症例は70歳代,男性。眼の違和感に対し市販の点眼薬を使用開始したところ,眼囲に瘙痒を伴う紅斑,腫脹が出現した。点眼薬の使用を中止の上,ステロイド軟膏の外用を行い,数日間で症状は軽快した。パッチテストでは,点眼薬で陽性。成分パッチテストではアミノカプロン酸で陽性だった。アミノカプロン酸はかつては止血剤として使用されていた抗プラスミン剤だが,現在医療用剤は販売終了している。しかし,止血,抗アレルギー,抗炎症作用と様々な作用を有するため,現在でも医薬品や医薬部外品の成分・添加物として汎用されている。日常生活品にも多岐にわたって使用されており,アミノカプロン酸に対しアレルギーを有する患者は注意が必要である。 (皮膚の科学,17 : 245-249, 2018)

This review focuses on the suppression of physical performance in a cold environment and the underlying physiological mechanisms. There are many situations where humans have to perform physical activities in a cold environment. Cold environments often limit exercise and working performance by impairing functions such as force production, velocity, power and manual dexterity. A muscle temperature of around 27°C is assumed to be a critical temperature below which maximal voluntary isometric force starts to decrease. The endurance time of submaximal isometric contractions peak at muscle temperatures of 27 to 28°C and decrease rapidly above and below these temperatures. Dynamic exercise performance, especially fast velocity movement, is generally more disturbed by cooling than isometric contractions. Additionally, the effect of cold adaptation on exercise performance, and the potential related mechanisms are summarized here based on a limited number of studies. Since the involuntary muscle contraction of shivering disturbs fine motor control, habituation of shivering, which is an example of cold adaptation, potentially improves exercise performance. Higher hand skin temperatures, induced by greater cold induced vasodilatation after local cold adaptation, could improve manual dexterity. Since there have been few studies testing the effect of cold adaptation on exercise performance in a cold environment, further studies seem warranted.

本稿ではインターネット調査に基づき,主観的最低生活費の検討を行った。その目的は三つある。第一は一般市民が想定する最低生活費を計測することである。第二は主観的最低生活費が,尋ね方によりどれほど幅のある概念なのか,ということを確認することである。第三は生活保護制度と比較し,計測された主観的最低生活費がどのような特徴を持っているか把握することである。その結果,生活保護基準額は,単身世帯を除きK調査(切り詰めるだけ切り詰めて最低限いくら必要か質問)とT調査(つつましいながらも人前で恥ずかしくない社会生活をおくるためにいくら必要か質問)の主観的最低生活費(中央値)の間に位置すること,単身世帯では生活保護基準はT・K両調査を下回る水準となっていること,世帯所得が1%上昇しても主観的最低生活費は0.2%しか上昇しないこと,主観的最低生活費の等価尺度は小さい(=世帯に働く規模の経済性を大きく見積もる)こと,等を明らかにした。

我が国の食事は, 白飯, みそ汁, おかずを交互に食べる食事様式である. 本研究では, みそ汁に, 白飯, あるいは白飯と塩分濃度の異なるおかずを組み合わせ, みそ汁の食べ始めと食べ終わりの塩味の強さ, 好ましさについて検討した. その結果, 食べ終わりは食べ始めよりも, みそ汁の塩味を弱いと感じる組み合わせがあり, 同じ塩分濃度のおかずでも, 種類により影響が異なることが示された. 一方, みそ汁の塩味の好ましさは, 白飯とおかずを組み合わせた場合の食べ終わりと, 食べ始めとの間に有意な差は認められず, 弱く感じさせたみそ汁の塩味をおかずの塩味が補い, みそ汁の好ましさが維持されていた. また, 組み合わせるおかずが同じ場合では, みそ汁の塩味の好ましさに, 0.6%と0.8%のみそ汁の間に有意な差は認められず, 「塩味が薄くてもおいしい食事」は, みそ汁では0.6%で可能であることが示唆された.

This study investigated the impact of sea surface temperature (SST) on a snowfall event of late January 2018 on Kanto, Honshu, Japan, associated with an extratropical cyclone (south-coast cyclone) along the Pacific side, during the Kuroshio large meandering period by a regional atmospheric model. We addressed SST impacts on the snowfall event under almost the same synoptic-scale conditions, such as extratropical cyclone, in a framework of lateral boundary forcing. We conducted control experiments (KLM) with SST of the Kuroshio large mean-dering, and sensitivity experiments (NKLM) prescribed non-Kuroshio large meandering SSTs. Observational results showed that SST anomalies south of Tokai, and east of northern Kanto and Tohoku could be candidates to modify a snowfall event. As simulated results, timings of snowfall and precipitation associated with the extratropical cyclone was reproduced. Unlike what has been suggested, the SST impact south of Tokai on snowfall was not clear. However, the SST east of northern Kanto and Tohoku showed a significant impact. This impact was explained by southward advection of modified air-mass from east of northern Kanto and Tohoku. Additional sensitivity experiments supported this result. This study implies the importance of the SST anomaly east of northern Kanto and Tohoku for snowfall in Tokyo.

近年,推薦精度以外の評価尺度を重視した推薦システムに関する研究が注目されている.推薦精度以外の評価尺度の一つであるセレンディピティは,推薦システムがいかにユーザにとって意外かつ有用なアイテムを発見できるかを表す評価尺度である.本研究では,セレンディピティ指向情報推薦システムとしてフュージョンベース推薦システムを提案する.提案システムは,外発的および内発的偶然を発生させる機構をもち,ユーザがこれらの偶然の中から価値あるアイテムを察知できるような仕組みをもつ.我々は,セレンディピティ指向情報推薦においては,これらの機構をもつインタフェースを要件としたシステム設計が必要であると考える.提案システムは,ユーザが任意に選択した二つのアイテムを混ぜ合わせることで,セレンディピティなアイテムを発見するという,フュージョンベースアプローチの考え方に基づく.本研究の貢献は次のとおりである:セレンディピティ向上のためのフュージョンベースアプローチを適用した推薦システムを提案した.実際の楽天ブックスの書籍データセットを用いた被験者実験による推薦システムの有効性評価を行った.既存の推薦システムの一つである Amazon とのセレンディピティの観点から比較評価を行った.

This essay examines the ideological and even libidinal investment in a child's innocence in Richard Powers's fourth novel Operation Wandering Soul (1993). Criticisms of Powers's novels have concentrated on their so-called "post-modernist" aesthetics and their political sensitivity, which might be regarded as a liberal humanist one. However, instead of following the common path of praising their politics without reserve, this essay demonstrates how his alleged humanism reflects the cultural obsession with the child's innocence in contemporary American society. Symptomatic of this cultural obsession is the recent heated debate on the "recovered memory syndrome." This epidemic discourse of the recovery of the memory of childhood sexual abuse articulates the problem of child molestation in the fixed triangulation of victimized child-monstrous pedophile-benevolent rescuer, or, "us." My argument follows James Kincaid's contention that such representation enables "us" to "produce and circulate the details of child molesting not only with impunity but with righteous fervor" (Kincaid 180). Published in the midst of the epidemic of "recovered memory" discourse, Soul incorporates and reflects this cultural enthusiasm for the innocent child. The text consists of three multi-layered narratives: the predominant realistic narrative of a relationship between the pediatrician Richard Kraft and the nurse Linda Espera; fragmentary narratives of "escaping children" interpolated into this; and the metafictional narrative of a novelist writing Kraft's narrative. Central to these narratives are repeated images of suffering children. The emphasis on their vulnerability and morality, which Kraft has failed to confront, is intended to evoke the readers' humanistic sympathy and responsibility towards them; hence the text closes by calling for the readers' intervention: "Someone donates their organs, all of them. You" (351). The text's humanist politics that have hitherto been praised, then, rely on the intense association of the images of the child with vulnerability and innocence. Such representation resonates with, and is symptomatic of, the ideological and libidinal investment in a child's innocence that has sustained the contemporary epidemic discourse of child molestation. Not surprisingly, Kraft's intimacy with the others is shadowed by the subtext of child/childhood sexual abuse. As the relationship with Kraft reminds Espera of the molestation she has suffered as a child, the emotional entanglement between them, and that between Kraft and his girl-patient Joy, is underpinned by a thorough unevenness of power; hence the line between their attachment to Kraft and the Stockholm syndrome, or the abductee's involuntary identification with the abductor, in sexual abuse is rather contingent. Moreover, Kraft's characterization itself obscures the boundary between pedophilic desire and "pure" affection towards children. Kraft is characterized by his misanthropy, which critics like Joseph Dewey have generalized as a "withdrawal" from reality. However, the fact that the rhetoric of his operation of Joy (273-4, 286) is explicitly sexualized seems to suggest that there lies behind his figure the subtext of pedophilia; in other words, that his misanthropy might be a symptom of his sexual orientation not towards adults but towards children. From the viewpoint of child/childhood sexual abuse, then, the text's ethics interconnected with its metafictional structure turns out to be problematic. The metanarrative of the novelist writing Kraft's narrative negates Kraft's "withdrawal" by relativizing him as a fictional character even within the framework of the text, in order to evoke the readers' responsibility. 直立二足歩行は地球上でヒトのみが行う特異なロコモーション様式である.この歩行がいつ,どこで,どのように,なぜ獲得されたかを知ることは人類進化過程最大の課題の一つである.ロコモーションという動きを知るためにヒトと類縁であるサルのロコモーションを調べる比較運動学の研究が進み,化石の証拠と相まって二足獲得過程を明らかにしてきた.ヒトはアフリカにおいて700 万年ほど前に二足歩行を行うことでサルと分岐した.二足歩行能力はサル特有の樹上三次元での生活へ適応する中から発達してきた.これにより,ヒトは樹上より地上に降り立った時点から,すでにかなり優れた二足能力を持っていたと考えられる.これらの考え方は化石の証拠と矛盾しない.なぜヒトが二足歩行を始めたかに関してはいまだ不明なところが多い.

本稿では,大都市圏の経済活動と経済格差について,6つの論点から研究課題と政策課題を整理し研究動向を踏まえつつ批判的検討を加えた.第1は,格差,不平等,貧困という概念の整理と研究意義についてであり,貧困のみを重視せず格差研究との両輪が必要であることを主張した.第2は,社会的格差と空間的視角についてであり,反映論的な都市・地域研究の問題点を指摘し,弁証法的な観点を取り込む重要性を述べた.第3は,大都市圏の社会的分極化をめぐり,日本の大都市圏についても分極化の観点だけでなく専門職化など多面的な接近が必要なことを論じた.第4は,就業,所得,消費の大都市圏内における格差についてであり,包括的指標として所得の重要性は認めつつも,就業,所得,再分配,消費という経済循環の諸側面をより意識する必要性を述べた.また,アメリカ合衆国のように社会的かつ地理的な分極化あるいは分断化が進むのか展望した.第5に,所得格差と資産格差との関わりを取り上げた.日本において分極化が明瞭でないのはなぜか,資産の側面から問題を提起した.第6は,経済格差との関わりからみた大都市圏経済の再編の方向性について展望した.

The neurovascular unit (NVU) is a conceptual framework used to better understand the normal physiological interaction between neurons and microvessels as well as the pathophysiology of cerebral ischemia. The major components of the NVU consist of neurons, microvessels, and astrocytes that are interposed between the neuronal synapse and the microvasculature. In addition, other types of glial cells (microglia and oligodendroglia), pericytes, and the extracellular matrix (ECM) play important roles in the neurovascular coupling that enables microvessels to supply adequate glucose and oxygen to neurons when activated. The close proximity of microvessels to astroglial end–feet and the metabolic support of astrocytes for neurons suggest that communication could also be directed from microvessels to the neurons. Regarding alternative energy substrates other than glucose, the highly active astroglial glycolytic pathway provides neurons with lactate as a TCA cycle substrate. In addition, ketone bodies are thought to be generated mainly in astroglia to fuel neurons. The shunt pathway of glycolysis, the pentose–phosphate pathway, plays a pivotal role in the maintenance of NADPH, which that is necessary to protect both neurons and microvessels against reactive oxygen species (ROS). In fact, ROS induces neuronal pyruvate dehydrogenase complex (PDHC) dysfunction and ECM degradation, leading to hemorrhagic transformation after reperfusion. Astrocyte–derived ketone bodies can be utilized by the neuronal TCA cycle even after ischemic insult, since PDHC is not required for their metabolism. The concept of the NVU has expanded our understanding of pathophysiology and aided the development of novel therapeutic strategies against neurodegenerative diseases, neuro–immunological disorders, and stroke.
