【目的】当院では、診療報酬請求をより適正に行なうため各種対策を行なってきた。具体的には、請求漏れと保険査定減対策、当月分未請求レセ・月遅れ請求レセ管理対策である。<BR>【方法】(1).診療報酬請求漏れ対策は平成15年から医事入院係各自がそれぞれ各請求担当分野の請求漏れをあらためて検証した。<BR>(2).平成16年からは医事外来係まで拡大し、請求診療科ごとに同じように請求漏れを検証した。<BR>(3).(1)(2)の方法、結果はいずれも各自・各グループからパワーポイント資料を使った発表会を行い医事職員全員に周知した。<BR>(4).保険査定減対策は、査定事前対策として医師による請求時のレセプトチェック、対策委員会での再請求審査、委員会作成の医師用簡易レセプトチェックリスト周知などである。これら委員会の情報は院内情報システムで閲覧できる。<BR>また、医事知識を得る上でも病棟カンファレンスにも参加した。<BR>(5).当月保留レセ・月遅れ請求レセ管理対策も新たに開始した。<BR>【結果】請求漏れ対策では入院関係がH15年、H16年の検証テーマ数はそれぞれ13題、外来の検証テーマ数は16年12題である、テーマは豊富である。<BR> 保険査定減対策はH15年、H16年年間平均比較で保険査定率平均(1か月)で0.43%から0.30%に、保険査定金額では同様に5,543千円/月から3,953千円/月に減少した。<BR> 当月未請求レセ・月遅れ請求レセ管理対策では月毎に多少の凸凹はあるものの件数、点数共に減少傾向である。<BR>【考察】請求漏れ対策については、テーマが豊富であるということは漏れがないとはいえない。しかし職員がその請求漏れを掌握しつつあることは確かである。保険査定減対策は大幅に減少している。顕著な成果がでてきた。返戻レセまたは月遅れレセの早期提出も以前より停滞が減少した。管理が行き届いた成果と考える。<BR>【まとめ】診療報酬請求の適正化対策は、どの対策も結果は良好であった。今後も引き続き適正請求を積極的に展開したい。<BR>

個人情報保護のため削除部分あり大隈条約改正交渉の収拾策としての明治二二年一二月一○日付閣議決定は、井上毅法制局長・伊東枢密院書記官長らの批准拒絶方針と、井上馨農商相・青木外相代理らの修正要求提起方針とを、後者を柱にしつつも両論併記した。一方、日本政府のこの弱体・分裂状態にドイツ等他の列強が取り入ったり、第一議会を強硬な条約廃棄論が席巻して政府を暴発に導いたりすることを危ぶむイギリスは、外国人裁判官任用要求を放棄した対案の早期提出で主導権をとる決断を下す。責任転嫁の対象を「英政府対案」で奪われ、対等条約の主張への配慮から本交渉開始にも踏み切れず、山県首相は対応に苦慮した。結局、第一議会後に青木外相が本交渉開始へ暴走して山県は政権を放棄し、青木が事実上更迭されて条約改正問題は凍結される。ただその結果は、日本は大隈交渉の白紙撤回、イギリスはスケープゴートにされる危険の回避、という各自の目標を一応達成したことをも意味していた。

The objective of this study was to clarify variations of the ceramide (CER) profile in human stratum corneum (SC) in different regions of the body and to estimate the contributions of CERs to the SC barrier and water holding functions. Based on the information that there are great variations of SC functions among body sites, we compared the CER profiles obtained from 11 different anatomical sites in healthy Japanese females. Not only the physiological parameters of SC but also the CER profile showed body region variations. Especially, strong (significant) regional variations of the CER profile were shown in the lip and the palm; the total CER level, the composition of CER [NP] contains non-hydroxy fatty acid and phytosphingosine and CER [NH] contains non-hydroxy fatty acid and 6-hydroxy sphingosine, the levels of longer species of CER [NS] contains non-hydroxy fatty acid and sphingosine were less, but the composition of CER [NS], CER [AS] contains alpha-hydroxy fatty acid and sphingosine and the levels of shorter species of CER [NS] were more than those of the upper arm. The total CER level on palm displayed strong correlations with the values of capacitance. Our results indicate that there are the regional variations of the CER profile in the healthy Japanese female and which may contribute to the SC functions.

The high grade of cobalt characterizes the deposit of the Shimokawa copper mine which is one of the most famous bedded cupriferous pyrite deposits of Japan. Unfortunately, however, the cobalt has not been recovered, because the concentration of cobalt is difficult. In order to show the distribution of cobalt in the ore, the microscopic observation and the electron microprobe analysis have been carried out. The main ore minerals are pyrite, chalcopyrite, monoclinic pyrrhotite, sphalerite, cobalt pentlandite, and cobaltian mackinawite in the descending order of abundance. Among them, pyrite is the most important carrier of cobalt, and cobalt pentlandite is subordinate. The cobalt of the copper concentrate depends upon the small mingled grains (less than 20μm) of cobalt pentlandite which is closely associated with chalcopyrite. On the other hand, the pyrite concentrate owes its cobalt amount to cobalt-rich pyrite. On the basis of the texture of ore and the crystal form of pyrite, the mode of occurrence of pyrite is divided into seven types as shown in Figs. 1 (a)-(g). The R-type pyrite having a rounded form is the richest in cobalt among them, and contains a average cobalt concentration of 0.8 wt.%.These natures of ore suggest that the cobalt of the coppem concentmate may be recovered as a by-pmoduct of the copper smelting, and that the cobalt of the pyrite concentrate may be recovered as a cobalt concentrate consisting of R-type pyrite.

In this paper, an impedance control system which uses together the inner torque control system with the disturbance observer is proposed for the pneumatic robot manipulator. It is assumed that the generated torque by a pneumatic actuator can be estimated from the pressure signals and the discharge volume. And we define that the disturbance torque includes the friction torque, load change and modeling error. At first in order to improve the control characteristics of pneumatic actuator driven by meter out method, we construct the inner torque control system by feeding back the generated torque. Secondly, to reduce the influence of the defined disturbance torque, we compose the impedance control systems with the disturbance observer which can estimate the defined disturbance torque based on the generated torque, the angular velocity and the reaction torque. In order to realize the robust impedance to the defined disturbance torque, we construct the torque based impedance control system which uses together the inner torque control system with the disturbance observer. The real command to the torque control system is generated from the impedance model and the positioning error. From some experiments, it is verified that the proposed impedance control system is effective to adjust the impedance of pneumatic robot manipulator.

1.目的<BR>各種の文字情報の入手に制約がある視覚障害者にとっては、インターネットを活用することで、情報の収集が容易になる。しかしながら、パソコン初心者の視覚障害者にとっては、一般のホームページはその構造が比較的複雑で、そこからいろいろな情報を得るのはかなり難しい作業となっている。<BR>そこで、<BR>a.文書構造をできるだけ分かりやすくして、リンクやフレームといった概念を理解しやすくする。<BR>b.リンク間の移動やリンクによるページ移動、フレームの切り替えおよびフォーム入力などといった操作の練習を集中的に行なえるようにする。<BR>ということを考慮して、ホームページ閲覧の練習用ホームページを開設した(http://homepage2.nifty.com/cosmosvision/)。<BR>2.ホームページ作成にあたって留意した点<BR>フレームの概念の理解とその切り替えの練習のために、随所に故意にフレームを使ってページを作成した。また、実際のホームページの中には、リンクを開くと勝手に別ウィンドウで開くものがある。そこで、そういう状況に遭遇した場合でも冷静に対処できるように、別ウィンドウで開くようにしたリンクも随所に設けた。<BR>また、インタラクティブなサイトでは、掲示板をはじめ、フォーム入力が必要になる場合が多い。そこで、フォーム入力練習用として、メールフォームや掲示板なども用意した。<BR>3.フィードバックとサポート<BR>必要に応じて、定例の勉強会の機会を利用するほか、メール等により寄せられる質問に対して随時回答し、回答の結果をホームページ上に掲載してメンバー間で情報の共有化を図っている。<BR>4.経過<BR>フォーム入力に対して抵抗感が少なくなったメンバーの中には自身のブログを開設して記事投稿を定期的に行ったり、他のメンバーが開設しているブログにコメントするなど、徐々にではあるが、ネットの世界が広がりつつあるメンバーも現れてきている。<BR>

うちばり(打鍼)・くだばり(管鍼)・けいはり(経鍼)中国では元代に「子午八穴鍼法」*12が登場するなど,四肢(手足)の要穴を多用するのに対して,日本では五大思想(五輪発生説・諸虫病因論)のバイアスによって体幹(背腹)の要穴を重視するよう変容を遂げた。 日本でいつから「打鍼」(図6)が行われるようになったのかは詳らかでない。正親町天皇・後陽成天皇に仕えた鍼博士・御薗意斎(1557〜1616)の遠祖で,花園天皇(1297〜1348)に仕えた多田次郎為貞を創始者とする説は時代が早すぎて首肯しかねるが,遅くとも戦国期に実施されていた状況証拠がある〔煙蘿子針灸法青樵齋道丹自序;1530〕。「蓋し,牡丹根は甜(甘)うして,螙蟲*13これを損ず。その旁ら,常に小穴あり。すなわち蟲の所為の処なり。花工,硫黄を点して其の樹に鍼し,艾炷を以て其の根に灸す。これ花を医するの法と謂う」は,諸虫の退治を目的とした「打鍼」の隠喩と考えられるが,宋代の園芸書を典拠としている点は興味深い〔洛陽牡丹記三・風土記;1072迄〕。なぜなら,打鍼のアイデアの源泉が法具や神器の延長線上ではなく,園芸からの技術転用という新たな可能性が開けるからである*14。

Objective The clinical characteristics and chest imaging findings of viral pneumonia and several interstitial lung diseases (ILDs) overlap, and viral pneumonia may be underrecognized and misdiagnosed as certain ILDs. To clarify the frequency of viral pneumonia among patients with acute progressive clinical courses that required a differential diagnosis between ILDs and pneumonia, and to determine the most frequent ILDs misdiagnosed in cases of viral pneumonia. Patients and Methods We retrospectively analyzed patients hospitalized from 2010 to 2017 with an acute clinical course (≤30 days) who underwent bronchoalveolar lavage (BAL) for the differential diagnosis of infection and ILDs. We performed a multiplex PCR for respiratory viruses using the patients' preserved BAL fluid. The final diagnosis was made by a multidisciplinary approach and after considering the PCR results. The diagnosis at discharge was compared to the final diagnosis. Results Among the 109 patients, 53 were diagnosed with viral pneumonia. Viral pneumonia and other diseases showed some differences in symptoms and laboratory data; however, the differences were small or overlapped. Viral pneumonia was misdiagnosed on discharge as acute fibrinous organizing pneumonia, cryptogenic organizing pneumonia, or chronic eosinophilic pneumonia (AFOP/COP/CEP) (n=22), acute interstitial pneumonia (n=5), connective tissue disease-related ILDs (n=3), unclassifiable interstitial pneumonia (n=2), drug-induced ILD (n=1), and pneumonia (n=20). Conclusion Approximately half of the patients who underwent BAL had viral pneumonia. The most common ILD-related misdiagnoses were AFOP/COP/CEP. Differences in symptoms and laboratory findings between viral pneumonia and other diseases were small, and viral pneumonia should be included in the differential diagnosis when physicians encounter cases in which the abovementioned ILDs are suspected.

タバコの受難時代 人とタバコとの永いかかわり合いの歴史上,後者の受難がつづく時期は今日を措いてない.私はパイプ党だが,つぎつぎにだされるタバコの有害性の証拠のまえには脱帽するしかない.2月19日の朝日新聞によると,日本でのタバコによる損夫は,火事や病気で年に1兆1千億円をこすというが,これも愛煙家には気分のよくない数字だ,石油蛋白の有害論が盛んだが,もしタバコに永い歴史がなく,新顔の嗜好品だったら,ひとたまりもなく追放されてしまうことだろう. ところが,それほど悪者扱いされるタバコをのむ人々が罹りにくい病気がただひとつだけ知られている.専売公社の喜びそうなこの病気はパーキンソン病で,患者の既往歴を調べると,不思議にもかえって喫煙歴が少ない.パーキンソン病は初老期以降に起き,振戦,筋の固縮,無動症などを呈する病気で,本誌16巻2号の特集でとりあげられている.

文部科学省は、ICT環境整備を進めるとともにIT活用による学力向上を通して、次世代を見据えた人的基盤づくりを図るために先導的な研究事業等を行う「学校教育情報化推進総合プラン」を推進してきた。本プランは「確かな学力」の向上を図るには、授業においてICTを効果的に活用することは不可欠であるとしている。これに対して、文科省が実施している、各県の教員のICT活用指導力と全国学力テストとの相関分析の結果、相関関係がないことが分かった。ICT環境を整備し、教員のICT活用指導力が向上すれば学力も向上するという文科省の見解は間違っていたのであろうか。調査研究等の課題について分析した。

本課題では,フラットスピンを利用した小型固定翼UAVの自動垂直着陸を実現する方法を研究した.通常の機体であってもフラットスピンにエントリーさせるために,バラストを利用して必要な時のみ機体の重心位置を後退させ,フラットスピンにエントリーさせる方法を考案し,飛行実験によってその有効性とスピン中の姿勢がほぼ水平になることを確認した.また,スピン中の姿勢がフラットになることにより,降下中の速度がかなり小さくなることが確認された.さらに,フラットスピン中の機種が所望の方向を向いた瞬間に推力を与え,飛行経路を変更する方法を考案し,飛行実験によって所望の方向に経路移動が可能であることを示した.
