同種造血幹細胞移植(allogeneic hematopoietic stem cell transplantation, allo-HCT)時には低K血症を高頻度に認め,非再発死亡(non-relapse mortality, NRM)との関連も報告されている。しかし,日本のK注射製剤の添付文書を厳格に遵守すると,補正が困難な場合が多い。今回我々はallo-HCTにおける低K血症とK補充療法について検討した。当科で施行したallo-HCT症例75例を後方視的に解析した。低K血症は92%に認め,grade3以上は40%であった。Grade3以上の低K血症を認めた症例は有意にNRMが高く,予後不良であった(1年:30% vs 7%,p=0.008)。75%の症例で,添付文書の範囲を超える補充療法が必要であった。K注射製剤の添付文書の基準は1988年以降見直されておらず,現状に即した改定が望まれる。

われわれの施設では帝王切開術における脊髄くも膜下麻酔後低血圧を予防する目的でフェニレフリン持続投与を行っている.今回,持続投与を行っていなかった期間と比較して,その影響を後方視的に検討した.フェニレフリンの持続投与は麻酔施行直後より1mg/hで開始し,必要時に昇圧薬のボーラス投与を行った.フェニレフリン1mg/hの予防的持続投与は,麻酔後低血圧の発生率や追加治療介入の必要性を有意に減少させた.また,持続投与を行わなかった場合と比較して反応性の高血圧や徐脈などの発生頻度を増加させることなく使用できた.

City of Kyoto, during the first half of the "Edo" period, expanded its urban area more rapidly than before. At this period, the already acomplished urban area also had re-developed, and also new urban areas were formed by having changed agricultural or barren fields into housing areas. As example, this study treats "OHTO" and "KAWARA" districts whose development had advanced by the impetus of "KAMOGAWA" new bank construction during "KANBUN" period (166-173), and has made investigations on the purpose of newly-urbanizing development of this area, and its procedure, its development subjects, etc.. As the result, I found the evidence of the presence of the specialists who practically engaged in the urbanizing development as agents of feudal lords. Also, I have made investigations on their profession as the specialist and their economical rewards. Then, having studied the constitution of one "CHO" which was formed by newly-urbanizing development, I made some reflections on the historical situation of newly-urbanized area in the urban constitution of the early modern period.

This study aims to clarify the building regulations in agricultural district of Yamashiro province through the Edo period. In Yamashiro province, official notices by Kyoto machibugyosho (magistrate's office) obligated every farmers to present the application forms for building up to Kyoto machibugyosho till 1767. But, in south Yamashiro province, there are some documents prove the existence of buildingregulations by Kyoto daikansho (regional office of administrative official) or Jito (manner's lord ) till the late 1730's. This paper takes up them.

ポピュラー音楽の制作では,マスタリングの際に曲の音量レベルを過剰に高めようとする傾向がみられる.しかし,このようにして作られたダイナミクスの小さな曲は,近年のリスニングスタイルに適していないことが多いと考えられる.そこで本研究では,ラウドなポピュラー楽曲のスペクトログラムからマスタリング前のラウドネスを推定することで,ダイナミクスの復元を目的とする.

A 25-year-old Japanese female was admitted with fever, headache and nausea. She had recurrent fever every 48 hours and a CT scan revealed splenomegaly. An automated hematology analyzer displayed an error message of platelet aggregation. The laboratory technician checked a blood smear with a microscope and found Plasmodium parasites. Suspecting malaria infection, we reconfirmed her travel history and found that she had visited India and Nepal 8 months ago. The PCR test revealed that she was infected with Plasmodium vivax (P. vivax) malaria. The average incubation period is 2-4 weeks for P. vivax malaria, but some cases of P. vivax malaria show a much longer incubation period up to a maximum of about 12 months. When a patient has malaria-like symptoms, it is of utmost importance to confirm his/her travel history within at least the previous 1 year.

南イタリア・ヴェスヴィオ山麓は紀元後1世紀の火山噴火で埋没したポンペイ等の古代都市が発見され、近代考古学・古代史研究の礎が築かれた「領域 (イタリア語 territorio)」である。本研究計画は(I)2002年以来東京大学が当地域で行うソンマ・ヴェスヴィアーナ古代ヴィラ遺跡調査を総括し、(II) 歴史学、美術史、パブリック・アーケオロジー、文献学、文化史の専門家である分担研究者が協働し、当該「領域」の「古代の記憶」に位置付ることで新たな知見を獲得し、(III)国際シンポジウム等を通して文化遺産の研究・保存活用をめぐる国際的議論において、広く統合的人文知の新たな展望と社会的役割を呈示する。

Ephedra Herb is defined in the 17th edition of the Japanese Pharmacopoeia (JP) as the terrestrial stem of Ephedra sinica Stapf., Ephedra intermedia Schrenk et C.A. Meyer, or Ephedra equisetina Bunge (Ephedraceae). The stems of Ephedra Herb contain greater than 0.7% ephedrine alkaloids (ephedrine and pseudoephedrine). Despite its high effectiveness, Ephedra Herb exert several adverse effects, including palpitation, excitation, insomnia, and dysuria. Both the primary and adverse effects of Ephedra Herb have been traditionally believed to be mediated by these ephedrine alkaloids. However, our study found that several pharmacological actions of Ephedra Herb were not associated with ephedrine alkaloids. We prepared an ephedrine alkaloid-free Ephedra Herb extract (EFE) by eliminating ephedrine alkaloids from Ephedra Herb extract (EHE) using ion-exchange column chromatography. EFE exerted analgesic, anti-influenza, and anticancer activities in the same manner as EHE. Moreover, EFE did not induce adverse effects due to ephedrine alkaloids, such as excitation, insomnia, and arrhythmias, and showed no toxicity. Furthermore, we evaluated the safety of EFE in healthy volunteers. The number of adverse event cases was higher in the EHE-treated group than in the EFE-treated group, although the difference was not significant. Our evidence suggested that EFE was safer than EHE.

独居高齢者の増加が社会の趨勢として避けられない中、独居しつつも周囲との繋がりを実感でき、生活の質を保ったまま老いていくことができる社会を実現することは喫緊の課題である。本研究の目的は、独居高齢者の生活行動と孤独感の関係を解明し、独居であっても孤独を感じずにいる人々の生活行動の特徴を明らかにすることにある。それによって、孤独感の解消に資する都市空間を実現していくために、介入の根拠となる基礎的知見を構築する。特定の住宅団地を対象にアンケート調査を行った結果、対人交流と外出行動に関する項目が孤独感と相関を示す要素として明らかになった。さらに、補足的なヒアリング調査から、様子確認ができる親密な人の存在に加えて、生活に余暇外出(とりわけ、徒歩30分圏内で行う日常的な散歩)を取り入れることが重要であるということが、孤独感を低減させる上で重要であると示唆された。「社会的孤立」の解消には家族以外による共助・公助の確立が課題となっている一方、「孤独感」は散歩など余暇外出を生活に取り入れることで改善できるとすれば、余暇外出行動を促進する都市環境を整えることで、一人暮らしの「孤独感」を低減できる可能性がある。

Our target are unemployed living alone seniors in the apartment complexes. We conducted a questionnaire survey and extracted 3 patterns of lifestyles. We analyzed lifestyle characteristics according to the presence or absence of loneliness and subjective health status. We also asked what kind of facilities and events they wanted in their apartment complexes. And we compared the features of them by levels of loneliness, subjective health and lifestyles. Finally, we identified some features in terms of facilities and events which are aligned to certain lifestyle that can contribute low level of loneliness and high subjective health.

三重重県のアサリの主漁場である伊勢湾は、海岸線延長660km、水域面積2,342km2の規模を持つ日本最大の内湾であり、西岸(三重県側)には木曽三川や宮川をはじめ約20の河川が流入し、河口域の干潟や半自然海岸にはアサリの好漁場が形成され、採貝漁業が営まれてきた。アサリの漁獲量は1970年代以降、年間10,000トン前後(日本3位)で推移していたが、1990年代半ばから急減し、2000年代以降は年間3,000トン前後に低迷している。伊勢湾の沿岸漁業は、他の漁業が衰退した南部を中心に年々アサリ資源への依存を高めており、漁獲量の減少は漁家経営に深刻な事態を招いている。本稿では、伊勢湾西岸のアサリの漁場環境の推移や漁場行使の現状を整理しつつ、今後の資源管理への展望を考察した。

Industrial microorganisms have been improved mainly by mutation and selection for their productivity, but recent development of gene manipulation would offer more effective tool for microbial breedings. Since the primary gene products are polypeptides, the gene manipulation techniques have been mainly applied to produce human peptide hormones, interferons and enzymes. These polypeptides have been expected to be useful for therapy of human diseases in near future. The technique will be available also to improve biomass production including microorganisms and plants. Further application is planned in the field of chemical industry, such as the conversion of ethylene to ethylene oxide. In addition, this technique would be developed as tolls for breeding of industrial microorganisms, especially in Japan.
