2011年3月11日に発生した東日本大震災では,宮城県の広い範囲,特に東部沿岸地域は津波による市街地の流失,電気,上水の供給停止,通信の途絶など,大きな被害を受け,災害拠点病院への救急患者集中,通院手段の確保困難,生活環境の悪化が生じ,被災地外での支援透析を要した.災害支援透析において,被害が大きい依頼側に,通常の臨時透析と同レベルの情報提供を求めることは,災害支援の基本概念に沿わない.また,支援側は,業務増大の中で初診の多数の透析患者の診療を行わなければならないが,過酷な環境から避難してきた透析患者への対応は,平時とは異なる視点が必要となる.そこで,われわれは震災後に多人数の入院支援透析を行うにあたり,災害時透析入院クリニカルパスを作成し使用した.クリニカルパスの運用によって,避難患者の容態把握を共通化,標準化することが可能で,二次避難先への引継ぎにも利用した.過去に例をみない大災害であったが,このクリニカルパスの活用は災害時入院支援透析における診療に有用であったので,ここに報告する.

我々は最近,1975年頃の日本食は現代の日本食に比べて健康有益性が高いことを示した.1975年日本食の特徴のひとつに肉類の摂取量が低いことがあげられる.そのため,肉類を他の食品と置換することで,健康有益性の増加が期待できた.そこで本研究では,現代の日本において広く食べられている「ソーセージ」を伝統的な日本の食品である「かまぼこ」に置換することによる効果を,ラットを用いて検討した.凍結乾燥·粉末化した「ソーセージ」または「かまぼこ」を通常飼育食に重量当たり20%混合し,SD系ラットに4週間与えた.その結果,「ソーセージ」群に比べて「かまぼこ」群において,血漿と肝臓における脂質量と過酸化脂質量が低下した.次に,「かまぼこ」のタンパク質·脂質·炭水化物のエネルギーバランスと塩分を精製飼料のみを用いて再現した「mimicかまぼこ」を作製した.これを通常飼育食に混合し,ラットに4週間与えた.その結果,「mimicかまぼこ」群に比べて「かまぼこ」群で脂質量と過酸化脂質量が低下した.以上より,「ソーセージ」を「かまぼこ」で置換することは脂質量と過酸化脂質量を低減するために健康有益性が増加することが示され,この効果は「かまぼこ」のエネルギーバランスのみに依存しないことが示唆された.

David Young's Glenn (1992) is a play about the Canadian pianist Glenn Gould (1932-82). There are four characters in the play, which portray the eras of Gould's fifty-year life and his thoughts: Prodigy (6-22), Performer (22-31), Perfectionist (31-48), and Puritan (48-50). These characters act and symphonize their voices in the framework of J.S.Bach's The Goldberg Variations according to the voicing and structure of the music. The use of the framework (the aria, 30 variations and the aria da capo) is a successful device of transfering a musical form into a literary form which is independent of the time-space conventions of the theatre. The play also employs successfully the compositional technique named "contrapuntal radio," which overlaps parallel dialogue and makes coincidental conjunction between voices, because it makes the four Goulds disguise other theatrical Gould-like personae who enliven the inter- actions. For all the complicated plot, the play can be analized clearly if one realizes that the crucial hero is Puritan: the plot is the journey of Puritan who goes back through the whole of Gould's life. The recomposed figure of Glenn Gould in the play is a typical Canadian according to the model of Canadian identity in Margaret Atwood's Survival (1972). Glenn Gould was a man of escape who fled from the "South" to the "North" in order to survive himself, but happened to be a hero in the world as well as in Canada, and is by definition a Canadian anti-hero.

A sudden acceleration of the Kuroshio jet appears off Cape Shionomisaki in the high-resolution (horizontal resolution of 1/36°) JCOPE 2 ocean reanalysis data. Using this dataset, we investigated the structure of the Kuroshio acceleration. The increase in the velocity of the current is accompanied by a downstream flow separation from the coast and an outcrop of cold temperature inshore. The acceleration of Kuroshio appears when it takes a near-shore path. Cape Shionomisaki amplifies the responses to the Kuroshio flow by creating the zonal velocity acceleration toward the downstream region when the Kuroshio flows closer to the coast. The Kuroshio acceleration coincided with the topographic ridge on the continental shelf near Cape Shionomisaki. This relation suggests that the dynamics of the acceleration is linked to the topographic feature. We proposed an explanation of the Kuroshio acceleration using a hydraulic control theory. An analytical solution was applied to the coastal topography around the Kii Peninsula. The solution captured some aspects of the Kuroshio acceleration.

The 2004 Mid Niigata Prefecture Earthquake (MJMA 6.8) occurred on 23 October 2004. The mainshock was followed by four aftershocks with MJMA≥6.0. This earthquake is located in the Niigata-Kobe Tectonic Zone in which large strain rates (>0.1ppm/y contraction) have been observed by GPS data. We deployed three temporary online seismic stations in the aftershock area. Combining data from the temporary stations and from permanent stations around the aftershock area, we determined aftershock locations, and estimated the structures and the stress change in and around the aftershock region. Based on these results, we suggested a generating process of the 2004 Mid Niigata Prefecture Earthquake supposing that a very weak region exists in the weak zone in the lower crust just beneath the seismogenic fault.

本学食物栄養学科では,平成20年度愛知県農林水産部食育推進課募集による「学園で食育・モデル事業」に採択され,「三世代を紡いでいくイキイキ健康クッキング」というテーマで,幼稚園児とその母親(保護者)と高齢者を対象に食育講座や料理教室,講演会という手法で活動を行った. ここでは,幼児の食生活の実態を報告する.平成20年6月N 市のS 幼稚園年長児109名の母親(保護者)に対して園児の食生活に関するアンケート調査を行った.この結果,朝食は「毎日必ず食べる」が96%,間食については「ほとんど毎日1回以上食べる」が9割以上をしめていた.食べ物の好き嫌いについては「少しある」「ある」が73%と多く,嫌いな食べ物は野菜やきのこ,牛乳が高率であった.食育については,幼児期までに始めるべきとの回答がほぼ10割で,「今よりよりよくしたいが時間的に難しい」が7割を超えた.

症例は53歳の男性で,農作業中に足底に釘を刺し,抜去しないで放置した.約1週間後に39度台に発熱したが,右肩関節炎と診断されて整形外科にて洗浄術を施行,排出された膿からは黄色ブドウ球菌が検出された.胸部X線像で心胸郭比の拡大,心電図で広範囲の誘導におけるST上昇,心エコー図で左室後壁側の心嚢液貯留が認められ,入院となった.発熱後の6日頃より収縮期雑音が聴取され,胸部X線像で肺うっ血が生じ,心エコー図で僧帽弁に逸脱と疣贅と思われる異常エコーが認められた.血液培養では黄色ブドウ球菌が検出され,感染性心内膜炎と診断された.緊急手術では,化膿性心膜炎と膿性心膜液貯留,さらには前後尖上の疣贅を伴う僧帽弁閉鎖不全が認められ,弁置換術が施行された.心嚢液からは黄色ブドウ球菌が検出され,抗生剤による治療を加えたが,術後2週間で多臓器不全(MOF)で死亡した.本例は日常的な外傷を放置したことによって化膿性肩鎖関節炎および化膿性心膜炎をきたし,さらには抗生剤治療法の発達した最近ではまれな感染性心内膜炎を併発した症例であり,報告する.

症例は61歳, 男性. 糖尿病性腎症による慢性腎不全により, 週3回の血液透析 (HD) 中であり, 透析後半に頻回の血圧低下を起こしていた. 身体所見や心胸郭比, 循環血液量モニタリングからは適正な体液の状態と判断していた. 本人の同意を得たうえで, 脳内局所酸素飽和度 (rSO2) のモニタリングを行った. HD中のモニタリングでは, HD開始120分後まで緩徐に平均血圧が低下した際, 右前額部において脳内rSO2の低下が観察された. 脳内の虚血性病変を疑い, 核磁気共鳴画像を撮影したところ, 右内頸動脈の高度狭窄が確認された. 単一光子放射断層撮影では安静時の脳血流は保たれていたため, 手術には至らず, 現在も経過観察中である. HD患者の脳内rSO2のモニタリングは脳内虚血性疾患の診断に寄与する可能性がある.

こめ油には不飽和ビタミンEトコトリエノール (T3),フェルラ酸(FA),γ-オリザノール, 植物ステロールなどの食品成分が豊富に含まれていることから,こめ油の健康機能性が注目を集めている。 我々はT3の抗血管新生やテロメラーゼ阻害を報告し,T3が高い抗がん作用を有することを明らかにしてきた。T3は4種の異性体の間で生理活性の強さが異なり,δ-T3が最も高い抗がん効果を示す。T3は通常のビタミンEであるトコフェロールと比較してバイオアベイラビリティが低いため,T3の生理活性を相乗的に高める化合物が精力的に探索されている。本総説では,こめ油成分であるT3とFAを活用した相乗的ながん抑制作用について紹介する。

This paper proposes the evaluation method of seismic performance of shear resisting elements in timber construction with shear walls. Proposed new evaluation method uses two parameters of horizontal stiffness and potential energy of the shear walls. These two design values provide horizontal damage limit and safety limit strengths of stories that have equal quality to those by the summation of load-deformation curves of the shear walls. These strengths by this method have less variety than those by existing simple evaluation method. This method also provides Ds values of stories that consist of different type of shear walls. Ds values of the stories have not been provided by the existing simple evaluation method. Precise safety limit strengths and Ds-values of the stories realize to apply horizontal load-carrying capacity design method for the structures with different type of shear walls. This evaluation method would be possible to apply for other structures than the timber construction as a simplified method.

近年、高齢者が増加し今後、日本は今までに経験をしたことがない高齢化社会を迎えようとしている。それに伴って、従来施行されてきた栄養サポートの理論が少しずつ変化してきている。特に高齢者の場合は、潜在的に生理機能が低下し栄養サポートでは極めて重要な「骨格筋」が減少して、器質性多疾患に陥っている患者が散見される。高齢者の栄養サポートのポイントは「動いて食べる」ことにつきるが、認知症の発症にて食行動の意欲が減少したり、長期臥床において十分な体動が得られない症例も少なくない。特に体動の少ない高齢者や長期臥床患者においてはエネルギー提供量の過不足による新たな問題も生じるため慎重に検討しなくてならない。本稿においては、高齢者の栄養学視点から見た特徴や従来、多くの施設で施行されているエネルギー必要量の算出式の問題点、身体計測等を概説する。

There has been considerable discussion about read-aloud activities in Japanese schools. However, few studies have attempted to investigate extracurricular read-aloud activities. The present study discusses how readers attach meaning to such activities, and, in particular, focuses on a theory of read-aloud activities concerned with how various readers attach meaning.The research was conducted at N elementary school located in the city of Tsuchiura. The school sponsors such activities and has a group of volunteers. There were seven participants consisting of three volunteers, two teachers, and two junior high school students.The research was conducted as follows. First, the researcher conducted semi-structured interviews with the participants. They were asked the following two questions: (1) What is the purpose of read-aloud activities in school? and (2) What is the meaning of read-aloud activities? Next, the researcher made transcripts of all the interviews. Finally, the researcher and a master's student analyzed the transcripts using the SCAT (Steps for Coding and Theorization) method.From this analysis, we identified 45 different meanings that make up “N elementary school's theory”. Up until now, people have undertaken read-aloud activities because they believe that such activities are good for the children being read to. In contrast, the results of this analysis indicate that these activities are good not only for the children as listeners, but also for the junior high school students, volunteers, and teachers as readers. In addition, these activities are good for both the school and the community to which the readers and listeners belong.

国民皆保険制度を維持している我が国において,臨床検査もまた医療保険の枠組みの中で運用されている.総医療費が年々増加する中,検体検査費用は微減しており,医療の根幹をなす臨床検査の質の維持が危ぶまれている.このような経済的圧力の中,臨床的有用性と検査効率を改善する努力が続けられているが,精度管理,検査データの標準化など,効率的運用を行うための問題点は多い.

症例は85歳,女性.ランソプラゾール内服中に水様性下痢が出現した.大腸内視鏡検査で下行結腸,S状結腸から直腸に縦走傾向を呈する線状の引っかき傷様の所見が存在し,いわゆるcat scratch colonの内視鏡所見を呈していた.生検で粘膜上皮直下にcollagen bandを認め,collagenous colitisと診断.ランソプラゾール中止により臨床症状は速やかに改善した.Cat scratch colonを呈した示唆に富むcollagenous colitisの1例と考えられた.
