腸内細菌はNASH病態の1st-hitである単純性脂肪肝において,リポ蛋白リパーゼの活性化などを介して肝臓や脂肪組織に脂肪蓄積を増加させ,脂肪肝の形成に重要な役割を果たす.また,NASH病態の2nd-hitである肝炎の病態形成では腸内細菌は,1)腸内細菌の質的量的異常,2)腸管透過性亢進,3)肝臓におけるエンドトキシン応答性亢進,の3つの機序で重要な役割を果たすと考えられている.治療においては種々のプロバイオティクス,プレバイオティクスの投与が有効であることが臨床試験で示されている.今後はNASH病態に強く関与する腸内細菌の同定と治療への応用が求められている.

From our previous reports, it is clarified that the fluctuations in the jet center have an important effect on opposed turbulent jet flow fields. In the jet center, where symmetrical flow fields are characterized, the turbulent structure will be different from those in other parts of the jet. In the turbulent opposed jet center, the fluctuating velocity plays role equal in importance to that of the mean velocity. The momentum which locates the impinging surface comprises three parts: that is, a mean velocity, a static pressure and a fluctuating velocity. The present paper deals with the influences of the variations of the fluctuating velocity, employing the turbulent lattice in the center of turbulent opposed jets, to the locations of the impinging surface.

今回,われわれは犬の歯科X線検査における二等分面法の新たなX線入射角度決定法を考案した.この方法では,フィルム面の口腔外への延長線と撮影対象歯の歯軸で成す角を二等分する角度でX線を照射する.この新しい方法と従来から用いられてきた基本的な方法,及び近年提案された別の方法の3つの方法を用いて,頭蓋及び模擬フィルムで作成したモデルにおける入射角度決定の検証を実施した.作業開始から入射角度決定までの時間を簡易性,得られた入射角度による画像長変化率を正確性,及びそのばらつきを精度の指標とし,それぞれの初回実施時の傾向及び習熟の関与について比較検証を行った.われわれの考案した方法は角度決定時間が最も短く,得られた角度は他法と同等で,ばらつきも少なかった.このことから初心者にも理解のしやすく応用しやすい方法と考えられた.

This paper proposes a new feedback-type dead-time compensation method. The proposed method compensates the output voltage by employing two operations. First, the normal compensation matches the width of the output pulse to that of the input pulse with reduced delay time from the input pulse to the output pulse compared with the conventional method. Second, the short pulse compensation generates an output pulse after a few pulses are input so that the average value of the output signal equals that of the input signal. The voltage utilization factor of the proposed method is almost 100%, because there is no limit to the compensation and the decrease in the fundamental output caused by dead time is eliminated. The experimental results using a 100-kHz input signal show that there is no discrepancy between the output pulse width and the input pulse width. Furthermore, the THD of the compensated output current, when the output frequency is 50Hz, is eliminated, compared with that of the output current without compensation. These results show that the proposed method has low distortion and high voltage utilization factor characteristics.

In the course of our search for new antitumor agents from microorganisms, the fungus Acremonium sp. KY4917 was found to produce novel compounds. Two novel compounds, UCS1025A (1) and B (2), were isolated from the culture broth by chromatographic methods. Molecular formulae of 1 and 2 were determined to be C_<20>H_<25>NO_5 and C_<20>H_<25>NO_6 from HRFAB-MS data, respectively. Structures of 1 and 2 were elucidated on the basis of spectroscopic methods, mainly by detailed analyses of their NMR spectra. Absolute stereochemistry of 1 was established by an X-ray crystallographic analysis of the 3'-bromo derivative 3'. As a result of spectroscopic analyses and chemical transformations of 1, the unique chemical equilibrium of 1 was found out. Three tautomeric isomers of 1 have been identified to be ketone 1a in CDCl_3 by NMR analyses, enol 1b by an X-ray crystallographic analysis, and enedione 1c in an aqueous buffered solution, respectively. UCS1025A (1) exhibited antimicrobial activity against Gram-positive bacteria, Staphylococcus anreus, Bacillus subtilis and Enterococcus hirae, and Gram-negative bacterium, Proteus vulgaris with the MIC values ranged from 1.3μg/mL to 5.2 μg/mL, and antiproliferative activity against human tumor cell lines (A431 and MCF-7) with the IC_<50> values 55 μM and 21 μM, respectively. In contrast, UCS1025B (2) showed weak antimicrobial and no antiproliferative activity.

In the past, when boiling spinach, it is suggested that one should use boiling water. It is said that this removes the oxalic acid ingredient and other ingredients with a non-taste, and improves the color. In this study the temperature of the boiling water and the influence of the boiling time were evaluated.1) The oxalic acid content decreased when the temperature of the boiling water was high and when the boiling time was long.2) There was no difference in the color of the boiled spinach.3) Even if the boiling time was long at 60 and 70°C, the oxalic acid content and the change in its strength were low and the heating was insufficient.4) At 80°C with a sufficient boiling time, a moderate condition was attained. For the oxalic acid content and softness evaluation, heating was from 2 to 3 minutes at 100°C which was considered moderate in the past.5) At 100°C, the range in the boiling time which can make spinach have a moderate condition was short. From 80 to 90°C, the time period was long. When boiling spinach is always kept in the hot water, boiling is not necessary and it is possible to say that the heating power should be maintained to provide an 80°C temperature or higher.

大学生における食習慣に影響を与える要因を検討する目的で,現在の食生活,食習慣の変化,食に関する体験について,フォーカスグループによる振り返り調査を行った.対象者は18歳から21歳の文化系または,運動系のサークルに所属している大学生9人(男2人,女7人)であった.大学生の食習慣に影響を与える要因として,経済状況,調理器具などの料理をする環境,料理に費やす時間や手間,生活スタイルの変化,嗜好の変化,運動部に入ったことによる影響が抽出された.さらに,幼少時からの家庭環境も大学生の食習慣に影響を与える要因の一つであった.また,家庭や学校の授業での料理経験は,大学生になっても食の体験として記憶に残っていた.大学生の食習慣の形成には,経済状況,生活スタイルの変化,所属サークルなどの現在の生活状況からの要因と,これまでの食に関する体験が影響していることが示唆された.

分娩後における乳清及び血清中免疫グロブリンG(IgG)濃度、豚萎縮性鼻炎(AR)、豚丹毒(SE)凝集抗体価の経時的推移を、母豚24頭を用いて調査した。乳清中IgGは、分娩0日が最も高く、42.5mg/mlと血清IgGの約3倍の値を示した。この後16日までは血清と同レベルで推移し、以降低下し20日で血清の半分になった。乳清中のAR抗体およびSE抗体は、それぞれ分娩後20日、分娩後10日まで検出可能であった。豚オーエスキー病ラテックス凝集抗体を、ワクチン接種豚の乳清38検体、未接種豚の乳清23検体を用いて調査した。ワクチン接種豚の乳清中抗体は分娩0日の9検体全てが陽性を示し、10日後の2検体も陽性を示した。酵素抗体法(ELISA)検査では、凝集抗体陽性の検体は全例ワクチン抗体と判断された。未接種豚の乳清23検体は全て抗体陰性であった。なお、野外抗体陽性6例の検査では、乳清中の抗体は血清と同じく野外抗体と判断された。<BR>初乳は血清と同様の抗体検査が可能であり、初乳を用いた抗体検査は繁殖母豚群の抗体スクリーニングや疾病清浄化に向けて有効な方法と考えられた。

短期間で幽門前庭部狭窄が増悪し,進行胃癌と鑑別に苦慮した胃十二指腸潰瘍の1例を経験したので報告する.症例は71歳男性で,頭部外傷の既往があり常時頭痛があるため近医より投薬を受けていた. 1989年10月16日吐血し当院入院となった.上部消化管検査にてBorrmann 4型の進行胃癌を疑診したが,生検結果で悪性所見を認めなかった.幽門前庭部狭窄が著明に進行したため11月22日幽門側胃切除,十二指腸切除を施行した.切除標本では胃体下部小弯,前後壁に三条の巨大帯状潰瘍(Ul-II),その肛門側に十二指腸球部にまで及ぶ長さ7cmの全周性狭窄部を認めた.病理組織像では粘膜の軽度の炎症所見と粘膜下層における膠原線維の増生,更に全周性狭窄部では固有筋層の著明な肥厚を認めた.幽門前庭部狭窄は慢性炎症の繰り返しによるものと考えられた.増悪の誘因として,薬剤,循環障害が考えられた.

40歳代,女性。幼少時期よりアトピー性皮膚炎と診断され加療を受けてきたが,最近では治療を自己中断していた。2015年春頃より全身のそう痒が増悪したため近医より当科に紹介受診となり,精査加療目的に入院した。季節の変わり目に皮疹が悪化するとの訴えがあり,うつ熱症状もあったため発汗機能異常を疑い全身温熱発汗テストを施行したところ顔面や体幹部の発汗低下を認めた。齲歯も複数あり,外分泌腺機能低下を疑い精査を進めたところ,抗 SS-A,SS-B 抗体陽性であり小唾液腺の導管に多数の細胞浸潤を認めシェーグレン症候群との診断に至った。アトピー性皮膚炎に対して外用治療を開始したところ,皮膚所見や好酸球の減少とは逆に肝逸脱酵素の数値が上昇した。シェーグレン症候群合併例のため,他の自己免疫疾患の併発についても精査したところ原発性胆汁性肝硬変が判明した。アトピー性皮膚炎では一般的に発汗機能異常が認められるが,発汗低下が著明な場合はシェーグレン症候群の合併も考慮に入れる必要があると考えられた。またアトピー性皮膚炎治療中に原発性胆汁性肝硬変に関連した肝逸脱酵素の数値が上昇した原因につき,Th1/Th2/Th17 バランスを含めて考察した。(皮膚の科学,15: 51-56, 2016)

<p>PD-1/PD-L1 immune checkpoint pathway, which maintains self-tolerance and limits collateral tissue damage in a physiological state, can be co-opted by cancer cells to evade immune surveillance. Immune checkpoint inhibitors, such as anti-PD-1 and anti-PD-L1 antibodies, can unleash anti-tumor immunity and mediate durable cancer regression. However, the complex biology of this pathway has not been fully elucidated, including the genetic basis of PD-L1 upregulation in cancer. Recently, we have identified a novel genetic mechanism of immune evasion associated with structural variations (SVs) disrupting the 3'-untranlated region (UTR) of the <i>PD-L1</i> gene in a variety of cancers. Despite a large diversity in SV type, these SVs are invariably associated with a marked increase of PD-L1 expression, which promotes tumor progression and immune escape. Here we present an overview of PD-1/PD-L1 immune checkpoint blockade therapy, and highlight the genetic mechanisms of PD-L1 activation, including copy number amplification, promoter replacement, as well as 3'-UTR disruption, with a special focus on their relevance as a biomarker.</p>

平成11、12年度に県内で生産されている31種類の地域農産物について、アイスクリームヘの加工適性を検討し、越生町特産の梅ジャム、ユズ、岡部町の味来トウモロコシのアイスクリームが商品化された。平成13年度には、地域農産物15種類について、豚肉ソーセージヘの加工適性を検討し、キヤッセ羽生のモロヘイヤソーセージが商品化された。平成15、16年度に商品性を高めた加工食品の開発をテーマとした取り組みでは、高品質肉用鶏タマシャモでソーセージ、燻製、プレスハムなど、新ブランド「愛彩豚」ネギ入りソーセージ、県内産大豆を使用した豆乳、豆腐アイスクリームなどを試作し、商品化を目指している。

反回神経麻痺による嗄声は,副甲状腺機能障害と並び,甲状腺摘出後に生じる主要な合併症の1つとなっている。片側反回神経麻痺により声帯の内外転運動が障害されると,発声時の声門のスリット形成が妨げられ,ベルヌーイ力と声門下圧の交互作用による声帯振動が生じにくくなる。甲状腺摘出後の反回神経麻痺の頻度は,一過性および永続的なものを含めて1~13.3%と報告されており,再手術,悪性腫瘍,頸部郭清の併施,反回神経麻痺を同定しない手術などがリスク因子とされている。反回神経麻痺による嗄声の治療に関して,麻痺が一過性である可能性があるために,発症後6カ月以内の姑息的治療とそれ以降の永久的な治療がある。姑息的治療として,音声治療および局所麻酔下声帯内注入術の有効性が報告されている。しかしながら,局所麻酔下声帯内注入術については,本邦の医療保険の事情からあまり行われていないのが現状である。永久的な治療法として,甲状腺手術後の頸部の瘢痕形成のために通常の喉頭形成手術の施行に難渋するため,甲状軟骨翼状板外側下方を開窓して行う披裂軟骨内転術,神経吻合術,神経筋移植術などの術式が選択されることがある。
