Concomitant therapy with acetaminophen (APAP) and low-dose aspirin is often used in clinical settings; however, it is unclear whether this combination is involved in the progression of chronic kidney disease (CKD). We hypothesized that concomitant therapy with APAP and low-dose aspirin may cause CKD progression. We carried out a retrospective 6-year cohort study that included all patients who received low-dose aspirin from January 2011 to December 2016 at Kaetsu Hospital. Primary outcome was defined as CKD progression at the end of the study compared with baseline. Among the 441 patients treated during the study period, we identified 89 cases of CKD progression. Multivariate regression analysis showed that exposure to APAP>50 g [odds ratio (OR), 2.68, 95% confidence interval (CI), 1.08-6.70], age increase by 1 year (OR, 1.05, 95% CI, 1.02-1.08), and diabetes mellitus (OR, 2.40, 95% CI, 1.41-4.08) had positive associations with CKD progression. Our findings suggested that concomitant therapy with APAP and low-dose aspirin increased the risk of CKD progression. Therefore, we recommend more thorough monitoring of serum creatinine when patients are on such concomitant therapy. Moreover, it is important to advise users of low-dose aspirin to avoid unnecessary use of APAP, in order to reduce the risk of CKD progression.

西田らは、主体価値測定アプリを開発し、思春期大規模コホート研究に導入した。その結果、思春期のロールモデルの獲得・更新と自己制御性の発達が、自律性の成熟の基盤となり、さらにその自律性が基盤となって主体価値が形成されていくことを解明した。山崎らは、全英出生コホートデータを用い、思春期の主体価値と自己制御の発達の相互作用が高齢期のウェルビーイングを予測することを解明した。川上らは、国内外のコホートデータを用いた一連の研究から、思春期主体価値の2要因モデルを提唱した。

アトピー性皮膚炎の増悪因子の一つとして黄色ブドウ球菌は重要である。黄色ブドウ球菌表面のタイコ酸はTh2型免疫応答へシフトさせ,protein Aは表皮角化細胞からIL-18を持続的に産生させる。Enterotoxin A(SEA)とB(SEB)は,正常表皮角化細胞にICAM-1 やHLA-DRを発現させる。また,アトピー性皮膚炎患者の半数以上はSEAまたはSEBの両方あるいはいずれか一方に対するIgE型抗体を有する。SEBの経表皮的感作によって真皮に好酸球や単核球細胞浸潤を誘導でき,Th2型サイトカインであるIL-4 mRNA発現を起こすが,Th1型サイトカインのIFN-γは発現しない。最近,スーパー抗原によって制御T細胞(Treg)の機能である抑制効果が失われることが示された。黄色ブドウ球菌はその菌体成分や外毒素によってアトピー性皮膚炎を増悪させる。しかし,皮膚に定着している黄色ブドウ球菌を完全に除菌し,無菌状態に保つことはできない。角層内でバイオフィルムに包まれて静止期にあるような定着(colonization)した黄色ブドウ球菌に対しては,抗菌療法も消毒も十分な効果発現は期待できない。皮膚を清潔に保ち,適切なアトピー性皮膚炎治療を実施することにより,黄色ブドウ球菌を増えすぎないようにコントロールして,正常細菌叢と仲良くするストラテジーが理想的と思われる。

Twice crops of rice a year appeared in the middle of the Edo era owing to the favorable climatic condition to make for the staple food. But a period of decadence set in the end of Edo era and early in the Meiji due to the poor cultivation and the difficulty of the gradeimprovement. Since the middle of Meiji at the first crop the Kinugasa-wasa had been planted from the end of March to the end of July and at the second crop the Hayakawa shu had deen planted from early in August to early in Novemder. And from early in Taisho the management has been kept eqilibrium. Those days the twice crops a year was cultivated on the seashore plains but early in Showa it has been limited to the Kochi-Aki Plain, due to labor problem.For the twice crops of rice a year the climatic condition is essetial. It can be cultivated by utilization of the possible-climate, optimatic climate and the sunshiny-hour for grow and distribution of the Typhoon. The latter is the direct motive especially. Topography is the second condition. In Kochi Prefecture the rice-farm is scanty due to the mountainious topography, so the intesity of the rice-farm is very high. This geve impitus to appearance of the twice crops of rice a year. But the grade is low, and the fertilizer and the lador are much required. So there is a tendenoy that the second crop has changed in to the realization crop. The agricultural management of the twice crops of rice a year is changing its areal character. The growing area was most large and the crop was high in 7-8th of Showa.

フラクタル画像符号化では,レンジブロックを最も類似したドメインブロックで近似するため,どのドメインブロックで近似するかによって復号画像の品質が左右される。本論分では,復号画像の高品質化を目指して,各ブロックにおけるフラット/ノンフラットブロックの判定閾値を用いた符号化制御の設計法を提案する。本設計法により,フラットレンジブロック判定閾値と高品質の復号画像を得るときのフラットドメインブロック判定閾値は比例関係を示し,その近似誤差が最大0.15dBの範囲であることから,ブロック閾値による符号化制御の設計が有効であることを示す。For fractal image coding, range block approximated by the best matching domain block. So the quality of reconstructed image will be influenced by which domain block is set as the best matching domain block. In this paper, we propose the method that using threshold value of flat block, to attain high quality of reconstructed image. As a result of a simulation experiment, the relations between threshold value for flat rage block and threshold value for the flat domain block which obtains high quality reconstructed image is expressed by propotrionality relation, and the approximation error is suppressed within 0.15dB. As a result the affectivery of the proposed method using these threshold value is shown.

Okino-Shima is a small island, belongs to Kochi prefecture, on the southern part of the Bungo Channel. As this island is on the border line of Kochiand Ehime prefecture, it has been being to keep the peculiar characteristics as the historical, economical and cultural point of contact between the southern power and the northern one. This is also due to the double-sided natural characteristics of the island. The southern part of the Bungo Channel on which the island lies, is the transitional area between the Pacfic and the Seto Inland Sea. Temperature is high in this area, therefore this island has the oceanic characteristics as the wild growing place of the subtropical plants.They got the abundant haul near the island, and fishing was the most important industry with agriculture. But now the main industry is agriculture with few fishing. They cannot extend their farm on the island, population comes also to the maximum. As the island is mountainious, the small terrace farm and the residential land is on the sharp slope. The most houses are low due to many starms.Historically the island was included in the market area of Uwajima before the war, but during the war it was included into that of Sukumo artificially owing to the rationing system. As the sea transportation between the island and Sukumo has been facilitated, their economic relation has been closed. But now it is going to be included into the market area of Uwajima.

近年,仮想化環境は広く使用されるようになっている.複数のマシンを動かすためにはハードウェア等の物理資源を仮想化して使用する必要があり,ハイパーバイザ型の仮想化ではハイパーバイザが物理資源を管理する.この様な環境ではスピンロックを獲得することができない仮想 CPU (仮想 CPU) が長期間ビジーウェイトしたままになってしまう Lock-Holder Preemption (LHP) という問題が発生することが知られている.この様な問題に対して,いくつかのハードウェアでは一定時間以上のビジーウェイトした場合にはハイパーバイザに制御を移すような機能が追加されている.Intel ではこれを Pause Loop Exiting (PLE) という.しかし,PLE を利用するだけでは LHP の完全な対策になっておらず,PLE が頻発することによってオーバーヘッドが掛かることがある.本論文では PLE の発生状況と,それに対するスピンロックの獲得状況を分析し,より効率よく PLE を使用するための新しい仮想 CPU スケジューリングの方法について提案する.

肝癌切除後の孤立性リンパ節転移を摘除することで,術後2年6か月の現在,無再発生存中の症例を経験したので報告する.症例は58歳の男性で,C型慢性肝炎に伴う肝癌に対して肝切除術を2回施行されていた.経過観察中のCT像上,肝尾状葉に約4cm大の腫瘤性病変を認め,AFP, PIVKA-II値の著明な上昇がみられた.腹部血管造影像では腫瘤は中肝動脈および左胃動脈より栄養される腫瘍濃染像として描出され,肝癌の尾状葉再発と診断し開腹した.腫瘍は肝尾状葉に接するように総肝動脈の腹側に存在していたが,肝臓からは独立しており肝癌の総肝動脈幹リンパ節転移と考え摘除した.病理組織学的検査では中分化型肝癌のリンパ節転移と診断された.AFP, PIVKA-IIは術後2か月日に標準値範囲内へ低下し,以来,再発徴侯を認めていない.原発巣がコントロールされた肝癌の孤立性リンパ節転移は摘除により良好な予後が得られる可能性が示唆された.
