国立大学法人が設立されてから今日に至るまで,かなりの時間が経過している。その期間にわたってどのような変化が見られたのであろうか。組織の動きを明らかにするには財務データを丁寧に追っていくのが一番わかりやすい方法である。そこで,長期にわたる財務データを用いながら,国立大学法人の特徴を確認していきたい。その際,学校法人の私立大学と比べるのがよい。同じ大学であっても設立の趣旨が違っているだけでなく,会計基準も違っている。本論文ではそうした視点に立ちながら国立大学法人の姿を追っていきたい。

Post COVID-19 sequelae are a constellation of symptoms often reported after recovering from COVID-19. There is a need to better understand the clinical spectrum and long-term course of this clinical entity. The aim of this study is to describe the clinical features and risk factors of post COVID-19 sequelae in the North Indian population. This prospective observational study was conducted at a tertiary healthcare centre in Northern India between October 2020 and February 2021. Patients aged >18 years with laboratory-confirmed COVID-19 were recruited after at least two weeks of diagnosis, and details were captured. A total of 1234 patients were recruited and followed up for a median duration of 91 days (IQR: 45-181 days). Among them, 495 (40.1%) had persistent symptoms post-discharge or recovery. In 223 (18.1%) patients, the symptoms resolved within four weeks; 150 (12.1%) patients had symptoms till 12 weeks, and 122 (9.9%) patients had symptoms beyond 12 weeks of diagnosis/symptom-onset of COVID-19. Most common symptoms included myalgia (10.9%), fatigue (5.5%), shortness of breath (6.1%), cough (2.1%), insomnia (1.4%), mood disturbances (0.48%) and anxiety (0.6%). Patients who were hospitalized were more likely to report fatigue as a feature of long COVID. Hypothyroidism (OR: 4.13, 95% CI: 2.2-7.6, p-value < 0.001) and hypoxia (SpO2 ≤ 93%) (OR: 1.7, 95% CI: 1.1-2.4, p-value 0.012) were identified as risk factors for long COVID sequelae. In conclusion, long COVID symptoms were common (22%), and 9.9% had the post COVID-19 syndrome. Myalgias, fatigue and dyspnoea were common symptoms. Patients with hypothyroidism and hypoxia during acute illness were at higher risk of long COVID.

浮腫とは細胞外液が血管外の間質に生理的な代償能を超えて過剰に貯留した状態である.その原因は多岐にわたり,①毛細血管内の静水圧上昇に起因するもの,②毛細血管内の膠質浸透圧低下に起因するもの,③血管透過性の亢進に起因するもの,④リンパ系およびリンパ系が流入する静脈系の還流障害に起因するものが挙げられる.浮腫の診察に際しては,浮腫の性状と分布を知ることが重要であり,圧痕があるか,ないか,圧痕の回復時間の遅い slow edema か,早い fast edema か,全身性浮腫なのか,局所性浮腫なのかをしっかり評価する必要がある.浮腫の病態は個々の症例ごとに,また病期により変化する.したがって浮腫に対しては,診察,検査所見の評価に基づいた適切な対応が求められる.

岩手県気仙地方の言語ケセン語には文字がなかった.著者はこれにケセン仮名という固有の文字を与え,さらにその不足を補ってケセン式ローマ字を考案し,文法的構造から音調までも正確に記述できる正書法を確立した.その文法体系を総合的に記述し,これを学習するための教科書を作成した.またすべての語彙に豊富な用例がついている,3万4000に反ぶ語彙項目を収載する辞書を編纂した.これらの研究によって,ケセン語による文学の創出が可能になり,多くの作品群を生み出した.方言差別による劣等感に悩んできた気仙衆は大きな勇気と誇りを回復している.ケセン語による詩作,歌曲の創作,ケセン語演劇の劇団の活動,ケセンそのものについての数多くのテレビ・ラジオ番組などが次々に生まれ,最近はギリシャ語原典から直接ケセン語に翻訳された,朗読CDつきのケセン語訳新約聖書も発行されている.文字は文化を発展させるのだ.

A 69-year-old man developed bilateral polyarthritis, edematous extremities, and skin desquamation on the fingers and ears. He did not meet the criteria for any connective tissue disease, including rheumatoid arthritis. An examination revealed advanced lung cancer. His systemic manifestations were attributed to paraneoplastic Bazex syndrome and remitting seronegative symmetrical synovitis with pitting edema (RS3PE) syndrome. Treatment with pembrolizumab (an anti-programmed death-1 antibody) for lung cancer relieved his symptoms and shrank the lung tumor. Bazex and RS3PE syndromes are rare paraneoplastic diseases. We herein report this unique case of synchronous development of these two paraneoplastic syndromes in the presence of advanced lung cancer.

Aujourd’hui, quelle est la situation de L’absolu littéraire, présentation et commentaire détaillés des textes théoriques du « premier » romantisme allemand qu’ont proposés Philippe Lacoue-Labarthe et Jean-Luc Nancy ? Paru en 1978, traduit en anglais en 1988, puis dans les années 2010, en espagnol, chinois, coréen et allemand, ce livre ne cesse d’attirer l’attention et de provoquer des débats chez les curieux de la théorie littéraire comme de la théorie comme littérature. Le travail proposé ici ne sera pas une recherche généalogique de cette histoire littéraire ni une réévaluation d’une éventuelle « actualité » des jeunes avant-gardistes d’Iéna, mais un diagnostic qui nous permettra d’identifier dans ce mouvement « notre lieu de naissance » et d’y dévoiler « notre » inconscient romantique.

本原稿では,女子及びジェンダーマイノリティの大学生・大学院生(以下,女子学生)によるプログラミングの学習が,プログラミングの動機づけにどのように影響するのかを検討することを目的に調査を行った.女子学生を対象に2週間のプログラミングのコース(Waffle Collegeエントリーコース)を実施し,コース実施前後に期待-価値モデルを適用した,プログラミングの動機づけに関する質問紙調査を行った.その結果,成功期待,実践的利用価値が有意に増加し,心理コストが有意に減少した.また機会コストは有意に増加したことが示された.

日常生活において麻痺手の使用頻度が乏しい回復期脳梗塞患者に対し,3Dプリンタで作製した筆記自助具を麻痺手機能の改善状況に合わせ段階的に変更を加え提供した.同患者へは,日記をつける際に自助具を用いた麻痺手で行うことを促した.結果,上肢機能評価において臨床上意義のある最小変化量を超えた改善が得られた.また,福祉用具満足度評価では,症例の上肢機能改善に応じて,修正を加えた3Dプリンタの自助具に対して高い満足度が示された.以上の結果は,上肢機能の変化に応じて自助具の形状を変更する方略が上肢機能訓練において有用な手段であることを示唆した.

カプサイシン受容体TRPV1は1997年にクローニングされ,感覚神経特異的に発現し,カプサイシンのみならず私達の身体に痛みをもたらすプロトンや熱によっても活性化される多刺激痛み受容体として機能することが,TRPV1発現細胞やTRPV1遺伝子欠損マウスを用いた解析から明らかにされた.さらに,TRPV1は炎症関連メディエイター存在下でPKCによるリン酸化によってその活性化温度閾値が体温以下に低下し,体温で活性化されて痛みを惹起しうることが分かった.このTRPV1の機能制御機構は急性炎症性疼痛発生の分子機構の1つと考えられている.TRPV1は消化管の感覚神経にも多く発現することが明らかになっているが,消化管では疼痛発生以外に粘膜保護などの消化管の生理的機能に深く関わることが明らかになりつつあり,それは長い歴史をもつトウガラシの消化管機能への影響に関する研究成果を説明する.TRPV1やTRPV1発現神経の消化管機能制御の詳細な作用メカニズムの解明が待たれている.

Most scholars interpret, this "Political Authority" as a "Governmental Power." However, today in Japan, we cannot imagine religious groups will exercise a "Governmental Power" literally. Therefore, this interpretation has only a historical meaning. Recently many problems have been caused by religious groups such as OUM Shinrikyou. Considering the many problems surrounding religious groups, "Political Authority" should be properly interpreted based on such new movements.

近年,都道府県別の貧困の大小に関する要因分析が進みだしている。そうしたなか,本稿は,初めて都道府県別子どもの貧困率を対象にして,その高低についての要因分析を行った。分析方法は,従属変数を子どもの貧困率にして,独立変数には,「最低賃金」,「捕捉率」,「非正規率」,「共働き率」,「三世代同居率」,「失業率」,「女性有業率」という7つの項目を用いて重回帰分析を行った。子どもの貧困率に影響を与えている要因は,大きい順に失業率,三世代同居率,最低賃金,共働き率,非正規率となっていた。このうち,子どもの貧困率にプラスの影響を与えている要因が,失業率,共働き率,非正規率であり,マイナスの影響を与えている要因が,三世代同居率と最低賃金であった。これらの分析結果を基に,子どもの貧困率が最も低い福井県について独立変数のデータがどうなっているのかを調べ,その特徴と課題を明らかにした。

Objective: Persistent proatlantal artery (PPA) is a primitive carotid-vertebrobasilar anastomosis (CVA); acute ischemic stroke due to basilar artery (BA) occlusion via a PPA is extremely rare.Case Presentation: An 84-year-old female developed disturbance of consciousness (Glasgow Coma Scale E2V1M5) and quadriparesis with a National Institutes of Health Stroke Scale score of 35. Head CT revealed early ischemic changes in the right temporal lobe, and a hyperdense vessel sign in the BA. Cerebral angiography showed that the left vertebral artery (VA) did not originate from the left subclavian artery or aortic arch. A left common carotid artery angiogram showed the presence of the left PPA originating from the left external carotid artery. Mechanical thrombectomy (MT) with contact aspiration using a Penumbra 5MAX ACE 60 aspiration catheter was performed, and successful recanalization was achieved after clot retrieval in the first attempt (thrombolysis in cerebral infarction scale 2b). MRI performed the following day, however, revealed a newly developed large hemorrhagic infarction in the pons, with no improvement in her symptoms (modified Rankin Scale score of 5 at 90 days).Conclusion: Although MT achieved successful recanalization of the BA via the PPA, her clinical symptoms did not improve, probably because of poor collateral circulation or the long length of the occlusion. In patients with acute vertebro-BA occlusion, if the VA does not originate from the subclavian artery or aortic arch, the presence of a primitive CVA should be considered.
