“At-Risk Mental State, ARMS(精神病発症危険状態)”とは、精神病発病へのリスクの高い状態を意味する。一見軽症で、注意深く診察しないと一般的なうつ状態などと鑑別できない。精神科診療所へはうつ状態や不眠、不安などを主訴とする患者が数多く受診している。ARMSの具体的な診療について、国内に6000を超えるとされる精神科診療所で共通の認識を得ているとは言いがたい。そこで、銀座泰明クリニックを受診したARMS 3症例、診断基準は満たさないものの可能性ある2症例を提示して考察した。精神科診療所は精神医療の「ゲートキーパー」としてARMS診療、早期発見・早期治療に寄与できる。夜間・土日、駅前・街中など、いつでもどこでも気軽に受診できるのは診療所の強みであろう。高次医療や救急医療を求められる場合は大学病院や精神科病院へ紹介し、良好な連携を取っていくことが望まれる。そのためには、日ごろから大学病院や精神科病院との「病診連携」を高めるため、診療所の医師が学会や研究会などへ積極的に参加し、お互い顔の見える関係を構築・維持することが望まれる。最後に「精神科医」として最も大事なことは「診断」という「ラベリング」ではなく、苦痛を訴える患者に寄り添い、できる限り「援助」を提供していこうとする、「治療者」の「マインド」である。

本年度は、前年度にまとめきれなかった論文執筆に取り組み、認知言語学の範囲でできる研究をまとめた。日本の手話研究は、先行研究が乏しく、手法も確立しておらず、手話話者と研究者の関係も乏しい状態であったことから、海外で先行している研究をまとめ、啓蒙的な論文を書くことが重大なミッションの1つであることが徐々に明らかとなり今年度はこれに取り組んだ。認知言語学の観点からの手話研究のレビューとして、『認知言語学大事典』に「手話と認知言語学」という項目を執筆した。また、手話言語の社会言語学的な状況と言語記述の必要を示した論文を、国立国語研究所論集に「危機言語としての日本手話」として出版した。さらに昨今のろう・難聴児を取り巻く急速な政策の展開を踏まえ、言語政策誌に「人工内耳時代の言語権―ろう・難聴児の言語剥奪を防ぐには」という共著論文を出版した。これらは日本手話が社会とどのように関わっているか、そして研究や手話話者への支援が、国外の状況・研究成果を踏まえたうえでどのように解釈できるかについて論じたものである。手話研究の分野はアメリカが先行しているが、アメリカでも著名な研究者が来日する機会があったので、国際的な研究業界の状況とその基礎についての講演会・ワークショップをコーディネートして、手話研究の重要性とその具体的な内容について、手話通訳をつけたうえで、手話話者と研究者に国立国語研究所と国立民族学博物館で開催した。フィールドへの還元を確保する一方、国際認知言語学会での発表や手話言語学最大の国際学会であるTISLRでの発表を行い、国際的な手話研究の土俵に日本手話のデータを上げることができた。前者は因果推論の構文の分析で、後者は語彙の構音的なネットワークを否定とメタファーの観点からまとめたものであり、どちらも認知言語学というフレームワークが手話研究でどのように生かせるかを示すものとなった。

Distigmine bromide is an anticholinesterase used to treat urinary retention. We describe a case of decreased serum cholinesterase caused by a usual oral dose of distigmine bromide for urinary retention. An 84-year-old man, who had been received chemotherapy for carcinoma of the lower gingiva, suddenly decreased serum cholinesterase to 21 IU/L in 7 days of administration of distigmine bromide 5mg without clinical symptoms. Serum cholinesterase improved after stopping the administration of distigmine bromide. This case shows that oral surgeons should be aware of the possibility of decreased serum cholinesterase in patients taking distigmine bromide.

The Seasonal Agricultural Workers Scheme (SAWS) in the UK has been designed to allow farmers to recruit low-skilled overseas workers to undertake short-term agricultural work. Before 2006, the unique feature of SAWS was that the workers were full-time students pursuing higher education in areas such as agriculture or the English language in their home countries. This indicated that their work experience in the farms in the UK would provide them with significant educational benefits. SAWS changed the eligibility for applicants in 2006 and now specifies that applicants must be either Bulgarians or Romanians but need not necessarily be students. In the present study, interview surveys were conducted on two SAWS operators and these surveys revealed that the quality of the workers has deteriorated after 2006 because of their age and knowledge.By comparing the former SAWS with the revised one, this study recommends that, in the future, the Japanese government draw upon the former SAWS while considering the regulations governing foreign workers.

これまで, 耳小骨筋反射がCMや中耳インピーダンスに及ぼす影響は報告されているが, 耳小骨振動に及ぼす影響については明らかにされていない。 そこで, 我々は, 耳小骨各部位の振動速度を直接計測することが可能なレーザドップラ振動計を用いて, モルモットおよびウサギの対側刺激による耳小骨筋収縮前後での, 耳小骨の振動挙動変化を計測した。 そして, 刺激音圧の増大に伴い, 耳小骨振動振幅が減少し, また, 耳小骨振動の周波数が低いほど, 振幅の減少量が大きいことを明らかにした。 さらに, 反射前後で, 耳小骨の振動様式に生じる変化を推察した。

単層カーボンナノチューブの表面に窒化ホウ素ナノチューブやMoS2ナノチューブを化学気相成長することで,異種ナノチューブの同心複合構造であるヘテロナノチューブの創出が実現した.2次元物質におけるファンデルワールスヘテロ構造の研究が近年盛んであるが,本成果は擬1次元的なナノチューブにおいて同様の自在な物質設計の可能性を開くものであり,ナノチューブ構造に特有の新たな物性の発現やデバイス応用が期待される.本稿では,ヘテロナノチューブの合成と構造評価,およびその物性計測や応用について紹介する.

遷移金属錯体は,中心金属イオン周りの配位子場を調整することで,その酸化還元特性やd電子スピン軌道を変えうる.その特徴を基に,酸化還元活性架橋配位子と連結することで,d-p軌道が共役した電荷移動型の錯体多次元格子,いわゆる,電子ドナー(D)と電子アクセプタ(A)の交互格子を構築できる.このようなDmAn格子でも,D部位のイオン化ポテンシャルとA部位の電子親和力と電子対反発エネルギーを分子・配位子修飾による可変数として捉えることで格子内電荷移動を制御することが可能である.同時に,格子で囲まれた“空間”を利用することで,ゲストの脱挿入による構造変調や格子‐ゲスト相互作用をトリガーとする動的な電荷移動を誘起できる.本稿では,主に電荷移動型錯体格子の電子・スピン制御について,格子上の電荷移動設計と空間利用という観点から解説する.

The strategic justification for supporting scientific activities has largely depending on the understanding that research provided both national security in a time of global hostility involving the improvement of the economy, greater social equity, better health, better race relations, and enhancement of environmental quality, and widespread health benefits. In terms of curriculum in the Earth Sciences, the important point here is that study courses must evolve to accommodate the changing, enlarging body of knowledge as well as the ultimate career needs of the future degree holders. Growing subdisciplines needed in America include hydrology, hydrogeology, neotectonics, continental margin oceanography, engineering geology, geologic hazards, environmental science, aqueous geochemistry, and material sciences. Metamorphic petrologists are especially well-equipped with the fundamental knowledge from physics and chemistry as well as geology to treat the problems society wishes to address and solve. The situation provides us with a natural advantage, but to realize it, we must move our emphasis away from the traditional topics of metamorphic petrology.

An 85-year-old woman presented with ataxia and deterioration of cognitive functions. She had no history of autoimmune diseases or viral infections. Magnetic resonance imaging showed a solitary mass lesion at the cerebral falx on contrast-enhanced T1-weighted imaging. Gross total resection of the lesion involving the dura mater was performed by bifrontal craniotomy. Histological examination showed diffuse infiltration of small lymphocytes and plasma cells. There was also some proliferation of large lymphocytes with folded nuclei, high-density chromatin, and inconspicuous nucleoli. The large atypical B lymphocytes did not demonstrate diffuse dense sheet findings. Meningothelial components were not detected. Immunohistochemistry was positive for pan B-cell antigens. The analysis of the kappa/lambda ratio indicated kappa immunoglobulin light chain-restricted B-cell proliferation. The final histopathological diagnosis was mucosa-associated lymphoid tissue lymphoma. Systemic screening examinations were then performed. Histological findings of the bone marrow showed normal findings without atypical lymphocytes. A chromosomal study of the bone marrow showed 46, XX. 18F fluoro-2-deoxyglucose positron emission tomography showed high accumulations at the left pterygoid muscle and the right transverse processes of the thoracic vertebrae, and mild accumulation at the right ilium bone, which indicated disseminated lesions. One year later, thickening of the dura mater was detected. Therefore, gamma knife surgery was performed. Two years later, she was alive without neurological deterioration, and magnetic resonance imaging showed no evidence of recurrence.
