Children with autistic spectrum disorders (ASD) are diagnosed by deficits in social interaction and linguistic skills, and restricted interests and stereotypic pattern of behavior. They have difficulties in motor, perceptual, cognitive, linguistic functions. In the present article, we proposed the comprehensive view point for understanding children with ASD; Relationships between various psychological functions, interaction between individual and environment, developmental mechanisms and developmental change by the appropriate intervention. We first described the profiles of ASD, prevalence, diagnostic criteria and resent assessment measures with the possibilities for very earlier screening in one year. Recent advancement of early intervention studies and the guideline for the application showed that early intensive treatment, pivotal behavior treatment, structured environment and parent-support program were effective for promoting the development of children with ASD. The developmental mechanism and treatment curriculum were analyzed in positive social interaction, joint attention, imitation, auditory comprehension, vocal production, verbal behavior and conversation. Children with high-functioning autism and Asperger syndrome have deficits in understanding ambiguous context, higher-ordered linguistic rules, social interaction and other's “mind”. We reviewed the treatment studies for these difficulties and found that the effective treatment included the “visualized” procedures, because children with ASD have “strength” in visual thinking. Video modeling and in-vivo role playing were effective for generalizing the acquired social skills to everyday life situation. We, as scientist ⁄ practitioner, discussed the future direction of evidence-based studies for developing children with ASD.

和漢薬が奏効した線維筋痛症 (FMS) の二例を提示し, FMSの臨床像と甘草附子湯証との類似点を考察した。症例1は52歳, 女性で2001年左手関節痛を自覚, その後両側の肘, 肩, 足関節痛とその周囲の筋痛が出現。近医リウマチ科で精査を受けたが異常はなくFMSと診断された。NSAIDsが無効のため, 04年当科を紹介受診した。桂枝二越婢一湯加苓朮加防已黄耆湯葛根を投与, 内服2ヵ月で疼痛は半減。06年3月VASは20%となりNSAIDsは不要となった。症例2は58歳, 女性で10年前から左肘痛を自覚。2004年から項頸部痛や両側上肢, 肩の疼痛が出現し, 近医整形外科を受診。頸部X-rayや神経学的に異常がなかったためNSAIDsで経過観察となった。05年3月症状が不変のため4月に当科を紹介受診。炎症反応が陰性でACRのFMS分類基準に適合した。甘草附子湯の3ヵ月の服用でVASは30%となりADLは向上した。

化学熱傷とは酸やアルカリを代表とする様々な化学薬品によって引き起こされる熱傷様の皮膚・粘膜障害である.その病態は刺激性接触皮膚炎の範疇に位置付けられている.原因となる化学物質は膨大な数におよぶ.そのため,初期の対応にとまどうことが少なくない.本稿では原因となる主な物質の分類と作用機序,診断時のポイント,および一般的な治療について述べ,さらには本邦でこれまで報告された症例をもとに具体的な事例についてまとめた.

2006年の容器包装リサイクル法改正により, 排出抑制に関する措置が施行され, 特にレジ袋について, 地方自治体と小売店等が連携し, 有料化やポイント制などの導入により削減を進める取り組みが全国的な広がりを見せるようになってきている。有料化を実施した地域では高い効果が上がっており, これは行動経済学の知見によって説明が可能であることを示しつつ, その前提として関係者の連携体制の確保が重要であることを示す。また, 2008年のG8神戸環境大臣会合で合意された神戸3R行動計画において, レジ袋対策をとることが盛り込まれた。20ヶ国以上の取組状況を概観し, 各国の取り組みが相互に影響し合っている様子を紹介する。こうした国内・国際両面でのレジ袋対策の進捗が持つ政策手法的な示唆にも言及する。

発達障害の親であり親のための相談者であるペアレント・メンターが、自らの体験の語りについて、聞き手である他者との関係の中でどのように捉え、価値づけしているのかを検討するため、ペアレント・メンター52名(51名が子どもに自閉スペクトラム症の診断あり)に対し、自分の体験を話した機会とその肯定的体験、否定的体験及びその理由について自由記述の質問紙で回答を求め、その内容をKJ 法に準拠した手続きにより整理・分析した。本研究の結果から、ペアレント・メンターがメンター活動の中で自己体験を語るさまざまな機会が示された。肯定的体験や否定的体験になりやすいカテゴリ項目が存在すること、また同じカテゴリの項目でも関与する要因によって、肯定的体験にも否定的体験にもなりうることも明らかとなった。またこれらの体験に関与する要因として、【聞き手からの共感】【聞き手との交流】【達成感】【語りへの抵抗】という4 つが得られた。これらの結果をもとに、ペアレント・メンターが自己体験を話すことの意味とメンターの語りという活動への支援のあり方について考察した。

When conducting widespread medical examinations throughout our municipality, our team of clinicians discovered many subjects who exhibited symptoms of zinc deficiency despite having serum zinc levels above 65 µg/dL, the commonly accepted limit of zinc deficiency. In this paper, we propose that the range used to express zinc deficiency be adjusted upwards to a new limit of 80µg/dL. When our research team used this higher value in our diagnoses of community members involved in the study, we correctly diagnosed zinc deficiency 91% of the time, with only 9% of our diagnoses being false negatives. Using the new limit, 25% of patients may be falsely diagnosed with a zinc deficiency (false positive) if their complaints or physical signs are not appropriately considered.

Coronary artery disease (CAD) remains a leading cause of mortality and morbidity in developed countries. Although urgent revascularization is the cornerstone of management of acute coronary syndrome (ACS), for patients with stable CAD recent large-scale clinical trials indicate that a mechanical ‘fix’ of a narrowed artery is not obviously beneficial; ACS and stable CAD are increasingly recognized as different clinical entities. We review the perspectives on (1) modifying the diagnostic pathway of stable CAD with the incorporation of modern estimates of pretest probability, (2) non-imaging evaluations based on their availability, (3) the optimal timing of invasive coronary angiography and revascularization, and (4) the implementation of medical therapy during the work-up.

世界金融危機後も依然として強い影響力を保ち続けているネオリベラリズム (新自由主義) の持続性は, 近年の研究における一つの焦点となっている。本稿は, ネオリベラリズムがどのように危機後も生き延びたか, またオルタナティヴな構想が真剣な議論の対象とならないのかについて, 行為遂行的効果という観点から考察する。経済社会学における行為遂行性論と, 「経済による政治の脱魔術化」 としてのネオリベラリズムというW・デービスの定義にもとづいて, 本稿では経済的な尺度が自律した政治的思考を抑圧する形で成立するネオリベラリズムが, 観念の次元で政策を規定するだけでなく, 市場の諸装置にまで及んでいることで半ば自然化されていることを論じる。そこでは, 経済学自体が統治メカニズムの一部となることによって, 経済合理性や道具的理性に合致しないものは規範理論として退けられる。行為遂行性をめぐる政治は, ネオリベラリズムの下では常に非対称的な関係の中でしか行われないのである。

Since the implementation of feed in tariffs (FIT: a subsidy policy to promote the spread of renewable energy), the external diseconomies of solar panels installed in excess, to earn income from the sale of electricity from photovoltaic power generation, have become apparent. The purpose of this study, therefore, is to identify the impact of the installation of photovoltaic power generation facilities on the living space of citizens. Using data collected through a web survey of residents living in five prefectures in the north-eastern Kanto region of Japan, a spatial autocorrelation analysis was conducted to identify the spatial distribution of discomfort caused by photovoltaic power generation facilities. The results clearly indicated that the spatial discomfort of these residents living in clusters, increased with the installation of the solar panels in their living space. Some of the residents intend making radical demands for corporate action to alleviate their discomfort; such actions can lead to environmental conflict. The results demonstrate that radical solutions are necessary to reduce the spread of this discomfort. By further utilizing the data obtained in this study, it will be possible to estimate the regions at risk of solar panel-related conflict more objectively.

1990年代の選挙制度改革が,国政における政党システムや政党と候補者の関係を再編しつつあることはしばしば指摘される。しかし再編は国政だけで起こったのだろうか。本稿では,1990年代に進展した重要な制度変化である選挙制度改革と地方分権改革が地方政治にどのような影響を与えるのかについて,先行研究を概観しながら検討していく。そのうえで,1990年代以降に44の道府県で続けられた統一地方選挙における道府県議会議員選挙の結果を記述的に分析する。この分析から,選挙制度改革後に国政レベルで自民党に対抗することになった民主党が二大政党の一翼として地方政治を国政と同様に分極化させつつも,地方分権改革の進展とともに地方政治が実質的な意味を持つようになる中で,地方議会における選挙制度や地方政府の二元代表制という地方政治レベルの要因が存在するために,選挙区の集合体としての地方政治レベルでは国政に連動した再編成が必ずしも進んでいないことを指摘する。

Background:The purpose of this study was to clarify the current status and issues of community collaboration in heart failure (HF) using a nationwide questionnaire survey.Methods and Results:We conducted a survey among hospital cardiologists and general practitioners (GPs) using a web-based questionnaire developed with the Delphi method, to assess the quality of community collaboration in HF. We received responses from 46 of the 47 prefectures in Japan, including from 281 hospital cardiologists and 145 GPs. The survey included the following characteristics and issues regarding community collaboration. (1) Hospital cardiologists prioritized medical intervention for preventing HF hospitalization and death whereas GPs prioritized supporting the daily living of patients and their families. (2) Hospital cardiologists have not provided information that meets the needs of GPs, and few regions have a community-based system that allows for the sharing of information about patients with HF. (3) In the transition to home care, there are few opportunities for direct communication between hospitals and community staff, and consultation systems are not well developed.Conclusions:The current study clarified the real-world status and issues of community collaboration for HF in Japan, especially the differences in priorities for HF management between hospital cardiologists and GPs. Our data will contribute to the future direction and promotion of community collaboration in HF management.

鶏レバー,牛レバー,豚レバーの3種類を用い,調理温度によってにおいを軽減できるかということを目的に,アンケート調査,ニオイセンサー,官能評価,化学的成分の測定を行った.アンケート調査の結=果,「生臭さ」や「味」が嫌いな理由の上位にあることが分かった.ニオイセンサーでは,レバーにより加熱温度によるにおいの強さが異なったので,官能評価を行ったところ,180℃の高温短時間加熱により生臭さが抑えられ,味も好まれた.また,高温加熱によりサラダ油の浸透が進み,高度不飽和脂肪酸の酸化も抑えることができた.鉄含有量が高いヘム鉄のレバーを高温短時間加熱することにより鉄分の損失は抑えられ,味もにおいも好ましくなることで,貧血の鉄補給源など栄養的効果を期待される食品として,レバーの利用範囲を広くすることができるのではないかと考える.
