本プロジェクトでは、人が音の空間性を認識するときの脳活動に着目し、基礎研究を行なうとともに、このような空間性を有する音の聴取時に観察される特徴ある脳活動を利用したブレインマシンインタフェースの開発を行なった。今年度は、空間聴覚刺激に対する事象関連電位の統計的特徴に基づいた電極と潜時の選択手法を提案し、識別率を向上させた。音の出力法の試みでは、スピーカによる実音源と仮想音源でP300反応に個人差があること、視覚刺激より振幅が小さいこと、頭部伝達関数を利用した空間聴覚刺激が後頭部にP300を誘発させることを確認した。

Intractable nausea and vertigo induced by opioid treatment are occasionally difficult to treat. It has been reported that antiemetic drugs and opioid rotation may be effective in treating nausea in such cases; however, this approach has been occasionally ineffective. Symptomatic treatment has not been developed for vertigo induced by opioid treatment. Here, we report a case study where combined treatment with perospirone and a histamine H1 receptor antagonist was used in 2 patients who developed intractable nausea and vertigo induced by opioid treatment. Treatment with a histamine H1 receptor antagonist drug (tablet form, containing 40 mg diphenhydramine salicylate and 26 mg diprophylline) suppressed the nausea and vertigo. However, increasing the opioid dosage exacerbated the symptoms, and treatment involving the histamine H1 receptor antagonist and opioid rotation was ineffective. Subsequently, combination treatment with the histamine H1 receptor antagonist (3 tablets per day) and perospirone (maximum daily dose, 16 mg) improved the symptoms. The results of the present study suggest that combination treatment with a histamine H1 receptor antagonist and perospirone might improve intractable nausea and vertigo induced by opioid treatment.

In order to expand the use of renewable energy, a shift from "selling" to "self-consuming" of surplus electric energy generated by photovoltaic power generation (PV) is necessary. As for the means of the self-consumption, water heater with a hot water tank, which can store energy at relatively low cost as hot water, is attracting attention. Hybrid water heater, that has both an electric heat pump and a gas heat source, stores hot water to a relatively small tank compared to a simple electric heat pump water heater at less electric energy. Since it also has a gas heat source, there is no need to worry about running out of hot water even with a small tank. By combining a hybrid water heater and a storage battery system, an improvement of self power consumption ratio of PV energy is expected compared to the stand-alone usage of a hybrid water heater. This article reports the improvement result in an actual house. The self-consumption of PV energy used at the hybrid water heater was improved by 11.6 % in winter, 23.8 % in mid seasons, and 4.9 % in summer. Then the CO2 emission is reduced by 33 % a year.

We encountered a case of hyoid bone elongation characterized by a clicking sound on swallowing. A 32-year-old man complained of a foreign body sensation and painless clicking sound on the left side of his neck when swallowing. CT scan of the neck revealed that the left greater cornu of the hyoid bone was elongated. Both greater cornu were surgically removed, and his symptoms disappeared.

腫瘍の生物学の研究の進展があれば, その診断法や治療法の改良・進歩が期待される.アポトーシス研究の進展に伴い, 放射線治療の基礎として放射線誘発アポトーシスに対する関心が高まるのは当然である.本稿では, まず, 放射線細胞死研究と, アポトーシス発見とその基本概念の確立との関係, さらに, p53, bcl-2などアポトーシス関連遺伝子の機能と放射線感受性との関係など, 最新の知見を紹介する.最近, DNA損傷に加えて, 放射線細胞死の標的としての膜損傷が, 放射線誘発アポトーシスの分子機構ならびに細胞内情報伝達の研究の進歩に伴い, クローズアップされてきた.さらに, アポトーシス実行過程の引き金となるタンパク質分解の役割と関連遺伝子, 放射線障害・リスクの軽減におけるアポトーシスの役割も注目されている.最後に, 放射線誘発アポトーシス研究において発見された増殖死型アポトーシスの意義について言及する.

【目的】緩和ケアに携わる医療提供者のターミナルケア態度の実態とそれに関連する要因を明らかにすることを目的とした.【方法】がん治療医と緩和ケア医を含む緩和ケアに携わる医療提供者を対象にFrommelt Attitude Toward Care Of Dying Scale Form B日本語版(FATCOD B-J)を用いて質問紙調査を実施した.【結果】解析対象は223例であった(回収率42.2%).FATCOD B-J総得点を目的変数とした重回帰分析の結果,偏回帰係数は年代で40代と比較して30代以下が低く(−3.8),業務から得られる満足感を感じているほうが高く(+5.7),緩和ケアへの関心が強いほうが高かった(+6.2).【考察】緩和ケアへの関心と業務から得られる満足感がターミナルケア態度の涵養に重要である可能性がある.

定額制サービスを提供しているオンラインストアが収益をあげるためには,ユーザの契約期間をできるだけ延ばすことが必要である.従来レコメンド法では,購入される確率を最大化するためにユーザの嗜好に合致する商品を提示する.しかしながら,従来法により必ずしも契約期間が延びるとは限らない.本研究では,定額制サービスを想定し,契約期間が延びる確率を最大にするレコメンド法を提案する.提案法では,まず契約期間の長いユーザに特徴的な購買パターンを抽出する.そして,抽出されたパターンと同じような購買行動になるように商品をレコメンドする.生存時間解析を応用し,ログデータから効率的に購買パターンの抽出を行う.また,効果的なレコメンドにするため,最大エントロピーモデルを用いてユーザの嗜好を推定する.契約期間が延びることはユーザがサービスに満足した結果であるため,提案法はオンラインストアだけでなく,ユーザにとっても好ましいレコメンドである.携帯電話用漫画配信サイトのログを用い,提案法の有効性を示す.Online stores providing subscription services need to extend user subscription periods as long as possible to increase their profits. Conventional recommendation methods recommend items that best coincide with user's interests to maximize the purchase probability, which does not necessarily contribute to extend subscription periods. We present a novel recommendation method for subscription services that maximizes the probability of the subscription period being extended. Our method finds frequent purchase patterns in the long subscription period users, and recommends items for a new user to simulate the found patterns. Using survival analysis techniques, we efficiently extract information from the log data for finding the patterns. Furthermore, we infer user's interests from purchase histories based on maximum entropy models, and use the interests to improve the recommendations. Since a longer subscription period is the result of greater user satisfaction, our method benefits users as well as online stores. We evaluate our method using the real log data of an online cartoon distribution service for cell-phone.

Than doing the actual experiment, in order to evaluate the safety of passenger flow and passenger evaluation station, the consideration by the simulation model is realistic. System for safety evaluation of the station in the event of failure by utilizing simulation technology, in this study, we have examined the specifications of sub-models that are required there. In addition, conduct a case study that assumes a situation where using a part of the sub-model for the models studied, underground station is flooded due to the tsunami and heavy rain guerrilla, we have confirmed the effectiveness of the system.

全IP(Internet Protocol)技術を特徴の一つとする次世代ネットワーク(NGN:Next Generation Network)における音サービスの主たる品質要因は,時間連続的な符号化ひずみ,時間離散的なパケット損ひずみ,及び信号の周波数帯域制限ひずみである.筆者らは,これまで楽音信号に対して,連続ひずみ,離散ひずみ,帯域制限ひずみを考慮した客観品質評価法を検討してきた.本稿では,同じ構造の客観品質推定尺度を用いて,音声信号の客観品質推定について検討し,楽音と音声の双方に適用できる客観品質評価尺度の開発を目指す.

本研究では、震災直後を想定し、地域住民と自治体との協働による速やかな被害情報の収集・共有を可能とする体制作りを行い、「まちなか発災対応型訓練」(町内に被災状況を模擬的に構築して行なう発災対応型訓練)を活用した震災対応力の向上と同時に、住民・自治体間の情報共有を可能とする防災訓練を実施した。協力頂いたのは愛知県豊橋市であり、住民・市職員による協働体制を構築するために、地域点検マップを作成する防災ワークショップと防災訓練を行なう活動を2005~2006年に実施した。地域点検マップによって地域の地震防災上の現況を把握し、実状に即した発災対応型の防災訓練を企画した。さらに防災訓練では、まず住民による「まちなか発災対応型訓練」を行い、その後で校区の避難所を拠点として地域被災マップを作成し、市の災害対策本部へ速やかに伝送した。一方、対策本部では市全域の被災像を把握し、延焼・避難・交通シミュレーション結果などから住民へ避難勧告の発令など、重要な情報を市から住民に伝達する訓練を行った。さらに自治体担当者を主とする訓練参加者にアンケート及びヒアリング調査を実施し、協働体制および訓練の有効性と今後の課題を確認した。

本稿では,関係データ解析への応用を目的として,無限サイズを持つ行列の長方形分割を行う確率過程について議論する.関係データ解析法の一つとして、与えられたデータを行列として表現し、その行列を少数の長方形クラスタに分割する手法が広く利用されている。長方形分割を表す確率的生成モデルとして従来Chinese restaurant processの積やMondrian processなどが用いられてきたが,これらは限られたクラスの長方形分割しか表現することが出来なかった.より一般に任意の長方形分割を生成しうる確率モデルとしてGilbert tessellationが知られているが,これは統計的な振る舞いの解析が困難であることが知られている.そこで本稿では,有限確率モデルの無限拡張によって長方形分割のための確率過程を構成する方法を提案する.はじめに,確率過程構成の常套手段であるKomogorovの拡張定理を用いた方法を示し,その問題点を明らかにした後,より洗練された構成法として有限のベイズ階層モデルに関する射影系をOrbanzの拡張定理によって無限拡張する方法を提案する.

重度発達障害児の夜泣き, 興奮, 暴力などの対応には療育面での工夫が必要なことはもちろんであるが, 特に夜間は人手の問題もあつておもうにまかせない. 抗痙攣剤を必要とする児が大部分であり, トランキライザーの併用は, 副作用をさらに増やすおそれもある. そこで漢方エキス剤を用い, 一応満足すべき状態を得ている. 今回夜泣きには甘麦大棗湯・抑肝散を, 興奮・暴力のある女児に加味逍遥散を用いた. 障害児の随証療法は正しく行うことが困難なので, 方剤の構成生薬の薬理作用と, 先人の経験の踏襲で方剤を選択したが, 投与に難渋することなく, 副作用もみられず, 一応の成果がえられた. 漢方の特性を生かせる分野として今後も広く活用されるべきものとおもわれるので報告した.

要旨: 秋田県で新生児聴覚スクリーニング (Newborn hearing screening: 以下 NHS と略) が開始されてから17年が経過し, 2012年からは受検率が94%を超えている。 NHS システムを構築する中で, NHS 後も聴こえに関心を持ち続けるための啓発や, 関係機関との連携ができ, NHS 後に難聴児が発見された場合の対応も確立されたと考えられる。 今回の調査で, 聴力型により NHS では発見できない難聴児がいること。 遅発性や進行性の難聴児がかなりの数存在することが示唆され, NHS パス後も引き続き聴覚に気を配り, 関係機関との連携を強化していく必要性が示唆された。 今回画像診断が不完全な症例が含まれたことや, 今後遺伝子診断をする例の増加が予測されること, 先天性サイトメガロウィルス感染症のフォローアップ例が増えている事などから, 今後は遅発性及び進行性難聴の原因について明確にしていく必要性があると考えた。

Linezolid (LZD), an antimicrobial agent against methicillin-resistant Staphylococcus aureus, demonstrates good bone and joint penetration, and is used for prosthetic bone and joint infections. Recently, we observed vomiting in several patients administered LZD. However, there are few reports on the incidence rate of, and risk factors for, LZD-induced nausea and vomiting. In this study, we aimed to verify the relationship between LZD administration and vomiting. Patients administered LZD at the Department of Orthopedic Surgery of Hokkaido University Hospital between November 2008 and December 2017 were enrolled in the study. The primary endpoint was the comparison of the vomiting rate between patients administered LZD (LZD group) and those administered other antibiotics (non-LZD group). For the secondary endpoint, to verify the risk factors of vomiting, a univariate logistic regression analysis was performed. In total, 130 patients were included in this study; 77 patients in the LZD group, and 53 in the non-LZD group. Vomiting occurred in 18 patients in the LZD group and 4 patients in the non-LZD group (23.4% and 7.5%, respectively); this was significantly higher in the LZD group. In the univariate logistic regression analysis, LZD administration, gender (female), age ≥65 years, renal impairment (creatinine clearance <60 mL/min) and concomitant use of rifampicin were extracted as potential risk factors of vomiting. The results of this study reveal a possible relationship between LZD administration and vomiting.

本稿では,SLP雑音下音声認識評価ワーキンググループの活動成果として,雑音 下音声認識評価用共通データベースAURORA-2Jと,その標準評価スクリプトによるベースライン評価結果について述べる.AURORA-2Jは,AURORAプロジェクトの AURORA-2データベースの日本語版として設計され,標準評価スクリプトも AURORA-2で配布されているスクリプトをベースとして開発されている.この共通 評価フレームワークにより,各機関における雑音環境下音声認識手法の性能を容 易に比較することが可能となり,雑音環境下音声認識手法の発展を促すことがで きると考えられる.また,自動車内における数字/コマンド発声データベースで あるAURORA-3Jの開発進捗状況についても述べる.This paper introduces a common database, an evaluation framework, and its baseline recognition result for noisy speech recognition, AURORA-2J, as an outcome of IPSJ-SIG SLP Noisy Speech Recognition Evaluation Working Group. AURORA-2J is designed as Japanized version of the AURORA-2 database and the evaluation framework is based on the AURORA-2 baseline scripts. This common evaluation framework enables to compare various noisy speech recognition techniques on a common ground. We hope more development of noisy speech recognition techniques using this evaluation framework. We also describe about AURORA-3J, digits and speech command database in car environments.
