抗菌薬適正使用の推進は院内感染対策において最も重要な課題の一つである.当院では2006年4月より第4世代セフェム系,カルバペネム系,ニューキノロン系抗菌薬と抗MRSA薬を対象として使用届出制を開始した.使用届出制の開始後,特に第4世代セフェム系,カルバペネム系抗菌薬の使用量が減少し,また投与期間が14日以上に及ぶ長期投与の処方件数も減少した.また2008年4月からはpharmacokinetics/pharmacodynamics理論に基づく抗菌薬の投与方法に関する資料の配布を開始した.資料の配布開始以降,cefozopran (CZOP)では1000 mg×3回/day及び2000 mg×2回/dayの投与方法が,meropenem (MEPM)では500 mg×3回/dayの投与方法がそれぞれ増加した.緑膿菌のCZOPに対する耐性株率は2005年度から2006年度で一時増加したが,2007年度では2005年度と同程度まで減少し,またMEPMに対する耐性株率は年々減少が見られた.このように,抗菌薬適正使用の推進及び抗菌薬耐性菌の増加防止において,infection control teamによる積極的な介入は重要であると考えられる.<br>

CS-570, a chemically stable prostacyclin derivative, inhibited decrease of platelet counts and fibrinogen level, increase of FDP, prolongation of prothrombin time and activated partial thromboplastin time and deposition of fibrin in the renal glomeruli dose-dependently in the endotoxin-induced DIC model of rats. Ticlopidine, dazoxiben (thromboxane synthetase inhibitor) and BM 13177 (thromboxane antagonist) also showed inhibition of changes in these parameters. CS-570, a potent platelet stabilizer and vasodilator, would probably have inhibited thrombocytopenia and coagulation disorders by physiological antagonism against TxA2 produced by endotoxin. This would suggest beneficial effects of CS-570 in endotoxin-induced DIC in humans.

So far, the use of superconducting magnets has been limited to chemical analysis and medical inspection. To develop a new use, we have tried to apply common MRI devices as nondestructive sugar content detector of watermelon. To estimate the sugar content, a new technique using multiple regression analysis of two NMR relaxation times was developed. It was found that the coefficient of multiple regression at the center of the watermelon exceeded 0.9 and the standard error of prediction was around 0.5, and that the over-ripened part of flesh called ‘nieka’ in watermelon could be distinguished because its T2 was much longer than that of the oridinary part. An evaluation rate of sugar content could be below 6s per watermelon. It is concluded that multiple analysis of T1 and T2 on intact watermelon can be applied as a non-invasive, nondestructive indicator of sugar content.

要旨:【目的】健側大脳への低頻度反復性経頭蓋磁気刺激(以下,RTMS)と集中的作業療法(以下,OT)は,いずれも脳卒中後上肢麻痺に対する有効な治療的介入である.本研究では,これら2つの介入の併用療法の安全性と有用性を検討した.【方法】全国8つの施設に入院し本併用療法を施行された上肢麻痺を呈する脳卒中患者1,008人を対象とした.各対象は15日間の入院下で,20分間の低頻度RTMSと120分間の集中的OTからなる併用療法を計22セッション施行された.【結果】全患者が有害事象をみることなく本併用療法を完遂した.治療によりFugl-Meyer Assessment点数,Wolf Motor Function Testの課題遂行平均時間,Functional Ability Scale点数が有意に改善した.【結論】我々が考案した併用療法は安全であり,脳卒中後の上肢麻痺を改善する可能性が示唆された.

The ductility factors of nonlinear SDOF systems at Service Limits Ds “using elastic analysis design” in JEAC4601 are investigated, and it was confirmed that the ductility factors depend on the natural frequencies of systems, seismic motions and constant loads. Based on the above results, an acceptance criterion of components to prevent ductile failure and plastic collapse is proposed. The criterion is given as a limit of ductility factor for Service Limits Ds “using elastoplastic analysis design”. The proposed limit of ductility factor allows single state for nonlinear systems, and doesn't depend on the natural frequencies of systems, seismic motions and constant loads.

レストランの顧客満足度は,料理の品質だけではなく,料理提供の順序とタイミングにも強く関係している.料理の品質は,料理ごとに定められている調理手順に従うことで維持できるが,料理提供の順序やタイミングは,注文された複数料理の組合せにより,各々の調理手順を組み立てなければならない.本研究では,調理手順を組み立てるスケジューリング問題を扱う.まず,スケジュールを行う調理手順を簡素化したモデルを作成し,提供までの時間とグループ客への同時提供を重視したルールを提案する.そして,評価実験により提案したルールを比較し,ルールの有効性と結果にもたらす影響を考察する.

レーザ変位計を用いた非接触法により、臨床に近い状態での模型材の凝結膨張を測定し、凝結膨張率と模型材の寸法精度との関係について検討した。ニューフジロック、インディックダイストーン、ニュープラストーン、モデルマティリアルの凝結膨張率は、それぞれ0.09%、0.10%、0.29%、0.76%であり、各メーカーの値よりも大きくなったが、こらは非接触法による測定法を用いたためだと思われる。

日本の技術倫理教育では、倫理を知識として獲得する事に重点を置く傾向がある。しかし、倫理問題を解決するには、型にはまった技術倫理の知識だけでは不十分といえる。そこで、我々のグループが開発しているe-ラーニングソフトウェア『技術倫理討論会』は、「ケーススタディ」と「ディスカッション」を中心に据えて設計・構築されている。具体的には、倫理的判断が難しい事例を「ケーススタディ」としてFLASHムービーを用いて表示した。加えて、ケーススタディ別に電子掲示板を用意し、ユーザーがケーススタディの解決方法に関する「ディスカッション」を行う場を提供した。ここで討論することにより、ユーザーは自分自身では思いつかなかったような対処法や他人の価値観を知り、独り善がりな考え方に陥る事を予防する。本ソフトウェアをWWW上で公開し、その結果を検討・考察し、より効果的な教育プログラムの開発を進めていく。

Joining of plastics and light metals contributes to the reduction of a product weight. In this study, the punching rivet method was applied to joining of an acrylic resin sheet and an aluminum alloy sheet. The punching rivet method can join the sheets without drilling. The riveting process of this method is constituted of the punching process of the sheets using the rivet shank and the fastening process of the sheets using the rivet and the rivet holder. The sheets are fastened by using the plastic deformation of the rivet shank. From the observation of the joints made by the punching rivet method, it was found that the acrylic resin sheet of the joint had no crack and out-of-plane deformation of the joint was small. From the results of the joint strength tests, it was considered that the joint made by the punching rivet method had high strength due to the effect of the pressures on seating faces of the rivet and the rivet holder. As a result, the punching rivet method was effective to join the acrylic resin sheet and the aluminum alloy sheet.

(1) サツマイモ黒斑病に対する薬剤の防除効果を検定するために,厚さ7~10mmのサツマイモ・スライスを所定濃度の供試薬液に浸漬, 風乾後, 黒斑病菌を接種し,薬剤の効果を調査した。<BR>(2) 本法により,高い抗菌力のみられたベンレート水和剤500倍,トップジン500倍および,抗菌力の低くかったウスプルン800倍処理によりほ場試験を行なった。その結果,ベンレートの接種2時間前および2時間後処理の防除効果は顕著であったが,24時間後処理では効果の低下がみられた。ウスプルンとトップジン処理の効果は低くかった。<BR>(3) 試験結果から,サツマイモ・スライスを用いた簡易検定法により抗菌力の低い薬剤を除外することができ,有効薬剤についてのみほ場試験を行なってその効果を確認すればよく,本法は,簡易で実用的である。

The movie Black Swan is directed by Darren Aronofsky. The title of this movie comes from the ballet performance, Swan Lake. The title refers to the third act of Swan Lake in which two swans, one black (Odile), and one white (Odette), feature. The black swan appears as a mysterious princess who betrays her Prince. In the story the audience is required to speculate as to whether the two swans are different, or they represent the divided selves of the same swan who was previously female. In the movie there are a number of recurring themes, among which are: mirrors;doubles; the colors; the mother-daughter relationship; the referential motif of fairytales; and fantasy and reality. These motifs are emphasized by their repetition. Afterbeing appointed as the principal dancer of a new production of Swan Lake by an artisticdirector of a ballet company in New York City, the heroine, Nina Sayers, meets a series of doubles many times, in a variety of real life situations and as reflected images inmirrors. As the title suggests, colors are also important motifs in the movie, especially,black, white, and red. Red symbolizes the color of blood. The mother-daughterrelationship is also explored, with this relationship having echoes of Snow White.Nina's mother has a conflicting and simultaneous love for and jealousy of her daughter.She has always supported and helped her daughter, but after Nina is chosen as the principal dancer of the company, her mother comes to envy her.

In this study, vibration characteristics of a single-degree-of-freedom linear oscillator with the fractional order derivative are examined in terms of the critical damping over a wide range of the order of the fractional derivative by using numerical analysis. Two types of the definitions of the critical damping used in the previous studies are considered. It is shown that (i) the critical viscoelastic damping ratio changes according to the order of the fractional derivative and its minimum value for both types of the critical damping is less than 1; (ii) no critical viscoelastic damping ratio is observed in a certain range of the order; (iii) the difference in the existence of the critical damping between the oscillators with the derivative of order 1/3 and 2/3 is caused by the change of the behavior of a component of the response corresponding to one of the roots of the characteristic polynomial for the oscillator. Finally, the impulse response characteristics are classified into three classes depending on the order of the fractional derivative and viscoelastic damping ratio of the oscillator.

A high-resolution display panel comes to practical use, but the resolution of the indicated contents does not change. The up-sampling processing is applied to indication of the low-resolution contents. In the up-sampling process, the super resolution enables an up-sampling process which estimates information of high frequency components lost by sampling while analyzing input images is noticed. In this paper, we aimed at reconstructing an image of normal resolution in which the influence of statistical noise is reduced by applying super resolution after down-sampling processing is applied to positron emission tomography (PET) image with many statistical noises. To evaluate the noise reduction effect, we compared it with the Gaussian filter which is frequently used to reduce the influence of the statistical noise of the PET image. A 3D Hoffman brain phantom was used to evaluate objectively by peak signal-to-noise ratio and power spectral density. The objective index of the PET image applying super resolution is positive results, suggesting the possibility of being useful as compared with the conventional method.
