症例は50歳の男性。視力障害,意識混濁により医療機関を受診した。当初心房細動,大脳基底核の両側対称性病変,視力障害からtop of the basilar syndromeを疑われ,脳血管造影を施行したが否定された。その後昏睡状態に陥り,CT,MRI上の両側対称性の被殻病変,重度の代謝性アシドーシスの存在から,メタノール中毒が疑われた。集中治療室へ入院し,気管挿管,血液浄化療法,エタノール投与,活性型葉酸投与などを行った。意識は回復したものの,ほぼ全盲であり見当識障害が残存した。その後妄想性障害のため精神病院へ転院となった。メタノール中毒において,治療の遅れは重篤な後遺障害につながる。メタノールの血中濃度は迅速に測定することができないため,臨床症状,血清浸透圧較差,CT及びMRIの特徴的な病変から疑い,迅速に治療を開始する必要がある。

67歳の男性が結婚40年後にC型急性肝炎を発症した.その妻(68歳)がC型慢性肝炎患者であり,C型肝炎ウイルス(HCV)RNAが高力価陽性(>5,000 KIU/ml)であった.両者のHCV遺伝子型はともに1b型で,NS5B領域の1,087塩基長の配列において99.7%の一致率を示した.それに対し,これまでに報告されているHCV/1b株との一致率は最高でも96.7%に過ぎなかった.分子系統樹解析によっても,夫婦の持つHCV株は一つのクラスター(bootstrap値:100%)を形成し,同一株である可能性が強く示唆された.詳細な病歴聴取を行ったが,性交渉(月1, 2回)以外の感染経路はいずれも否定された.高齢夫婦間の感染には加齢に伴う生体側因子が関与していると考えられるが,高齢化社会を迎え,HCV感染者頻度の高い高齢者層でかかるHCV感染が起こる可能性を念頭に置く必要があると考え報告する.

We present three cases in whom mental disorders appeared after orthognathic surgery.The first case was a 35-year-old female who had a history of depression. After the operation, a manic state appeared and she was diagnosed with bipolar disorder. Her medicine was changed from an antidepressant to a mood stabilizer, after which her mental condition stabilized.The second case was a 34-year-old male. He could not accept the appearance of his postoperative face, and it took about three months for him to finally accept his new appearance.The final case was a 37-year-old female. She could not accept the appearance of her postoperative face because it was not what she had expected. She began to complain about her face and became depressive, and was finally diagnosed with major depressive disorder. After administration of an antidepressant, her mental condition gradually improved.We must take into consideration the possibility that patients will develop mental disorders after orthognathic surgery, and adequate explanation and patient assessment before the operation are therefore important.

We previously reported the association of the estimated glomerular filtration rate (eGFRcreat) calculated from the serum creatinine level (S-Cr) measured using the Jaffe method with the GFR (eGFRcys) estimated from the serum cystatin C level (CysC). However, few studies have compared the eGFRcreat using the enzymatic method with the eGFRcys. It is unclear whether there are differences in the results of renal function assessment. The purpose of this study was to compare the eGFRcreat calculated from the S-Cr with the eGFRcys calculated from the CysC in patients in whom the S-Cr and CysC were simultaneously measured using the enzymatic method, examine the correlations of respective parameters, and clarify physiological factors involved in differences among the parameters. The subjects were 1334 patients treated in 5 institutions. The mean values and correlation coefficient were statistically analyzed using Student's t-test and Pearson's test, respectively. Influential factors between formulae were analyzed using multiple regression analysis. The mean eGFRcreat was 67.0 mL/min/1.73 m2, being significantly higher than the mean eGFRcys (63.2). Multiple regression analysis showed that factors influencing differences in the S-Cr and CysC included the sex, age, serum albumin, and blood urea nitrogen BUN/S-Cr. Furthermore, factors involved in the overestimation of the eGFRcreat in comparison with the eGFRcys included the serum albumin and BUN/S-Cr. The differences between the eGFRcreat calculated from the S-Cr and eGFRcys were less marked than when adopting the Jaffe method in our previous study. However, the eGFRcreat were higher than the eGFRcys in patients with malnutrition or dehydration.

「ザイ」は鬼剣舞の中で演じられる特徴的な動作のひとつである。これは踊り手個人が任意でする動作であり、「振付け」の範疇として習うものではないとされている。従って習得していく過程において振付けと同レベルで教えられるものではなく、ザイを切るタイミングは大体決まってはいるが、決められたタイミングで全員でしなければならないという決まりもない。あくまで踊りの型がからだに入った者が、それからさらに踊りを高度な次元で表現する術としての、「わざ」の領域の動作なのである。本稿では、「基本」と「わざ」の間にある関係性について、舞踊人類学的な調査に基づく質的分析と情報学的な動作計測に基づく量的な分析の両面からの考察を行う。

乳頭癌488例の治療成績をStage別に調べ,日本の現状における乳頭癌の初期治療は如何にあるべきかについて検討した。StageⅠ,Ⅱにおける再発例は21例(7%)であり,原病死したものは1例(0.3%)であった。一方StageⅣではⅣcを除いた84例中44例(52.4%)が再発を起こし,原病死したものはStageⅣ全体で 29例(27.6%)に及んでいた。StageⅢはStageⅠ,ⅡとStageⅣの中間的なものであった。再発例の転帰を調べたところStageⅠ,Ⅱでは再発がみられても再手術などによりsalvageが可能であることが多いのに対し,StageⅣでは再発例の多くは生命予後に直接影響を与えるものと考えられた。以上よりStageⅡ以下では初診時より遠隔転移を認めるもの(M1),明らか周囲臓器へ浸潤しているもの(EX2),大きなリンパ節を触れるものなどを除き(準)全摘は必要なく片葉切除などで十分と考えられた。一方StageⅢ,Ⅳでは(準)全摘は必要と考えられたが,特にStageⅣでは遠隔転移が多く,生命予後も不良であることより131Ⅰアブレーションを可能な限り追加すべきと思われた。

Peripheral facial nerve palsy following orthognathic surgery is a rare complication. We discuss the incidence of this complication, its possible causes, subsequent treatment and ultimate outcomes.A total of 910 patients underwent orthognathic surgery in our department during the period from 2001 to 2015. Five (0.55%) of those patients had peripheral facial nerve palsy postoperatively. The postoperative day when facial nerve palsy occurred ranged from day 1 to day 13. All of the cases were unilateral and included lip motility disturbance, difficulty in closing the eyes and inability to wrinkle the forehead. All of the patients received medication with an adrenocortical steroid and vitamin B12. Stellate ganglion block and physical therapy were also used for the patients who had moderate to severe facial nerve palsy. Complete recovery was achieved in all but one of the patients, in whom slight palsy remained. There were several possible causes of facial nerve palsy including abnormal bleeding, postoperative swelling, and handling of operating instruments.In conclusion, peripheral facial nerve palsy after orthognathic surgery is relatively rare. If it occurs, however, complete recovery can be expected in most cases.

近年,リニア中央新幹線の名古屋以西の建設ルートに関して活発な議論が行われている。伊勢湾地域(愛知県,三重県)では,三重県の亀山市周辺に新駅設置が噂されているが,最も適切であるのか疑問が残る。本研究では,リニア中央新幹線の新駅を,中部国際空港(セントレア)と鈴鹿市周辺に建設することを提案する。セントレアと鈴鹿市を経由することで,伊勢湾地域に限らず,リニア中央新幹線沿線住民の利便性の向上や,経済効果が期待されることを説明する。また,本研究の提案が実現すれば,成田および羽田空港の過密解消や,首都圏や関西圏の災害時の旅客輸送のバックアップにも貢献するため,国策として検討する価値があることを主張する。

ロドプシンファミリーは多様なメンバー (オプシン) から構成され, 視覚やそれ以外の様々な光生理現象に関与する。筆者はこれまでに様々なオプシンの解析を通じて, 動物の光受容細胞の機能と進化に関する研究を行ってきた。本稿では筆者らの行った以下の研究について紹介する。(1) ヤモリの桿体に含まれる視物質が錐体タイプであることを明らかにし, ヤモリ桿体が錐体から進化したとする仮説を検証した。(2) ホタテガイの繊毛型視細胞において新しい光情報伝達系を発見し, 視細胞の光情報伝達経路は脊椎動物と節足動物の分岐以前にすでに多様化していたことを明らかにした。(3) ゼブラフィッシュの脳内において2種類の新規オプシン (非視覚型オプシン) を同定し, その一つ (エクソロドプシン) は松果体, もう一つ (VALオプシン) は脳深部と網膜の二次ニューロンに存在することを示した。また, エクソロドプシン遺伝子プロモータの機能解析を行い, 松果体特異的なシス配列の同定に初めて成功した。
