本稿のテーマは,ライフストーリーがもつ時間と空間の基本的な枠組みを示すことである.語りの古典的モデルは,標準時間にもとづくクロノロジー的編成と語りが直接リアリティを指示しているという考え方にたっていた.それに対して,P. Ricoeurは語りの結末からの筋立てによるストーリーの時間のW及的特質を指摘した.これらは語りの時間の近代的秩序を表している.また,語りにはさまざまな社会的空間に応じたモードがある.それらは「制度的モード」「集合的モード」「パーソナル・モード」であって,語りの空間的枠組みを表している.ところが,こうしたライフストーリーの時空間の枠組みにあてはまらない〈中断された語り〉や〈ストーリーのない語り〉などの脱近代に特徴的な語りが登場してきている.ライフストーリー・インタビューにおいては,どのようなモードの語りであるかに注意を払いながら,インタビューにおける相互行為をリフレクシブ(reflexive)にとらえ返すことで脱近代に特徴的な語りを聞くことが求められている.

脳卒中片麻痺患者において,長下肢装具の使用は重要である.しかし,種々の理由により長下肢装具の処方を躊躇することがある.そこで,短下肢装具へ容易に変更可能な長下肢装具があれば,用途に応じた使用が可能になると考え本装具を作製した.本装具を症例に使用したところ,訓練中は長下肢装具と日常生活は短下肢装具での使い分けが可能となり,日常生活が車椅子で自立した.また,麻痺の状態に応じて長下肢装具,短下肢装具の使い分けも可能だったため,カットダウンがスムーズに行え,短下肢装具での退院が可能となった.

The purpose of this paper is to discuss close friendship from social, cultural, and psychological viewpoints. The definition of friend is made by Brain R (1976) as follows: person who is connected by means of affection or dearness, not by blood relation or Eros. In order to consider the meaning of close friendship, we referred to the research of comparative anthropology of Brain. Through his study, we could see how close friendship worked in some culture and society, especially in some tribes in Africa where friendship played very important role. Historically, friendship was formed as a social system in the Middle Ages in Europe. Moreover, trust and affection between friends are emphasized in almost all culture. Thus, friendship is regarded as a fundamental aspect of human nature. Friendship is a relationship which has equality and complement nature, and it has played the role of exchange in some social systems. Also, some societies regard friendship as important as kinship, because it proceeds economical and social negotiation outside the kinship system. Several examples are shown in this paper. In order to see friendship from a psychological point of view, the image of twins was proposed in connection with close friendship. In some society, a close friend is recognized as a substitute for twin, which represents half of one’s soul. The image of twin-ship represents “a pair of souls”, or “the other self.” Brain says that the fundamental aspect of friendship is equality, the form of a pair in soul, and “the other self.” So he thinks the image of twins is connected with friendship. Finally, we discussed gender difference in close friendship, and close friendship between the sexes. Men and women can form close friendship with each other. Equality, or the basic nature of friendship, enables men and women to communicate with each other apart from their sexual drives.

大宮台地の西縁部では,荒川の河川敷に存在する沖積土を台地上の火山灰土畑に運びこむ「ドロツケ」という客土作業が続けられていた.本研究は,ドロツケによる人為的土壌生成過程における土壌特性の変化を明らかにすることを目的として,長年のドロツケにより,沖積土が元来の土壌の上に厚く堆積した埼玉県北本市の圃場の土壌,台地上の火山灰土,および河川敷沖積土において,土壌断面調査と土壌理化学性の分析を行い相互に比較した.ドロツケにより,土壌の固相部が増大し,最大容水量および水分含量が減少した.沖積土中のAl_oおよびSi_o含量は,火山灰土の1/10以下であった.この沖積土の客土により,ドロツケ畑では客土層上層ほどリン酸吸収係数が減少し,可溶性無機態リン酸の内のCa型リン酸の割合,有効態リン酸量が増大した.沖積土には多量の交換性Caが含まれることにより,ドロツケ畑にCaが供給され,土壌pHも上昇した.ただし,ドロツケ畑では,Caの溶脱傾向が著しく,とくに,作土層では,土壌pHが低い値を示した.客土層の厚さと乾燥密度ならびにSi_oの分析結果から,この圃場への客土投入量は,1ha当たり5000t程度と試算された.

大動脈弁狭窄症に対する経カテーテル的大動脈弁植込術(TAVI)を透析用シャント肢と同側の鎖骨下動脈アプローチを用いて行った4例を報告する.症例は70~79歳,左上肢に透析用シャントが作成されていた.いずれも大動脈弁置換術は髙リスク(STSスコア6.292~12.624)と考えられ,TAVIを行うこととなった.腸骨動脈の狭窄・石灰化もしくは総大腿動脈の高度石灰化のため,左鎖骨下動脈アプローチを選択した.術前大動脈弁圧較差は30.2~44.2mmHgであったが,術後3.0~5.8mmHgへと減少した.いずれの症例でも術後透析は問題なく行うことができ,シャント不全は認めなかった.透析患者では末梢血管疾患の合併が多く,大腿動脈アプローチが困難であることが少なくない.しかし,透析用シャント肢と同側の鎖骨下動脈アプローチの安全性について十分なデータは存在しない.自験例では術後透析経過に問題なく,TAVI時のアプローチサイトとして安全に使用できる可能性が示唆された.

Glucokinase is a glycolytic enzyme that catalyzes the phosphorylation of glucose to glucose-6-phospate in the first step of the glycolytic pathway. It also regulates the threshold for insulin secretion from pancreatic beta cells by catalyzing the phosphorylation of glucose and plays an important role as a glucose sensor. Pathogenic variants in the glucokinase gene (GCK) cause non-progressive but persistent mild fasting hyperglycemia, also recognized as maturity-onset diabetes of the young 2 (MODY2). This report presents the case of two Japanese siblings with MODY2, who were initially diagnosed with impaired glucose intolerance at 20 and 17 years of age, and later developed diabetes mellitus. They had no history of obesity, were negative for islet-related autoantibodies and their serum C-peptide level were within the normal range. Diabetic complications were not observed. Next-generation sequencing revealed a novel heterozygous variant in GCK (NM_000162.5: c.1088A>G, p.Asp363Gly) in both siblings. This variant has not been reported previously. In silico functional analyses, using SIFT and MutationTaster, suggested that the variant was damaging. To confirm the functional impact of the mutated GCK, the HiBiT-tagged p.Asp363Gly variant and the wild-type GCK were transiently expressed in HEK293T cells. The cells expressing the variant GCK exhibited 79% less bioluminescence, compared to those expressing the wild-type GCK, suggesting that the pathophysiology of the variant was a result of haploinsufficiency.

私は一般枠での正社員の転職を7回経験し,8回目の転職で障害者枠での職を手にし,現在に至る自閉症の当事者である.19年間一般枠にこだわって,システムエンジニア,経理職などに就いたが,周囲との人間関係が上手くいかず退職に追い込まれ転職を繰り返した.はじめに,一般枠で就職してから退職に至るまでの経緯について,産業医やキャリアカウンセラーとの相談,カミングアウトにより上司や同僚から受けたパワーハラスメントの実情も含めて実体験を記載した.その次に,障害者枠による現在の職場で得られている合理的配慮について,実際に起こったトラブルと会社側の支援への動きについて述べた.最後に,一般枠と障害者枠に関する,就労のメリットやデメリットをそれぞれの支援体制の違いに着目してまとめ,継続就労を実現するための職業上の課題と対処方法について考察した.そして,ダイバーシティ&インクルージョンに期待を寄せて結びとした.

2008,2011,2014,2016年夏季に経験したヒトパレコウイルス3型感染に伴う成人の筋痛症17症例(男性14例,女性3例)について検討した.年齢は21歳から50歳.全例,四肢に強度の筋痛,筋力低下,握力低下を認めた.14例(82%)に発熱,8例(47%)に上気道炎症状,4例(24%)に胃腸炎症状,男性4例(男性の29%)に陰部痛を認めた.血清CKが1例を除き上昇していた.骨格筋MRIは5例中2例に大腿筋に異常信号を認めた.神経伝導検査は9例中5例でF波の誘発が不良だった.7例で同時期に家族内の乳幼児に発熱,感冒様症状があり家族内感染が疑われた.全例1~2週間で軽快した.
