快適な都市空間づくりが主要なテーマとなってきた現在,都市の熱環境が人の温冷感に及ぼす影響を正しく評価することも重要である.その評価のための温熱指標確立のための要素は種々であるが,屋外環境では,特に,日射の影響が地物での反射も含めて大きい.そこで,本研究では,多重放射を考慮して,日射と建物など地物の影響を組み入れた平均放射温度の導出を試み,その評価法として提案する.また,多重放射の影響については,射度と本研究で定義するIncidence Factorにより考慮し,それぞれによる平均放射温度を導出し,その特性について検討する.

本研究では、uTAS分野において活用可能な、微少量の液体をハンドリングする分注機構を搭載した超小型ロボットの開発を行っている。前報までに振動モータを利用した微少液滴塗布機構を開発してきたが、振動の振幅制御を手動で行っていた為、作業に熟練を要した。今回、微小変位を高速に測定できる渦電流式変位センサ用いて自動的に振幅制御を行う手法を新たに開発したので、この原理および性能について報告する。

In the introductory philosophy class at Nagaoka Institute of Design, students are required to make philosophical experiments, one of which is a meditation of listening to the surrounding sounds very intently. Through it students can experinece various forms of nonordinary modes of experiencing the world. We can see the beginning steps ofphilosophical questions in those experineces. We also analyse them by applying the phenomenology of M. Merleau-Ponty and other theories.

目的:医療機関受診者に比べ一般健康人により近い健診受診者を対象として,胃食道逆流症(gastroesophagealreflux disease:GERD)の実態調査を施行した.方法:上部消化器疾患で治療中の人を除いた胃内視鏡検査受診者659名(男性368名,女性291名)を対象とした.内視鏡所見とfrequency scale for the symptoms of GERD(FSSG)問診票のスコアを基に対象者をびらん性胃食道逆流症(erosive gastroesophageal reflux disease:e-GERD),非びらん性胃食道逆流症(nOn-erosivegastroesophagealreflux disease:NERD),非GERDの3群に分け,各群の性別発見頻度と血糖,血清脂質,高感度C-反応性蛋白(CRP)などの生活習慣病関連因子,血圧,ならびにメタボリックシンドロームの合併率を比較検討した.結果:NERDの頻度は男女ともe-GERDより多かった.e-GERDは女性より男性に高頻度でみられ,NERD,非GERDに比べ,血圧が高く,血糖,血清脂質,高感度CRPなどがより高値を呈し,メタボリックシンドロームの合併率が高かった.一方,NERDはやぜ気味の人に多くみられ,血糖,血清脂質,高感度CRP,血圧は非GERDよりもさらに低値を示し,メタボリックシンドロームの合併率も3群のなかで最も低かった.結論:e-GERDは肥満に起因する生活習慣病の1つと考えられたが,NERDはやぜ気味の人に多くみられ,生活習慣病関連因子の異常が少ないことから,e-GERDとは異なる病態を有することが示唆された.

喉頭肉芽腫は声帯突起部に好発する炎症性の腫瘤であるが,しばしば再発し治療に難渋することのある疾患である。今回我々は喉頭肉芽腫の9症例に対しレバミピドとセラペプターゼの2剤併用の内服加療をおこなった。その結果,9例中5例(56%)で肉芽腫が消失した。肉芽腫消失までの期間は最短で21日,最長で91日であった。5例中3例は術後再発の症例であった。また1例はPPIの無効例であった。また5例全例とも投与後に最短10日,最長28日で肉芽腫の縮小効果が確認された。消失した5例で,内服終了後に再発したものはなかった。<br>今回の結果から,喉頭肉芽腫が消失した原因は主として血中レバミピドの炎症抑制作用が喉頭粘膜に影響を及ぼしたためではないかと推察した。また無効であった4例では,血中濃度が充分でなかった可能性と,レバミピドでは胃酸逆流の防止が困難なほど重度なGERDが潜んでいた可能性を推察した。

今回我々は,北海道上砂川町にある地下無重力実験センター(Japan Microgravity Center: JAMIC)を利用する機会を得たので,これが身体平衡系の研究における新しい実験手段として有用であるかを検討する目的で,平衡系実験セット(姿勢•行動観察用及び筋電図記録用)を作製し,カエルを用いた予備実験を行った.1) 姿勢•行動観察では,無処置群において頭部が背屈し,四肢が伸展するという特徴的な姿勢が観察できた.また前庭神経切断群では,落下前の姿勢変化はそのまま持続し,障害側を下にする回転運動が見られた.2) 筋電図では,落下直後に前庭脊髄反射のためと考えられる筋活動の亢進を認めた.また前庭神経切断例では,術側の潜時が,やや遅延した.3) カエルは,落下実験施設を利用した平衡系実験の実験動物として適当である.4) 微小重力暴露時の身体平衡系の研究において,落下実験施設は非常に有用な実験手段である.

Objective : Odontoid fractures are the most common cervical injuries among the elderly population. We evaluated treatment outcome in 14 patients with odontoid fractures who were treated at our institute between 2003 and 2016. Additionally, in this report, we discuss the optimal treatment option for odontoid fractures in the elderly population and provide a literature review.  Results : Patient age ranged 19-90 (average, 60.6) years, and the patients included five (36%) octogenarians. According to Anderson’s classification, four cases (29%) were type II and 10 cases (71%) were type III odontoid fractures. Surgical intervention was performed in 12 cases, including anterior odontoid screw fixation in nine, posterior cervical instrumented fusion in four (including reoperation after anterior screw fixation in one), and conservative treatment with halo vest fixation in two. Moreover, 13 patients received radiographic follow-up, and 11 (85%) attained bone fusion. In octogenarians, three (60%) of five cases attained bone fusion through anterior screw fixation.  Conclusion : In the elderly population, tailor-made treatment is necessary according to systemic diseases, activities of daily living, and radiographic assessment of odontoid fractures in addition to Anderson’s classification.

哺乳動物において射出精子は,透明帯との結合,先体反応および卵子との受精のために雌性生殖路内で複雑な生理学的,機能的変化をとげねばならない.この精子が受精可能となる変化を受精能獲得(capacitation)と呼ぶ.capacitationを完了した精子は,透明帯を貫通するための特殊な鞭毛運動である超活性化(hyperactivation)と呼ばれる変化を示す.またcapacitationの過程で,精子表面や精子細胞膜の変化,Ca ²⁺やHCO₃^&ndash;イオンの変化,アデニル酸シクラーゼ/cAMP/PKA経路そして精子タンパク質リン酸化の変化を起こす.将来的に,詳細なcapacitationのメカニズムの解明により男性不妊症における精子機能検査法や男性不妊治療法を開発できるかもしれない.<br>

薬理学と関係の深い創薬は医薬品産業政策と密接なかかわりを持つ.本稿では,医薬品産業政策の課題を,経済の基盤である市場における主体の行動と経済の関わりをもとに整理する.医薬品産業政策を考える上での問題の1つは,医薬品産業政策において重視される複数の目的が必ずしも両立可能でないということである.たとえば,現在の政策の1つの流れである医療費の抑制は,その目的においてしばしば中・長期的な経済への正の影響とトレードオフの関係にある.中・長期的な経済への正の影響とは,消費者の将来の健康そのもの,または健康に起因する経済への効果のことであり,さらに生産者である医薬品業界における研究開発投資が経済成長を起こす潜在的な機動力となる,ということである.これらの医薬品の使用が健康に及ぼす影響や創薬の経済効果は,長期間にわたり発生するため定量的な評価が著しく困難であるが,医薬品産業政策を考えるうえで重要な要素である.

<p>慢性早剥羊水過少症候群(chronic abruption oligohydramnios sequence;CAOS)は,一般に周産期予後は不良で初回出血の週数が早いほど予後が悪いとされている.今回,われわれは異なる経過をたどり,CAOSの予後規定因子について示唆に富む2症例を経験したので報告する.症例1は妊娠14週4日より性器出血と絨毛膜下血腫(sub-chorionic hematoma;SCH)を認め,妊娠23週4日に腹痛と多量の性器出血により当院に救急搬送され入院管理となった.入院後も性器出血は持続し,妊娠24週1日で羊水過少を認めCAOSと診断した.妊娠31週6日に陣痛が発来し経腟分娩となり健児を得た.胎盤病理は絨毛膜羊膜炎(chorioamnionitis;CAM)を認めなかった.症例2は妊娠15週4日に性器出血を認め当院に受診し,SCHを認めた.妊娠16週1日より持続する性器出血と子宮収縮を認め入院管理とした.同時期より羊水過少を認めCAOSと診断した.妊娠21週3日より子宮内胎児発育停止を認め,妊娠24週1日で子宮内胎児死亡となり,妊娠24週6日に経腟分娩した.胎盤病理はCAM III度であった.2例の経過を比較すると,CAOSにおいてもCAMという炎症の長期持続が児の予後不良因子であった可能性が示唆された.SCHに羊水過少を認めた場合はCAOSを念頭に,児の関連合併症に注目するとNICU併設の高次医療機関での周産期管理が推奨できる.〔産婦の進歩70(2):134-142, 2018(平成30年5月)〕</p>

"Mon Emouvant Amour", c'est une chanson avec les paroles et la musique de Charles Aznavour. Il l'a chante parlant par gestes fascinants, c'est-a-dire avec la langue des mains, au theatre de l'Olympia a Paris en 1980 et a eu un tres grand succes. Voila l'histoire : Un brave homme sympathique aime une jeune fille qui est tres jolie, mais sourde-muette. Il lit dans ses regards et ses sourires interpretant ce qu'elle veut dire par ses mains. Quand elle lui murmure de bout des doigts : "je t'aime", il est tout a fait emu, mais quant a lui, il n'a aucun moyen de parler a son aimee; elle lui semble tout etrangere, malgre qu'elle soit tout pres de lui. Alors il se decide a apprendre a son tour le langage des mains. Us reussissent tous les deux a jouer une pantomime. C'est ainsi que leurs deux coeurs se fondent en un seul. Quel miracle ! Ce miracle de pantomime evoque en nous l'opera "La Muette de Portici" (1829, livret de Eugene Scribes) mis en musique par Daniel-Francois-Esprit Aubert. Que l'heroine, fille de pecheur de Portici, est muette, cela a un effet important, agite, touche et gagne le coeur comme "Mon Emouvant Amour". Or, Charles Aznavour, auteur-compositeur-interprete, est ne a Paris en 1924. il est fils d'Armeniens emigres de Turquie. C'est pouquoi il l'a ecrit sur la musique de G.Garvarentz "Ils sont tombes" a la memoire des Armeniens massacres.

イネにおいて,夏期の高温による玄米品質の低下が問題となっている。特に2010年の夏期は北日本から西日本にかけて記録的猛暑となった。そこで,2010年に当農場の大規模圃場において播種期を変えて作付けした複数の品種のイネについて,玄米の外観品質の変異とそれをもたらした要因を解析し,水稲栽培における温暖化対策を検討した。2010年における玄米品質は例年よりも大きく低下したものの,作期や品種によって大きな変異があり,穂揃い後20日間の登熟初期の気温の影響を強く受けていた。今後,より多くの品種,栽培条件下でデータを蓄積していく必要がある。

A 52-year-old male was found to have fever and complained of myalgia and joint pain.<BR>On physical examination by admission, his blood pressure was 128/68 mmHg, pulse 66 per minute, regular, temperature 38.5&deg;C. The patient was pale and appeared chronically ill. Bilateral temporal arteries were extended and moderately tender by percussion. Cardiac dulness was normal and heart sound was clear. Liver and spleen were not palpable. Muscles of extremities were slightly atrophic. Raynaud's phenomen negative. Purpuras on backs of feet were noticed. ECG showed normal sinus rhythm, no axis deviation, ST, T waves normal. X-ray of chest normal.<BR>At 2 weeks after admission, he had been noticed his blood pressure was elevated (BP 210/100 mmHg) and abdominal bruit audible. Following comfirming of polyarteritis nodosa by muscle biopsy of right quadriceps, steroid therapy bigan. On the 26th hospital day, suddenly he experienced severe pain in left hypochondrium which radiated into the back. He was explored surgically because the pain was persistent and anemia revealed progressively.<BR>Retroperitoneal hematoma was found along abdominal aorta and left kidney with subcapular bleeding enlarged as child's head sized but could not be removed because his general condition got worse. Peripheral plasma renin activity (PRA) on restricted salt intake (5g/day) while the patient kept in supine position was 11.9 ng/ml/h, renal vein renin analysis was as follows; r-renal vein 12.1 ng/ml/h, 1-renal vein 14.2 ng/ml/h, upper IVC 12.1 ng/ml/h, lower IVC 8.3 ng/ml/h. Angiotensin II analogue test was carried out using in-fusion of 1-Sarcosine 8-Isoleucine Angiotensin II. Blood pressure before test was 180/130 mmHg and reduced to 130/100 mmHg at 20 minutes after the biginning of infusion at the rate 300 ng/kg/min.. The blood pressure rose gradually to control level over subsequent one hour after the cessation of angiotensin II analogue. PRA level was 8.4 ng/ml/h before administration of angiotensin II analogue and similar value during infusion, then 8.7 ng/ml/h at 110 minutes after cessation of the infusion.

内示生産システムは,製造開始に先立って取引先から入手できる内示情報を生産に活用する仕組みであり,製品仕様の多様性とリードタイムの短縮,コスト削減等を図るものである。すでに,内示生産システムについて特性の明確化,生産管理方式の分類および生産計画法の提案が行われてきている。近年では,種々の業種・業態のなかで内示生産システムの様々なバリエーションも見出されている。また,リスク意識の高まりとともに,レジリエンスの向上に向けた取組も重要である。そこで,本論文では,種々の業種・業態にみられる内示情報の提示パターン,内示注文と確定注文のブレパターンについてのいくつかのバリエーションを示し,かつ,レジリエンス向上のために従来では,考慮されることが少なかった注文の充足不可能性などのリスクを把握する指標について考察を行う。最後に,内示生産システムを規定する基本属性項目を更新し,体系化を図る。In 'NAIJI' production planning system,advanced demand information which is called 'NAIJI' plays an important role for suppliers to produce customer's products in advance to delivery due date. And they realize the cost down, the variety of the product specification and shortening of the lead time. Recently, various variations of 'NAIJI' production planning system are found out and a production system with resilience against risks is indispensable.In this paper, therefore, we show some variations about 'NAIJI' production planning system for sending 'NAIJI' information to suppliers and the difference of order quantity between 'NAIJI' and firm order. Wewill investigate their impacts to production management and purchasing. Finally we will update the key items in order to classify the 'NAIJI' production planning system.
