Aberrant female and male medusae of Turritopsis rubra and T. sp. (most frequently those with three oral lips and three gonads, but with normal four radial canals) were found (c 13% incidence in female of both species, n = 107, 85) in November, 2016 from Iwaki City, Fukushima Prefecture, Japan. In both species, parasitic trematoda was found; in T. sp. maximally eight individuals/normal medusa and five ones/aberrant medusa. Moreover, in the same place in Fukushima Prefecture and in the other place in Ibaragi Prefecture aberrant Turritopsis rubra mature medusae without oral lips or with 1, 2, 3, 5 oral lips, were also found (18.3 %, n = 142 in Otsu harbour in Ibaragi Prefecture) in October, 2017

The vocal cord paralysis may be caused by the occult type malignancies. The tumor occupies along the route of the vagus nerve or recurrent laryngeal nerve. To detect such a malignant tumor, it is advocated that the diagnosis should be performed with various kind of figures, such as X-ray picture, CT and ultrasonography. Consequently 13 cases were diagnosed to the malignant tumors among 31 vocal cord paralysis cases. They were thyroid cancer, lung cancer, esophageal cancer and others. CT and ultrasonography were very useful for the evaluation of the mass lesions in the head and neck area. Additionally, the fine needle aspiration biopsy under the ultrasonographic imaging was available to diagnose whether the mass lesions were benign or malignant. Consequently 13 cases were diagnosed to the malignant tumors among 31 vocal cord paralysis cases. They were thyroid cancer, lung cancer, esophageal cancer and others.CT and ultrasonography were very useful for the evaluation of the mass lesions in the head and neck area. Additionally, the fine needle aspiration biopsy under the ultrasonographic imaging was available to diagnose whether the mass lesions were benign or malignant.

【目的】 脳卒中治療ガイドライン2009で脳卒中後の半側空間無視(USN;Unilateral Spatial Neglect)に対し、根拠がある方法が紹介されているが、十分科学的な報告は少ない。現在のUSNの治療法の多くは、左空間を意識させる、頚部への電気刺激、無視側への体幹の回旋、プリズム眼鏡の利用などが挙げられる。その中で、視覚探索訓練はエビデンスレベルBを得ている。今回、近年普及しているタブレット型コンピューターのipadで新規に治療も可能なアプリケーションを作成した。これは科学的に認められている視覚探索訓練をもとに作成し、ゲーム感覚に実施できることが特徴である。また、ipadのため持ち運びが可能であり、病室でも自主練習が可能である。そこで今回、USN患者の新しい治療法となるかを検討すべく、ipadでの治療前、直後での即時効果を検討した。本研究は理学療法士協会の研究助成の一部を用いて行った。【方法】 対象は脳血管障害により左片麻痺・半側空間無視を呈した右利き7名(年齢58~88歳;70.7歳;男性5名,女性2名)、平均発症日数94.7(SD76.2:27-209日)、平均FIM64.6(SD8.2)、平均MMSE20.2(SD1.3)であり、治療アプリケーションが使用できる症例であった。ipadの治療アプリケーションは、左右端から5つの円(円列)が毎秒100mm以下のゆっくりした速さで青色の円が画面に現れるように設定した。画面を横切る5つの円のうち1つが途中で赤色に1施行中2回変化させた。円列は左右からランダムに10回モニターを横切り、どの位置でどの円の色が変更されるかはランダムとした。検査者は対象者に「色が変化したら、その円を画面上でタッチしてください」と指示した。ipadの治療前、直後で線分末梢試験と線分二等分試験を実施した。線分末梢試験は末梢した本数、線分二等分試験は中央をより偏位した距離を右に偏位するほど正の値で算出した。統計処理はSPSS 19.0 for Windowsを用い、ipadでの治療前後の線分二等分試験と線分末梢試験の数値をウィルコクソンの符号付順位和検定にて分析を行い、なお有意水準は5%とした。【倫理的配慮、説明と同意】 全対象者に対して、事前に本研究の目的と方法を説明し、研究協力の同意を得た。本研究は首都大学東京荒川キャンパス倫理審査委員会の承認を得た。【結果】 ipadにおける治療前の線分二等分試験は中央より右に23.86±22.52mm偏位していたのが、治療直後の線分二等分試験は中央より右に12.43±22.50mm偏位しており、有意に中央に偏位した。(p<0.05)ipadにおける治療前の線分末梢試験は22.86±15.53本消去したのが、治療直後の線分末梢試験は26.57±12.91本消去し、有意に消去する本数が増加した。(p<0.05)【考察】 視覚探索訓練は科学的に推奨される治療法の1つである。今回、簡易的に持ち運べるipadを利用して、治療ソフトの開発を行い治療前、直後の即時効果を検討した。治療効果の検討には現在までに使用されている線分二等分試験と線分末梢試験を適応した結果、両方の試験において治療前、直後で有意な差があった。現在までの多い報告では、右より左に動き光や動く物を目で追わせることでUSNが改善したものが多い。今回も色の変化を右または左から動く円を注視させるために、注意を促すことが可能であり、左への注意が改善したことが考えられる。また、右からだけではなく、左から移動する円にも注意を払わなければならず、右からの追走以外にも左から移動するものにも注意することで空間注意が即時的に改善したと考えられる。【理学療法学研究としての意義】 近年普及しているタブレット型コンピューターのipadを利用して、視覚探索訓練のアプリケーションを作成した。即時効果の検討を行った結果、治療効果が認められた。また、今学会にて同時に信頼性と妥当性の検討も行った。持ち運び可能で、本人のペースで実行可能であり、ゲーム感覚があるために取り組みやすい。今後は効果の継続や、対照群を設定することで効果の更なる検証が必要だと考えられる。さらに即時効果の影響や、長期的な影響の結果がADLに対する影響を詳細に検討することで、科学的に効果のあるリハビリテーション手段として確立することが可能になるのではないかと考えられる。

終末期の意思決定に関する法制度・ガイドライン等を批判的に検討した結果、以下のことが明らかとなった。①医師ー患者関係に信頼性があり、透明性が担保されていれば、すべり坂の仮説はおこらないこと、②緩和ケアと安楽死は、相互に排他的なものではなくて、よき生の終結ケアの不可欠の要素であること、③それにもかかわらず、「すべり坂の仮説」を完全に払拭しえないのは、通常の医療である治療の差し控えや中止、緩和医療を施行するとき、患者の同意を医師が必ずしもとらないことにあること。したがって、通常の治療を含むすべての終末期ケアを透明にする仕組みの構築こそが『死の質の良さを』を保証する最上の道であると、我々は結論した。

We reported here several sting cases by tentacles of Tamoya haplonema (Cnidaria, Cubozoa) collected in the Inland Sea of Japan in these four years (2009-2012). All cases experienced by us were not severe and swollen portions cured within a week because of hands were stung.

A Japanese Black calf six and a half months old showed signs of anorexia and constipation for eight days. It failed to respond to any medical treatment during this period. The right sublumbar fossa was mildly distended. A resonant ping was audible by simultaneous auscultation and percussion over the whole right flank on the sixth day of illness.A tentative diagnosis of cecal volvulus was made. laparotomy was performed through the right flank under local anesthesia. Manual examination of the abdominal cavity detected the distended and displaced cecum filled with a large amount of gas and gastric juice, and adhesions of the omentum to the intestinal wall around the ileocecocolic junction.As the site of adhesion was thickened with proliferated connective tissue and the intestinal lumen narrowed at this site, the cecum and a proximal part of the colon were resected and then end-to-end anastomosis was made between the ileum and the colon.Postoperative recovery was uneventful with defecation on the day after operation and the appetite resumed gradually. During the postoperative period of four and a half months, the serum magnesium level was high temporarily and returned later to a normal one.

<b>【目的】</b>揚げる・炒める分科会ではこれまで揚げ種重量の10倍程度の油を用いた一般的な揚げ方法での様々な研究を行なってきた。しかし、最近では環境問題や健康への配慮から少量の油で揚げる「シャロウフライ」と呼ばれる方法が注目されている。そこで今回は揚げ種が浸る程度の少ない油量で揚げた場合の揚がり具合について検討した。<br>&nbsp;<b>【方法】</b>試料として豚カツ(業務用冷凍豚一口カツ)およびサツマイモ、揚げ油はキャノーラ油を用いた。油の量は「ディープ」(油の深さが揚げ種の厚さの2倍)、「シャロウ1」(同1倍)、「シャロウ1/2」(同1/2倍)とした。温度調節付きガスコンロと26cm径のフライパンを用い、豚カツは180℃、5分間、サツマイモは160℃、6分間揚げた。揚げ操作中に揚げ種の中心温度と油温の変化を測定し、揚げ操作後の重量変化より吸油率と脱水率を算出した。官能評価はパネル6~8名で、評点法(外観、油臭さ、におい、味、揚がり具合、テクスチャー、総合)と順位法で行った。<br>&nbsp;<b>【結果】</b>脱水率は、豚カツ、サツマイモともに「シャロウ1/2」で低かった。吸油率は、豚カツでは油量による差はみられなかった。中心温度は「シャロウ1/2」では温度上昇が遅かった。官能評価の評点法は、豚カツ、サツマイモともに外観、におい、揚がり具合、テクスチャーおよび総合において「シャロウ1/2」が有意に低い評価であった。順位法では、豚カツは、「ディープ」、「シャロウ1」、「シャロウ1/2」の順に好まれた。サツマイモの「シャロウ1/2」では仕上がりが不均一で水っぽく感じられ、脱水率の結果と一致していた。以上の結果から、油量の違いは揚がり具合に影響を及ぼすことが明らかになった。

<b>【目的】</b>近年、簡便な揚げ調理として、少量の油で揚げる「シャロウフライ」が注目されている。前回、「シャロウフライ」は通常の揚げ調理に比べて、官能評価が低いことを報告した。しかし、これは官能評価に影響を及ぼす要因である脱水率が統一されていない条件下であった。そこで今回は、脱水率を統一して「シャロウフライ」の再評価を実施した。<br><b>【方法】</b>試料は豚カツ(業務用冷凍豚一口カツ)、揚げ油はキャノーラ油を用いた。油量は、通常の揚げ調理である揚げ種の厚さの2倍の深さ(D)、厚さの1倍(S1)、および厚さの1/2倍(S1/2)とした。温度調節付ガスコンロを用いて180℃で揚げ調理を行い、揚げ種の中心温度と油温の変化を測定した。揚げ油の物理化学的性状値として、酸価、カルボニル価、粘度、極性化合物量、および色を測定した。揚げ種について脱水率を算出し、分科会のメンバーをパネル(n=18)として官能評価を評点法(外観、油臭さ、におい、味、揚がり具合、テクスチャー、および総合)により実施した。<br><b>【結果】</b>通常調理(D、揚げ時間5分)と同じ脱水率を得るために、S1は6分、S1/2は8分を要した。官能評価の評点(外観、におい、揚がり具合、テクスチャーおよび総合)において、S1とS1/2は揚げ時間の延長により、有意に高い評価が得られた。揚げ油の化学的性状値は、カルボニル価、極性化合物量、および色においてS1/2では揚げ時間の延長とともに上昇した。以上の結果から、シャロウフライは的確な揚げ時間の延長と揚げ作業の継続により、Dと同等の風味評価を確保できることが明らかとなった。一方、シャロウフライでは、使用油の劣化は大で、油の劣化に繋がることが明確になった。

本研究の目的は,初年次教育の学習活動において学生自身が到達目標を設定することが,学生の自律的な学習態度へどのような影響を与えるのかを明らかにすることである.初年次教育の授業において学生がルーブリックを作成し評価に用いる実践を行った.質問紙調査の結果,ルーブリックを自分達で作成することにより,多くの学生が目標と省察を意識しながら学習活動を行ったことが分かった.また,学生の自由記述を分析した結果,ルーブリックを自分達で作成することは「目標への意識」「課題に対する動機づけ/責任感」「課題の成果に対する省察」「評価に対する公平感」「多様な評価の観点の気付き」と関連していたことが分かった.

The Mid-Pleistocene Transition (MPT) is the well–known interval that the dominant periodicity of earth’s climate cycles shifted from 41 to 100 ky rhythms (e.g., Elderfield et al., 2012). This study will discuss paleoceanographic changes during the Early Pleistocene before the MPT around the central part of the Pacific side of Japan based on calcareous nannofossil assemblages. We studied the Kiwada Formation in the Kazusa Group, distributed the Boso Peninsula. Two different counting techniques were used to clarify biostratigraphic and paleoceanographic events based on calcareous nannofossils. The age model for this section was proposed by Kuwano et al. (2019a, b). Investigaed ages is Marine Isotope Stage (MIS) 41 to 36. Spectral analyses using the PAST3 software were also applied in order to extract paleocanographic signals from nannofossils.At least 13 species and 13 genera of calcareous nannofossils were identified in the examined section. Umbilicosphaera sibogae(Kuroshio water index) increased at the glacial–interglacial boundaries, Florisphaera profunda (stratified, warm offshore water index) and Helicosphaera spp. (freshwater inflow index) increased in the interglacial period. On the other hand, Calcidiscus leptoporus (cool offshore water index) and very small Gephyrocapsa spp. (eutrophic freshwater index) increased during the glacial period. In particular, Coccolithus pelagicus (eutrophic cool water index) abundant at the end of the glacial period. The power spectra of F. profunda, U. sibogae, very small Gephyrocapsa spp., and C. leptoporus show 55–57 kyr periodicity, which also appeared in benthic foraminiferal δ18O. The periodicity of 22–23 kyr was recognized from relative abundances of F. profunda, U. sibogae, and Helicosphaera spp. Those sequential fluctuations of nannofossils indicate that northward/southward of the Kuroshio and Subarctic Front around the Pacific side of Japan.
