10%ポビドンヨード製剤中の添加物の違いによる殺菌効果・細胞毒性の差を検討した.その結果, 殺菌効果は製剤間の違いは認められなかったが, 細胞毒性に大きな違いが認められた.この毒性は, グリセリン, ポリビニルピロリドンでは認められなかったことから, 添加界面活性剤の違いと考えられた.ポビドンヨード原末の毒性は低いものの, 創傷部等に頻回に使用する際には, 添加物の毒性にも注意が必要と考えられた.

敗血症の際には,血管内血栓形成が進行し,しばしば播種性血管内凝固症候群(disseminated intravascular coagulation, DIC)を合併する。血管内血栓形成は,血流を悪化させ,臓器障害の原因となりうることから有害事象と考えられているが,感染を局所に封じ込めるうえで重要な役割を果たしている可能性が指摘され,“immunothrombosis”という概念で注目されている。しかしながら,感染を局所に封じ込めることができずにimmunothrombosisが全身に拡散した場合,多臓器が障害され,生体防御機構であったはずの血栓形成が宿主にとってむしろ有害なものになってしまう。このように,immunothrombosisが全身に拡散して制御不能に陥った状況が,敗血症性DICの病態基盤であると考えられる。本稿では,immunothrombosisで重要な役割を果たしている好中球細胞外トラップ(neutrophil extracellular traps, NETs)の放出機序や意義について概説し,NETsから遊離してくると考えられている細胞外ヒストンが敗血症性DICの病態に及ぼす影響について考察する。

近年,普通学校に学んでいる難聴の児童生徒が増えていると言われている。因みに平成26年度の文部科学省の報告に,聴覚支援学校の在籍者は減少が続いているが,普通学校で通級による指導を受けている小・中学生は平成7年度以降増え続けていることが示されている。最近の仙台市における学校健診の結果から,高度難聴の児童は人工内耳や補聴器による聴覚補償を受け普通学校に在籍し通級指導を受けていること,また軽度・中等度難聴児の一部は発見が遅れ適切に対応されていないことが判明した。補聴器,人工内耳をすでに装用している難聴児および学校健診で発見される難聴の中,学校生活に影響があると考えられている軽度・中等度難聴,一側高度感音難聴,機能性難聴への対応について概要を述べた。文献的考察を踏まえ,難聴の児童生徒が普通学校で不自由なく学校生活を送るために,今後耳鼻咽喉科の学校保健活動として取り組んでいくべき課題を提唱した。

R.A.Fisher studied the concept of Likelihood in stead of the methods of Mathematical Statistics given by Neyman and Pearson. The writer also studied likelihood as the measure of odds or its measure of guessing error instead of distribution of unknown parameter. Fisher made a mistake introducing Lebesgue measure in parameter space. There is no such Lebesgue measure in parameter space. Likelihood should be used for comparing the odds among each value of parameter values instead of distribution of parameter values.This paper introduced the new ideas to compare only likelihood among each value of parameter.

In December 2019, a cluster of cases of acute respiratory illness, novel coronavirusinfected pneumonia, occurred in Wuhan, Hubei Province, China. The false-negative nasopharyngeal swabs of SARS-CoV-2 caused the delayed diagnosis of COVID-19 which hindered the prevention and control of the pandemic. The transmission risk of SARS-CoV-2 in negative nasopharyngeal swabs cases were little addressed previously. This study evaluated two clusters of COVID-19 in six patients. Four of six (66.7%) showed negative RNA of SARS-CoV-2 by nasopharyngeal swabs. All epidemiological, clinical and laboratory information was collected. The first cluster was a nosocomial infection of four health care providers at early January. One of them made sequential familial cluster of infection. All patients received either selfquarantined at home or were admitted to hospital for isolated treatment. All recovered and had anti-SARS-CoV-2 IgG and/or IgM positive (100%) for serological detection of SARS-CoV-2 at recovery stage. Our study provides a cautionary warning that negative results of nasopharyngeal swabs of suspected SARS-CoV-2 infection can increase the risk of nosocomial infection among health care providers. Serologic detection for anti-SARS-CoV-2 IgG and/or IgM is an important test in the assistant diagnosis of COVID-19.

In this essay, I will first describe what constitutes a model in natural science both in the physical and biological disciplines and give a brief overview on its relationship to constructive experimental approaches. Interesting parallels are noted in the key defining inventions and models starting from Voltaic for the early exploration into electrophysiological mechanisms behind muscle contraction, Lillie’s ironwire for nerve impulse conductance, Belousov-Zhabotinsky reaction for feedback regulation in the Krebs cycle. The were not meant to faithfully reproduce the intended phenomena as a whole, however sucesstully captured the very essence of nonlinear dynamics as we know them today. Drawing from the basic concepts that eventually emerged from these studies, the present day natural science awaits further exploration into the constraints and geometries in high dimensional systems. The present day natural science is witnessing closer encounter to their unique dynamic properties or life ‘process’ at the cellular level such as cell growth and cell deformation, but how close are we from understanding such complex phenomena?The new challenge may require further inteination of machine and nature that drives synthesis of classic hypothesis-driven approach with data-driven quantification.

Schwannomas (neurilemmomas) are benign neoplasms derived from Schwann cells of the neurilemma and appear most frequently on the auditory nerve or peripheral nerves of the skin. They arise in the oral and maxillofacial region infrequently, and very rarely in the center of the jaw. We herein present a case of a rare mandibular intraosseous schwannoma derived from the main trunk of the inferior alveolar nerve in a 33-year-old man. Fusiform expansion in the mandibular canal was observed and a mass showing the target sign in the mandibular canal was confirmed on T2-weighted and Gd contrastenhanced T1-weighted MRI. Based on these findings, an inferior alveolar nerve-derived schwannoma or other benign nervous system neoplasm was diagnosed. A buccal side cortical bone flap in the mandibular molar region was removed to expose the mass, which was then peeled away from the nerve fibers and completely removed. Some inferior alveolar nerve fibers that were connected to the mass were removed at the same time, but the remaining nerve fiber bundle was preserved. Histopathology confirmed the diagnosis of a schwannoma with Antoni type A and Antoni type B regions. Although the patient experienced extremely mild paresthesia in the skin over the mental region and mental foramen at immediately after surgery, this had almost entirely disappeared at 7 years and 4 months later, and there has been no tumor recurrence.

本研究では,現在の小学校高学年において有効であると考えられる正負の数のカリキュラム開発を行うことを目的とする。そこで本稿では,正負の数の教育の歴史的変遷・先行研究・諸外国の教育(一部の州)・認識調査などの分析から,現在の子どもに有効であると考えられる正負の数のカリキュラム(試案)を開発し,カリキュラムの小学校高学年段階の内容(一部)について,実際に,小学校6 年生(正負の数を未習)を対象に教育実験を行うことからその妥当性を検証した。その結果,小学校6 年生で十分に理解可能であることから,開発したカリキュラムに一定の妥当性が示唆された。

19世紀後半から経済成長を加速させたロシアは,二度にわたる急速な工業化の期間を経て,1960年には西欧先進国の発展レベルに接近した.長期にわたるロシア・ソ連経済のキャッチアップの過程は,サイモン・クズネッツのいう「近代経済成長」の一例といえよう.しかし,1960年頃から経済成長と技術革新の主要推進力としての「上からの圧力」を失ったソ連は,目標とした先進国の水準に到達することなく解体に至り,その「近代経済成長」は挫折に終わることとなった.

症例は,72歳,女性.1998年2月初旬より感冒様症状があり,近医で内服治療を受けていた.2月9日胸痛が出現し同医院を受診したところ,心電図上ST上昇を認め心筋梗塞の疑いにて当院に救急搬送された.心筋梗塞を疑い冠動脈造影を施行したが,異常所見を認めなかった.スワンガンツカテーテルにて心拍出量の低下および心臓超音波検査による壁還動のび漫性低下の所見を合わせ急性心筋炎による心不全(フォレスターIII度)と考え,昇圧薬と利尿薬による心不全治療を開始した.第3病日突然に心原性ショックに陥ったため,大動脈内バルーンパンピング法,経皮的心肺補助装置による補助循環を開始した.心機能の回復がなく,第5病日に人工呼吸器管理,さらに第8病日には腎不全のため透析を開始したが,第12病日に多臓器不全のため死亡した.ペア血清でインフルエンザA型(H3N2)のウイルス抗体価の有意な上昇を認めた.さらに剖検心組織からRT-PCR法によりインフルエンザA型ウイルスが検出された.97年度はインフルエンザが流行した.そのインフルエンザウイルス感染が原因で心筋炎を発症し激烈な経過をたどり,剖検心筋組織からウイルスが証明された症例を経験したので報告する.

In this paper, we would like to clarify the way of differentiation among the dwelling unit plans by their spatial configuration in super-highrise condominiums developed in Tokyo Metropolitan Area. In the former part, through the interview to the architects, we illustrate their understanding toward the livingroom-connected type, which is used as relatively compact units, and corridor-connected type, which is used as larger and more privacy-secured units, is structured. In the latter part, we analyze that the variation among the livingroom-connected, intermediate and corridor-connected type is used for the ordering of unit plans, and that the order of the three unit plan types form an axis of values. We also point out that there are a few patterns of strategic unit arrangement in an apartment building and discuss that the architects use the axis to fulfill the various request from their clients.

本研究では,大学生および成人が,「男らしさ」や「女らしさ」をどのように考えているのか,また,「異性に求めることは何か」を質問紙の自由記述欄において尋ねたものをまとめた。その結果,「男らしさ」としては,包容力,頼りがい,たくましさ・強さ,決断力・判断力,行動力・積極性,リーダーシップなどが,「女らしさ」としては,上品,気遣い・繊細さ,家庭的,かわいさ,愛嬌,色気,美しさといった内容が上位にあがっていた。これらの多くは,従来から示されているものとほぼ同様であり,男性性,女性性に関するジェンダー意識に余り変化がみられなかった。また,「男らしさ」「女らしさ」に共通してみられた内容としては,優しさ,包容力,忍耐強さ,誠実さなどがあげられる。さらに,異性に求めるものとしては,男女共に優しさが最も多かった。次いで,男性が女性に求めることとして,癒し・安心感,ルックスのよさや,自分にないものをもっている,話が合う,従順さや気が利くことなどが上位にあがっていた。一方,女性が男性に求めることとして,包容力,経済力,頼りがい,楽しさ・ユーモア,誠実さなどが上位にあがっていた。

真言密教(東密)系の阿弥陀如来像とされる紅頗梨色阿弥陀如来像は、頭上に五智の宝冠を戴き、身色を紅くして、阿弥陀如来の三昧耶形である金剛杵を茎とする蓮華を台座としてその上に坐す、特異な形式の像として知られている。その典拠は弘法大師空海作とされる『無量寿次第御作』『紅頗梨秘法』とされるが、宝冠を戴く阿弥陀如来像の成立は、空海時代よりもかなり下がると考えられる。また阿弥陀法次第の道場観では、種子・三昧耶形・本尊形の三種を順次変化させて観念する次第であって、三昧耶形と本尊形とが同時に同一画面上に現れることはあり得ない。そこで現在の紅頗梨色阿弥陀如来像がどのようにして成立したのか、儀軌から道場観へ、道場観から実際の図像へという過程を、図像の直接的典拠である道場観を中心に考察したい。

ニュートン力学は,アリストテレス的世界観から新しい世界観への転換を示してくれた体系であり,その考え方を是非とも学生たちに理解させておきたい,そのために花火の運動を考察することによってニュートン力学の特徴を理解させることを試みている.このとき花火の運動を三段跳び(ホップ,ステップ,ジャンプ)にたとえて,慣性の運動,重力による自由落下の運動,空気の抵抗(速度に比例)の3段階でとらえ,その役割と特徴をあきらかにする.花火のかけら(星)の形は,どの段階でもつねに円(球)となって落下することが導けるので意外性があり印象的である.

Optics-based surveys for large unicellular zooplankton were carried out in five different oceanic areas. New identification criteria, in which “radiolarian-like plankton” are categorized into nine different groups, are proposed for future optics-based surveys. The autonomous visual plankton recorder (A-VPR) captured 65 images of radiolarians (three orders: Acantharia, Spumellaria and Collodaria) and 117 phaeodarians (four taxa: Aulacanthidae, Phaeosphaerida, Tuscaroridae and Coelodendridae). Colonies were observed for one radiolarian order (Collodaria) and three phaeodarian taxa (Phaeosphaerida, Tuscaroridae and Coelodendridae). The rest of the radiolarian orders (Taxopodia and Nassellaria) and the other phaeodarian taxa were not detected because of their small cell size (< ca. 400 µm).

We conducted ground penetrating radar survey on the Chashi Sites of Shibecharigawa basin of Shinhidaka in Hokkaido, with the purpose of investigating the internal structure of the Chashi. The surveyed sites were Shibechari-Chashi and Hoinashiri-Chashi. Shibechari-Chashi was thought to be the fort of Shakushain, leader of Hidaka Ainu. There was the anomaly likely to be a ditch around Chashi. In the case that it was a buried ditch, Shibechari-Chashi would come to have the largest area in Hokkaido. In Hoinashiri-Chashi, the reflection related to the chashi was gained at the main/center part of the site. Also, the anomaly detected on the west side was thought to be the reaction of the buried ditch. These results showed the Chashi was surrounded with the ditch and strengthened its protection.
