本研究は、大学生の精神的健康に対する集団アクセプタンス&コミットメント・セラピー(G-ACT)の効果の検討を目的として行われた。大学生17名のうち9名(男性4名、女性5名)をG-ACT群、8名(男性1名、女性7名)を通常介入群に割り付けた。G-ACTはアクセプタンスとマインドフルネスに関わるワーク、コミットメントと行動変容に関わるワークを含む全6セッションから構成された。通常介入は学生相談サービスの利用であった。測定は、G-ACT実施前、実施後、実施1カ月後時点で実施した。線形混合モデルによる解析の結果、G-ACT群は通常介入群と比較して、実施後時点において不快気分の改善、および行動の活性化が、実施1カ月後時点において不快気分の改善および心理的柔軟性の向上が認められた。価値の明確化および回避行動は、G-ACT群と通常介入群の変化量の差異は認められなかった。以上の結果を踏まえ、大学生に対してG-ACTを実施するうえでの留意点について考察を行った。

Histone H3 mutations are frequently found in diffuse midline gliomas (DMGs), which include diffuse intrinsic pontine gliomas and thalamic gliomas. These tumors have dismal prognoses. Recent evidence suggests that one reason for the poor prognoses is that O6-methylguanine-DNA methyltransferase (MGMT) promoter frequently lacks methylation in DMGs. This review compares the epigenetic changes brought about by histone mutations to those by isocitrate dehydrogenase-mutant gliomas, which frequently have methylated MGMT promoters and are known to be sensitive to temozolomide.

After tightening a bolt, technical knowledge on how far the thread of the bolt should protrude from the nut is not organized. Therefore, in addition to the surveys of existing standards, surveys were conducted in consideration of thread strength at bolt tightening, sealing performance of flange joints, and corrosion. As a result, it was found that the protruding length of the screw thread of the bolt had little influence on the strength of the screw and the sealing performance of the flange joint. However, if it is below flush with nut face, there is a possibility of promoting corrosion by dust and moisture, so one or more protrusion lengths may be required.

ろう児への人工内耳手術が論争を呼んでいる。人工内耳手術とは内耳に小さな電極を挿入し、音を電気信号に変換、聴神経に直接、電気刺激を伝える不可逆的な手術である。ろう者の組織の多くは各国でろう児への人工内耳手術に反対する運動を繰り広げ、95年の世界ろう者会議は「ろう児に人工内耳手術を勧めない」と決議した。手話を確固たる言語として認識する動きと、ろう文化の主張が背景にある。日本でも93年の「Dプロ」の結成を契機にろう文化運動は上げ潮である。ろう児への人工内耳手術に対しては、(1)現技術レベルの人工内耳は中途半端であり、音声言語、手話言語共に身につかないという批判と、(2)聴者である親が本人の自己決定抜きで、ろう者を聴者に変えようとするのは許されないという倫理的な批判がある。ろう者としての独自の世界があることを、聴者の親に伝える努力が求められている。ろう者自身の組織から、ろう児の親への積極的な情報提供、相談の役割が期待される。

要旨: 1985年に我が国で最初に人工内耳手術が行われてから30年近くになり, すでに7000人以上の方が手術を受けている。人工内耳は高度難聴に悩む方への福音となっている。各人工内耳機器の改良, 適応の拡大などによって, さらに今後手術を受ける方が増加していくものと考えられる。一方, 現在の人工内耳の適応が適切か, 手術後のリハビリテーション施設数, 言語聴覚士の数の問題, 聴覚以外のコミュニケーション手段との関係, 人工内耳機器の取り換え (アップグレード), 人工内耳に関わる費用の問題など検討すべき課題も多い。本稿では表1の各項目に関し, 人工内耳医療の現状とその問題点を, 医学のみでなく, 社会医学的な問題として解説する。

近年,ごみ焼却施設は,公衆衛生の向上,環境保全といった目的だけでなく,地球温暖化防止,資源・エネルギー消費の抑制等,低炭素・循環型社会に果たす役割が強く求められている。本研究は,ごみ焼却施設の低炭素・循環型社会への適合性を評価する手法の開発を目的として,15の焼却施設に対してLCA等の詳細調査を行い,評価指標の検討を行った。その結果,評価指標として,投入されるエネルギー量,CO2排出量,搬出残渣量等を抽出した。また,施設から外部へ供給する電気と熱という質の異なるエネルギーを同じ尺度で評価できる,外部へのエネルギー供給率を指標として提案した。さらに,これらの指標を用いて,発電効率の高い97焼却施設に適用評価し,ベストプラクティスである焼却施設の実態を把握するとともに,ベンチマーキングの基礎情報を得ることができた。また,これらの結果をわかりやすく示すことができるスコアリングおよび表示方法を提示した。

B型,C型慢性肝疾患の治療はめざましい進歩を遂げ,肝炎ウイルスの制御が可能となった.しかしながら,肝硬変,アルコール性・非アルコール性脂肪肝炎(non-alcoholic steatohepatitis:NASH),原発性胆汁性胆管炎や原発性硬化性胆管炎等肝線維化が鍵となる疾患に対する治療はunmet medical needsにとどまっている.これを解決するためには,肝線維化の病態を分子細胞論的に細密に解析し,その情報をもとにした標的治療薬の開発が必須である.肝臓における細胞外マトリックス物質(extracellular matrix materials:ECMs)の産生細胞は星細胞や門脈周囲の線維芽細胞が主体であり,それらが活性化すると筋線維芽細胞(myofibroblast:MFB)として線維化の増幅のみならず,炎症や免疫反応制御,さらには肝癌の微小環境構成の主役となる.従って,肝線維化の治療には,肝細胞障害の阻止と同時に活性化星細胞の機能制御が必要である.近年,線維化誘導分子の解析が進展し,それらをターゲットとした臨床試験が始まっている.

The Road Act and its application will be revised in order for roads function as public property convenient and useful for people. It is beneficial to revise the occupancy permit requirements to recognize the diverse purposes of road space, which cover not only the traffic viewpoint but also the revitalization of communities, as the essential purposes of the road. We should pay attention to the relationship with urban space and city planning.

Steroid 5α-reductase type 2 deficiency (5αRD2) is a congenital disorder of sex development caused by impairment of conversion from testosterone (T) to 5α-dihydrotestosterone (DHT). DHT deficiency leads to various degrees of undervirilized external genitalia including micropenis, primarily correlated with mutations of the SRD5A2 gene that encodes 5α-reductase type 2. Four Japanese boys with isolated micropenis were diagnosed as 5αRD2 by elevated ratios of serum T/DHT, and decreased ratios of urinary 5α/5β-reduced steroid metabolites. Genetic analyses for SRD5A2 identified that the four patients shared a hypomorphic mutation R227Q that has a residual activity related to the mild-form of 5αRD2. For prepubertal micropenis, DHT was transdermally applied to the four patients at the ages of 4–11 year, increasing a median of stretched penile lengths (SPLs) from 2.6 cm (–2.5 SD) to 4.4 cm (–0.2 SD). Nevertheless, the post-pubertal penile growth was apparently retarded, despite normal levels of T secreted from well-developed testes. The second course of DHT treatment underwent at ages of 12–18 year, but unable to normalize SPLs at a range of 6.0 to 7.0 cm (–3.4 to –2.4 SD). The prostate volumes of two patients were variable at 8.1 and 21 cm3, and a sperm cell count of one patient was normal as young adult. DHT treatment contributes to development of the penis and prostate, which are favorable for the potential fertility of 5αRD2 adults. Meanwhile, the retarded penile growth and a risk of prostate overgrowth may complicate the post-pubertal management with DHT for 5αRD2 males.

Using 2-layer model which is the special case of the multiple layer model defined by Charney and Phillips, investigation is made, from the view point of numerical weather prediction, on the role of the tilt of trough relative to the vertical for the development of the pressure pattern. At the same time, the mechanism of the transition between the tilted and the non-tilted trough is discussed. Finally, corresponding to the above development, the energetics of westerly wave, i. e., the energy exchange between the potential and the kinetic energy is investigated. Here, the simple expression for the change of these energies is obtained.

先住民(族)を「異類の他者」として客観的な表象としてのみ扱ってきた学問の倫理的姿勢を、哲学者の植木哲也(2011)氏は「学問の暴力」と呼び、それを厳しく糾弾する。遺骨返還運動の抗議の矛先は自然人類学に向けられているが、このような歴史的な負債を負っているのは、はたしてわれわれのキョウダイ学問だけではあるまい。私は文化人類学もまた、過去の歴史からの反省し学問が倫理的にノーマライズすべきであることを主張する。

ノーベル賞は,大きな賞金の額と,推薦を国際的に集める選考過程に特徴がある。同賞の最初の授賞は1901年に行われたが,この年すでに,日本の医学者,北里柴三郎の名前が候補者の中にあった。ノーベル賞を初めて受賞した日本人は湯川秀樹であったが(物理学賞,1949年),それ以前から同賞と日本の関わりは始まっており,人工癌の発生に成功した山極勝三郎のように,1920年代に受賞に近づいた者もあった。湯川,朝永振一郎(1965年)と続いた理論物理学者の受賞は,20世紀初頭の物理学の変革の産物であるとも解釈できるが,その影響は日本人初の化学賞を受賞した福井謙一の研究にも及んでいると考えられる。

Metallic surgical implants have been widely used in orthopedics and dentistry. This paper reviews the biodegradation of metallic implants and its local and systemic effects on patients. The review discusses typical damage of 316L stainless steel, Co-Cr alloy, commercially pure Ti, and Ti-based alloy implants due to corrosion and wear and factors affecting the biodegradation of these implants. Macrophage mediation of cellular and humoral regulatory pathways in inflammatory and immune responses to metallic ions and wear debris released from the implant is summarized. Implant-related factors influencing the susceptibility to local infection are also discussed.
