Exercise affects human immune functions through the hypothalamus-pituitary-adrenal (HPA) axis and reward systems in the brain. Most immune cells, including T cells, B cells and phagocytes, express functional hormone and neurotransmitter receptors and are regulated through the neuroendocrine immune network. Light-to-moderate exercise improves the defense activity against pathogens and prevents metabolic disorders. Many epidemiological and experimental studies have suggested there is an inverse relationship between moderate exercise training and illness risk. Furthermore, habitual exercise might improve immune regulation, reduce the activity of autoimmune disorders and delay age-related dysfunctions, such as sarcopenia. On the other hand, high-intensity exercise suppresses immune defense activity, especially against upper respiratory infections. Thus, high-intensity training and competitions possibly increase the risk of infection in athletes. Appropriate nutritional intake and hygiene control are also essential to protect both athletes and non-athletes.

Objectives: Recently, attention has been paid to the impact of cigarette smoking on skeletal muscles, as its underlying pathophysiological mechanism has been gradually elucidated. In this study, we aimed to examine whether cigarette smoking is associated with muscle mass reduction and low muscle strength in elderly men.Methods: The study participants comprised 417 community-dwelling elderly men (aged 73±6 years) without severe glucose intolerance, chronic kidney disease, or liver disease. Bioelectrical impedance analysis was performed to estimate appendicular skeletal muscle mass (ASM), which was normalized for height (ASM index, kg/m2). Handgrip strength (HGS) was measured using a Smedley grip dynamometer. Cumulative smoking exposure level during a lifetime was expressed in pack-years, which is a product of the average number of packs of cigarettes smoked per day and smoking duration in years.Results: When the participants were stratified on the basis of cumulative smoking exposure (<10 pack-years, 10–39 pack-years, ≥40 pack-years), the ASM index and HGS progressively decreased with increasing exposure level (P for trend <0.01). In multiple regression analysis, heavy smoking (defined as ≥40 pack-years) was found to be a significant determinant of the ASM index and HGS, independent of potential confounding factors. Among former smokers, the subgroup that quit smoking for ≥20 years had a significantly higher ASM index and HGS than the subgroup that quit smoking for <10 years. The duration of smoking cessation was significantly associated with the ASM index and HGS, even after adjusting for cumulative smoking exposure.Conclusions: These findings suggest that cigarette smoking contributes to the loss of muscle mass and function in elderly men and that smoking cessation could reverse the impact of cigarette smoking on skeletal muscles.

症例は41歳の女性. 手術歴はない. 平成3年と5年に下腹部痛で当院を受診. 平成11年11月7日夕方突然間欠的な右季肋部痛が出現, 次第に増強して当院内科を受診した. 右上腹部に強い圧痛を認め腸音は亢進していたが, 反跳痛や筋性防御はなく, 白血球数, CRP値も正常であった. 腹部X線写真, およびCTで肝前面の横隔膜下に鏡面像を伴った小腸の拡張を認めた. 踵吐も出現し, イレウスの診断音で内ヘルニアを疑い緊急手術を施行. 肝と腹壁の間にviolin string状の検状物を伴つた著明な線維性癖着を認め, その間に小腸が入り込んでいた. 小腸を引き出し線維性癒着を切除してイレウス解除できた, 子宮附属器に軽度の炎症像を認め, 術後の採血でクラミジアIgA抗体は1.38, TgG抗体は5.41と陽性でクラミジア感染による肝周囲炎が原因のイレウスと考え報告した.

京急蒲田駅付近連続立体交差事業における高架橋築造工事では,営業線直上に高架橋を構築するため,移動式直接高架施工機とハーフプレキャスト(以下,HPCa)工法を組み合わせた直接高架工法を適用した。移動式直接高架施工機は,基礎杭打設と部材架設が行える施工機である。HPCa工法は工場製作のプレキャスト部材を現地で組み立てラーメン高架橋を構築する技術で,梁とスラブに適用した国内2件の工事実績がある。本稿では,柱に初めてHPCa工法を適用した本事業における,工法選定理由,HPCa工法の概要,工事概要および施工概要について報告する。

新生子および成獣ライオン Panthera leo の舌表面を肉眼的および走査型電子顕微鏡で観察した.ライオンの舌の先端は円く,舌背側面には糸状乳頭,茸状乳頭および有郭乳頭が観察され,舌尖の腹側面の一部にも糸状乳頭が見られた.糸状乳頭は舌表面全域に見られ,舌尖の周辺部の糸状乳頭はそれ以外の場所の糸状乳頭に比し小型であった.また,糸状乳頭は舌の場所により異なる形態を示した.新生子の糸状乳頭は形態的に未発達で,その先端部が浅く陥凹しており,突起状を示さなかった.茸状乳頭は,糸状乳頭の間に散在して見られ,その分布は舌体に比し舌尖周辺部において密であった.有郭乳頭は舌体と舌根との境界領域に観察された.さらに,糸状乳頭の発育は茸状乳頭および有郭乳頭に比し遅かった.なお,葉状乳頭はいずれのステージでも観察されなかった.

We evaluated a 20-item self-administered odor questionnaire for assessing olfactory function, proposed in 2003 by the Japan Rhinologic Society committee on olfaction tests. The 20-items selected based on results of olfactory studies in Japan were steamed rice, miso, seaweed, soy sauce, baked bread, butter, curry, garlic, orange, strawberry, green tea, coffee, chocolate, household gas, garbage, timber, stercus (shit), sweat, flower, and perfume. Subjects were 302 people —179 men and 123 women (average age: 35.7 years)— having no history of nasal or paranasal disease and tested between December 2004 and December 2007. Subjects were asked to score items as follows: “always smelled” (2 points); “sometimes smelled” (1 point); “never smelled” (0 points); or “unknown or no recent experience” (no score). Scores were calculated and represented using a percentage. Response was 99.3% (300/302), with two subjects excluded for reporting more than 10 “unexplainable” items. The mean score was 95.2% (n=300). Of the 302, 281 (93.0%) agreed on the number and 252 (83.4%) on the content of items. Scores correlated statistically significantly with those of a visual analogue scale (rs=0.501, p<0.0001, n=300). We concluded that the self-administered odor questionnaire is useful in assessing olfactory function in normal subjects. The next step will be to administer the questionnaire to diseased or otherwise compromised subjects to determine whether it is useful for clinically diagnosing such olfactory dysfunction.

As many as four hundred words have been borrowed into English from Japanese, most of which have their doubtless etymologies. There are, however, nine dubious words which are presumed to be borrowed from the Japanese language. They are bonze, soy (soya), mebos, gingko (ginkgo), funny, ramanas, rumaki, sharawaggi (sharawadgi) and hobo. Bonze, soy (soya), mebosu, and gingko (ginkgo) are definitely the words of Japanese origin, but they were indirectly borrowed into the English language. That is to say, they were borrowed from Japanese through Spanish or Portuguese or Dutch to English. The other five words are very difficult to ascertain their etymology.

本講演では、産業用ドローンで世界最先端の技術と実績を持つ自律制御システム研究所(ACSL)の早 川研介氏にお話しいただいた。労働人口の減少とインフラの劣化が同時進行している現在の我が国にお いて、省人化、無人化による社会システムの再構築は国家的課題である。産業用ドローンを用いたイン フラの劣化診断をはじめとする各種ソリューションを提供している当社の仕事は、今後ますます重要に なりそうである。

目的 震災による疾患発生・死亡への影響を検討した疫学研究(保健医療統計調査を含む)を対象に系統的な文献レビューを行い,疾患別に震災からの経過時間とその影響の関連を検討した。方法 文献検索には MEDLINE を用い,発行年が1990年 1 月 1 日から2012年10月30日,政府統計もしくは500人程度以上の集団,を検索条件として実行した。文献レビューでは重複文献,少数例の調査,動物実験,実験的研究など不適切な文献を除外した。最終的に抽出した文献から,震災からの経過時間と震災前後の疾患発生・死亡の増減に焦点をあてチャート図にまとめた。なお経過時間については災害サイクルに基づき,発生~3 日,4 日~3 週間,1 か月~5 か月,6 か月から 1 年未満,1 年,2 年以降とした。疾患分類は精神障害,自殺,感染症,外傷,循環器疾患とした。結果 文献検討の結果54件が抽出された。精神障害では,震災直後からのうつ症状の有訴率,心的外傷後ストレス障害などの精神的ストレス評価指標は高い得点を示す割合が高く,震災後 6 か月以降緩やかに減少傾向を示したものの,震災 3 年後でも依然高い得点を維持する傾向がみられた。自殺では震災後 1,2 年間は減少傾向を示し,その特性は中高年男性のみ減少傾向,男性で減少傾向を示す一方で,女性では増加傾向を示すなど,性・年齢・被災地域での違いで認められた。感染症では震災の影響は震災直後から数か月間と限定的であり,理由として衛生状態の悪化などが示されていた。外傷では震災発生時から 2,3 日間死亡および入院が激増し,それ以降は激減した。循環器疾患では,急性心筋梗塞は発症・死亡数のピークが震災後24時間~数日で夜間発症例が多く,3~6 か月間から最長 1 年間は継続し,震災規模や被災状況により増加する期間に違いが認められた。脳卒中は急性心筋梗塞と同様のパターンを示し70~80歳での発症・死亡が多かった。その他,突然死,たこつぼ型心筋症の報告があり,震災直後から 1 週間~1 か月程度は平時に比べ増加する傾向にあった。結論 疾患により震災発生からの経過時間と疾患発生のパターンに大きな違いがあり,震災直後の疾病の増加抑制のためには,疾患に応じた介入タイミングがあることが示された。

2018年度は、高齢者財産管理に関する諸問題を、.死亡時の財産処理、生前の財産保全・運用、消費者被害の分野ごとに、社会問題班が、具体的な社会問題について、A. 手続班、B. 後見班、C. 消費者班ごとに検討し、カテゴリ班Iが、公的介入の在り方についての総論的検討を行った。具体的には、A. 手続については今津が近時話題となる子の引渡事件を中心に家事事件について公的介入の要否・在り方を検討し、B. 後見班では、国家の後見的介入についてインプットの代表例の税制について分担者渋谷、藤岡が相続税及び資産税を検討し、かかる税制・国民負担の関係を踏まえた上で、アウトプットの代表例である社会保障について分担者嵩が所得保障や医療・介護保障の問題点の分析を行った。また、A.,B.両面にわたる問題として、分担者石綿が近時の相続法改正における生存配偶者の保護を素材に、遺産分割の交渉のスタートとなり、かつ、「後見的保護」となる民法のルールについて分析を行った。C. 消費者班では、消費者法のみらず消費者保護を一部取り込んだ民法(債権法)も含めて分担者渡辺が総論的な検討を示し、分担者中原が具体的なトピックを素材に検討を深め、分担者得津は投資商品(金融商品)の売買契約のプロセスの瑕疵について検討を行った。これらの諸問題の分析手法について、分担者森田が、法制度の現実社会に与える影響(相関関係・因果関係)の析出の検討を深め、その具体的な成果を海外で公表した。これに対して、総論的検討を行うカテゴリ班Iでは、代表者水野が、A.手続、B.後見、C.消費者のすべてにわたる高齢者の財産管理を保護する制度的インフラの不存在を指摘し、諸外国を参考に、いかなるインフラが不足しているのか、国家的保護の「類型」の候補を指摘した。そして、水野、中原、石綿は2019年度比較法学会のシンポジウム報告の準備を進めた。

ウイスキー,ブランデーやワインなどの酒類と樽の関係については実用面,科学技術面から多数の知見が集積されてきている。これは,樽がこれらの酒類の品質にいかに大きな影響を及ぼすかを示すものと思われる。今回,樽の専門家である筆者に,樽材となる世界の主要なオークについて,その特性,酒類製造への利用の歴史,現状からオーク資源の今後にまでわたり詳細に解説していただいた。

Generally vitreous hemorrhage (VH) is detected in 2.2% to 13% of subarachnoid hemorrhage (SAH) patients. VH with SAH (Terson's syndrome) is known to occur frequently in patients with severe SAH or re-ruptured aneurysms. We retrospectively analyzed 20 patients diagnosed with Terson's syndrome out of a total of 881 patients treated for SAH in our department from July 1995 to October 2004. Our study group comprised 15 male and 5 female patients ranging in age from 38 to 77 years (mean 51.2 years). Each patient was classified in Hunt & Kosnik (H&K) grades on admission. One patient was classified in Grade 2, 3 patients in Grade 3, 7 patients in Grade 4 and 9 patients in Grade 5. Each patient was further classified in a Fisher group: 1 patient was in Group 2, 9 patients in Group 3, and 10 patients in Group 4. Regarding the aneurysmal location, 4 cases had ICA aneurysms, 6 had AcomA aneurysms, 4 had MCA aneurysms, 4 had VA or BA aneurysms, and 2 had ACA aneurysms. Re-rupture of aneurysm occurred in 4 cases. Two patients underwent external ventricular drainage because of acute hydrocephalus immediately after CT on admission. Seventeen aneurysms were treated by surgical neck clipping, and 3 aneurysms were treated by intra-aneurysmal coil embolization as the final treatment. Seven patients underwent external decompression because of severe brain swelling, and 6 patients underwent V-P shunt for chronic hydrocephalus. Symptomatic vasospasm occurred in 1 case. Glasgow Outcome Scale (GOS) at discharge showed that 8 patients were GR, 10 were MD, and 2 were SD. VH occurred in only 1 patient on the contralateral side to the ruptured aneurysm among those who had obvious hemilateral VH. Vitrectomy was performed for the 17 VH of 10 patients, and the duration from VH onset to treatment was 8-24 weeks (mean 16.4 weeks). Conservative therapy was done for 15 VH of 10 patients, and the follow-up duration was 12-102 weeks (mean 27.0 weeks). Comparing these 20 VH patients with 311 favorable-outcome (GR or MD) patients who were not considered to have VH, H&K grade or Fisher group scales were significantly higher in VH patients. No significant difference existed between the groups with regard to the number of ruptures or the location of the ruptured aneurysms.

岩手県下の沢内村, 雫石町および滝沢村において採集された成虫と幼虫の標本にもとづいて, 新種Atylotus hasegawaiハセガワキイロアブを記載した。本種はA. horvathi (Szilady)ホルバートアブとA. bivittateinus Takahasiフタスジアブに酷似しており, 外部形態による区別は容易でないが, その生態は明らかに相違している。これら近縁な3種の主な形態的特徴点についても比較を行なった。本種名は成虫を最初に採集された長谷川勉氏に献名したものである。
