目的 高齢者の歩行量を維持・増加させることには多くの健康上望ましい効果が期待できる。しかし,健康日本21(第二次)中間評価では,高齢者の歩数が目標値まで達成できなかったことが報告されている。そのため,従来とは異なるアプローチに建造環境(街路ネットワーク,施設や居住密度,土地利用など人工的に造られる環境)を通じた身体活動量や歩数の維持・増加をもたらすゼロ次予防が注目されている。本研究では,建造環境の1つである生鮮食料品店の変化と歩行時間の変化との関連を明らかにすることを目的とした。方法 日本老年学的評価研究(JAGES)が27市町の要介護認定を受けていない65歳以上を対象に実施した自記式郵送調査データを用いた2016・2019年度の2時点での縦断パネル研究である。目的変数は,歩行時間の2時点の変化(増加あり・なし)とし,説明変数は追跡前後の徒歩圏内にある生鮮食料品(肉,魚,野菜,果物など)が手に入る生鮮食料品店の有無の2時点の変化を5群にカテゴリー化(なし・なし:参照群,なしとわからない・あり,あり・あり,あり・なしとわからない,その他)したものである。調整変数は2016年度の人口統計学的要因,健康行動要因,環境要因,健康要因の計14変数とした。統計分析は,ロバスト標準誤差を用いたポアソン回帰分析(有意水準5%)で歩行時間の増加なしに対する歩行時間の増加ありとなる累積発生率比(cumulative incidence rate ratio:CIRR)と95%信頼区間(confidence interval:CI)を算出した。分析に使用する全数のうち,無回答者などを欠測として多重代入法で補完した。結果 歩行時間の増加ありが13,400人(20.4%)だった。追跡前後で徒歩圏内の生鮮食料品店の有無の変化が「なし・なし」(6,577人,10.0%)と比較した場合,「なしとわからない・あり」(5,311人,8.1%)のCIRRは1.12(95%CI:1.03-1.21)だった。結論 徒歩圏内の生鮮食料品店が増加していた者で,高齢者の歩行時間が増加した者の発生が12%多かった。暮らしているだけで歩行量が増える建造環境の社会実装を目指す手がかりを得られたと考える。

Although a strong floral scent is typical of hybrid lilies, Asiatic hybrid lilies (Lilium spp.) have only a weak scent. Therefore, developing new cultivars with pleasant floral scents is an important objective in Asiatic hybrid lily breeding programs. Among the wild species contributing to Asiatic hybrid lily establishment, L. cernuum has a characteristic scent that the nose can perceive. We crossed Asiatic hybrid lily cultivars twice with L. cernuum to introduce floral scent traits into Asiatic hybrid lilies, and scent emission in the F1 plants and their progeny lines was evaluated instrumentally. Flowers of L. cernuum emitted benzenoids/phenylpropanoids (mainly 2-phenylethanol and benzaldehyde), monoterpenes (predominantly linalool and linalool oxides), and a fatty acid (iso-valeric acid). Lilium cernuum gave off unpleasant odors mainly because of iso-valeric acid emission. The F1 flowers also emitted these three chemical classes. Among the lines derived from the crosses between Asiatic hybrid lily cultivars and the F1 plant, lines that emitted all three chemical classes and lines that released one or two of the three chemical classes were segregated, and some lines emitted benzenoids/phenylpropanoids or terpenoids without emitting fatty acids. We successfully selected elite breeding lines that predominantly emitted pleasant scents. Therefore, this study demonstrated the introduction of pleasant scent production capability into Asiatic hybrid lilies using interspecific hybridization with L. cernuum.

2018年台風第12号(ジョンダリ)は7月29日の日本上陸前に、対流圏上層寒冷低気圧の円周に沿った異常な経路をとった。大気海洋相互作用および対流圏上層寒冷低気圧とジョンダリの相互作用が台風経路に及ぼす影響を調べるため、3kmメッシュ非静力学大気モデルと大気波浪海洋結合モデル及び異なる初期時間を採用して作成した初期条件に基づく数値シミュレーションを実施した。シミュレーションされた対流圏上層寒冷低気圧は355K等温線上の高ポテンシャル渦度、低い気圧、低い相対湿度の特徴をもつ。7月25日から27日にかけて対流圏上層寒冷低気圧はジョンダリの北側を南西方向に移動し、この期間にシミュレーションされたジョンダリは対流圏上層寒冷低気圧の円周に沿って反時計回りに移動した。ジョンダリが西に移動し始めてから、大気波浪海洋結合モデルによるシミュレーション結果において、経路に沿って海面冷却が生じていた。日本上陸後にジョンダリは勢力を弱めると、対流圏上層寒冷低気圧も日本の南側で勢力を弱めた。特に潜熱フラックスと対流による対流圏上部における加湿が勢力の弱化に影響を与えていた。ジョンダリが九州の南海上で再び発達したとき、台風域では渦位は柱状に高くなり、一方で対流圏上層寒冷低気圧付近の対流圏上層渦位は相対的に低い値であったことから、台風域の渦は対流圏上層寒冷低気圧と合体する様子がシミュレーションされた。大気波浪海洋結合モデルのシミュレーション結果では、寒冷低気圧付近の対流圏上層部における高渦位は維持される一方、柱状の台風域の渦位はその高さを下げつつ弱まり、台風中心からの対流圏上層への外出流が弱まった。この結果、対流圏上層寒冷低気圧から変形した高渦位の折り返し位置に影響を与える様子が見られた。ジョンダリの経路に影響を及ぼす指向流は、対流圏上層寒冷低気圧下の地衡風の影響を受けていたため、実際は上記海洋結合の効果よりも大気初期条件の違いがジョンダリと対流圏上層寒冷低気圧両方の経路と強度により強い影響を与えていた。

Hidden bow hunter's syndrome (HBHS) is a rare disease in which the vertebral artery (VA) occludes in a neutral position but recanalizes in a particular neck position. We herein report an HBHS case and assess its characteristics through a literature review. A 69-year-old man had repeated posterior-circulation infarcts with right VA occlusion. Cerebral angiography showed that the right VA was recanalized only with neck tilt. Decompression of the VA successfully prevented stroke recurrence. HBHS should be considered in patients with posterior circulation infarction with an occluded VA at its lower vertebral level. Diagnosing this syndrome correctly is important for preventing stroke recurrence.

本研究の目的はジュニア競泳選手の短距離泳パフォーマンスに影響を及ぼす要因を分析するとともに, それらが年齢に伴ってどのように変化するかを明らかにすることである. 被験者は8~10歳, 11~12歳, 13~14歳, 15~18歳の4グループに区分された8~18歳の男子114名, 女子130名である. パフォーマンス (個人の50m泳のベストタイム) には体格, 筋力, 柔軟性, ストローク効率が強く影響していると仮定した. 11項目の測定値に因子分析を実施し, 体格因子, 筋力因子, そして柔軟性因子を抽出した. 因子分析から抽出された3因子とストローク効率で構成される重回帰モデルに多母集団の同時分析を性別, 年齢群別に実施した. 男女とも14歳以下では, ストローク効率のパフォーマンスへの影響が最も強かった. 15歳以上では, 男子は筋力因子のパフォーマンスへの影響が最も強くなり, 女子では体格因子が最も強くなった. 競技歴がストローク効率に及ぼす影響は小さかった. この結果, ストローク効率のパフォーマンスへの影響は低年齢で強く, 年齢とともに弱まるが, 反面, 体格因子と筋力因子は低年齢で影響が弱く, 年齢とともに強まることを示唆している.

介護負担度に影響する因子を明らかにする目的で, 老年者の問題行動の有無や介護者自身の要因と介護負担度との関連をバーンアウト・スケールを用いて検討した. 対象は当院総合内科に入院した65歳以上の患者のうち, 家族が何らかの介護をしていた73例 (男性31例女性42例, 平均年齢82.7±6.9歳) で, 主たる介護者へのアンケートにより, 患者・介護者の属性や一日の介護時間, 徘徊などの問題行動の有無などを調査した. また, アンケートには Pines のバーンアウト質問項目が含まれ, 計算式からバーンアウト・スコアを算出した. 介護者の属性では, 約3割が配偶者であり, 介護者が高齢であるとバーンアウト・スコアは有意に高値となった (p<0.01). これに加えて介護者自身の健康状態に基づき介護負担を重く感じるほどバーンアウト・スコアは高値となった (p<0.001). 老年者の問題行動のうち, 夜間の介護が必要 (p<0.05), 監視が必要 (p<0.01), そして介護拒否 (p<0.01) があるとバーンアウト・スコアは有意に高値となった. バーンアウト・スコアを説明変数とする重回帰分析では, 老年者の Basic ADL・問題行動・介護者自身の要因のうち介護者自身の健康状態による介護負担, 夜間の介護そして排泄の介助が独立した因子でバーンアウト・スコアの約4割を説明し得た (R2=040: p<0.0001). 介護保険サービスの導入によって, 上記の家族の介護負担を重くしている要因が改善され, バーンアウト・スコアを低下させるか検討することが今後必要である.

帯状疱疹後神経痛の治療開始1カ月後に自殺企図を起こした1症例を報告する.79歳の女性で,帯状疱疹のために,急性期より強い痛みが持続し,不眠となっていた.鎮痛薬で痛みが軽減せず,副作用で日常生活動作が著しく障害されていたので,当科を紹介された.当科受診後,薬物による副作用は改善したが,痛みの治療に難渋した.神経ブロックによる治療を後日予定し,退院した.退院の翌朝に自殺目的で服薬し,意識が消失した状態で家人に発見された.本患者は,精神神経疾患の既往はなく,入院中に自殺念慮を示唆する明らかな言動はみられなかった.再入院後,頸部硬膜外ブロック,星状神経節ブロックを施行しながら精神科医による治療を受け,痛みは軽減した.高齢者の慢性痛患者では,自殺の可能性に留意する必要があると考えられた.

本研究では高次脳機能障害者の相談支援体制に関して, 国立障害者リハビリテーションセンター発行の報告書 (2011 年) を基に統計的分析を加えて定量的に検討した。41 都道府県のうち外れ値として特定した 2 地域を除外し, 39 都道府県を分析対象とした。支援拠点機関における相談件数の年平均は, 直接相談 527.2 (±526.4) 件, 間接相談 269.3 (±301.2) 件, 総計796.5 (±735.0) 件であり, 人口 10 万人あたりに換算した総計は年 47.0 (±38.3) 件であった。また, 当該地域の人口と相談件数との間に有意な相関を示した。さらに, 39 都道府県を高次脳機能障害支援モデル事業 (以下「モデル事業」) に参加した 12 都道府県とそれ以外の 27 都道府県とに分けて群間比較をおこなったところ, 人口 10 万人あたりの件数に有意な差は認められなかった。したがって, モデル事業実施の影響は減少し, 高次脳機能障害者に対する支援体制の均霑化が図られたと示唆される。

Il libro fu da lui scritto con l' intendimento che fosse usato dai mercanti al servizio dei Barbi. Esso e una guida per esercitare la mercatura in Europa ed in Asia. Nella prima parte l' autore da utili indicazioni per poter commerciare con la Cina e l' Asia Centrale, allora possibile grazie alla Pax Mongolica. Nel suo libro egli dimostra quanto desse valore al Commercio della seta. Ed e proprio della stoffa di seta cinese che si puo riconoscere nelle opere di Simone Martini. Nel libro il Pegolotti tratta nei particolari dal valore delle varie monete e dei pesi e misure usati in molti paesi d' Asia ed Europa. Abbiamo provveduto alla traduzione della parte del libro che tratta del commercio con la Cina, cioe abbiamo tradotto i primi otto capitoli allo scopo di sottolineare come il commercio tra Italia e Cina fosse assai sviluppato. In Italia esistono pochi studi recenti su tale tema. Esiste, come e noto, la traduzione inglese di H. Yule e Cordier e lo studio di A.Evans del 1936. Con questa nostra traduzione, vogliamo incoraggiare studi ad alto livello sui rapporti commerciali tra l'Italia e l' Asia nella questa epoca.

The purpose of this study was to investigate whether ROM on pronation of the transverse tarsal joint affected both pelvic movement and EMG activity of gluteus medius muscle on hemi-standing. We researched two experiments about ROM limitation on pronation of the transverse tarsal joint. One was the inherent limitation: it was classified as the hypo-mobility side (ROM on pronation of the transverse tarsal joint <10°) or normal-mobility side (ROM on pronation of the transverse tarsal joint ≥10). Another was artificial limitation by meanes of taping. It discriminated between the non-taping group and the taping group (ROM on pronation of the transverse tarsal joint=0°). In each experiment, subjects were instructed to maintain hemi-standing with 90 degree hip flexion on the unsupported side. We compared the degree of eversion of the subtalar joint, pelvic movement and EMG activity of the gluteus medius muscle on the supported side on hemi-standing on both the hypo-mobility side and the normal-mobility side; and taping group and non-taping group in two experiments, respectively. In both of the two experiments on ROM limitation on pronation ofthe transverse tarsal joint, pelvic movement tended to increase pelvic elevating to the opposite side on hemi-standing or the degree of eversion of the subtalar joint on the supported side significantly increased on hemi-standing, and EMG activity of the gluteus medius muscle significantly increased on hemi-standing. We would like to emphasize that pelvic movement was altered by ROM limitation on pronation of the transverse tarsal joint. The main reason was that ROM limitation on pronation of the transverse tarsal joint on the supported side caused excessive eversion of the subtalar joint on hemi-standing. And excessive eversion of the subtalar joint caused the obstruction to shift the pelvis in the direction of the supported side for hemi-standing. We suggest that the pelvic elevation to the opposite side was caused to compensate for obstruction when shifting the pelvis in the direction of the supported side.

マスク着用は感染症拡大防止に有効だが,その着用で表情が読みとりづらくなる。こうしたマイナス面の報告は,成人被験者を対象とした実証研究の知見に依拠しており,学童期の子どもを対象とした知見は極めて少ない。本研究では,マスクなし,不織布マスク,あるいは透明マスクを着用した顔表情画像を小学生に提示し表情弁別を求めた。そして成人の既データと比較した。実験の結果,成人では,マスクなし顔に比べて,不織布マスク着用顔において「喜び」「無表情」「悲しみ」の弁別パフォーマンスの低下が認められた。対して小学生では,「嫌悪」「恐怖」「喜び」「無表情」「悲しみ」「驚き」とより多くの表情でその低下が認められた。なお,このような低下は,透明マスク着用顔では総じて認められなかった。本研究の結果は,不織布マスク着用による顔表情認知の妨害は小学生でより大きい可能性,その対処として透明マスクの活用可能性を示している。

Water temperature directly affects the physiological phenomena of fish categorized into heterothermic animal. In particular, the water temperature has been known as a major factor that controls the reproductive phenomena, such as sexual differentiation, gametogenesis, spawning, etc. In the present paper, we describe about the effects of water temperature on the reproductive phenomena in fish.
