【目的】高齢患者を対象に頭部屈曲運動を実施し,嚥下能力に与える影響を検討することを目的とした.【方法】対象は,嚥下障害のない65 歳以上の患者70 名とした.除外基準は,頭部屈曲運動に対するリスクを呈する者,口頭指示が理解できない者とした.入院順に介入群と対照群を交互に割り付ける準ランダム化比較試験を実施した.介入群の介入内容は,背臥位での頭部屈曲反復運動を1 日に30 回×3 セット,2 週間実施した.対照群の課題は通常のリハビリテーションのみとし,嚥下能力,機能への介入はしないとした.主要アウトカム指標を3 回唾液嚥下積算時間とし,副次的アウトカム指標を舌圧,開口力,General oral health assessment index(以下,GOHAI)とした.介入群,対照群のアウトカム指標の介入前後の値の変化を以下のように検討した.正規性が認められた場合は,二要因(時間×群)の反復測定による分散分析および対応のあるt検定(Bonferroni 補正)で検討した.正規性が認められなかった場合は,各群の前後の差をWilcoxon の符号付順位和検定(Bonferroni 補正)にて検討した.有意水準は5% とした.【結果】介入群35 名,対照群35 名となった.各群の基本属性,アウトカム指標はすべての項目において有意な差を認めなかった.3 回唾液嚥下積算時間は介入群にのみ有意な減少を認めた.舌圧は介入群にのみ有意な増加を認めた.開口力は介入群のみ有意な増加を認めた.GOHAI は,両群とも有意な差を認めなかった.【考察】頭部屈曲運動は嚥下能力を向上させる効果的な運動方法であることが示され,嚥下障害を呈する高齢患者へ応用可能な有益な情報になることが示唆された.

Chrysanthemum morifolium is one of the most popular ornamental plants in the world. However, as C. morifolium is a segmental hexaploid, self-incompatible, and has a sizable heterologous genome, it is difficult to modify its trait systematically. Genome editing technology is one of the attractive methods for modifying traits systematically. For the commercial use of genetically modified C. morifolium, rigorous stabilization of its quality is essential. This trait stability can be achieved by avoiding further genome modification after suitable trait modification by genome editing. Since C. morifolium is a vegetatively propagated plant, an approach for removing genome editing tools is required. In this study, we attempted to use the piggyBac transposon system to remove specific DNA sequences from the C. morifolium genome. Using the luminescence as a visible marker, we demonstrated that inoculation of Agrobacterium harboring hyperactive piggyBac transposase removes inserted 2.6 kb DNA, which harbors piggyBac recognition sequences, from the modified Eluc sequence.

Members of the genus Rhinogobius are widely distributed in Far East Asia, at least 11 species having been recognized in Japan. Of the six species recorded from Okinawa Island, two are land-locked (“aobara-yoshinobori” R. sp.BB and “kibara-yoshinobori” R. sp.YB) and the remainder amphidromous. Embryonic development and morphological changes in the two land-locked species are described from artifically-reared eggs, larvae and juveniles. Survival and growth of the two species were compared under different salinities (0-35%0) dur-ing a 21-day period following hatching. Although egg diameters of both species were almost the same (“aobara-yoshinobori”; 4.3±0.1 mm, “kibara-yoshinobori”; 4.3±0.2 mm), newly-hatched larval morphology, and larval and juvenile salinity tolerance clearly differed. Newly-hatched larvae of “aobara-yoshinobori” had a flexed notochord and caudal fin rays already developed. The yolk was completely consumed 3 days after hatching. By comparison, “kibara-yoshinobori” hatched at an ealier developmental stage, having a straight notochord. The yolk disappeared 7 days after hatching. Larvae and juveniles of “aobara-yoshinobori” were less toler-ant in high salinity than those of “kibara-yoshinobori”. The results suggested that the two land-locked gobies have become adapted to a wholly fluvial life cycle, through somewhat different strategies.

[目的]超音波画像計測装置を用いて測定した大腿前面の筋厚が,膝関節伸展筋力を含めた身体機能と関係しているのか,また筋厚から膝関節伸展筋力の推定が可能かを調査した。[対象]地域在住の女性高齢者98名とした(平均年齢72.7 ± 5.4歳)。[方法]対象者の大腿前面の筋厚を超音波画像計測装置にて計測した。また身体機能として,等尺性の膝関節伸展筋力(N),握力(kg),通常歩行速度(m/分),開眼片足立ち時間(秒),下肢のしなやか度を測定した。[結果]大腿前面の筋厚は膝関節伸展筋力,握力,開眼片足立ち時間と有意な正の相関が認められた。一方,重回帰分析の結果では,膝関節伸展筋力を予測する因子としては,大腿前面筋厚の寄与率は低いことがわかった。[結語]超音波画像計測装置で計測した大腿前面の筋厚は,地域在住女性高齢者の身体機能を反映する指標とはなりうるものの,膝関節伸展筋力を推定するためにはさらに検討が必要と考えられた。

本論文は、日本での手話歌をめぐる対立の背景にある音楽に関する問題領域、すなわち「聾者にとって歌とは何か」「それは聴者とどのように異なるのか」という問いについて、聾者カテゴリー内の多様性(先天性/中途失聴/難聴)も踏まえながら理論的に論じるものである。本稿で注目するのは、手話における身体運動のラインである。文化人類学者ティム・インゴルドによるラインと知覚に関する議論を参照し、彼の「全感覚的な知覚経験の中で環境と呼応するライン生成過程」という視座を導入することで、聾歌を「身体運動ライン生成過程そのもの志向」としてとらえる可能性を提示する。そしてこうしたライン生成への視点の欠如こそが手話歌の音楽的問題を招いていると結論づける。

Electronic display devices such as iPad and Kindle have recently attracted considerable attention as, reading media. Although the display performance and, business model concerning the contents have been discussed, there has been little discussion on comfortability, and - kansei like readability. In this paper, we conducted task performance investigation and impression evaluation among electrophoretic displays, liquid crystal displays and paper in order to examine readability for electronic displays, especially electrophoretic displays. In the task performance investigation, the reading speed, error detection rate and comprehension were measured. In the impression evaluation, the impression of the display, reading media and sense of fatigue were examined. As a result of task investigation, it was shown that the electrophoretic display was more comprehensible than that of the liquid crystal display. In the impression evaluation, the following results were obtained; 1) Although the display properties for the electrophoretic display almost passed muster, there was room for improvement in the beauty of the display. 2) The electrophoretic display was easier to read and concentrate on than the liquid crystal display. 3) The electrophoretic display was less likely to cause eyestrain and stiff-neck than other media. Principal component analysis revealed that the synthetic readability for the electrophoretic display was superior to the liquid crystal display, and moreover, the novelty of the electrophoretic display was superior to that of the other media.

児童虐待の予防対策の充実は喫緊の課題となっているが、そのために必要な虐待の前段階を含む連続的な虐待の評価指標はこれまで開発されていない。このため本研究では、母親の虐待的行為の項目数と頻度からなる得点について、虐待の前段階を含む虐待評価指標としての妥当性を検討することを目的とした。本研究では、3-6歳の子どもを持つ母親を対象としてアンケート調査を実施し、虐待的行為得点と虐待の強力なリスク要因である社会経済的状況並びに育児感情との関連性を分析した。結果、虐待的行為得点は、十分な内的整合性を有するとともに、母子家庭、世帯収入の低さ、母親の低学歴と関連することが示された。また、虐待的行為得点は、育児負担感、育児不安と関係することが示された。したがって、本得点は、虐待の前段階を含む連続的な虐待の実態を把握するための指標として、一定の妥当性を有することが示唆された。

Considerable research is being directed toward active debris removal (ADR) in space, especially by small, cost-effective satellites. In the descent phase, thrust vector control (TVC) is often utilized to maintain the satellite’s trajectory and thrust vector by gimbaling the thrust vector. A common challenge in performing TVC is that its effectiveness depends on the mass, moment of inertia and location of the thrusting point with respect to the center of mass. Since these properties change with fuel consumed during a mission, a designer needs to consider such parameters as they strongly affect the resulting dynamics. This paper introduces spacecraft dynamics with a gimbaled thruster and proposes an adaptive controller for TVC with model uncertainties. Through numerical examples, the effectiveness of the proposed controller is demonstrated for improved control performance.

In tensile testing, polycrystalline materials generally fail at relatively low total elongations but under some limited conditions it is possible to achieve exceptionally high elongations of up to and exceeding 400%. This superplastic condition is important scientifically but also it has important uses through the industrial development of superplastic forming operations. This overview traces the historical development of this superplastic effect and it provides a summary of the main characteristics of the superplastic flow process. Conventional thermomechanical processing is not able to produce exceptionally small grain sizes within the submicrometer or nanometer range but this limitation was effectively overcome through processing using severe plastic deformation (SPD). The advantages of SPD processing are discussed and examples are presented. Finally it is demonstrated that the experimental data may be easily and effectively displayed through the construction of deformation mechanism maps based on combinations of stress, grain size and temperature.

微気圧変化による津波の発生および規模の検知の可能性を探るために,まず平常時の微気圧変動観測を新潟大学の建物屋上で行った.観測は,微気圧変化,風向・風速および雨量の同時観測を行った.観測の結果,微気圧変動は降雨や寒冷気流の降下等によって発生していることが分った.津波発生・伝搬に伴う微気圧変動の3次元数値解析では,津波の初期水位の上昇領域では正の微気圧波が,水位の下降領域では負の微気圧変動が発生することが分った.また,微気圧変動が津波よりも速い速度で広範囲に伝搬する様子が再現された.さらに,地殻変動時間が短いほど強い微気圧波が発生する結果が得られた.このことにより,微気圧変動の観測により津波の発生の有無および規模を推定できることが分った.

術中にアナフィラキシーを生じた肝門部胆管癌症例を経験した.アレルゲンは術中には確定できなかったが,後日行った薬剤リンパ球刺激試験でタココンブ®と判明した.タココンブを貼付したまま閉腹したためか,術後もアドレナリンの持続投与が必要な状態が続き,約20時間後にアドレナリンを離脱できた.血漿中ヒスタミンとトリプターゼの定量測定では,両者とも検出できず,これらを介さない機序でのアナフィラキシーを生じたものと考えられた.

本研究は,美顔器使用による顔面の機能や形態的な変化について解明することを目 的として行った。対象は,発症後1年以上経過した慢性期脳卒中片麻痺患者6名であった。 運動機能評価として口を閉じる筋力とまばたき回数を計測し,感覚機能評価として触圧覚 閾値と二点識別覚を測定した。また,同時に顔面の形態的変化を画像解析により評価した。 美顔器を使用しない期間にみられる各測定項目の変化量をコントロールデータとし,その後の美顔器介入期間にみられる変化量と比較した。美顔器プログラムは1試行9分間,朝夕1日2回行わせた。その結果,コントロール期間の変化量と比較して,介入後の触圧覚閾値や2点識別覚の間隔距離の変化量が有意に減少した。しかし,運動機能に関しては有意な変化は認められなかった。一方で,形態的な顔面の左右差は介入期間後に減少した。 本研究結果は,美顔器の刺激が顔面の感覚機能や形態の変化に影響することを示唆した。

背景.ノカルジア感染を合併した囊胞状気管支拡張を伴う肺分画症の1例を報告する.症例.46歳,男性.感染性肺囊胞の診断で紹介となる.喀痰のグラム染色でノカルジアを検出し,肺ノカルジア症の診断で,2年間加療するも症状の改善,増悪を繰り返した.右下葉に病変が限局することより切除の方針とした.術前検査の造影CTで下行大動脈から右下葉に流入する異常血管を認めた.これにより,ノカルジア感染を合併した肺分画症の診断に至った.胸腔鏡下右下葉切除を施行したところ,呼吸器症状は改善し,術後5年経過したが症状の再燃は認めない.結語.ノカルジア感染症は日和見感染症の一つであるが,肺の器質的異常もリスク因子と報告されている.ノカルジアの分画肺への感染はとても希である.しかし,難治性のノカルジア感染が下葉縦隔側に限局する場合には,肺分画症を念頭に置いて積極的に造影CT検査を行うべきと考える.
