Model Core Curriculum for Pharmacy Education (2013 version) presents eight disease areas as the minimum medical conditions, and it requires student trainees to be continuously engaged in pharmacotherapy for these diseases. We developed an “Individual Student Report Form” (Report Form) in order to investigate and evaluate each student trainee's engagement and continuity with the eight diseases. The Report Forms were shared between the community pharmacy, hospital, and university, and the effectiveness of collaboration among these institutions was examined.Participants were 25 student trainees who were planning to do their clinical practicums in our hospital in the second term of 2017. During the earlier pharmacy practicum, the trainees were asked to complete the Report Form regarding the diseases they handled. Trainees took notes on the Report Form during their daily reflection time in their hospital practicums about their engagement with the eight diseases. We calculated the cumulative total numbers and percentages of their engagement with the eight diseases to examine engagement rates and continuity across terms.The percentages of trainees who were engaged with all eight diseases were 36% (9/25 persons) at the pharmacy, 24% (6/25) at the hospital, and 68% (17/25) overall at the two locations. The average numbers of the eight tracked diseases that trainees were engaged with were 2.3 at the pharmacy, 3.8 at the hospital, and 4.5 in total. The findings of this study suggest that this tracking tool could help facilitate and efficiently transfer information about the disease engagement of trainees during their two pharmacy practicums.

近年、コーポレートブランディングへの関心が高まる一方で、その実践に苦労する企業が多い実態が 示されている。本稿は、関連するアイデンティティ、ブランド、レピュテーションの概念を整理し、実践の ためのフレームワークを提言する。コーポレートブランディングとは、組織のアイデンティティをマネジメ ントするプロセスであり、その目的はステークホルダーとの間に好ましいレピュテーションを形成すること にある。この定義に基づき、コーポレートブランディングの役割と手法について論じる。

Fuji-bashi, completed in 1915 over the Fuji River in Sizuoka Prefecture, is probably the longest span suspension bridge as well as only one railway suspension bridge in Japan before the World War II. However it's existence has been forgotten for a long time. The reason of oblivion will be as follows; that is, it was constructed by a private paper-manufacturing company, and it was used only four years until it was destructed in the stormy night with intent to save disaster. The purpose of this paper is to regain its reputation, and try to emphasize that Fuji-bashi is one of the important structures in the history of civil engineering of the modernized era in Japan. The paper contains lots of original data concerning its construction.

目的:ジャカルタに住む日本人の母親の現地生活,育児とその支援への思いを滞在3・4年目の駐在員の妻に焦点をあてて明らかにする。方法:2018年9月に,ジャカルタに住む日本人の母親に半構造化面接を実施し,質的統合法を用いて分析した。結果:ジャカルタに住む日本人の母親たちは,①【特有の生活事情への懸念】をもっていた。加えて②【医療への不安感とその対応への変化】を経験し,③【子育て支援への期待感】や④【生活環境への適応感】をもつに至った。さらに母親たちは,⑤【本帰国への不安感】をもちながらも,その反面で⑥【密な人間関係に基づく情報伝達意欲】という肯定的な思いをもつに至った。結論:日本人の母親たちの現地生活,育児と支援への思いは上記の構造をもつことが明らかになった。母親たちは,現地生活が長引くにつれて生活に適応し,否定的な思いから徐々に肯定的な思いに移行していくことが示唆された。また母親たちは,本帰国が近づくにつれて帰国後の日本の生活に対する不安が高まりながらも,情報発信意欲をもち,情報の受け手から担い手へ徐々に移行していくことが示唆された。

「辺境と異境」という視点からロシア文化の研究を行った。具体的には、第一に、極東、サハリンなどの辺境と中央(モスクワ、ヨーロッパ・ロシア)との文化的相互作用を研究した。第二に、中国・日本やヨーロッパにおける亡命ロシア社会の文化活動について、文学、美術、宗教など多岐にわたる分野で、その特徴を調査研究した。非中心といえる「辺境と異境」を視点とすることによって、ロシア文化を包括的に捉えることができた。

In order to investigate the role of progesterone in the maintenance of pregnancy, an anti-progesterone agent, RU486 (RU) was injected subcutaneously into pregnant rats on day12 (D12), and morphological changes of the uterus as well as endocrinological changes were observed.In all rats injected with RU, abortion occurred with macroscopic and microscopic intrauterine hemorrhage and degeneration or delivery of conceptuses. Endocrinologically, the levels of progesterone decreased rapidly 48 hours after the injection, while the levels of estradiol showed a tendency to increase.As progesterone is mainly produced by the corpus luteum but not by the placenta in rats, the decrease in progesterone is suspected to be due to luteolysis. Then in order to clarify the mechanism of luteolysis induced by RU and the effects of progesterone on this phenomenon, the dynamics of the luteotrophic factors (estradiol, LH, PRL) and specific binding capacity of the ovaries to LH/hCG were investigated in D7 pregnant rats treated with RU 1 mg/kg alone (RU group) or with both RU 1mg/kg and progesterone 50mg/kg (RU+P group).The serum levels of progesterone in the RU group decreased significantly after 72 hours of administration, while those in the RU+P group remained within the levels of the control group. However, serum levels of luteotrophic factors in the RU group did not de-crease, and some of them were even higher than those in the control group. In the RU+P group, luteotrophic factors remained within control levels.On the other hand, the specific bindings of LH/hCG to ovarian homogenates decreased significantly after 72 hours in the RU group. But in the RU+P group, the specific bindings were kept at the same levels as the controls. Scatchard analysis of these results disclosed that in the RU group, both affinity and numbers of receptors decreased compared to the controls, and that in the RU+P group only affinity decreased transiently and afterwards recovered quickly.From these results, it is concluded that deterioration of affinity and numbers of ovarian LH/hCG receptors seems to be one of the factors which induce luteolysis in pregnant rats treated with RU, and that progesterone can spare the effect of RU on the corpus luteum during pregnancy.

37歳の女性. 第 2子を正常分娩後 4日目に意識を消失し, 救急外来を受診した. 血圧40/ -mmHg, 心拍数126/分, チアノーゼを認めショック状態であった. 心電図上は, 完全右脚ブロック, 左側胸部誘導中心のST上昇を示し, 胸部X線では肺水腫を認めた. 心エコー上は, 前壁中隔から側壁にかけて広範な低収縮であった. 緊急冠動脈造影にて左冠動脈主幹部より左前下行枝, 回旋枝にかけての解離を認めた. 主幹部から回旋枝にベアメタルステントを留置したが, 前下行枝の血流は確保されなかった. 緊急冠動脈バイパス術を予定したが, 血行動態の悪化により施行されなかった. 最大CK値は17,742 IU/Lと広範な梗塞であり, 大動脈内バルーンパンピング, カテコラミンの投与によりショックから離脱した. 第61病日に施行した冠動脈造影では, 左前下行枝は再開通し, 解離は自然修復されていた. 妊娠に関連した急性心筋梗塞は非常に稀であるが, 最近の妊婦の高齢化により増加している. 特に, 産褥期は冠動脈解離に注意する必要がある.

【目的】Lipoblastomaの適切な診療方針について検討する.【方法】1981年4月から2019年3月の期間に当院で外科的切除を行い,病理組織学的にlipoblastomaと診断された51症例を対象とし,発生部位,症状,手術所見(被膜・癒着・浸潤・全摘の有無),再発の有無,術後合併症,再発腫瘍の病理所見について後方視的に検討した.【結果】発生部位は四肢と体幹に多く,無痛性の増大する腫瘍として気づかれるものがほとんどであった.体腔内に発生した症例は4例でうち3例は咳嗽や嘔吐など周囲臓器の圧排症状を呈した.2例に術後2か月と5年で再発がみられ,いずれも被膜不明瞭もしくは周囲に癒着がみられたが全摘された症例であった.不完全切除となった4例に再発はなかった.周囲の正常組織も含めて腫瘍を全摘した症例の中には術後瘢痕による機能障害を残した症例もあった.再発腫瘍の病理組織はいずれも初回手術時より分化が進んでいた.【結論】Lipoblastomaは局所再発のリスクがあるが,良性腫瘍であり,また経過とともに消失したり組織が分化する可能性もあるため,癒着や浸潤傾向の強い症例では,全摘に執着せず,術後機能障害を起こさない程度の切除に留めることも考慮してよいと考える.また,全摘の有無に関わらず術後長期間経過してから再発することもあるため,術後は最低5年以上の慎重な経過観察が必要である.

【要約】 本邦の歯科医院における全身的合併症発症時の院内救急体制の整備状況と実態を明らかにするため,全国の郡市区歯科医師会および歯科医院を対象にアンケート調査を行った.歯科医院および回答した歯科医師会は392団体(回収率51.2%),歯科医院は392施設(回収率25.5%)であった.救急薬剤を配布している歯科医師会は48.5%,歯科医院における救急薬剤の常備率は74.5%であり,歯科医師会による配布薬剤,歯科医院における常備薬剤ともアドレナリン,アトロピン,ニトログリセリンが多かった.救急薬剤を投与した経験のある歯科医院の割合は5.6%であった.また,医療用酸素,生体情報モニタ,AEDの配備率はそれぞれ82.7%,66.3%,71.5%であった.「救急薬剤を配布している」と回答した歯科医師会190団体のうち,救急薬剤投与法の講習/研修会を開催していた歯科医師会は75.8%であった.また,過去5年間に緊急時対応・救急蘇生法の講習/研修会を開催していた歯科医師会は68.9%を占めた.歯科診療においては,生命に関わる重篤な全身的合併症を予防することはもちろん,その発症時に適切に対応するには,救急薬剤および器材を常備し,研修を怠らないことが医学的および倫理的見地から重要である.歯科医師が現実的に使用可能な救急薬剤の選定とその適正使用についての卒前・卒後教育の充実が今後の課題であると考えられた.

小児がんの治療成績向上に伴い,晩期合併症として二次がんの発生が問題となっている.今回小児固形腫瘍治療後に発生した二次性甲状腺癌の4例を経験したので報告する.一次がんはanaplastic sarcoma of the kidney・atypical teratoid/rhabdoid tumor・胸膜肺芽腫・卵黄嚢腫瘍であり,全例に手術・術後化学療法が施行された.2例に術後放射線照射が施行され,うち1例は頸部も照射野に含まれていた.二次性甲状腺癌の発生までの期間は中央値7年6ヵ月(4年4ヵ月~8年9ヵ月),組織型は乳頭癌が1例,濾胞癌が3例であった.二次性甲状腺癌の発生の原因として,放射線照射・化学療法・遺伝性素因等が挙げられる.高リスク群に対しては長期にわたって触診や超音波検査による甲状腺の観察が必要である.

Previous studies demonstrated that Mindfulness based Cognitive Therapy (MBCT) is effective for reducing rumination. However, little is known about the effectiveness of mindfulness on the process of getting worse of rumination. The present study conducted cross-sectional and 6 months longitudinal studies to replicate a previous finding that mindfulness traits buffer the influence of causal analytic rumination on uncontrollability of rumination. However, there was no evidence that mindfulness traits prevent the development of rumination. Additional analysis showed Nonreact aspects of mindfulness predicted low uncontrollability of rumination 6 months later. These results suggest that mindfulness contributes to inhibit cognitive reactivity before occurring of rumination rather than prevents worsening of rumination which already occur. We discussed the importance of subdivided examination of mindfulness and directed future studies.

Previous studies have indicated that rumination is related to the difficulties in controlling interference during updating working memory. The current study examined the relationships between interferences during working memory updating, depressive rumination, and depressive symptoms by using the 2-back paradigm. The 2-back paradigm included two forms of interferences, n+1 lure in which a 3-back item corresponded to the currently presented item instead of the 2-back item and a n-1 lure in which a 1-back item corresponded to the currently presented item instead of the 2-back item. The result indicated that the high brooding group with high scores on the brooding subscale of the Ruminative Responses Scale showed greater interference scores in response to the n+1 lure than low brooders, which was not the case in response to the n-1 lure. These findings indicate that high brooders have difficulties in removing relevant representations that are no longer needed from working memory. This finding expands those of previous studies and also extends our understanding of the mechanisms of ruminations.

本研究では,日本人大学生45名を被験者とし,第二言語習熟度に関連すると思われる社会認知能力と運動能力を検討した。具体的にはまず,英語習熟度をTOEICのリスニング,リーディングおよび合計スコアに加え,読解速度と語彙力により求めた。社会認知能力は,Baron-Cohenが開発した3種の社会性指標で求め,さらにワーキングメモリ容量を算出した。学習者の運動能力については,反復横跳び,垂直跳び,握力,長座体前屈,12分間走により,5種類の基本身体能力を測定した。検査課題で得られた英語習熟度と両能力の結果を相関解析した。社会認知能力に関し,男性では共感指数は語彙力と負の相関を示し,語彙力の高い学習者は対人関係に敏感でない傾向が示唆された。運動能力に関し,12分間走と垂直跳びが英語習熟度に関連していることから,第二言語習得に関し学習者は根気強さとエネルギーが必要であることを明らかにした。言語習熟度と他の能力との相互関係に,男女差が影響を与えている可能性も示唆した。
