症例は56歳,男性。単独で登山中,富士山8合目(海抜3,100m)で突然昏倒した。居合わせた外国人医師が救助に当たるとともに同行者が8合目救護所に通報した。自動体外式除細動装置(automated external defibrillator; AED)を持って出発し,昏倒から30分後胸骨圧迫を受けている傷病者と接触した。AEDを装着したところ適応があり,除細動を施行した。まもなく呼吸開始,脈を触知した。呼吸循環が安定したところでキャタピラ付搬送車(クローラー)で下山を開始。5合目で救急車とドッキングし,約2時間後山梨赤十字病院に到着した。第1病日に施行した心臓カテーテル検査で前下行枝の完全閉塞,右冠動脈からの側副血行路による灌流を認めた。低体温療法を施行せずに第1病日に意識レベルJCS I-1まで回復し,とくに神経学的後遺症なく4日後に退院となった。富士山吉田口登山道では7合目,8合目に救護所があるが,2007年から全山小屋にAEDを装備し,山小屋従業員に対してBLS講習会を施行している。今回の事例はこれらの取り組みの成果であり,healthcare providerに対するBLS・AED教育の重要性が再確認された。

自動車の運転支援や自動運転の技術開発は目覚ましい.自動車の運転操作は,人間にとっては容易に可能なものである.しかし,自動車は多主体の交通参加者が複雑に関与しあう道路交通システムにおいて移動を行うものであることから,自動化は容易ではなく,少しずつ機能を向上させつつ段階的に進展させざるを得ない.運転支援・自動運転のシステムは,ドライバのニーズにマッチした商品性を訴求しつつ,人とシステムとのインタラクションにおける齟齬が発生しないように慎重に設計されなければならない.本稿では,自動車分野における運転支援システムの安全性に関するヒューマンファクタの課題を示し,それらの課題に対する取り組みの動向の一端を紹介する.自動車と有人宇宙船は大きく異なるが,自動車分野の動向を知ることは有人宇宙船開発に有用と思われる.

人体をコンピューターグラフィックスで表現する場合, 単に人体の表面の計測のみで表現するとでは腕や脚に力が入ってる状態や, 筋肉が緊張しているといった状態を表現しにくい. そこで, 人体の各部位の断面情報をもとに骨格や筋肉の輪郭線図をモデル化して3Dの表面データに加えることにより, 人体の状態をより精密にモデル化することを試みた.

SIP第2期自動運転(システムとサービスの拡張)の“走行環境条件の逸脱や自動運転システムの機能低下における適切な運転引継のためのHMI等に関する研究開発”と“運転者や歩行者等が習得すべき知識とその効果的な教育方法に関する研究開発”の取り組みを概説する.前者では,自動運転から手動運転への運転交代前におけるドライバーの周辺監視状態の評価指標の検討や,HMIによるドライバーのシステム理解への効果の検討等に取り組んでいる.後者では,主な研究目的に基づき,(1)個人特性を踏まえた教育方法の提案,(2)動機づけ手法の提案,(3)部分教育を意識したモジュール化可能な完全教育教材の開発の3つの研究テーマを設定し研究を行っている.また,試作した教材を用いて,自動運転に関する一般的な知識を事前に提供することの効果についてドライビングシミュレータを用いた検証を行った.これらの成果をもとに,日独連携として自動運転と教育についてのワークショップを担当した.

This note was summarized when optimizing numerical calculations. Under the personal environment supposing PC which recent years formed into the multi-core, the important points about fundamentals of vectorization, parallelization, memory access, etc. were arranged for the double loop of simple matrix calculation for the example.

Emacs is one of the editors well used on UNIX system. Originally an editor is used for the input of a text and a program. However, Emacs offers the environment where it was unified in the case of using a computer. This note discusses Emacs as not input support of a program but execution environment of a program.

The tablet has reached almost practical use and had functionalites as a handwriting device. When scientists write numerical expessions using tablet with pen device, it is possible to write similarly that it writes by hand for an ordnary note. Furthermore, the influence on schools was also considered.

The keyboard is an essential device when using a computer. To perform editing tasks efficiently, it is important to know what functions are bound to the shortcut keys. Those familiar with Emacs-like key bindings would, most likely, find operating a computer eve easier with the use of Emacs-like key bindings. Using xkeysnail on Linux and keyhac on Windows makes it possible to operate a computer with Emacs-like key bindings on both Linux and Windows. I also demonstrate how to achieve Emacs-like key bindings on a virtual machine under Linux.

血液培養よりAerococcus urinaeを分離した重症感染性心内膜炎の1症例を経験した。患者は大動脈弁閉鎖不全症による大動脈弁置換術,慢性膀胱炎の既往のある70歳代の男性。来院時の血液培養よりCluster状のグラム陽性球菌を認めた。分離培養を行ったところ血液寒天培地に小型のα溶血性レンサ球菌様のコロニーが発育し,質量分析装置による同定を実施したところA. urinaeと同定された。心エコー検査によって感染性心内膜炎と診断され,VCMやSBT/ABPC,カルバペネム系抗菌薬などによる加療を行ったが,心原性脳梗塞を合併し第26病日に死亡した。A. urinaeはGram染色ではブドウ球菌様の形態を示すのに対し,血液寒天培地上ではα溶血を呈するレンサ球菌様のコロニーとして発育するため,Staphylococcus属やStreptococcus属と誤同定されやすい。高齢者の尿路感染症の原因となりうるほか,稀に敗血症や感染性心内膜炎を引き起こす。本菌による感染性心内膜炎は臨床経過の急激な悪化をたどり,重症化しやすいとの報告がある。血液培養にて本菌を疑う所見を得た際は迅速に同定し,早期に適切な治療を開始することが重要である。その点で質量分析装置による同定は非常に有用であると思われた。

The estimation of the number of toilet fixtures is discussed here through the investigation of daily activity of in-patients. The results are as follows : (1) Two urinals for men are sufficient to meet demand in each nursing unit including about 50 beds. (2) Three closets are to be provided for both male and female patients in every nursing unit. These are the results of application of the "α method" for cases of Bernovlli Distribution.

<p>前庭感覚系は, 体の傾きを察知するのに重要な役割を担っており,視覚系を始めとする複数の感覚系から情報を受け取っている.しかしながら, 前庭感覚系を調べる脳計測実験を設計するのは技術的に難しく,前庭感覚情報と視覚情報の相互作用については不明な点が多い.そこで本研究では,前庭感覚入力を操作するために反転台,前庭感覚入力の手掛かりを視覚的に与えないために Virtual Reality (VR) Display を使用し,反転台によって実験協力者の体勢を変えた上で VR Display を介した落下映像に対する脳波の計測を行った.事象関連電位解析において,視覚的な落下方向に関わらず, 倒立状態では直立状態と比較して落下開始後 100-200 ms から 視覚活動の減衰が確認された. また,Granger Causality 解析において,頭頂前庭領域から視覚領域への有意なコネクションが確認された.以上の結果により,異常な前庭感覚情報によって視覚活動が抑制され,視覚処理の比較的早い段階から視覚と前庭感覚の相互作用が始まることが示唆された.</p>

Despite a negative result for syphilis in a maternal serological test in the first trimester, we experienced a case of congenital syphilis. The mother was a 23-year-old primipara who had been undergoing regular prenatal care since early pregnancy ; a serological syphilis test at 10 weeks of gestation was negative. She was admitted to our hospital with preterm labor, fever, liver dysfunction, and skin rash at 29 weeks of gestation. Her fetus had ascites and hepatomegaly, although the cause was unclear. The mother delivered a male infant by emergency C-section at 32 weeks of gestation due to concerns over fetal status. The neonate had severe persistent pulmonary hypertension. He also had epidermolysis, hepatomegaly, and thrombocytopenia. CRP value and serum IgM level were abnormally elevated. Considering the clinical findings, and the mother’s previous medical history, we checked the serological syphilis tests for both infant and mother. As both tests were positive, we made a diagnosis of congenital syphilis and commenced treatment with penicillin. Treatment was effective and the patient was discharged from the neonatal intensive care unit at 75 days of age ; developmental follow-up is ongoing. Our case shows that even if a syphilis test is negative during the first trimester, it is important to consider congenital syphilis in infants with suspicious clinical findings.

Japanese capitalism has formed a vicious circle among multiple socio-economic crises under neoliberalism. This paper reexamines the historical origin of these crises, their structural relations, and identify their core, fundamental issues. It will be shown also how three different types of natural disasters also deepen socio-economic crises, thereby endangering the metabolic reproduction of human lives and nature. How can we overcome such an age of deep crises? More than ever, it is indispensable to reconsider the historical significance and task of transforming neoliberal capitalism in the world and in Japan. (1) Six multiple crises in a structural, vicious circle The inflationary crisis of 1973-75 initiated a long downturn of Japanese capitalism. In addition to the excessive supply of currencies and credit in the process of transition from the Bretton Woods international monetary system to the floating exchange rates system, over-accumulation of industrial capital in relation to the limitation of flexible supply of both labor power within major capitalist economies and primary products such as crude oil in the world market inevitably caused sharp rises in wages and prices of primary products and squeezed profit rates as a fundamental factor behind the crisis. In this regard the inflationary crisis revealed anew the basic contradiction of capitalist economy in the commodification of labor power, together with the instability of credit mechanisms.
