ヘモクロマトーシスは鉄が体内の臓器に過剰に蓄積する遺伝性代謝性疾患であるが,サラセミアや,再生不良性貧血,鉄芽球性貧血などの難治性貧血に対する頻回輸血等の二次性要因によっても発症する.ヘモクロマトーシス患者の75~80%に耐糖能障害が起こり,このうち50~60%が顕性糖尿病である.本疾患が進行するとインスリン分泌障害が生ずるが,グルカゴン分泌は保たれており,膵β細胞への選択的鉄沈着がその原因と考えられている.

経肛門的直腸異物は,性的嗜好などが原因で肛門から異物が挿入され,抜去不可能となったものである.われわれは平成18年~平成19年の2年間で5例の経肛門的異物を経験した.5例中2例は同一の患者であった.平均年齢は56.2歳(30~67歳),全て男性であった.主訴は5例とも異物摘出困難であった.挿入の動機は不明1例,自慰3例(2名),Sadomasochism(以下SM)行為1例であった.異物はプラスチック製の蓋,電動歯ブラシおよびスプーン,プラスチック製の洗剤容器が2例(同一患者),とうもろこしであった.診断はいずれの症例も腹部単純X線で確認出来た.摘出方法は無麻酔下経肛門的摘出1例,腰椎麻酔下経肛門的摘出3例,全身麻酔開腹下経肛門的摘出術1例であった.術後合併症は特に認められなかった.平均入院期間は5.4日(3~10日)であった.

We explored the association between understanding a proper use of over-the-counter cold medicines and a choice of information source to purchase such medicines. We conducted an internet survey in March 2011 regarding false perceptions about cold medicines as well as understanding of package inserts. Respondents (20〜69 years old) to the main survey were individuals who purchased cold medicines based on TV commercials (CM group) and those who purchased these medications in consultation with pharmacists (consultation group). We surveyed 121 and 159 respondents in the CM and consultation groups, respectively. We calculated the principal component scores for false perceptions of cold medicines and the respondents’ understanding of the package inserts. The median scores were 0.20 (CM group) and -0.08 (consultation group) false perceptions, and -0.18 (CM group) and 0.15 (consultation group) for understanding. The Mann-Whitney U-test revealed that the CM group had more misconceptions regarding cold medicines compared with the consultation group (P<0.001). Furthermore, the consultation group acquired greater understanding of the package inserts than the reference CM group (P=0.005). These results suggest an association between understanding the proper use of over-the-counter cold medicines and choice sources of information on purchasing these medicines. They further suggest that a lack of knowledge or understanding of cold medicines does not lead to consultations with a pharmacist about the purchase.

参議院選挙は,戦後日本政治の変動に,何度も重要な役割を果たしてきた。しかし,従来の日本政治研究,特に選挙過程の分析は,そのほとんどが衆議院を対象にしたものであり,日本政治の理解もこの範囲を出なかった。参議院選挙データはほとんど分析されず,参院選が日本政治に与えた影響は解明されてこなかったのである。その状況をもたらした原因の一つは,参院選データが十分に整備されていないことである。そこで我々は,参院選の市区町村別データを収集し,データベースを開発した。参院選研究を活性化させ,日本政治に対する参院選の意義や,二院制の意義そのものの再検討を促したり,選挙研究の理論モデルを発展させたりすることが狙いである。本論文は,その参院選データベースの内容と利用方法を説明するものである。

札幌の「YOSAKOIソーラン祭り」で用いられる鳴子を具体的なターゲットとして, 踊り手のパフォーマンスをアシストするIT楽器「サイバー鳴子」を開発した. サイバー鳴子には衝撃センサが組み込まれており, バチの動きに合わせてフルカラーLEDを発光させることで, 踊り手のパフォーマンスに同期したイルミネーションを演出できるようになっている. 本発表では, サイバー鳴子の開発過程について述べた後, YOSAKOIソーラン祭り2004におけるモニター試験について報告する.

粘着剤を2種の剥離フィルムで挟まれた構造の基材レス粘着シートは,耐溶剤性,耐熱性に劣るフィルムに,粘着剤のみがシート化され貼り合わせることが出来るため,テレビや携帯電話に代表されるディスプしイを製造する際の偏光板等とガラスを積層するために使用されている。粘着剤と剥離剤を剥離する剥離力は,粘着剤種により変動するため,弾性率を変化させた粘着剤を用いた基材レス粘着シートにおいて,剥離角度や剥離速度の条件を変動させ剥離力を測定した。その結果より,粘着剤と剥離剤の界面の剥離エネルギを解析した。また,剥離角度を変更した条件で剥離先端部の可視化を光学顕微鏡で行うことで,剥離時の粘着剤の変形を観察した。これらの結果を併せることで,粘着剤の弾性率の違いによる剥離のメカニズムを予測した。

歌手は自らの身体が楽器であるため、歌唱にかかわる諸器官の動きを視認することができない。本研究では、歌手の身体感覚と実際の現象との対応関係を検証することを目的として、歌手へのインタビューを行い、歌唱技術の要とされる横隔膜の動きを実時間での磁気共鳴画像法(MRI動画)を用いて可視化した。その結果、プロ歌手の制御された横隔膜の動きが観察され、その現象が歌手の主観的な身体感覚の説明とも一致することが明らかになった。また、声道や舌の形状、ヴィブラートにも歌唱技術を反映すると思われる特性がみられた。本研究で提案されたMRIによる撮像法と分析法は、歌唱技術を解明する上で有用な手段のひとつとなりうるであろう。

In 1988 we recognized glycosylated components in pyrodextrin and started a study to obtain an amylase-resistant ingredient. We succeeded in establishing a series of processes for industrial-scale separation of the indigestible component with superior appearance and taste by roasting starch (pyrolysis), enzymatic hydrolysis, purification, chromatographic fractionation and spray drying. The component was named indigestible dextrin (ID). In order to utilize ID as a source of dietary fiber, a low-calorie ingredient, and a physiologically active ingredient, we first confirmed that ID is a highly safe ingredient by conducting an acute toxicity study, a mutagenicity study, long-term administration studies in both rats and humans, and a study of diarrhea caused by long-term consumption. A novel determination method using the enzyme-gravimetric method in combination with high performance liquid chromatography (enzyme-HPLC method) was proposed to the Association of Official Analytical Chemists (AOAC) and approved as Final Action Method AOAC 2001. 03 in January 2005. Animal and human studies showed the energy value of 1 kcal/g dietary fiber fraction. Based on these results, ID has been approved in many countries. Moreover, it has been confirmed by both animal and clinical studies that ID has physiological functions such as intestinal regularity, moderating postprandial blood glucose level, lowering serum lipid, and reducing body fat. As for the physicochemical properties of ID, it is similar to DE10 maltodextrin in sweetness and browning property. The properties of not being fermented easily by yeast or lactobacillus impart an interesting characteristic to beer and lactic acid drinks. At present ID is commercially available for use in a wide range of food products not only in Japan but also in many countries around the world.

Most malignant thyroid nodules are surgically treated. This report reviews changes in size of malignant thyroid nodules; none of them received any medical and surgical treatment. Three hundred and three nodules were diagnosed as malignant using fine needle aspiration biopsy (FNAB) and ultrasonography from 1997 to 2003. In 17 patients (19 nodules), they were observed for more than 6 months. We examined the transition of their major axis by ultrasonography. Large nodules (>10mm) and nodules in younger patients (age<50) tend to increase in size. As compared with benign nodules, malignant nodules tend to increase in large nodules (>10mm).

よく知られたボードゲーム「チェッカー」は,両プレイヤが最善を尽くせば引き分けになることが,アルバータ大学(カナダ)のJonathan Schae er教授を中心とする研究チームによって,計算機を用いて証明された.本稿では,チェッカーの解明に利用した技術と筆者がプロジェクトの一員として参加した経緯,およびチェッカー解明までの道程について述べる.

Since the early 2000s, Japan has been frequently noted as being the only country among about 20 major countries where the publication of academic papers has stagnated. During this period, there have been major changes especially with regards to the Japanese pharmacy schools, such as the shift to a six-year schooling system and the rapid increase in the number of private pharmacy schools. However, few studies have focused on academic productivity specifically among pharmacy schools. Therefore, this study analyzed the outputs associated with the publication of peer-reviewed scholarly articles in Scopus search engine per faculty member in pharmacy schools nationwide in 2020. Professors, associate professors, and lecturers were considered as faculty members. The average number of papers published per the faculty member was 3.13 for national and public universities and 1.15 for private universities, with a significant difference between these universities. Dividing the pharmacy schools of private universities into schools established after 2003 and schools established before that, the production coefficient of the former was 0.98 and that of the latter was 1.33. Newly established pharmacy schools were thus found to be slightly less productive in terms of published papers than traditional schools. The paper productivity of private university pharmacy schools is stagnant; the increase in the number of private schools that have adopted a six-year schooling system has contributed to large extent to the creation of clinically competent pharmacists, but it has also brought to light concerns related to the decline in basic research capabilities of Japanese pharmaceutical academia.

健常成人を対象として,各種糖質負荷後の血糖およびインスリン分泌応答に及ぼす難消化性デキストリンの影響について検討した。さらに,食事負荷試験により,食後の血糖上昇に及ぼす影響について検討した。(1)10gの難消化性デキストリン摂取ではショ糖100g負荷後の血糖およびインスリン分泌応答を抑制し,頂値のレベルを非添加の88および69%に低下させた。また,グルコース509およびマルトデキストリン50g負荷に対して,難消化性デキストリンはインスリン分泌のみを抑制し,頂値のレベルは非添加のそれぞれ52および60%に低下した。(2)うどん定食(糖質104.2g,565kcal)摂取後の血糖応答は全般に急峻であり頂値は183.9mg/dlに達した。これに対し,菓子パン(糖質137.2g,553kcal)摂取時の頂値は145.3mg/dlであり,血糖応答は緩やかであった。これらの試験食負荷において難消化性デキストリンを緑茶あるいはコーヒーに添加して摂取させたところ,いずれも有意な血糖上昇抑制効果が認められ,頂値は88および84%に低下した。 以上の結果から,難消化性デキストリンは健常成人において食後の血糖上昇に対して一定の抑制効果が期待できるものと考えられた。

This research examines how providing Japanese consumers with fair trade information affects their willingness to pay (WTP) for fair trade bananas. We estimated the WTP before and after providing information with 86 young consumers in their 20s through an online survey by a conjoint analysis. The results show that providing information increased WTP by those who have understood fair trade or have been interested in the poor without information. Especially, the information about the benefits of fair trade is more effective than the information about the disadvantages of non-fair trade.
