当院でTPNに関連したと考えられた肝障害が認められた55名を対象とした。TPN開始から肝障害発症までの期間は3日まで6名、7日まで19名、14日まで12名、21日まで6名、28日まで4名、28日以降8名であった。グルコース投与速度は3mg/kg/min未満が16名、3~5 mg/kg/minが21名、5mg/kg/min以上が18名であった。脂肪製剤を14名で使用し、血液培養の結果から原因菌が同定された感染症合併は12名であった。肝胆道系酵素の最大値はAST:160±160 IU/L、ALT:162±169 IU/L、ALP:618±299 IU/L、γ-GTP:230±348 IU/Lで、肝障害の分類では混合型39名、肝障害型8名、胆汁うっ滞型8名であった。輸液内容変更のみの15名中14名が肝機能改善し、1名のみに改善が認められなかった。輸液内容の変更と経口・経腸栄養を開始した18名全員で改善が認められた。輸液内容を変更しなかった22名のうち経腸栄養を開始した2名で改善が認められた。また、内容を変更せずそのまま輸液を継続した20名中10名で改善が認められた。

形態シソ(perilla nankinensis Decne)とまったく同一であるにかかわらずシソ臭を有せず,エゴマ(P. frutescens Brit.)と同じ臭気を有するものがしばしば存在する。 このものの精油を検索すると,收油率生草の0.03~0.05%,油分はマツタケアルコール(1-オクテン-3-オ-ル) 6.8~7.1%,リナロール7.9~8.4%,エルショルチアケトン8.8~10.4%,ツワブキ酸1.5~3.0%,ナギナタケトン48.6~50.8%,その他の成分23.0~23.7%からなることがわかった。 このものはおそらくシソとエゴマとの雑種であろうと考える。

Objective: Recurrent laryngeal nerve (RLN) paralysis was previously believed to be a major cause of dysphagia after esophageal cancer surgery. However, reports from recent years have indicated that dysphagia may be caused by decreased laryngeal elevation due to cervical lymph-node dissection (LND). For this reason, we studied whether a relation exists between postoperative decrease in geniohyoid muscle mass and postoperative dysphagia in patients treated for thoracic esophageal cancer.Methods and Results: Our study was retrospective and included 54 patients who underwent surgery for esophageal cancer at our hospital between April 2014 and August 2018. Computed tomography (CT) had been performed on postoperative days (POD) 5–8 and laryngeal video fluoroscopy on POD 7. The patients were divided between those with and without dysphagia and those with and without preoperative sarcopenia, and clinical variables were compared between the patient groups. The dysphagia group (n=12) had significantly lower postoperative prealbumin (PA) values (18±7 vs. 22±6; P<0.05) than the non-dysphagia group (n=42). Three-region cervical LND was performed in a greater percentage of patients in the dysphagia group than in the non-dysphagia group (9/3 (75.0%) vs 15/27 (35.7%); P<0.05). In addition, decreases in the psoas major and geniohyoid muscle cross-sectional areas were significantly less in the dysphagia group 93.0±5.1% vs. 98.4±8.3%; P<0.05 and 77.5±11.3% vs. 88.2±16.5%; P<0.05, respectively). The cross-sectional area of the geniohyoid muscle was significantly smaller in patients with preoperative sarcopenia than in those without sarcopenia 82.8±11.1% vs. 90.6±21.1%; P<0.05).Conclusion: Our findings suggest that a postoperative decrease in geniohyoid muscle mass causes the dysphagia seen in patients after esophageal cancer surgery. In addition, dysphagia may occur more readily in patients with pre-existing sarcopenia.

Almost all the people of any village or town in Nagano-ken in Japan imagine that the rainbow rises from the water. " Where does the rainbow rise from ? " Ask the question, and one almost always receive such an answer that the rainbow rises from a pond, a marsh, a river, depths, a lake, or the sea. Sometimes people tel・1 him even the name of a special and definite pond or river from which the rainbow rises. Such a traditiona] belief among people that the rainbow rises from the water is found not only in Nagano-ken, but also in Niigata-ken, Yamanashi-ken, and Chiba-ken, and even in such a far northern district as Akita-ken, according to the author's information. It can also be found in Ishikawa・ken, Fukui-ken, the urban districts of Ky6to, Hy6go-ken, Kyti-shiu, and even in Okinawa Isles. In short, this traditoinal belief that the rainbow rises from the water may be considered to be wide spread among people all over Japan. And moreover it may have exsited a'mong people since ancient times. One can find the evidence of its existence in such classics as Kojiki, Nihonshoki, and Mannyo. The documents of the Heian Dynasty show that it also exsited in that age. The origin of this traditional belief can be explained by an ancient, belief that the rainbow is a dragon or serpent that lives in the water. The ancient be]ief even now remains evidently in such prefectures as Nagano-ken, Yamanashi-ken, Akita-ken, and dita-ken, and also in Okinawa Isles. This queer ancient belief that the rainbow is a dragon or serpent which rises from its dwelling place in the water is found not only among Japanese, but also among the Ainu, Koreans, Chinese, the aborigines of Formosa, Malayans, Rumanians, and lrishmen in Europe. Africans, Austrarians, and the natives in America, in so far as it has been known to the author till now. It may be concluded that all the people in the world, perhaps, had once the ancient belief. There was another belief from old time that the rainbow was a bridge from the heaven to the earth over which various divine beings passed. This was believed not only in Japan, China, and Korea, but also among American Indians and many of Europeans. It was also a world-wide traditional belief. There was one more belief that gold, treasures, and good luck were hidden at the place from which the rainbow rose. This belief was probably world-wide from old timesand seems to have some close relation with a strange Japanese custom in the Middle Age that a fair was held at the place from which a rainbow had risen. What interests the author is the report indicating some relation between the rainbow and an old Melanesian trade custom. What has been mentioned above is the result drawn from the data ivhich the author collected and arranged, but it is too great a riddle for him to solvb why there are such traditional beliefs about the rainbow that seem to be common to all the people in the world.

がんが発生すると, がんの増大に伴う機械的刺激やがんが誘導した炎症細胞から放出される発痛物質により持続的な侵害刺激が発生する. やがて疼痛伝達系全体に感作が発生すると, 病巣から離れた皮膚の痛覚過敏, アロディニアなどの知覚異常, 立毛筋収縮や発汗などの交感神経刺激症状, 筋肉の収縮, 周囲の圧痛といったいわゆる関連痛が発生する. 内臓や骨などの深部体性組織に発生したがんでしばしばみられる現象で, 内臓や骨が侵害刺激を入力している脊髄レベルに同様に侵害刺激を入力している皮膚, 同脊髄レベルに遠心路核をもつ筋肉, 交感神経に症状が出現する. 痛みの部位に病巣がない場合は, 関連痛を念頭において異常のある皮膚, 筋肉のデルマトーム, マイオトームから責任脊髄レベルを同定し, そのレベルに侵害刺激を入力する内臓や骨の異常を検索することが病巣の早期発見につながる. また, 関連徴候として神経障害性疼痛様の知覚異常が出現することを知っておくことは鎮痛薬選択の意味から重要である.

道路上を立体交差する鋼鉄道橋では,道路を走行するクレーン車等に衝突される事故が報告されている.この事故が発生した場合,現場技術者は即座に列車を抑止させ,鋼鉄道橋の損傷状況を調査し,抑止継続あるいは運転再開を判断している.軌道や支承部,主桁等に著しい損傷が発生した場合,抑止継続の判断は容易であるが,主桁下フランジの局部的な変形や面外変形のみが残留した場合,明確な運転再開評価法がなく,現場技術者の判断に委ねられているため,この評価法の策定が課題となっている.本研究は,この課題解決を目的に,過去の損傷事例の調査結果および鋼材の材料特性,鋼桁の耐荷力特性を検討した.その結果,下フランジの局部変形および面外変形の限界量を明らかにし,運転再開評価法を策定した.

アトピー皮膚炎(ADと略)の環境因子であるダニ抗原やハウスダスト(HDと略)の除去が, AD患者の経過にいかなる影響を与えるかを検討した。HEPAフィルターを内蔵した空気清浄化装置を設置した病室(クリーンルーム)を使用し, IgE-RAST法にてダニもしくはHDのスコア3以上を示すAD患者を対象とした。2∼4週間の治療の結果, 全例において皮疹および自覚症状に改善が認められた。IgE(RIST法)値1万IU/ml以上の症例ではIgE(RIST法)値の低下も認められ, 退院後の長期寛解も観察された。IgE-RAST法におけるダニ, ハウスダストのスコアは治療前後で変動を認めなかつた。ダニもしくはHDがアレルゲンと考えられるようなADに対して, クリーンルーム入院治療は試みる価値のある治療法であると思われた。

In developing countries, many people are unable to access basic healthcare services, resulting in many avoidable deaths and/or disabilities. The United Nations adopted the Millennium Development Goals in order to resolve this problem, and Japan has been contributing greatly to the achievement of these goals. In this context, in 2013 the Government of Japan proposed its Strategy on Global Health Diplomacy, and since then has been promoting Universal Health Coverage. Since the beginning of the 21st century, the particular importance of addressing neglected tropical diseases (NTDs) has been stressed by the international community. Nevertheless, of the 1 billion people world-wide who are currently living with NTDs, about three-fourths of these are living in poverty, and of these, nearly 65% are unable to acquire or access drugs for the prevention and treatment of these diseases. Under these circumstances, Japan decided to support the Global Health Innovative Technology (GHIT) Fund in order to support the research and development of drugs for people in developing countries, as well as the manufacture, supply and administration of these drugs. Over the last two years, the GHIT Fund has been supporting the research and development of five new candidate drugs for three NTDs (Chagas disease, leishmaniasis and malaria). Japan also hopes to stimulate domestic pharmaceutical industries in developing countries, as well as to increase international cooperation through various activities such the utilization of our capacity to research and develop new drugs.

Various problems concerning corrosion in ULSI (Ultra-large-scale Integration)'s are discussed. After a brief discussion on Al-line corrosion in plastic packages, two problems connected with ULSI manufacturing are explained and discussed. One is the selection of heat treatment atmosphere which does not oxidize tungsten gate electrodes, but can oxidize silicon at the same time. The other is the high-temperature stability of thin SiO2 films in refractory metal/SiO2/Si structures. Namely, the degradation of SiO2 occurs by H2 atmosphere and by the reaction between SiO2 and Si.

This research aims to analyze the "parklet" and "plaza" generated by the Pavement to Parks Program in U.S. San Francisco and to consider the main point in the case of planning, designing and using a street as a place for people's exchange and stationary. It become clear that the half of streets which parklet and plaza are installed have characterized by concentration of neighborhood shops and have the location which public access is easy to obtained. The design of parklet and plaza has attained pedestrian friendly and desirable amenity for people. And also it is the design as inclusive public space which people can continue to stay without interference mutually. The parklet as public space and the sidewalk seat in front of the cafe is articulated and both parklet and plaza serve as open space which includes people's various activities. In addition, sustainable design here assumes not to maintenance-free and durability of initial condition but to frequent maintenance and repair. It also become clear that the purpose of plaza project is the making people's place on the street and the purpose of parklet project are street beautification and to put the rest space. As for the plaza, the temporary one is built first and then improved in full-scale. And it is the issue that not only a control of the undesirable use such as crime prevention but also a promotion of the desirable use such as an event. As for the parklet, the maintenance issues for each cases, such as a cleaning, a burden of the repairing cost in a hardware, and so forth. It can be summarized in the problem of fund. It is considered that the pavement to parks program create a place of human-centered on the road. Seating space of parklet is designed so that many and unspecified persons may not carry out mutual interference but may continue to stay, and also to be a inclusive humane space on the street. The installer of parklet and plaza has been recognized as a host rather than maintenance administrator. Although maintenance of a parklet is managed by the shops in front, it is not use for commercial. Design and use of parklet and shops are separated. Although the parklet manual shows the guideline of its location criteria and design elements, the its design is diverse, in particular, the part of user interface. It would say that parklet create a public open space on the street in the user's point of view. In parklet and plaza design, priority seems to be given to humane and desirable amenity over the durability that assumes the repair and frequent maintenance. It is thought that this design offers comfortableness. Event use, safety, and aesthetic are contributed to comfort for people, too. The process of temporary plaza improvement promotes sympathy and agreement formation of a local community with verification of their various subjects through its experimental introduction. By trial and error, it enables the design which gives priority to atmosphere or creativity over durability. It is thought that these two points offer the process of both the publicness in a user viewpoint and the people's comfortableness.

This paper describes a composition technique based on two methods, the information theory and the genetic algorithm. Recently, we have needed a new development of the composition technique from variety of our life style on the music, so that it is useful to obtain fundamental rules for the composition by using intelligent informatics technologies. The main idea of our technique consists of the following two parts. The entropy obtained from scores contributes the determination of melody, and the genetic algorithm so as to fit the melody performs the code progression.Two examples of composition results have been given to demonstrate the usefulness of our method.

Scientific Online Letters on the Atmosphere (SOLA) has been a fully Open Access journal under the terms of the Creative Commons Attribution 4.0 International (CC BY 4.0) License (http://creativecommons.org/ license/by/4.0) since 2018, and has been added in the DOAJ (Directory of Open Access Journals) list. Last year, SOLA has updated the Guide for Authors and renewed its own web site (https://www.metsoc.jp/sola/). In response to the extreme weather events in the summers of 2017 and 2018, SOLA welcomed submissions of paper on the related topics and has published the special edition “Extreme Rainfall Events in 2017 and 2018” as Volume 15A. The published papers cover the large-scale atmospheric and oceanic features of extreme weather in the summer of 2018 (Shimpo et al. 2019; Sekizawa et al. 2019; Takemura et al. 2019; Takaya 2019), the mesoscale environmental properties of the extreme rainfall in July 2018 (Takemi and Unuma 2019), the predictability of extreme weather in the summer of 2018 (Kotsuki et al. 2019; Matsunobu and Matsueda 2019; Kobayashi and Ishikawa 2019), the relationship between a typhoon and the rainfall in July 2018 (Moteki 2019; Enomoto 2019), the use of a gridded precipitation product in investigating extreme rainfalls (Yatagai et al. 2019), and the impacts of global warming on the extreme hot summer in 2018 (Imada et al. 2019). We are growing. SOLA welcomes submission from the international community in meteorology, atmospheric sciences, and the related fields.

デジタル・テクストとして物理的存在を欠いた電子ブックの普及は,「書物」の再定義をせまっている。しかし,電子ブック登場以前から,書物はすでに大きな変容を遂げてきている。本稿では比較メディア論の視点から,書物の変容を1920年代のラジオ放送開始,1930年代のペーパーバック革命において検討した。それは「書物のラジオ化/雑誌化」,すなわち「書物の広告媒体(メディア)化」の系譜である。こうした「書物のメディア化」の最終段階として,広告料収入で運営されるメディア企業,Googleによるライブラリープロジェクトが登場する。ウェブ2.0時代のコミュニケーション状況において「書物のデジタル化」がもたらす問題点を整理した上で,電子ブックを既存の書物のリテラシーに接合する必要性を指摘した。

目的:WHOのヘルスプロモーション(HP)に関するオタワ憲章は世界の保健医療関係者にとってのバイブルであり,理念である.本報告は,1.HPの基本理念,2.HPを支える理論,3.HP推進の立役者,4.日本でのHPの登場・展開,5.国際的なHPの動向,6.今後のHP戦略について論じた.内容:ヘルスプロモーションとは,人々が自らの健康とその決定要因をコントロールし,改善することができるようにするプロセスである(オタワ憲章:1986年,バンコク憲章:2005年).HPの立役者は,Kickbusch IとNutbeam Dの二人である.HP活動は,人々の健康課題を共有し,解決し,共に推進することに焦点を置いている.その理由は,「共に生み出すものだ」と考えているからである.21世紀を生きる我々は,未来をコントロールし,人生をあらゆる面において豊かなものとすることが求められている.それゆえ,我々人間は自分の能力を全面的に発揮し,人生を楽しみながら,世界のすべての人々と共にヘルスプロモーション活動を実践しなければならない.結論:そのためには,HPと協働できる健康と幸福を目指した新しい健康教育理論の構築に向かっての努力が必要である.

From March 2012 to October 2013, 145 cases including 62 deaths of Middle East respiratory syndrome (MERS) have been identified in 9 countries (Saudi Arabia, UAE, Qatar, Jordan, France, Germany, Italy, Tunisia, and UK). Recent studies say, while MERS appears to be more deadly in those it infects, it also seems to be less contagious than severe acute respiratory syndrome (SARS) in 2003. All primary cases were connected to the Arabian Peninsula, and nearly half of the cases died due to severe lung inflammation. Nosocomial transmission was implied in 26 percent of the cases. Human-to-human-transmission was considered the likely source of infection in hospital. From these cases, the median incubation period was estimated as 5.2 days (95 percent confidence interval 2 to 15 days). At the same time, some asymptomatic or mildly symptomatic cases have been reported. All MERS-positive cases were diagnosed by using real-time RT-PCR targeting upE and Orf1a genes of MERS-CoV. Specimens were taken from the upper or the lower respiratory tract and blood. Even though over a year has passed since the emergence of the 1st case, many questions on the origin and transmission patterns of the disease remain. The pathogen of MERS belongs to the lineage C of the beta coronaviruses (CoV), which are genetically similar to various coronaviruses detected in bats in Africa and Europe. And two studies suggest dromedary camels in Oman, the Canary Islands and Egypt may have been infected with the virus or a MERS-CoV-like virus in the past. However, human cases have not been detected in these areas. With the Hajj, the Muslim pilgrimage to Mecca in Saudi Arabia, taking place in October 2013 and attracting 3 million visitors, international public health efforts to mitigate and possibly contain this outbreak need to be reinforced. On alert for a possible pandemic, we prepared the PCR system, and shared it to 74 locations of prefectural public health institutes and quarantines in Japan.

膝関節は,骨,靱帯,筋肉のそれぞれが合わさって屈曲と伸展だけではなく,屈曲に伴う大腿骨-脛骨間のrollback motionと外旋運動を認める.変形性膝関節症では,一部に内旋運動を認める症例があり,正常膝とは異なる動態を認めた.2D/3Dレジストレーション法を用いた人工膝関節術後の動態解析では,大腿-脛骨コンポーネント間の屈曲角度は平均120°程度であった.脛骨に対する大腿骨の外旋運動は認められるものの,その回旋量は約10°前後と正常膝に比べて少なかった.またキネマティックパターンはさまざまであり,インプラントの表面形状,手術手技により規定されている可能性が高かった.
