【目的】本研究の目的は,リンパ浮腫外来における圧迫下の運動療法の実施状況と,その実施が抱える問題点を把握することである。【方法】リンパ浮腫外来を実施している全国のがん診療連携拠点病院を対象に,リンパ浮腫ケアの実践内容,運動療法の実施内容,リハビリテーション(以下,リハビリ)スタッフとの連携の有無,運動療法の実施が抱える問題点についてアンケート調査を行った。【結果】リンパ浮腫外来で運動療法を実施している施設は14.2% であった。運動療法を実施できない問題点として「知識・技術のあるスタッフの不足」,「診療時間の不足」,「連携不足」などが挙げられ,運動療法を実施している施設はリハビリスタッフ数が多かった。【結論】今回の調査結果から,リンパ浮腫外来で運動療法を実施するためには専門的な知識をもったリハビリスタッフを育成,増員する必要があることが示唆された。

昭和46年から47年にMicrosporum canisによる小水疱斑状白癬の3例とケルスス禿瘡の2例を経験した。この菌による小水疱斑状白癬の皮疹の特徴は小指頭大の小型の皮疹が多発し, それらが中心性治癒傾向に乏しいことであるが, 3例のうち1例はその大きさが鶏卵大に及んだ。ケルスス禿瘡は2例とも数個の浅在性頭部白癬を併発した。<BR>同菌による本邦白癬例280余例を数えるが, 関西地方では自験例を含めて最近数年間に19例に達している。自験5例の感染源はペルシャ猫のほか雑犬, 雑猫であったが, 高価な動物ばかりでなく, 雑犬, 雑猫, にも同症が拡っていることは, 今後, 関西地方での症例の増加を強く示唆するものと言える。

保険請求データベース(DB)や電子カルテ DB 等,日常診療の情報が記録されたリアルワールドデータ(real-world data:RWD)は,薬剤疫学研究における重要なデータ源の一つである.日本において,2018年4月より製造販売後調査の新たなカテゴリーとして,医薬品の製造販売後データベース(製販後 DB)調査が追加された.以降,医薬品リスク管理計画(risk management plan:RMP)において製販後 DB 調査が計画され,製販後 DB 調査の実践が期待されているが,現時点で結果公表まで至ったものはほとんどない.一方,海外においては RWD を用いた DB 研究成果は現時点で多数報告されている.海外と日本では,DB 自体の特性(項目・構造等)の違い,医療環境・慣習の違い等を念頭におく必要はあるが,そのような前提を踏まえて海外 DB 調査論文を精読し,研究仮説,研究デザイン,手法等を吟味することは,日本における製販後 DB 調査の計画・実行・結果の解釈を実践するうえで参考価値があると考えた.本報告の目的は,海外 DB 調査論文の批判的吟味を通じて,DB 調査の特徴や注意点を考察すること,そして,日本における製販後DB 調査の実践に役立つ提言を行うことである.本稿が,今後の製販後 DB 調査を計画・実施するうえでの一助になれば幸いである.

River ecosystem is spatiotemporally dynamic under frequent and intensive geomorphic disturbances, characterized by its fluctuating habitat availability. The species inhabiting there adapt themselves to the shifting habitat by developing various life-history strategies. In regulated rivers, however, suppressed disturbance and habitat dynamics threaten the disturbance-dependent communities. The recognition of important geomorphic perspectives is still in its infancy in conservation of river ecosystems in Japan. Riparian tree species have developed life-history strategies to increase the likelihood of arriving regeneration sites, where disturbance regimes are closely linked to the spatial and temporal availability of regeneration sites. River damming and channel modification severely diminish the critical function of geomorphic disturbance sustaining habitat diversity and abundance. Channel migration ceases in flow-regulated rivers, which may temporarily promote expansion of pioneer forest in the active channels. However, new geomorphic surfaces suitable for establishment of seedlings are no longer created because of altered peak flows and sediment supply, thereby threatening the recruitment opportunity and ultimate survival of their populations. Suppression of geomorphic disturbance also leads to the degradation of in-stream fauna. Channel straightening particularly simplifies geomorphic features of a river reach and diminishes longitudinal and lateral habitat diversity in its active channel. Flood control reduces inundation habitat and disrupts hydrologic connectivity between floodplain habitats (e.g., backwater and oxbow lakes), impacting fish and benthic invertebrates that require those habitats in their various life stages. A channel re-meandering experiment in the Shibetsu River, Hokkaido, demonstrates that the recovery of geomorphic dynamics can play significant roles in restoration of river ecosystems. Meandering reaches develop shallow edge habitat with low hydraulic stress along the inside of a convex, providing stable substrate for macroinvertebrate communities. Fallen trees provided by lateral erosion also promote macroinvertebrate colonization and become in-stream covers for fish communities. Geoecology in Japan as well as Europe and U.S. has long focused on vegetation pattern in alpine regions and has unexplored terrestrial and aquatic ecosystems in the vicinity of human activities. Knowledge in habitat dynamics and geomorphic processes is vital to ecosystem conservation and restoration. We believe that participation of geomorphologists is essential in future development of conservation and restoration strategies.

【目的】消化器がん患者の周術期から自宅復帰後における身体運動機能とQuality of Life(以下,QOL)の経時的変化,各時期間の身体運動機能変化と自宅復帰後QOLの関連性について検討すること。【方法】対象は,周術期消化器がん患者42例(男性23例,女性19例,年齢60.6±11.3歳:平均±標準偏差)とした。本研究では,身体運動機能を等尺性膝伸展筋力,Timed "Up and Go" test,6分間歩行距離の3項目,QOL指標にShort-Form 36-Item Health Survey version 2のアキュート版を使用し,手術前,手術後,退院後の3つの時期に各々評価した。【結果】身体運動機能とQOLは,手術後一時的に有意な低下が認められた。退院後では,身体運動機能が手術前と同程度まで向上する一方で,QOLの身体的健康が手術前より有意に低かった。また,手術前後の身体運動機能変化比と退院後のQOLの間に有意な相関関係が認められた。【結論】消化器がん患者の周術期の身体運動機能変化が自宅復帰後のQOLと関連することがあきらかとなった。

わが国の肝移植は,かつてない大きな変革を遂げている.2019年,脳死肝移植は全移植数の1/5を数えるようになった.Allocation制度も大きく改変され,model for end-stage liver disease(MELD)制などが実臨床に大きな影響を与えている.ドナー情報を得られる機会が増加し,高MELDなど重症患者の増加も見込まれる中,脳死肝移植ナショナルデータ解析のプロジェクトもすすんでいる.さらに,働き方改革,互助制度,新型コロナウイルス感染症(COVID-19)など新たな移植のスタイルが成り立とうとしている.

Objective: We measured the formant frequency of vowels in retrusion patients and normal healthy control patients, and made a comparative study of the purpose in phonology. We wanted to know how orthognathic surgery has influenced the articulation of vowels in patients. We investigated how the relationships between the position of the maxilla and mandible and incisor occlusion affected vowel sounds. And because resonance space is believed to affect sound frequencies, we investigated the effect of the width of the airway preoperatively and postoperatively on vowel sounds.Methods: Five vowel sounds in 10 Japanese patients with mandibular retrusion requiring surgery for jaw deformity and malocclusion preoperatively and at 3 and 6 months postoperatively were recorded and compared with those in normal healthy control patients. Also, the effect of the positional relationship between the maxilla and the mandible and incisor occlusion on these vowel sounds was investigated by correlating skeletal and denture patterns of cephalometric measurements taken preoperatively and at 3 months postoperatively with these vowel sounds. The width of the airway was measured preoperatively and 3 months after the operation, and the correlation with the frequency of the vowels was examined.Results: In /e/ F1, /o/ F2-F1, significant differences were observed between the preoperative patients and the control patients. However, these differences disappeared at 3 and 6 months postoperatively.A significant difference was admitted with F1 of /a/ and F1 of /e/ in comparing the preoperative with the postoperative patients 6 months by a rank test with a sign of Wilcoxon.Both preoperatively and postoperatively, positive correlations were observed between F2-F1 the narrow, open vowels /i/ with SNA and SNB. In the preoperative patients, some correlations were observed between F1 the semi-narrow, open vowel /e/ and denture patterns.Significant increases were observed between the preoperative and 3 months postoperative patients in the width of the hypopharyngeal cavity. Many correlations were observed between the change of width of the airway and the vowel frequency, particularly in the posterior tongue vowel /u/. But other vowels had few correlations.Conclusion: A change of occlusion of the front teeth and a change of frame by the mandible correction operation greatly influenced close vowels and half close vowels that were a front tongue vowel, especially the influence of /i/ and /e/ in retrusion patients. It was an interior tongue vowel that the change in the airway influenced the vowel frequency of only /u/.

湿疹におけるかゆみの誘発に関してはヒスタミンが重要なメディエーターと考えられている。従って、外耳道湿疹におけるかゆみの軽減に抗ヒスタミン薬は有用であると予測される。今回、われわれは外耳道湿疹患者 (15歳以上) を無作為選択 (封筒法) により、ステロイド外用剤 (ベタメタゾン・ゲンタマイシン配合剤) 単独治療群 (30例) と第二世代抗ヒスタミン剤である塩酸フェキソフェナジン1201ng/日を併用した治療群 (39例) に分け、1週間の治療後に外耳道所見、ならびに自覚症状 (かゆみスコアにて評価) の改善度を比較し、塩酸フェキソフェナジンの外耳道湿疹における有用性を検討した。治療終了時における外耳道所見の改善度ではステロイド外用剤単独治療群と塩酸フェキソフェナジンを併用した群の間に有意差は認められなかったが、自覚症状 (かゆみ) においては併用群に有意に高い改善効果を認めた (p<0.05)。外耳道湿疹におけるかゆみに対して塩酸フェキソフェナジンの有意な改善効果が示されたことになり、外耳道湿疹におけるこの薬の有用性が示唆された。

In view of increasing trend of applying water cooling to electric machines, investigation has been made on corrosion of the copper pipe that is comprised in the internally water cooled power cable. Practical tests on corrosion of the copper pipe by flowing pure water were carried out using actual power cable under practical use conditions with regard to conductivity, temperature, and oxygen content of water, and flow velocity. The ranges of flow velocity were chosen to be much wider than could be encountered in the actual cooling system. Corrosion quantity of the copper pipe was determined from analyzed data on ion exchange resin and filter. The result of two times corrosion tests made about one year, in state where water temperature are below 80&deg;C and flow velocity are below 3m/s, are as fellows. (1) Corrosion rate of the copper pipe reached 2&mu;/year, but the traces of pitting corrosion was not detected. (2) By electrolytic reduction, predominant component of black corrosion products on the copper pipe surface was identified with CuO. (3) Corrosion products were found to have considerable effect on stifling corrosion rate.

【目的】柿の葉にはポリフェノール類が多く含まれ,その抗菌性や抗酸化性等が注目されている。古くから柿葉は柿の葉寿司や柿茶などに用いられてきたが,機能性に関する研究は未だ十分ではない。これまでの研究から柿の葉には&alpha;-アミラーゼ阻害活性が認められており,このことは,柿の葉に血糖値を抑制する働きがある可能性を示唆している。また,一部のヒト試験でもその傾向は認められている。そこで今回は柿の葉茶の血糖値抑制効果をさらに詳しく調べるために,食事性肥満マウスを用いて実験を行った。<br>【方法】B6J DIOマウス♂(8週齢)を1週間の予備飼育後,体重・血糖値を基に2群に分け,柿茶投与群には柿茶抽出濃縮物の0.5%溶液を,対照群には水道水を,飼料は固形の高脂肪飼料D12492 60kcal%Fat をいずれも自由摂取させた。なお,体重,飼料摂取量,飲水量は経日測定し,血糖値は1週間ごとに尾静脈より採血してメディセーフミニ(TERUMO社)にて測定した。柿茶投与6週間後に,18時間絶食後,ゾンデを用いてグルコース負荷試験を行い,屠殺・採血後,解剖した。膵臓,肝臓,腎臓については組織標本を作製し,観察した。また,HbA1c をDCAバンテージ(SIEMENS社)により,血漿インスリンをMercodia社 マウスインスリンELISAキットを用いて測定した。なお本実験は昭和女子大学動物実験委員会の承認を得て,規定を遵守して実施した。<br>【結果】非空腹時血糖値及び,グルコース負荷試験時の血糖値,HbA1c,血漿インスリン濃度は,柿茶群と対照群に大きな差は認められなかったが,柿茶濃縮物投与群は対照群に比べてグルコース負荷試験における血糖値曲線下面積が低値となる傾向が認められた。血糖値抑制効果の可能性が示唆された。

前縦隔に異所性甲状腺癌が発生した犬に外科手術を行った3例を経験した。3症例はCT検査を行い、他臓器への浸潤、転移、胸水を認めなかったため、細胞診、病理組織検査後、外科手術を行った。術後、症例1、3はそれぞれ1,050、1,420日経過しているが、再発転移なく良好に経過している。また、症例2は術後2,925日に腫瘍とは関連なく死亡した。症例の集積による検討が必要ではあるが、前縦隔に発生した異所性甲状腺癌は、他臓器に浸潤や転移がない場合、外科手術を行うことで良好な予後が得られる可能性が考えられた。
