千島海溝沿岸域は本邦屈指の地震多発地帯である.この地には未開の原野も多く,手つかずの自然が残されており,400~500年間隔で繰り返し発生した超巨大地震(17世紀型)によって生じた離水地形や津波堆積物が観察できる唯一無二の地域と言える.また北海道東部には偏西風によってもたらされた完新世テフラが1000年オーダーの頻度で挟在し,地震津波イベントの同時性を議論するうえで有用である.津波堆積物の自然露頭は北海道東部太平洋沿岸でも限られるが,この巡検で最初に議論する根室市西端部のガッカラ浜地域には小規模な沿岸湿原が存在し,その太平洋側には湿原堆積物の断面である泥炭層が,高さ約2m程度の海蝕崖が連続して露出している.この露頭では,6層の完新世テフラと過去4000年間に発生した12層の津波堆積物を確認することができる.一方,根室海峡に面する風蓮湖と野付半島には,我が国には珍しい現在も活動的なバリアーシステムが認められている.これらの沿岸地形を特徴づける分岐砂嘴(バリアースピット)の相互の分岐関係と7層の完新世テフラとの対比によって,過去5000年間の地形発達史が解読され,このうち過去3回分の離水(強制的海退)については,超巨大地震(17世紀型)に伴う数mオーダーの広域地殻変動が大きく寄与している可能性が示唆される.

Background: Robot-assisted valve surgery represents the latest development in the field of minimally invasive approaches. Robotic assistance may provide greater visualization, enhanced dexterity, and greater precision than traditional mini-thoracotomy aortic valve replacement.Methods and Results: Aortic valve replacement operations using the da Vinci Xi Surgical System (Intuitive Surgical Inc., Sunnyvale, CA, USA) were performed on 2 patients, 1 with severe aortic insufficiency and the other with aortic stenosis. Both patients had an uneventful postoperative course and were discharged without any adverse events.Conclusions: Robot-assisted assisted aortic valve replacement appears feasible and safe in limited cases.

Background: Recently, the function of high-density lipoprotein (HDL), rather than the HDL cholesterol (HDL-C) level, has been attracting more attention in risk prediction for coronary artery disease (CAD).Methods and Results: Patients with clinically diagnosed familial hypercholesterolemia (FH; n=108; male/female, 51/57) were assessed cross-sectionally. Serum cholesterol uptake capacity (CUC) levels were determined using our original cell-free assay. Linear regression was used to determine associations between CUC and clinical variables, including low-density lipoprotein cholesterol and the carotid plaque score. Multivariable logistic regression analysis was used to test factors associated with the presence of CAD. Among the 108 FH patients, 30 had CAD. CUC levels were significantly lower among patients with than without CAD (median [interquartile range] 119 [92–139] vs. 142 [121–165] arbitrary units [AU]; P=0.0004). In addition, CUC was significantly lower in patients with Achilles tendon thickness ≥9.0 mm than in those without Achilles tendon thickening (133 [110–157] vs. 142 [123–174] AU; P=0.047). Serum CUC levels were negatively correlated with the carotid plaque score (Spearman’s r=0.37; P=0.00018). Serum CUC levels were significantly associated with CAD, after adjusting for other clinical variables (odds ratio=0.86, 95% CI=0.76–0.96, P=0.033), whereas HDL-C was not.Conclusions: HDL function, assessed by serum CUC level, rather than HDL-C level, adds risk stratification information among FH patients.

症例は37歳, 男性. 主訴は意識障害. 既往歴は双極性うつ病で精神科通院中であった. 家族との会話中に突然興奮状態となりベンゾジアゼピン系薬剤を大量服薬したため当院に救急搬送された. 内服した薬剤は, ロヒプノール (1) 60錠, レンドルミン (0.25) 30錠, デパス (0.5) 14錠, レスリン (25) 14錠であった. 来院時所見ではバイタル等は問題なかったが, 入院後経過は服薬後11時間30分後に呼吸抑制が出現したため気管挿管を行った. ベンゾジアゼピン薬剤血中濃度遷延した原因は様々な理由が考えられるが, ベンゾジアゼピンは多く使用されている薬剤であり致死率も低いとされているが, 大量服薬後は血中濃度が遷延するため, 人工呼吸器が必要な症例もあり, 数日間は呼吸状態に注意すべきと考えられた.

In this paper we discuss the effects of Aira-Tn Ash (AT) eruption on ecosystem, based on fossil pollen and diatom assemblages obtained from central and northern Honshu in Japan. This eruption occurred during the Last Glacial Maximum (21, 000-22, 000 or 24, 000 years ago) in the Aira Caldera in southern Kyushu. The fossil pollen accumulation rate and pollen assemblages from AT deposits show that the ash fall occurred in the spring. We conclude that the AT fall occurred during, and accelerated, the last stage of climate change to a colder phase, and of attendant vegetation change to coniferous forests, that began about 30, 000 years ago. The AT fall also caused a rise in level and a change in pH from acid to alkaline of the sedimentary environment water. Some of these phenomena changed in proportion to volume or thickness of ash. It seems that the AT eruption caused widespread dry climatic conditions that were felt, at least to some degree, over long distances.

This study examined the relationship between the actions of piercing, cosplay, and self-mutilation, and self-image. The subjects were 157 female university students majoring in the field of clothing and fashion. The data was analyzed by factor analysis, a t test, and ANOVA. It was hypothesized that there would be differences between different groups of subjects:(1)those with piercings and those without;(2)those who had experienced cosplay and those who hadn't; and(3)those who had performed self-mutilation and those who hadn't, not only on the impressions others had of them, but on the impressions they had of themselves. The results showed that the hypotheses were valid and that the three actions of piercing, cosplay, and self-mutilation were related to one's self-image. Furthermore, the results revealed that there were differences among the three actions. Having piercings or not may have a “reverse impression" effect; that is to say, the impression one has of herself may be the opposite of the impression others have of her. Having experienced cosplay may result in having both a positive impression of oneself and being looked upon positively by others. Those who experienced self-mutilation tend to be seen negatively by others and have a poor self-image. As a result, it was clarified that these three alterations to one's appearance related to the way students saw and evaluated themselves and how they saw and evaluated others.

Background: Task-specific dystonia (TSD) confined to the lower extremities (LE) is relatively rare. This report describes dystonia confined to the LE only during forward walking. This case required careful neurological and diagnostic assessment because the patient was taking several neuropsychiatric drugs that cause symptomatic dystonia, such as aripiprazole (ARP).Case: A 53-year-old man visited our university hospital with a complaint of abnormalities in the LE that appeared only during walking. Neurological examinations other than walking were normal. Brain magnetic resonance imaging revealed meningioma in the right sphenoid ridge. The patient had been treated for depression with neuropsychiatric medications for a long time, and his abnormal gait appeared about 2 years after additional administration of ARP. After the meningioma was removed, his symptoms remained. Surface electromyography showed dystonia in both LE during forward walking, although his abnormal gait appeared to be accompanied by spasticity. The patient was tentatively diagnosed with tardive dystonia (TD). Although dystonia did not disappear clinically, it was alleviated after discontinuing ARP. Administration of trihexyphenidyl hydrochloride and concomitant rehabilitation improved his dystonia until return to work, but some residual gait abnormalities remained.Discussion: We report an unusual case of TD with task specificity confined to the LE. The TD was induced by the administration of ARP in combination with multiple psychotropic medications. Careful consideration was required for clinical diagnosis, rehabilitation, and assessment of its relevance to TSD.

The present study investigates the association between social skills and indicators of school adjustment: school satisfaction, loneliness, self-esteem, depression, and academic grades. A total of 1,042 junior high school students (boys = 513, girls = 529, Mage = 13.63, range = 12–15) from 33 classes in three schools participated in a questionnaire survey. The results of correlation analysis with multilevel modeling and Zou’s test revealed that (a) hairyo skills, consisting of compassion for others and maintaining relationships with peers, were positively correlated with academic grades, and (b) kakawari skills, consisting of assertion and developing relationships with peers, were positively correlated with school satisfaction and self-esteem. Both types of social skills were negatively correlated with a sense of victimization and depression. These results show multiple aspects of the effects of social skills toward school adjustment problems.

我々が住んでいる地球は大きな磁性体であり, 微生物ばかりでなく高等動物も大なり小なり磁場の影響を受けているが, 磁場が目に見えないエネルギーであるので気付ずに生活または生きている。ここでは微生物に対する磁場の影響, 新しい高磁場発生装置の開発, 今後の磁性の利用見通しと現在の研究現状を解説していただいた。
