オルベスコ®インヘラーは新規のプロドラッグ型ステロイドであるシクレソニドを主薬とする定量噴霧式エアゾール剤(pMDI)であり,本邦では初めての1日1回の用法による成人の吸入ステロイドとして承認された.本稿ではシクレソニドの非臨床成績および臨床成績について概説する.シクレソニドは肺に吸入された後,組織のエステラーゼによって活性代謝物である脱イソブチリル体(desisobutyryl-ciclesonide:des-CIC)へと変換される.des-CICは強力かつ特異的にグルココルチコイド受容体に結合し,既存薬に匹敵する抗炎症作用を有することがin vitroおよびin vivoの非臨床薬理試験で示されている.さらにシクレソニドの作用は持続的であることが肺細胞を用いた検討で示されており,そのメカニズムとして細胞内滞留性を有する脂肪酸抱合体の形成による可逆的な代謝経路の関与が示唆された.オルベスコ®インヘラーは完全溶解型の製剤であるために,デバイスから噴射されるエアロゾルは1μm程度の微細粒子の割合が高く,末梢気道まで効率よく到達する.そのため,本剤の効果は末梢気道にまで及ぶと考えられる.その一方で,薬剤の口腔沈着率が低いことからカンジダ症や嗄声などの局所副作用の低減が期待される.また,シクレソニドはdes-CICとしての消化管からの生物学的利用率が低いこと,全身循環に移行した場合においてもタンパク結合率が高いために遊離型薬物濃度が低いこと,さらに肝臓で速やかに代謝不活性化されることなどの特性により,全身性副作用の軽減も期待できる薬剤である.これらの特性に基づいて国内外で臨床試験を実施した結果,1日1回の用法で喘息患者の呼吸機能,症状,QOLが改善されること,および長期の安全性に問題がないことが確認された.現在,吸入ステロイドは国内外で喘息の薬物治療の第一選択薬とされているが,本邦ではその普及率は未だに低いのが現状である.オルベスコ®インヘラーが新たな吸入ステロイド薬の治療選択肢を提供し,吸入ステロイド治療の定着と喘息患者のQOL向上に寄与することを期待する.

This paper introduces our fourth workshop on fieldwork, following three workshops conducted by Imaizumi & Umezaki (2016, 2017, and 2018). This workshop focuses on the usage of cartography and photography; in fact, using maps and photographs for fieldwork is not a new approach. However, the use of maps or photographs that students have not investigated sufficiently is problematic as they are only interested in participant observation and interview methods. In brief, students enter the field without having done much preparation, thus making it difficult for them to understand the essence of fieldwork. Therefore, we designed a workshop using landscape photographs in order to build a strong “geographical imagination” and examined the resulting effects.

This paper introduces our third workshop on fieldwork using maps and conducts discussions regarding this in the university after first workshop introduced by Imaizumi & Umezaki(2016,2017). In fact, using maps for fieldwork is not a new attempt. However, some problems exist in using maps that students have not sufficiently investigated as they are only interested in participant observation and interview methods. Briefly, students would go into the field without much preparation, making it difficult for them to understand the essence of fieldwork. Therefore, we designed the workshop for building a strong “geographical imagination” and examined its effect.

飼育下にある大型類人猿の育児放棄は,母や育つ環境が影響していることが以前から報告されている。さらに育児放棄された個体はヒトが育てることになり,チンパンジーらしさが十分育たず,チンパンジーの群れに入れないという悪循環を繰り返してきた。近年,そういった負の連鎖を断ち切るためにさまざまな試みがなされるようになった。多摩動物公園では2008年に生まれた人工哺育個体(ジン,オス,GAIN識別番号0705)の群れ入りに成功している。 本研究では多摩動物公園で母親に育てられた7 個体の発達と群れ入り後のジンの行動を比較し,早期の環境剥奪が発達過程に及ぼす影響について検討した。ジンは群れ入り直後は他の個体に比べて,運動発達,認知発達,社会性発達に遅れが見られたが,いずれも半年後には改善していた。特に道具操作の発達は短時間で他の個体に追いついていた。一方,指しゃぶりや,肛門に指を差し込むなどの行動が観察されているが,日常生活に支障をもたらすものではなかった。大型類人猿の母子関係はヒトのそれと比較されてきた。本稿では,人工哺育個体の群れ適応の難しさや,適応の必要要件について取りあげ,ヒトの子育ての特性についても併せて考察した。

19世紀末アメリカの消費社会では、人々の購買意欲を刺激して消費へと向かわせるさまざまな戦略が登場した。その代表として挙げられるのが、豊富な商品を分類し、効果的に照明をあててショーケースの中に入れたデパート、さらにその展示の場を印刷物に移したカタログ商法である。消費社会の中で商品は必要に迫られて購入されるより、展示の方法や広告によってイメージが増幅させられた結果購入されるものへとその性質が変えられていく。言い換えれば商品はその本質的価値のために実用に供されるのではなく、豊かさや知的満足などへの欲求を充足するものとして流通し、人々の欲望を刺激する記号へと変化した。ヘンリー・ジェイムズの『ポイントンの蒐集品』では、集められた美術品や骨董品がそれ自体の価値よりも、登場人物たちの共通のコードの中で負わされた役割ゆえに争われると考えられる。本稿では、登場人物により意味が変化させられる蒐集品の性質を消費社会という文脈の中で読み、同時にそこに込められた作者ジェイムズのアメリカ消費文化に対する感情についても検討していく。

This paper aims to propose a risk assessment method on countermeasure against infectious disease with high fatality rate using social simulation. We constructed a novel model which are able to visualize social vulnerability against outbreak of infectious disease such as smallpox. In this model, disease state transition of smallpox, infection process, social structure, human activity, social countermeasure, simulation condition that include parameters, and simulation results are described. We evaluate the social protection policies by an agent-based model using Spot Oriented Agent Role Simulator (SOARS). This model is able to visualize the risk of social vulnerability against smallpox and social vulnerability is different among generations.

Recently, because the marked rise in medical expenses in Japan has become a major social problem, self-medication using OTC drugs in cases of minor health problems has attracted increasing attention. When people use OTC drugs for self-medication, they need support and/or advice from pharmacists on their proper use. This paper outlines recent revisions in the legal sales system of OTC drugs, the social background of self-medication, characteristics of OTC drugs and role of the pharmacists in providing consultation on OTC preparations. Next, consumers' views of self-medication and the OTC drug sales system are described based on the results of surveys performed after they attended an educational event on the proper use of OTC drugs. The survey of consumer views on the legal sales system of OTC drugs revealed that they were the most concerned about safety and convenience. From the survey of consumer views on self-medication, a significant percentage of the group who understood the meaning of the term “self-medication” practiced it in cases of minor health problems. Although no significant difference was seen between the groups who understood the term “self-medication” and those who did not in regard to the reading the drug package label and/or insert, a significant difference was found in their understanding of “The System for Sufferers from Adverse Drug Reactions”. Therefore, it was clear that the consumers familiar with “self-medication” not only practiced it, but also understood the contents of drug package labels and/or inserts.

Rossby wave propagation theory is reviewed under two kinds of non-uniform basic flows: the zonal mean (ZM) and horizontally non-uniform (HN) flows in this study. The diagrams in the wavenumber domain for stationary and non-stationary waves embedded in the ZM flow are given and discussed in comparison with previous studies. Then a circle diagram in the group velocity domain for waves embedded in the HN flow is derived from the formulas in forms of three vectors: the wavenumber, background wind and gradient of basic-state absolute velocity. Given the basic state, we can identify the maximum and minimum magnitude of group velocity and its departure from the background wind. These results provide insights into Rossby wave propagation behaviors in the real atmosphere.

1982年より1990年までの8年間に末梢性顔面神経麻痺204例に対して高気圧酸素療法を行い, その効果を検討した. 症例の内訳はベル麻痺185例, ハント症候群19例であった. 発症後2週以内に治療を開始した新鮮例の治癒率は, ベル麻痺82%, ハント症候群81%と高率で, 治癒までに要した日数の平均は, ベル麻痺28.3日, ハント症候群27.3日と短かった. また, 初診時の麻痺程度が軽いほど, 年齢が低いほど, 治療開始までの期間が短いほど治癒率は高かった. ベル麻痺陳旧例の治癒率は47%で, 前医の治療で回復しなかった症例に対しても有効であった. 本療法は, 特別な合併症はなく, 基礎疾患の有無や年齢にかかわらず安全に行うことができるため, 末梢性顔面神経麻痺に対してきわめてすぐれた治療法であるといえる.

研究ノート・資料(Note)2000年に創設された介護保険制度は、それまで主に女性が担ってきた家庭内での介護を、社会全体で支えることを目的に創設された。そして、わが国の介護保険制度は「介護を要する状態」の要介護者だけでなく、「介護を要する状態に陥るおそれがある」要支援者も介護保険での給付対象としている。2000年の介護保険制度の創設後、要支援や要介護1の軽度認定者が著しく増加した状況に対応するため、2005年の法改正で介護予防の拠点として地域包括支援センターが創設された。しかし、その後も要支援認定者等の軽度認定者は増加を続け、地域包括支援センターは介護予防支援業務に忙殺され、本来の業務である包括的支援事業に取り組む余裕がない状況が続いている。2000年に創設された介護保険制度の検討過程において、軽度者に対して予防的給付を行うことは、保険事故に対して給付を行う保険制度の原則に反するのではないかという意見も多く出された。しかし、最終的には要介護と要支援は一体のものとして取り扱うべきであると結論づけられ、要支援認定者に対しても予防給付が行われることとなった。介護保険の本来の目的である「介護の社会化」の「介護」の概念から逸脱しているケースが多くみられる現状に対し、本論文では、要支援者に対する予防給付が過剰に発生するメカニズムについて、申請時の状況、それに続く要介護認定における認定調査と介護認定審査会およびケアマネジメントの各プロセスの状況分析をもとに考察を行うことで、申請・利用の件数が増加する仕組みを明らかにしていく。

The recruitment process of planktic larvae of the nereidid polychaete Hediste diadroma, which has a catadromous life cycle, was examined in an estuary in Kagoshima Bay, Southern Japan. Our laboratory experiments showed that the phototaxis of the planktic larvae changed drastically from positive to negative during the 3-chaetiger nectochaeta, 3.5–4.0 days after fertilization. During this stage, the prostomial antennae, first peristomial cirri, and anal cirri appeared and the ciliary bands used for swimming were reduced; these results suggest that larvae shifted from pelagic to demersal stage at approximately 4.0 days after fertilization. Field sampling took place in the estuary, where reproductive swarming of mature adults was observed in late February to April; planktic larvae of, mainly, 5- to 8-chaetiger stages were collected in evening high tides during the spring tides of April and May 1989, 2013, and 2015. During the same period, 5- or higher chaetiger benthic juveniles were collected at low tide from intertidal flats from a wide area of the estuary where adults inhabited. The morphology of benthic juveniles of 5- and 6-chaetiger stages differed from that of planktic larvae of the same stages in the following characteristics: (1) the long larva-specific spinigers were lost; (2) the second peristomial cirri, originating from the parapodial lobes of chaetiger 1 of the planktic larvae, appeared; and (3) the anal cirri were elongated. Our results indicate that 5- to 8-chaetiger nectochaeta are critical phases for the successful settlement of planktic larvae, which come back into an estuary with rising tides.

現在,日本では,高齢者における肺炎の罹病率,死亡率が増加しており,これを背景として医療・介護関連肺炎という新しい疾患概念も生まれている.高齢者の肺炎は,非典型的な症状や所見を呈することが多く,また,誤嚥が大きな要素となっている.高齢者肺炎の原因微生物は,若年者に比して多様であり,薬剤耐性菌の頻度が高くなるが,検出菌が真の原因菌であるかを検討しなければいけない.高齢者肺炎の治療は,患者の医学的および社会的背景ならびに薬剤耐性菌のリスクを考慮して原因微生物を想定し抗菌薬を選択するが,過剰な治療にならぬよう,用量や薬剤併用に留意する必要がある.また,補液,栄養管理,呼吸管理等,抗菌薬治療以外の治療にも留意する必要がある.高齢者肺炎の中には,末期肺炎,老衰としての肺炎,嚥下機能廃絶例も含まれ,これらに対してどのように対処していくか,社会的なコンセンサス作りも必要である.

日本のある種のマンガ(いわゆる「少女マンガ」)には,一時期,他の芸術にはない独自の言語位相が認められた。図1における中央部白ヌキの言葉は,その一例である。1981年にこのページに驚いた私は,その後少女マンガをよく読むようになったが,それは主としてこの位相に引かれたためであった。これはマンガを描く能力を持たない私のような者には,なかなか実現できない位相である。このような言葉は,他の芸術ジャンルでは,たとえマンガの隣接領域である文学やアニメにおいてさえ,作り出すことが容易ではない。白ヌキの言葉(「ハンプティ/ダンプティ/死んでしまった/白ねずみ, くだけた/ガラス/食べちゃった/お菓子,すべて/もとには/もどらない」) それ自体はいうまでもなく『マザーグース』の歌詞であり,文学の領域においてすでに実現されているものだ。したがってこの言葉が少女マンガ独自の位相を帯びているというのは,言葉それ自体によるのではない。この言葉と作中人物たちとの関係や,この言葉が作品世界で果たしている役割が,他の芸術ジャンルにはあまり見られないようなものなのである。この位相を持つ言葉は,形態面についていえば,吹きだしや枠で囲まれることなく,いわばむき出しで画面上に配置されている場合が多い。本稿はこの少女マンガ独自の言語位相の考察を目的とする。ただしここで対象とする言葉の位相は,1970年代から80年代にかけて発達し,一部の少年マンガや劇画にまで影響を及ぼしたものの,1990年前後を境に急速に衰退して,現在ではすでに少女マンガにおいてさえアクチュアルな位相とは見なされていない。1990年代以降に登場した描き手たちの多くは,この言語位相の使用を好まない。
