The inhibitory effects against hyperglycemia of "alleged health foods" containing mulberry leaves as an ingredient were compared using an experimental model of digestion and absorption in rats. Rats, after gastric and portal venous catheterization, were continuously fed an aqueous sucrose solution by intragastric administration, and portal venous glucose concentrations were maintained at a constant level. The rats were administered either five types of mulberry leaf tea or three types of mulberry leaf granules, and the time period during which the portal venous glucose concentration showed levels significantly lower than baseline, i.e., the duration of the inhibitory effect on hyperglycemia, was compared. The results showed that although all of these health foods demonstrated an inhibitory effect against hyperglycemia, a comparison of the efficacy based on the duration of this inhibitory effect revealed large disparities among the products, of a magnitude of 17-fold among the mulberry leaf teas and 11-fold among the granular mulberry leaf products. This discrepancy in the efficacy of these types of products could not be discerned by analogy based on the label or the ingredients described on the product. Although alleged health foods are ordinary foods, and efficacy is not demanded, a uniform level of quality assurance is necessary, given that the general consumer purchases these products in expectation of a certain level of effect.

【目的】<BR> 我々は、高校サッカー選手における運動時腰痛の軽減を目的にメディカルチェックと体幹筋トレーニング指導を行っている。体幹筋機能検査として、Kraus-Weber test 変法大阪市大方式(以下、KW)と Side Bridge test(以下、SB)を用いている。KWは体幹筋機能検査として有用性が数多く報告されている。また、我々はSBの体幹筋機能と運動時腰痛との関連性を報告した。先行研究において体幹筋トレーニングを継続した結果、SBやKWの点数の向上に伴い運動時腰痛が軽減したことを報告した。しかし、腰痛が残存する選手も認められた。本研究では初回メディカルチェック時のKW 、SBの点数から低値である選手、高値である選手に分類しトレーニングを継続したことによるそれぞれの点数の変化と運動時腰痛の関連を調査した。<BR>【方法】<BR> 対象は某高校男子サッカー部員で、平成19年度の1年生10名(身長169.0±4.2cm、体重56.9±5.7kg)と、平成20年度の1年生14名 (身長167.6±6.4cm、体重56.8±5.6kg)の計24名。メディカルチェックにおいてKW 、SBの測定と腰痛に関する問診を実施した。KWは大阪市大方式に準じた。SBは姿勢保持の時間を最大60秒とし片側6点満点、左右で12点満点とした。KW、SB共に負荷量は体重の10%の重錘負荷とした。メディカルチェックは初回、中間時(以下、2回目)と約1年後(以下、3回目)に実施し、体幹筋トレーニングは初回メディカルチェック終了後より開始した。<BR> 平成19年度、平成20年度の初回のKWの点数を合計し、平均点を算出した。平均点が中間群に含まれるよう上位群、中間群、下位群の3群に分類した。SBも同様に3群に分類した。今回は上位群、下位群におけるKW、 SBの点数と運動時腰痛の保有者の変化を調査した。<BR> 統計処理は、多重比較検定にTukey-Kramer法を用い、有意水準を5%未満とした。<BR><BR>【説明と同意】<BR> ヘルシンキ宣言及び、個人情報保護法の趣旨に則り、被験者に研究の趣旨や内容、データの取り扱い方法について十分に説明し、研究への参加の同意を得た。<BR>【結果】<BR> KWは下位群9名、上位群8名であり、SBは下位群8名、上位群8名であった。<BR> KW下位群は初回15.1±2.4点、2回目18.0±6.8点、3回目22.3±7.4点であり、初回と3回目(p<0.05)において有意な増加が認められた。運動時腰痛の保有者は初回5名、2回目3名、3回目4名であった。KW上位群は初回24.6±2.4点、2回目26.8±5.3点、3回目27.0±5.4点であり有意な変化は認められなかった。運動時腰痛の保有者は初回7名、2回目5名、3回目4名であった。<BR> SB下位群は初回3.8±1.7点、2回目7.5±2.4点、3回目8.9±3.3点であり、初回と2回目(p<0.05)、初回と3回目 (p<0.01)において有意な増加が認められた。運動時腰痛の保有者は初回7名、2回目4名、3回目5名であった。SB上位群は初回11.3±0.9点、2回目8.9±3.0点、3回目10.9±2.1点であり有意な変化は認められなかった。運動時腰痛の保有者は初回4名、2回目2名、3回目4名であった。<BR><BR>【考察】<BR> 今回の結果からKW、SB下位群では点数向上に伴い運動時腰痛の保有者が減少した。初回メディカルチェック時の体幹筋機能検査において点数が低値である選手は、体幹筋機能の向上に伴い運動時腰痛の保有者が軽減したと考えられる。<BR> 一方KW、SB上位群では点数に有意な変化は認められなかった。KW上位群では運動時腰痛の保有者は減少したが、SB上位群では運動時腰痛の保有者に変化はなかった。我々は先行研究においてKW 、SB共に点数が高値でかつその点数を一定の期間維持することが、運動時腰痛改善の一要因となる可能性があると報告した。今回の結果からも、KW上位群では点数を一定の期間維持できたことで運動時腰痛の保有者が減少したと考えられる。しかしSB上位群では有意差が認められなかったものの点数を一定の期間維持できていないため運動時腰痛の保有者に変化はなかったと考えられる。<BR><BR>【理学療法学研究としての意義】<BR> 腰痛に対する体幹筋トレーニングの効果と体幹筋機能の客観的な評価や腰痛改善との関連性についての報告は少ない。そこで、我々は体幹筋機能を客観的に評価し、運動時腰痛との関連性について経時的に調査することで運動時腰痛発生の要因を検討してきた。本研究では初回メディカルチェック時の体幹筋機能の評価結果から、運動時腰痛の予防や改善のための具体的な方針を決定しうることが示唆された。

While teaching a class is the foremost task of an instructor, whether class preparation also counts as part of that duty have been unclearly defined. Yet recently, the Labor Standards Inspection Office issued a recommendation toward major educational institutions to recognize class preparation as labor entitled to payment. This, however, raises course management questions regarding what and how much instructors should actually prepare. Utilizing a various data, this paper discusses how much time is required for Japanese language instructors to conduct effective class preparation.

Some proteins which construct human hair were solubilized in aqueous solutioons of surfactants generally applied for shampoos. 1) The aqueous solution of sodium dodecylpoly (oxyethylene) sulfate solubilized smaller amounts of proteins from the hair than that of other surfactants used in this experiment. 2) The amount of solubilized proteins had the relationship with the degree of damage in hair. The higher damaged hair gave larger amounts of solubilized proteins. 3) Solubilization of proteins was influenced with pH of the solution. The minimum amount of solubilized proteins was obtained at pH 56 corresponding to the isoelectric point of human hair. 4) The hair of which proteins was extracted with an aqueous solution of the surfactant had poor water holding capacity and gave much splitted hair caused by combing. These facts have provided valuable imformation on the effect of surfactants on human hair.

本研究は、新たなMRI技術(位相差強調画像化法:PADRE)を用いて、脳内のamyloid beta沈着によるアルツハイマー型認知症による味覚障害を非侵襲かつ早期に見出す診断法の確立を目的とした。ヒトamyloid beta遺伝子導入トランスジェニックマウスのPADRE像と組織染色像とを比較した結果、鉄、老人斑の量、位置ともに月齢に応じた正の相関がみられた。トレーサー実験の結果、ラット大脳皮質味覚野は従来の味覚野および味覚野に接した背側、尾側からも投射があることが分かった。また、リッキングテスト装置の改良に取り組み、効率的な味覚行動実験プロトコルを検索した。

Once there were two Narazaka Slopes. There are two passes passing through Narayama Mountain (though it is only small hill) that extends from the east to the west on the frontier between the Province of Yamato and that of Yamashiro, one on the western ridge is called Utahime Pass and another on the east, Hannya Pass; they are both important roads connecting Yamato with Yamashiro. In this paper, I wish to pay attention to Narazaka Slope on the Hannya Pass.After passing the period when Heiankyo was the capital, in the Middle Ages when the center of Nara moved to the east, and even in the present time, the image of Narazaka Slope seems to be always spun from the bundle of the collective memory twining about this region. Such Narazaka seems to offer a very effective clue for someone who seek for circumstances of how the new memories about the scene are being born. This paper has a character of, so to speak, a preliminary study for the matters mentioned above.Thus, the interest of this paper is directed first to elucidate the character of the border given to Narazaka. While we continue to try to grasp the meaning of various messages about a peculiar “scene” called Narazaka, it is certain that the external image buried in Narazaka as the border will gradually surface to our eyes.However, the memory about the scene is not single. This paper seeks for the circumstances about the generation of various memories traveling through Narazaka in history from the antiquity to the contemporary period, by being led by the clue that gave us a strong impression, out of various memories that must have been accumulated in Narazaka, of its being as the border.Perspective that obtained in this paper wakes up our interest in the performing arts in wider sense of the word, played once around the Narazaka. The separate article entitled “Salvation as the performing art” will be elaborated for discussing such a subject.

クマネズミはthe <i>Rattus rattus</i> species complexとも称され,複数の種からなる種複合体である.日本には古くに移入した東アジア地域起源の<i>Rattus tanezumi</i>(2n=42)に加え,新規に移入したインド地域が起源の<i>R. rattus</i>(2n=38)の2系統が存在する.本研究ではこれらクマネズミ系統の日本列島における分布および移入の歴史を把握することを試みた.毛色関連遺伝子<i>Mc1r</i>(954 bp)をマーカーとし,奄美大島産を含む36個体の塩基配列データを新規に収集し,既存の配列データと合わせ,日本列島の17地点,さらに比較対象として用いたパキスタン産を含め,総計133個体のデータを基に系統学的解析を行った.その結果,小樽,小笠原諸島および東京の3地域で<i>R. rattus</i>型が認められ,これらの地点では<i>R. tanezumi</i>型とのヘテロ接合体も存在した.これらの結果から,既存系統への浸透交雑が一部の市街部,港湾部および離島で進行している実態が明示された.一方,琉球列島の自然林では,<i>R. rattus</i>型の<i>Mc1r</i>ハプロタイプは認められなかった.これは,新たな外来系統<i>R. rattus</i>の定着や浸透交雑を起こさない何らかの要因が存在する可能性を示唆する.琉球列島には独自の<i>Mc1r</i>配列の存在も認められ,他地域とは遺伝的に分化した集団として位置づけられる可能性も示唆された.<br>

深さ優先探索とスタックの利用を学習する CS アンプラグド教材を試作した.高等学校への出張講義で実践した結果を主に用いて評価を行い,その有効性について報告する.具体的には,迷宮に隠されている宝物を全て集めて帰ってくるアクティビティを試作した.これを導入として用いることにより,逆ポーランド表記の四則演算がスタックを用いて省スペース (少ないメモリ) で計算できることの理解を高めることができた.

外傷生胆管狭窄はまれな疾患であり, 悪性胆管閉塞との鑑別が問題となる.今回, 我々は交通事故後に発症した外傷性胆管狭窄の1例を経験したので報告する.症例は70歳の男性, 乗用車を運転中, トラックに追突し右尺骨骨折にて入院, 腹部症状はなかったが, 14日後に黄疸が出現した.超音波, CTでは腫瘍像は明らかではないが, PTCD造影では下部胆管の狭窄と肝内胆管の拡張を認めた.減黄後の胆管造影でも狭窄は改善せず胆管癌と考えられた.胆汁細胞診でも悪性が疑われ, 下部胆管癌を考え膵頭十二指腸切除術を施行した.摘出標本の所見では, 狭窄部では胆管壁が肥厚しなだらかな狭窄を示したが腫瘍性病変はなく, 組織学的にも悪性疾患は否定された.以上より, 交通事故の腹部外傷による胆管狭窄と診断された.本例は腹部打撲の程度が軽かったことも診断を困難とした一因と考えられた.

Early-stage cancer located on the upper part of the stomach, especially near the esophagogastric junction, is still difficult to diagnose, and frequently undiscoverd until it is too late. Minute cancers so far discovered, in particular, are few in number, and much remains obscure about their morphology, , including such problems as strophic and metaplastic changes in the mucous membrane over the said area. We looked into diagnosis and morphology of minute cancers of the said region in patients encountered by us. Subjects consisted of 12 patients with early-stage gastric cancer the size of 2 cm or less in maximum diameter, a margin of which was within distance of 2 cm from the junction. Total 13 such lesions were found in them. Our findings were as follows : (1) Straight-view fibescope is more useful in observation snd biopsy of small lesions adjacent to the junction, while lateral-view fiberscope has slight advantages with small lesions off the junction, (2) of small, early-stage gastric cancers near the junction, those adjacent to the junction are frequently protuberant, and those off the junction tend to be concave, (3) the majority of small, early-stage gastric cancers near the junction are histologically highly differentiated.

異常ヘモグロビンとはグロビン鎖のアミノ酸配列異常を呈するヘモグロビンの総称である.鎌状赤血球貧血患者の研究に端を発し,Hb M-Iwateの発見により日本人においても異常ヘモグロビン症が証明された.異常ヘモグロビン症は等電点電気泳動法の応用や技術改良による検出頻度の拡大を経て,現在ではヘモグロビンA1cの測定に用いられる高速液体クロマトグラフィの異常クロマトグラムより発見される例が増えている.今回われわれは,非血縁2家系,4例の異常ヘモグロビン症を経験した.同疾患は遺伝性疾患であり人口移動の少ない地域では出現に注意する必要がある.異常クロマトグラムであってもヘモグロビンA1cが測定される場合があり,クロマトグラムのパターンに注意を要する.遺伝子解析には十分に配慮する必要があるが,不要な検査,治療を避けるためにも患者が自身の病態について理解しておくことは重要と考える.普段から臨床医とコミュニケーションを図り,適切な情報を臨床側に提供していく必要があると考えられる.
