透析患者にとって, 便秘は隠された深刻な悩みの1つであり, 下剤を常用している患者は多い. 痔疾患は便秘が主因でくるために, 透析患者に多い合併症と思われるが, 今まで痔疾患についての報告は皆無であり, 当院での透析患者の痔疾患手術9例について検討した. 9例中男性5例, 女性4例であり, 内痔核8例, 痔瘻1例であった. 当院のこの2年間での健常者の痔疾患手術例は542例で, 63%は内痔核, 23%は痔瘻, 14%は裂肛であった. 従って透析患者では貧血や免疫能が低下しているに拘わらず, 予想に反して, 裂肛と痔瘻が少なくそめ考察を加えた. また手術例は透析導入後短期間であり, 痔疾の病悩期間はいずれも透析導入かなり以前より有し, 導入により増悪し易いが, 意外にも, 透析導入時に痔疾患がなければ, 便秘が頑固であっても, 痔疾患には罹患しづらいと思われる.

軽種馬の育成調教や競走成績に影響を及ぼす発育期整形外科的疾患(DOD)を回顧的に明らかにすることを目的に,国内の軽種馬生産牧場に対して,育成期全般に罹患したDODを含むすべての疾病について聞き取り調査を実施した.その結果,DODとして,腰痿,近位部関節の離断性骨軟骨症,骨端炎,肢軸異常,屈曲異常及び軟骨下骨囊胞があげられた.競走馬登録された個体の中で,種子骨炎,腱炎,外科手術を要する疝痛を罹患した個体は,病歴があげられなかった個体と比較して,初出走の時期が有意に遅延していた.一方で,購入前検査で発生率が高い遠位部関節の骨病変については競走への影響を訴える回答は得られず,深刻な症状を呈していない例が多いと考えられた.

本研究の目的は,うつ病等の休職者を対象としたリワーク・プログラムにおいてマインドフルネス認知療法(MBCT)をベースとした「マインドフルネス講座」を実施し,その有効性とワーク・ライフ・バランスへの影響について検討することであった。マインドフルネス講座は,1回につき2時間の集団によるプログラムを計3回により構成した。本研究の参加者は17名であり,無作為に介入群(9名)と待機群(8名)に割当てて有効性の検討を行った。その結果,セルフ・コンパッションのポジティブ因子が向上し,身体的症状が軽減され,ワーク・ライフ・バランスの家庭の重要度が増した一方で,仕事への労力度が低下することが示された。また,各回における感想から,マインドフルネスの体験が参加者にもたらした変化について検討し,復職支援におけるマインドフルネスの意義と今後の課題について考察した。

本研究の目的は,新人看護師の職業性ストレスを測定する尺度を開発することである。まず,2006年1月と2007年1月に就職後10 ヶ月目の新人看護師を対象に尺度原案を作成するための調査を行った。文章完成法により得られた記述を内容分析し,23項目5件法の質問紙を作成した。次に,3県の施設に勤務する新人看護師383名を対象に,就職後6~7ヶ月目である2007年9月~ 10月に質問紙調査を行い,尺度項目の選定とCES-D及びSEとの相関から信頼性と妥当性を検討した。因子分析の結果, 16項目3因子が精選され,『職場環境』『看護実践』『自己成長』と命名した。尺度全体のCronbach’s α係数は0.860であり,抑うつ自己評価尺度とは正の相関,自尊感情尺度とは負の相関が認められた。以上の結果から,本尺度の信頼性と妥当性は概ね確保されており,実用可能な尺度であると判断できる。

Re-experiencing trauma by overgeneralization is a hallmark of PTSD which is high comorbidity with depression. We thus studied the relation from overgeneralization to depression. Mice were subjected to context A with electric shock as a conditioning which was followed by unconditioned context B using a different, but similar box in 3h on Day1 and was followed by re-exposure to context A (Group ABA) or B (Group ABB) on Day2. Group ABA and ABB mice exhibited the longer freezing in context A and B on Day2. However, the mice which was not exposed to context B on Day1 (Group A-B) showed the shorter freezing time in context B on Day2, indicating that the overgeneralization was induced by experiencing the context B after the conditioning in 3 h. Group ABB mice exhibited the longer immobility time of tail suspension test (TST) than Group ABA and A-B. Freezing time in the first half of the test in context B and immobility time of TST in Group ABB were negatively correlated with the staying time of center zone in the box during the context B on Day1. In contrast, the freezing and immobility time of TST in Group ABA have positive correlation with the time of center zone during context B on Day1. These results suggest that the PTSD and depression may depend on the coping style during unconditioned context within a few hours after the trauma.

蜂刺症とムカデ咬症に対して,受傷直後から黄連解毒湯エキスと茵蔯五苓散エキスを中心とした漢方治療を併用し,翌日には著明に改善した5例を報告する。症例1は70歳男性,30分前に左手背をスズメバチに刺されて受診した。症例2は43歳男性,20分前に左顔面をスズメバチに刺され受診した。症例3は55歳男性,10分前に左下腿をスズメバチに刺され受診した。症例4は39歳男性,60分前に右大腿をスズメバチに刺され受診した。症例5は35歳男性,20分前に右第1趾をムカデに咬まれて受診した。5例とも受診後直ちに漢方治療を開始し,以後,数時間おきに間隔を詰めて治療を継続したところ,翌日には疼痛と皮膚の紅斑と腫脹が改善した。蜂刺症,ムカデ咬症に対して漢方治療を行うことで速やかな治癒に貢献できる。

アラビア語、ペルシア語、トルコ語、ヘブライ語など中東の諸言語で、中東地域で生産される作品のみならず、中東に歴史的出自を持つ者によって、欧米など地理的中東世界を超えた地域で、英語、仏語、独語、伊語などの西洋の諸言語で生み出される作品をも対象に、文学や映画などさまざまなテクストに現れた「ワタン(祖国)」表象の超域的な分析を通して、「ワタン」を軸に、近現代中東世界の社会的・歴史的ありようとそのダイナミズムの一端と、国民国家や言語文化の境界を越えた共通性および各国・各地域の固有性を明らかにすると同時に、近現代中東の人々の経験を、人間にとって祖国とは何かという普遍的問いに対する一つの応答として提示した。

Unusually, we can not recognize Qi energy which is assumed in oriental medicine or oriental philosophy with ordinary observation system. For this reason, Qi energy had scarcely been regarded as serious problem. From a Quantum mechanical point of view which has similar way of thinking to that of oriental philosophy, Qi energy is considered to influence quantum wave function nonlocally. This idea is opposed to the present outlook of natural science. Therefore it is difficult to research Qi energy in present science. Conversely elucidation of Qi energy has a possibility of changing viewpoint of natural science.

The concealed information test (CIT) is an information detection technique measuring differential responses of the autonomic nervous system caused by the knowledge of crime-related information. The purpose of this study is to compare physiological responses caused by crime-related knowledge with those caused by a questioning voice with a deviant pitch in the CIT and to research whether there is any difference between these responses. Participants were assigned to one of three groups: a “knowledgeable” group, an “unknowledgeable” group, and a “deviant-sound” group in which the unknowledgeable participants received a deviant-sound question of a crime relevant item. Each participant chose one of the two envelopes that manipulate to assign the participant to the predetermined group. The envelopes for the knowledgeable group contained instructions with the role of the thief and the item related to the theft, and the envelopes for the unknowledgeable group and a deviant-sound group contained instructions with the role of the innocent. Participants are asked to hide their roles and undergo the CIT. The knowledgeable group and deviant-sound group showed increased electrodermal activity, low heart rate (HR) and low normalized pulse volume (NPV) for relevant items. In the deviant-sound group, there was no suppression of respiration, and low HR appeared earlier compared to the knowledgeable group. The differential responses on thoracic respiratory speed and HR were consistently observed across question repetitions in the knowledgeable group, while the differential responses on skin conductance response, HR and NPV decreased along with question repetitions in the deviant-sound group. The different patterns of differential physiological responses found in the knowledgeable group and the deviant sound group suggested the possibility of distinguishing these two groups. In particular, the suppression of respiration caused by the crime-relevant item may be characteristic of knowledgeable persons. The practical implications are discussed.

本研究は、「地域包括ケア」「地域共生社会」が目指される時代に、地域において障害や病い、または、生きづらさなどを抱える人たちがどのように支援と関わりながら日常生活を送り、それを支える人たちがどのように支援実践や居場所を形成してきた/いるのかを、領域横断的な研究者の経験的調査によって明らかにする。この作業を通じて、現在、政策目標とされている地域包括ケアや地域共生社会に向けた課題の明確化を目指す。中心となる研究領域は認知症ケアおよび障害者支援であり、これらの研究を中核として、さらに他領域の支援実践に関する研究の知見を付き合わせていく。

〔目的〕消化器がん患者の術前後の倦怠感の質を検討するために調査した.〔対象と方法〕消化器がん患者33名にCancer Fatigue Scale(CFS)を用い,その小項目を因子分析することで倦怠感の質を検討した.〔結果〕3因子が抽出され,因子負荷量はそれぞれ24.5,17.9,17.3%であった.CFSで本来の要素とされる身体的,精神的,認知的倦怠感とは異なるもので,3因子を身体的耐久性,精神的,初動動作の倦怠感と解釈した.〔結語〕周術期消化器がん患者は身体的耐久性・初動動作の倦怠感が交雑しており,様々な要素が含む特有の倦怠感の質が存在している可能性がある.

This study was designed to investigate the changes of low back pain and saggital pelvic inclination over time after total hip arthroplasty (THA) in osteoarthrosis (OA) of the hip. Fifty OA cases were classified into three groups according to age: Adult; 40 to 64 years of age, 19 hips; Young-Old; 65 to 74 years of age, 16 hips; Old-Old; 75 years of age ∼16 hips. The incidence of low back pain before THA was about 60% in all groups, but the transition of low back pain after THA was that the low back pain tended to improve more in the younger group. No improvement was seen in the “Old-Old” group. The degree of saggital pelvic inclination was calculated from the transverse axis length (b/a) ratio of the radiological shape of the pelvic cavity in antero-posterior radiographs. As a result, we found that the pelvis tended to incline posteriorly with increasing age. Therefore, pelvic anterior inclination was found in all “Adult patients” and pelvic posterior inclination in most “Old-Old patients”. After THA, the pelvis in most patients tended to slightly incline posteriorly over time.

本研究は,養鰻業における経済的な変化を,産地における制度的・技術的な変化と,それらに応じた生産者間の関係性の変化に注目しながら考察した。事例地域として,日本有数の生産量を誇る愛知県西尾市一色町を設定した。一色町の養鰻業では,第二次世界大戦後の高度経済成長期に行政が中心となって生産基盤が整備された。各経営体も,この時期に生産力の強化を目的として,加温式ハウスに代表される様々な生産技術を導入した。一方で,各経営体の経営主間の関係性はこの期間に変化した。特に,集出荷作業における手伝い関係は,加温式ハウスの導入による作業の省力化によって解消された。このように,養鰻産地を取り巻く経済状況と個別経営体の経営状況,生産者間の関係性は相互に作用している。ただし,関係性の変化は経営主の経験によってそれぞれ異なるため,これらの相互作用を明らかにするためには,各経営体の個別具体的な状況を考察することが重要となる。

マイクロ波の送信モジュールには小型化と低コスト化が求められる.提案する送信モジュールは,高集積に適するSiデバイス上に制御回路を構成し,最終段の1W級の増幅器にSi-GaNスタック型増幅器を採用したことで,送信モジュールで使用するデバイスプロセスの数を減らし電源電圧の数を最少化することで電源回路を簡易化することが可能である.試作では,GaNチップを基板に内蔵し,Siチップと電源回路素子のチップインダクタを基板上に表面実装した3次元実装構造とすることで,モジュール面積7 mm x 7 mmの小型な実装構造を実現した.試作した送信モジュールは,L~C帯において広帯域な動作が可能であり,出力電力はL帯で34.8 dBm,S帯で32.0 dBm,C帯で25.8 dBmが得られ,0.6 dB-rms以下の振幅誤差と1.8 deg.-rms以下の移相量誤差の特性が得られた.
