日本の精神科医療現場において、昔に比べて人権が配慮されてきているが、長期入院患者も少なくなく、また隔離拘束となる場合も多い。最近のアメリカのトラウマ研究から、医療・福祉・行政・司法・教育など様々な領域でトラウマ理解に基づいた支援(トラウマインフォームドケア)が必要であることがわかって来ている。精神障害者が、発症以前にトラウマを抱えていたり、発症後にトラウマとなりうる不当な扱いを受けて病状を悪化させたりしていたことなどが、精神障害者施設のフィールドワークより認められた。今後、日本の精神科医療の中でもトラウマインフォームドケアが行なえる体制が必要である。

ハスカップのアントシアニン色素を塩酸メタノールで溶出し,Dowex50Wカラムを用いて精製した。ハスカップ色素の安定性に及ぼす糖,有機酸,L-アスコルビン酸の影響を検討した。1. 糖類添加では,5.0~20.0%濃度,いずれも吸光度の増大が認められ,増強された赤色は長時間安定であった。また,糖濃度が高い程,濃色化効果は大きかった。2. 有機酸0.8%添加でも濃色化効果を示し,その効果を長時間持続した。3. L-アスコルビン酸添加では.色素は低温(4℃),室温(20℃)ともに急激に退色が進んだ。

再帰下降構文解析は人気の実装方法であるが,バックトラッキングにより,最悪ケースにおいて指数時間を要することが知られている.現実のパーサでは,最悪ケースはほとんど発生しないが線形時間性を保証することは難しい.本研究では,再帰下降構文解析の形式モデルとしてPEGを用い,PEGの文法定義から線形時間性を判定する方法に取り組んだ.

Here, in comparison with community-dwelling middle-aged and older men, we investigated the skeletal muscle mass and bone mineral density of a Japanese alpinist (Mr. A) who, at the age of 80 years, is to date the oldest person to have climbed to the summit of Mount Everest (8,848 m). Using dual X-ray absorptiometry, we determined the appendicular skeletal muscle mass index (SMI), total bone mineral density (tBMD), whole body fat-free mass index (FFMI) and fat mass index (FMI) of Mr. A (84.6 yr) and 209 community-dwelling middle-aged and older men (50-79 yr, mean age: 68.1 yr). The SMI, tBMD, FFMI and FMI were 8.79 kg/m2, 1.075 g/cm2, 22.3 kg/m2 and 9.8 kg/m2, respectively, in Mr. A and 7.46 ± 0.81 kg/m2, 1.020 ± 0.100 g/cm2, 18.1 ± 1.9 kg/m2 and 5.5 ± 1.7 kg/m2, respectively, in the community-dwelling middle-aged and older men. The values in Mr. A were higher than those in the community-dwelling middle-aged and older men, with z-scores for the SMI and tBMD of 1.63 and 0.55, respectively. Mr. A maintained a high skeletal muscle mass and bone mineral density even at the age of 84 years, which may have been attributable in part to his long-term training for mountain climbing.

The purpose of this study was to elucidate the effect of high-temperature storage on the stability of ranitidine, specifically with respect to the potential formation of N-nitrosodimethylamine (NDMA), which is classified as a probable human carcinogen. Commercially available ranitidine reagent powders and formulations were stored under various conditions, and subjected to LC-MS/MS analysis. When ranitidine tablets from two different brands (designated as tablet A and tablet B) were stored under accelerated condition (40°C with 75% relative humidity), following the drug stability guidelines issued by the International Conference on Harmonisation (ICH-Q1A), for up to 8 weeks, the amount of NDMA in them substantially increased from 0.19 to 116 ppm and from 2.89 to 18 ppm, respectively. The formation of NDMA that exceeded the acceptable daily intake limit (0.32 ppm) at the temperature used under accelerated storage conditions clearly highlights the risk of NDMA formation in ranitidine formulations when extrapolated to storage under ambient conditions. A forced-degradation study under the stress condition (60°C for 1 week) strongly suggested that environmental factors such as moisture and oxygen are involved in the formation of NDMA in ranitidine formulations. Storage of ranitidine tablets and reagent powders at the high temperatures also increased the amount of nitrite, which is considered one of the factors influencing NDMA formation. These data indicate the necessity of controlling/monitoring stability-related factors, in addition to controlling impurities during the manufacturing process, in order to mitigate nitrosamine-related health risks of certain pharmaceuticals.

I hope that this lecture series will be helpful, not only for dermatologists, but also for paramedicals and cosmetic scientists.

The stratum corneum (SC) is the outermost layer of the skin and plays important roles in sustaining physiological activities. One of the roles of the SC is in the maintenance of skin moisture. The decrease in water content in the SC, which is a condition called dry skin, leads to a mildly rough skin and desquamation. Natural moisturizing factor (NMF) and intercellular lipids are essential for skin moisturization. NMF, which is composed of amino acids, pyrrolidone carboxylic acid, lactic acid, urea, and mineral ions, can hold water molecules in the SC. Intercellular lipids consisting of ceramides, cholesterol, cholesterol esters, and free fatty acids can prevent the evaporation of water molecules from the body, which is a barrier function. Moisturizers are broadly categorized into two types, humectants and emollients. Humectants such as amino acids, polyols, hyaluronan, and its derivatives moisturize the SC similarly to NMF. Emollients such as ceramides and oils are effective for improving the barrier function. Moisturizing products contain these moisturizers in various formulations. The moisturizing effect of products differs depending on the type of formulation. An adequate amount of a moisturizer in a product is important for providing both the moisturizing effect and the good texture of use, which affects continued use, because a large moisturizer amount impairs the texture of use.

Cosmetics are used to help the skin’s native function work normally with the aim of keeping and recovering healthy, beautiful skin and hair. Cosmetics have a variety of roles; their basic purposes include washing, wiping clean, and anti-dryness, anti-ultraviolet, antioxidation and stimulation functions. The purpose of bodily cleansing is to remove dirt on skin and hair and keep a body hygienic. Cosmetics can be classified in various ways, such as by their purpose and form, and body cleansing agents can be categorized by the body parts to which they are applied, such as cleansing agents for skin, the body, and hair. Skin cleansing agents, which are used to wash facial skin, remove metabolites produced by physiological actions of the skin, such as sebum, stratum corneum scales, oxidative degradation agents of sebum, sweat residue, dirt in the air, microorganisms, and makeup residue from the skin’s surface. Like skin cleansing agents, body cleansing agents are used to wash a large region of the body. Hair cleansing agents remove dirt on the scalp and hair and keep them clean. Although the kinds of dirt on the scalp and hair are similar to those on the skin, residues of hair care agents are also dirt to be removed. What is needed in common for these cleansing agents includes detergency, low stimulative effect on the skin, and foaming property, and these attributes mainly depend on the properties of surfactants. The evolution of cleansing agents is due largely to the evolution of surfactants. Looking back on the historical background of cleansing agents and the basis of cleansing, this article describes the kinds and ingredients of cleansing agents and the effects of cleansing agents by classifying the agents by the body parts they wash and focusing on surfactants.

虚血性心疾患の既往のない, 69歳男性の腹部大動脈瘤破裂の緊急手術前に, 中心静脈カテーテル挿入のため右内頸静脈穿刺時, 突然STの上昇と共に徐脈, 血圧低下を認めた. 腹部大動脈瘤患者では高率に冠動脈疾患を合併することから, 冠動脈硬化による冠動脈血管の tonus の亢進した状態下に, 中心静脈穿刺時の迷走神経刺激が誘因となって冠動脈スパズムが発生したものと考えられた.明らかな虚血性心疾患の既往がなくても全身的に高度な動脈硬化性疾患を有する患者の麻酔管理においては, 冠動脈硬化病変の存在並びに冠動脈血管の tonus の亢進によって, 冠スパズムや重症な不整脈の発生し易いことを十分に念頭におくべきである.
