Chinese Medicine helps to normalize and stabilize autonomic nervous system imbalances and endocrine and metabolic disorders.In the field of oto-rhino-laryngology, Chinese Medicine is very effective when it is applied according to the traditional Chinese medical diagnoses for: 1) Abnormal sensation in the pharynx in patients with autonomic dysfunction. 2) Chronic inflammation. 3) Recurrent otitis media and pharyngitis. 4) Tinnitus, vertigo and dizziness with circulatory disturbances.

73歳女性.夜間睡眠中の大声の寝言や日中覚醒時の異常言動,会話中に急に反応が乏しくなるエピソードが月単位で相次いで出現.他院にてレビー小体型認知症(dementia with Lewy bodies,以下DLBと略記)と診断されるも,これらの頻度が徐々に増加したため,当院にて精査入院.終夜睡眠ポリグラフィの“筋活動の低下を伴わないレム睡眠”および18F-FDG PETの辺縁系糖代謝亢進より,レム睡眠行動異常症を合併した辺縁系脳炎と診断.ステロイドパルス療法2コース実施後,症状は徐々に軽減した.本例は後に血清抗voltage-gated potassium channel(VGKC)複合体/leucine-rich glioma-inactivated protein 1(LGI1)抗体陽性が判明した.本抗体関連辺縁系脳炎は臨床像がDLBに重なることがあり,診断に際し注意が必要である.

根拠に基づくリハビリテーションの実践のため,帰結評価指標を用いた定期評価による治療効果判定の重要性が増している.そのためには適切な帰結評価指標を選択し,その情報を利用した臨床意思決定を行うことが求められる.本稿では臨床において最適な帰結評価指標を選択する基準である6 つの主要因子(評価の対象,評価の目的,指標のタイプ,測定尺度の種類と心理測定特性,対象者因子,時間・空間・物理的環境因子)について紹介するとともに,測定の標準誤差や最小検知変化などの有用な情報について説明する.さらにリハビリテーションにおいて対象とすることの多い身体動作能力に関する帰結評価指標であるFunctional reach test, 10m 最大歩行速度,脳卒中身体動作能力尺度を紹介し,その有効利用法について述べる.

第一次世界大戦中、日本に囚われていたドイツ兵捕虜のスポーツ活動の中でも、特にサッカー交流を取り上げ、その事実・背景と教育遺産について論じた。その内容は、おおむね、下記のごとくである。 1.似島、青野原および名古屋の捕虜収容所にいたドイツ兵のサッカー交流が、史料により確認されている。 2.交流相手は、主として、中学校、師範学校・高等師範学校であった。 3.サッカー試合を含む文化・スポーツ交流は、彼らの解放が近くなった1919 年に集中している。 4.ドイツ兵捕虜収容所があった自治体における、コンサート、スポーツ行事、展覧会の催行により彼らの諸活動を再現するとりくみ、および学校でドイツ兵捕虜の足跡を題材にした教材づくりや授業をする実践は、地域教育や平和教育、国際交流教育の観点から評価できる。

Medical workers are continually faced with determining the appropriate usage of disinfectants in the areas of hospital nursing and home health care. The present study has been designed to provide answers to such questions as “what kind of technique is best for intra-airway and oral aspiration ?” and “what is the most effective use of disinfectants during aspiration procedures to prevent infectious diseases ?” First, the microbial contamination of suction catheters and disinfectant solutions, which have been in use for 24 hours, were investigated. The results indicated contaminations in 50% of catheters and one of the disinfectant solutions, which were presumably caused by the different aspiration techniques of nurses.Experiments were designed to improve the manual, and the results presented to nurses and pharmacists. Additional items to the manual are : (1) The necessity of removing saliva and sputum from the catheter prior to rinsing with the disinfectant solution; (2) The appropriate volume of rinse water and disinfectant solution necessary to achieve a complete de-contamination of the catheter; (3) Preventive techniques used to avoid microbial contamination of disinfectant; (4) Disinfectant soaking techniques for the catheter; (5) Advice for hospital/home health care personnel regarding the proper bedside use of a catheter and disinfectant bottle to maintain appropriate hygienic conditions.The results and subsequent improvement of the instruction manual had a pronounced effect on both the aspiration techniques of nurses, and upon the attitude of pharmacists in our hospital. The pharmacists now realize their leading role in the effective and proper usage of disinfectants.

本稿の目的は 「新体制試案要綱」 の策定に関わる民間シンク・タンク国策研究会と陸軍省軍務局幕僚の関係を再評価すると共に, 軍務局幕僚の新体制構想の実像を明らかにすることにある。従来, 国策研究会は陸軍のブレーン・トラストと考えられており, 同会が策定した 「新体制試案要綱」 は, 未発見の陸軍の新体制構想を代替するものと明確な根拠を欠いたまま推測されてきた。しかし 「新体制試案要綱」 の策定過程を確認すると, 同要綱が多様性に富んだメンバーによって立案され, 審議の過程や結論が広く公開されたこと, その内容も議会や旧政党を尊重するものであったことが分かる。この事実は同要綱と軍務局幕僚の関係を一見否定するものである。にもかかわらず, 既存研究はこの矛盾について全く説明しえていない。本稿は, 当該期に軍務局幕僚が陥っていた政治的苦境を指摘し, 軍務局幕僚にとっては国策研究会の 「中立性」 や 「公開性」 にこそ同会の利用価値の本質があったことを指摘する。そしてこの考察の過程で, 当該期の軍務局幕僚の新体制構想が, 議会政治や政党政治に肯定的評価を与えるものであったことを明らかにする。

目的:本研究の目的は,若年労働者のメンタルヘルス不調の特徴を明らかにし,実効的なメンタルヘルス対策を検討することである.方法:国内の産業医386名に無記名の自由回答式質問票を送付し,109名から回答を得た.質問票は,産業医として対応したメンタルヘルス不調者の年齢階層別の特徴に関する設問と,若年労働者に対して実施しているメンタルヘルス対策とその効果の認知に関する2つのパートで構成された.全ての回答をデータ化し,質問毎に頻出語句とその出現数を数えた.前者に関しては,各年齢階層と頻出語句の関連性を検討するために統計学的処理を加えた.後者では,共同研究者及び研究協力者10名にて,若年労働者に対して実施しているメンタルヘルス対策とその効果の認知に関する記述を整理した.結果:コレスポンダンス分析において,20歳代の周辺には,性格,未熟,他罰的,発達障害,統合失調症,新型うつ病,不適応,入社,社会,上司,同僚などの語句が布置された.30歳代では業務の質的負担,量的負担といった仕事に関する語句,40歳代は家庭,子供,介護といった職場外要因に関する語句が布置された.若年労働者に対して実施した対策は,教育と面談に関する記述が頻出したが,その効果は不明であるという回答が最も多かった.複数名の回答者から,上司や人事担当者,産業医といった職場関係者と若年労働者の家族との連携により家族の支援の向上が認められたという回答が得られた.考察:若年労働者のメンタルヘルス不調は,職場への不適応や未熟で他罰的な性格といった個人的要因や精神障害,労働者の背景や職場組織に関する職業性ストレスといった様々な要因の影響を受けていることが示唆される.職場と家族との連携は若年労働者にとって重要なメンタルヘルス対策となる可能性がある.

In this study, authors participated in the process of re-designing an information system for civil activities. From viewpoint of Actor Network Theory, they mainly pointed out two findings. Firstly, designing an information system inevitably includes (re-)designing related social organization and activity itself. Therefore the requirement engineering approach has its limitations, for it implicitly assumes that activities and social organization are out there independently from information systems, user requirements already exist, and they can be obtained out with various methods. Secondly, the subject of the information system design should not be regarded as an isolated individual or a team. It is a fluid network including artifacts and people with various interests. Moreover, these actors sometimes participate in, and withdraw from, the project. A new approach, the network-oriented approach, is required to make a useful information system, which will not cause serious breakdown.

Biaryl synthesis continues to occupy a central role in chemical synthesis. From blockbuster drug molecules to organic electronics, biaryls present numerous possibilities and new applications continue to emerge. Transition-metal-catalyzed coupling reactions represent the gold standard for biaryl synthesis and the mechanistic steps, such as reductive elimination, are well established. Developing routes that exploit alternative mechanistic scenarios could give unprecedented biaryl structures and expand the portfolio of biaryl applications. We have developed metal-free C–H/C–H couplings of aryl sulfoxides with phenols to afford 2-hydroxy-2′-sulfanylbiaryls. This cascade strategy consists of an interrupted Pummerer reaction and [3,3] sigmatropic rearrangement. Our method enables the synthesis of intriguing aromatic molecules, including oligoarenes, enantioenriched dihetero[8]helicenes, and polyfluorobiaryls. From our successes in aryl sulfoxide/phenol couplings and a deeper understanding of sigmatropic rearrangements for biaryl synthesis, we have established related methods, such as aryl sulfoxide/aniline and aryl iodane/phenol couplings. Overall, our fundamental interests in underexplored reaction mechanisms have led to various methods for accessing important biaryl architectures.
