 females) aged 40-79 years underwent baseline surveys during the 1980s and early 1990s. The average follow-up period was 9.6 years. PAFs for all-cause mortality and individual tobacco-related diseases were estimated from smoking prevalence and relative risks.Results: The prevalence of current and former smokers was 54.4% and 25.1% for males, and 8.1% and 2.4% for females. The PAF of all-cause mortality was 27.8% [95% confidence interval (CI): 25.2-30.4] for males and 6.7% (95% CI: 5.9-7.5) for females. The PAF of all-cause mortality calculated by summing the disease-specific PAFs was 19.1% (95% CI: 16.0-22.2) for males and 3.6% (95% CI: 3.0-4.2) for females. The estimated number of deaths attributable to smoking in Japan in 2005 was 163,000 for males and 33,000 for females based on the former set of PAFs, and 112,000 for males and 19,000 for females based on the latter set. The leading causes of smoking-attributable deaths were cancer (61% for males and 31% for females), ischemic heart diseases and stroke (23% for males and 51% for females), and chronic obstructive pulmonary diseases and pneumonia (11% for males and 13% for females).Conclusion: The health burden due to smoking remains heavy among Japanese males. Considering the high prevalence of male current smokers and increasing prevalence of young female current smokers, effective tobacco controls and quantitative assessments of the health burden of smoking need to be continuously implemented in Japan.

The major department stores in Japan had been actively developed in the local cities in the early Showa Era. Under the Economic depression, conflicts arouse between the medium and small retailers who cherished traditional commercial styles and the department stores who held up modern management systems. Consequently, the retailers in the disadvantage position had to adapt themselves to the new age through various movements of the retail innovations. This paper analyzes the above-described problems in Tokushima City.

Twenty-three of 42 European rabbits (Oryctolagus cuniculus), belonging to the same rabbit colony, died in March 2020 (55% mortality) in Chiba prefecture, Japan. The disease course was extremely acute without indicators of death or hemorrhage. Necropsy revealed liver swelling, discoloration, cloudiness and fragility, and pulmonary edema. Histologically, severe hepatocellular necrosis (mainly peripheral) and intra-glomerular capillary hyalin thrombi were observed. On molecular-biological examination, reverse transcription polymerase chain reaction analysis of RNA from tissues detected a rabbit hemorrhagic disease virus, confirmed as a RHDV-2 VP60 fragment, which shared 99.42% nucleotide identity with the homologous fragment of RHDV-2 German isolate by nucleotide sequence analysis. This report shows the outbreak of rabbit hemorrhagic disease caused by RHDV-2, an emerging infectious disease, in Japan.

The year 2020 will be remembered for the coronavirus disease 2019 (COVID-19) pandemic, which continues to affect the whole world. Early and accurate identification of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is fundamental to combat the disease. Among the current diagnostic tests, real-time reverse transcriptase–polymerase chain reaction (RT-qPCR) is the most reliable and frequently used method. Herein, we discuss the interpretation of RT-qPCR results relative to viral infectivity. Although nasopharyngeal swab samples are often used for RT-qPCR testing, they require collection by trained medical staff. Saliva samples are emerging as an inexpensive and efficient alternative for large-scale screening. Pooled-sample testing of saliva has been applied for mass screening of SARS-CoV-2 infection. Current policies recommend isolating people with borderline cycle threshold (Ct) values (35<Ct <40), despite these Ct values indicating minimal infection risk. We propose the new concept of a “social cut-off” Ct value and risk stratification based on the correlation of Ct with infectivity. We also describe the experience of RT-qPCR screening of saliva samples at our institution. It is important to implement a scientific approach to minimize viral transmission while allowing economic and social activities to continue.

Recent studies have revealed that decline in cellular nicotinamide adenine dinucleotide (NAD+) levels causes aging-related disorders and therapeutic approaches increasing cellular NAD+ prevent these disorders in animal models. The administration of nicotinamide mononucleotide (NMN) has been shown to mitigate aging-related dysfunctions. However, the safety of NMN in humans have remained unclear. We, therefore, conducted a clinical trial to investigate the safety of single NMN administration in 10 healthy men. A single-arm non-randomized intervention was conducted by single oral administration of 100, 250, and 500 mg NMN. Clinical findings and parameters, and the pharmacokinetics of NMN metabolites were investigated for 5 h after each intervention. Ophthalmic examination and sleep quality assessment were also conducted before and after the intervention. The single oral administrations of NMN did not cause any significant clinical symptoms or changes in heart rate, blood pressure, oxygen saturation, and body temperature. Laboratory analysis results did not show significant changes, except for increases in serum bilirubin levels and decreases in serum creatinine, chloride, and blood glucose levels within the normal ranges, independent of the dose of NMN. Results of ophthalmic examination and sleep quality score showed no differences before and after the intervention. Plasma concentrations of N-methyl-2-pyridone-5-carboxamide and N-methyl-4-pyridone-5-carboxamide were significantly increased dose-dependently by NMN administration. The single oral administration of NMN was safe and effectively metabolized in healthy men without causing any significant deleterious effects. Thus, the oral administration of NMN was found to be feasible, implicating a potential therapeutic strategy to mitigate aging-related disorders in humans.

平成15年の地方自治法の改正によって公の施設に関して「指定管理者制度」が導入されることとなり,地方公共団体の所有する各種の施設と並んで都市公園もその対象となった。導入時のいきさつから招かれざるものとして公園管理者に受け止められた制度であるが,施行から2年を経過した現在比較的好印象で迎えられているように感じられる。そこで,都市公園管理史の観点から「指定管理者制度」の導入が都市公園の管理行政にどのような意味を持つかを検証した。考察の結果,「指定管理者制度」の導入は太政官布達第16号以来の都市公園管理史における転換期の特徴である外圧性と偶然性を持ち,近年の都市公園管理行政の閉塞感を打破する可能性,むしろ将来的に市民利用本位の公園管理のあり方を示唆する主要方策のひとつであることがわかった。具体的には ○1公園管理を再点検 ○2正確な数量把握による予算確保 ○3評価のための利用者意向把握等の動きが見られ,財政悪化時代を迎え危機的状況にあった公園管理行政の転換点となった。

During a cold-air outbreak, a broad cloud band is occasionally observed over the Japan-Sea Polar-Airmass Convergence Zone (JPCZ) that forms over the Sea of Japan from the base of the Korean Peninsula to the Japanese Islands. On 14 January 2001, a broad cloud band associated with the JPCZ (JPCZ cloud band) extended in a southeastward direction from the base of the Korean Peninsula to Wakasa Bay, and it stagnated for half a day. The JPCZ cloud band consisted of two cloud regions: one was a long cloud band extending along its southwestern edge (a developed convective cloud band), and the other was the region consisting of cloud bands normal to a wind direction of winter monsoon (transversal cloud bands). The structure and formation mechanism of the transversal cloud bands were examined on the basis of observations (e.g., satellite images, in situ measurement and cloud-pro.ling radar data from an instrumented aircraft and upper-air soundings from observation vessels) and simulation results of a cloud-resolving model with a horizontal resolution of 1 km.The transversal cloud bands had the following characteristic structures; they extended along a northeast-southwest direction, which was parallel to the direction pointed by the vertical shear vector of horizontal wind in the mixed layer, they mainly consisted of convective clouds, which slanted with height toward the down-shear side, and they widened and deepened toward southwest, as the depth of the mixed layer increased. An examination of simulation results presented that the transversal cloud bands were accompanied by roll circulations. The axes of rolls were oriented nearly parallel to the direction of the vertical shear vector in the mixed layer. An analysis of the eddy kinetic energy budget indicated that the roll circulations derived most of its energy from the mean vertical shear and the buoyancy.

Petrosavia amamiensis (Petrosaviaceae) is described as a new species from Amami-Oshima Island, Kagoshima Prefecture, Japan. The plants were previously identified as P. sakuraii. The flowers of P. amamiensis are autonomously self-pollinated at the beginning of anthesis, while those of P. sakuraii are self-pollinating at a later stage of anthesis. Petrosavia amamiensis has a shorter aerial stem, shorter internodes, more densely flowered inflorescence and smaller seeds than P. sakuraii. Moreover plastid and nuclear DNA differ greatly between P. amamiensis and P. sakuraii.

Background: Population ageing and stringent licensing policies will increase the number of older drivers who stop driving. Adverse health outcomes owing to driving cessation and their prevention are emerging concerns. Therefore, we longitudinally examined the impact of driving cessation and alternative transportation use after cessation on the risk of functional limitations in a cohort of community-dwelling people (65 years and older) in Japan.Methods: Using cohort data of those who drove as of 2006/07, we compared the risk of functional limitations between 2,704 current drivers and 140 former drivers (who stopped driving by 2010). Of the former drivers, 77 did not use public transportation or bicycles after driving cessation (thus losing independent mobility). We calculated the hazard ratios (HRs) for the incidence of functional limitations with 95% confidence intervals (CIs) based on the Cox proportional hazards model with the covariates influencing the functional limitations.Results: From 2010 through 2016, 645 people had functional limitations, which included 38, 82, and 119 per 1,000 person-years among current drivers, former drivers who used public transportation or bicycles, and former drivers who were only driven by others, respectively (HR 1.69; 95% CI, 1.15–2.49 and HR 2.16; 95% CI, 1.51–3.10, relative to current drivers).Conclusion: Driving cessation is associated with an increased risk of functional limitations among older adults, but this risk might be alleviated if they are able to maintain independent mobility using public transportation or bicycles after driving cessation.

The stings of box jellyfishes can be fatal, so knowing the fauna of a certain area is important to save lives. Five described and two still-undescribed species of Cubozoa have been reported from Thailand: Chironex indrasaksajiae, Chironex sp., Chiropsella sp., Chiropsoides buitendijki, Copula sivickisi, Morbakka fenneri, and Tripedalia cystophora. We made detailed observations of the morphology of two of the species newly recorded in Thailand: Alatina morandinii and Tripedalia binata. The molecular phylogeny of these species is also discussed. Additional investigations are needed to understand the diversity of Cubozoa in Thailand.

Low serum 25-hydroxyvitamin D (25(OH)D) levels are implicated as a risk factor for hip and spine fractures. Studies of the relation between 25(OH)D levels and fractures have primarily involved elderly osteoporosis patients or patients with fractures; however, the serum 25(OH)D and parathyroid hormone (PTH) status in younger adult populations remains largely unknown. We evaluated serum 25(OH)D and intact PTH levels in 411 women aged 39–64 years who were not receiving medication for osteoporosis or other bone diseases. Serum 25(OH)D levels were positively correlated with age (P = 0.019), whereas intact PTH levels were inversely correlated with 25(OH)D levels (P < 0.001). Thus, low vitamin D levels with high intact PTH levels were more common in younger than in older women. Our data show that serum 25(OH)D insufficiency could be a more serious concern in the younger population than had been previously anticipated. Because serum 25(OH)D insufficiency is reportedly a risk factor for hip and spine fracture, the number of fracture patients could increase in the future, suggesting that we may need to correct the serum vitamin D/intact PTH status to prevent future osteoporosis.

A new species of hesionid polychaetes, Leocratides kimuraorum sp. nov., is described based on material collected from the Shima Peninsula and Sagami Bay (middle Honshu, Japan), as well as museum specimens collected from Sagami Bay, Suruga Bay, and Shirahama. Leocratides kimuraorum sp. nov. is the third species in this genus and can be discriminated from the two congeners, L. filamentosus Ehlers, 1908 and L. ehlersi (Horst, 1921), by i) the length of the antennae, which are as long as palps, ii) the presence of pharyngeal terminal papillae, and iii) the absence of a papillose peristomial membrane. On the other hand, L. filamentosus, originally described from off Western Sumatra, Indonesia, was also collected from Sagami Bay, and represents a new record of this species for Japanese waters. Partial mitochondrial cytochrome c oxidase subunit I gene sequences from the holotype of L. kimuraorum sp. nov. and newly collected specimen of L. filamentosus are provided for reliable species identification in the future.

Background: Diuretics are the most prescribed drug in heart failure (HF) patients. However, clinical evidence about their long-term effects is lacking. The purpose of this study was to compare the therapeutic effects of furosemide and azosemide, a short- and long-acting loop diuretic, respectively, in patients with chronic heart failure (CHF). Methods and Results: In this multicenter, prospective, randomized, open, blinded endpoint trial, we compared the effects of azosemide and furosemide in patients with CHF and New York Heart Association class II or III symptoms. 320 patients (160 patients in each group, mean age 71 years) were followed up for a minimum of 2 years. The primary endpoint was a composite of cardiovascular death or unplanned admission to hospital for congestive HF. During a median follow-up of 35.2 months, the primary endpoint occurred in 23 patients in the azosemide group and in 34 patients in the furosemide group (hazard ratio [HR], 0.55, 95% confidence interval [CI] 0.32-0.95: P=0.03). Among the secondary endpoints, unplanned admission to hospital for congestive HF or a need for modification of the treatment for HF were also reduced in the azosemide group compared with the furosemide group (HR, 0.60, 95%CI 0.36-0.99: P=0.048). Conclusions: Azosemide, compared with furosemide, reduced the risk of cardiovascular death or unplanned admission to hospital for congestive HF. (Circ J 2012; 76: 833-842)

Background: P2Y12 inhibitor monotherapy without aspirin immediately after percutaneous coronary intervention (PCI) has not been tested in East Asian patients, so in this study we aimed to assess the safety and feasibility of reduced dose (3.75 mg/day) prasugrel monotherapy in Japanese patients presenting with chronic coronary syndrome (CCS).Methods and Results: ASET-JAPAN is a prospective, multicenter, single-arm pilot study that completed enrolment of 206 patients from 12 Japanese centers in September 2022. Patients with native de-novo coronary lesions and a SYNTAX score <23 were treated exclusively with biodegradable-polymer platinum-chromium everolimus-eluting stent(s). Patients were loaded with standard dual antiplatelet therapy (DAPT) and following successful PCI and optimal stent deployment, they received low-dose prasugrel (3.75 mg/day) monotherapy for 3 months. The primary ischemic endpoint was a composite of cardiac death, spontaneous target-vessel myocardial infarction, or definite stent thrombosis. The primary bleeding endpoint was Bleeding Academic Research Consortium (BARC) type 3 or 5. At 3-month follow-up, there were no primary bleeding or ischemic events, or any stent thrombosis.Conclusions: This pilot study showed the safety and feasibility of prasugrel monotherapy in selected low-risk Japanese patients with CCS. This “aspirin-free” strategy may be a safe alternative to traditional DAPT following PCI.

Here, we investigated the prevalence of Salmonellaenterica, with and without resistance to 17 common antimicrobial agents, in 706 green anoles (Anolis carolinensis) that were collected in Naha and Tomishiro Cities, Okinawa Main Island, Japan, between 2009 and 2014. Salmonella strains, including S. enterica Weltevreden and Enteritidis serovars, were identified in the large intestinal content samples extracted from 15 (2.1%) of the analyzed green anoles. No antimicrobial resistance was detected. Thus, the present study demonstrates that although the prevalence of Salmonella and the risk of its transmission from the green anoles to humans or other animals on Okinawa Main Island are relatively low, the green anole population nevertheless represents a potential source of Salmonella infection that could affect human health in this region.

本報告書は将来の宇宙活動の利用者の多様な要求に応え得るための宇宙輸送体系のあるべき姿の一つを提案した。将来の宇宙輸送体系では地球低軌道に到達する手段の効率の向上、低コスト化、運用の柔軟性が鍵となる。これらを実現すべく宇宙輸送系の「再使用化」を目標として今後のわが国の宇宙輸送体系の発展の一つのシナリオを提案した。打上げ機の下段は性能に関する重量増の感度が比較的低いので、「無人・自動化技術」を適用して「回収・再利用化」を図り、段階的にその範囲を拡大していくことを合理的と考えた。このシナリオに基づき既開発のH-2ロケットの構成部品を利用した再使用ブースタの概念の一例を示した。
