本研究は,リバース・ナレッジ・フローという現象,すなわち転職によって成員を失うことを契機として元組織に知識が逆流する可能性について検討する.日本の半導体関連技術の発明者の転職データを分析した結果,流動性の高い米国で指摘されてきたリバース・ナレッジ・フロー現象は,流動性の低い日本 においても確認できた.ただし,複数人の転職者が存在する場合には,この知識移転効果が制限される可能性が示唆された.

本研究の目的は, Lazarusらの心理的ストレス理論に基づいた対人ストレスモデルを提唱し, その妥当性を検証することである。本モデルではパーソナリティ→媒介過程 (認知的評価→コーピング)→精神的健康といった因果関係が仮定されている。大学生227名を対象に, パーソナリティ (統制の所在, 楽観性, 自尊心), 認知的評価 (重要性, 対処効力感脅威), 対人ストレスコーピング (ポジティブ関係コーピング, ネガティブ関係コーピング, 解決先送りコーピング), 精神的健康 (友人関係の満足度, 心理的ストレス反応) を測定した。パス解析の結果から, 対人ストレスモデルの妥当性が検証された。部分的に, パーソナリティからコーピングへ有意な影響が確認されたが, パーソナリティは認知的評価を媒介としてコーピングに影響を及ぼしていることが実証された。ポジティブ関係コーピングと解決先送りコーピングから友人関係に関する満足感に対して有意な正の影響が確認された。ネガティブ関係コーピングから友人関係満足感に対しては, 有意な負の影響が確認された。また, 解決先送りコーピングは心理的ストレス反応を減少させ, ネガティブ関係コーピングは心理的ストレス反応を増加させることが実証された。

症例は88歳男性.若年期に完全内臓逆位を指摘されていた.上部消化管内視鏡検査で胃前庭部小彎後壁に20mm大の0-Ⅱc型早期胃癌を疑う病変を認めた.左側臥位では病変の存在する前庭部の伸展が不良であったため,右側臥位で再度内視鏡検査を行った.右側臥位では左側臥位と比べ前庭部の伸展は保たれていた.しかし,右側臥位では通常と異なる術者姿勢と周辺機器配置を要し,ESDにおける繊細な内視鏡操作は困難であることが想定され,実際のESDは左側臥位にて行った.左側臥位におけるESDでは合併症なく一括切除可能であった.完全内臓逆位における早期胃癌に対しESDを行う際,左側および右側の側臥位における術前シミュレーションは有用であると考えられた.

以下に紹介するのは、20世紀前半のアゼルバイジャンを代表する民族運動の指導者マンメド・アミン・ラスルザーデ(1884-1955)の晩年の著作『あるトルコ民族主義者のスターリンと革命の回想』の冒頭部分の翻訳である。この回顧録は、もともと1954年にトルコの新聞《デュンヤ》紙に連載された。著者ラスルザーデは、20世紀の初めからロシア帝国内の産業都市バクーで政治活動に入り、その中で労働運動の組織化に携わっていたボリシェヴィキの活動家スターリンとも接触をもつようになった。彼はアゼルバイジャンのみならず、イランやオスマン帝国(とトルコ共和国)、さらにはヨーロッパでも幅広い活動に従事したが、今回はまず、スターリンとの出会いにいたるまでを扱った、回顧録の冒頭部分を訳出した。

Marchiafava-Bignami病 (以下, MBD) 自験8症例において, 急性期および慢性期の症候と病巣について検討した.8例中6例は意識障害にて発症し, 慢性期に構音障害と半球離断症候を呈し, 画像検査にて脳梁に限局した病変を認めた.これらは従来報告されているMBDの臨床像と一致していた.一方, 発症時に意識障害を呈さない例が2例みられた.これは, 発症時の意識障害の存在が必ずしもMBDの診断に必須ではないことを示している.また, 2例では肢位の異常, 固縮などの錐体外路症状がみられ, そのうちの1例ではMRIで両側の被殻に病変が認められた.同様の症例の報告は過去に4例のみであるが, この症候もMBDの症候の1つとして注目される.慢性期には意識障害, 前頭葉症状は消失するが, 錐体路症状が構音障害, 半球間離断症候とともに長期にわたり持続することが示された.以上から, MBDの臨床像は急性期, 慢性期ともに従来考えられていたよりも多彩である可能性がある.従って, アルコール多飲者が急性の構音障害, 歩行障害, 痙攣などを呈し, 局所徴候が明らかでない場合には, MBDの可能性も考え, 画像検査で脳梁病変の有無を確認する必要があると思われた.

[Purpose] This study aimed to assess the correlation between lateral thrust and clinical symptoms after high tibial osteotomy and determine lower limb alignments that may decrease lateral thrust. [Participants and Methods] We included 54 patients (73 knees) who underwent high tibial osteotomy. Clinical symptoms, including the Japanese Orthopaedic Association score and the hip–knee–ankle angle measured via radiography, were assessed 12 months postoperatively. Lateral thrust was measured using three-dimensional motion analyses. Logistic regression was used to calculate the cut-off values with good Japanese Orthopaedic Association score and lateral thrust as dependent variables and both lateral thrust and hip–knee–ankle angle as independent variables. [Results] The lateral thrust cut-off was 3.1° (sensitivity: 0.83; specificity: 0.74; area under the curve: 0.76), while that of the hip–knee–ankle angle was 1.9° of valgus (sensitivity: 0.71; specificity: 0.81; area under the curve: 0.72). [Conclusion] Good clinical outcomes after high tibial osteotomy can be expected with a lateral thrust of ≤3.0°, indicating that the target hip–knee–ankle angle should be 2.0° valgus. In cases where valgus alignment is insufficient, lateral thrust may develop, which should be assessed using gait analysis.

The difference in sex between the author, the narrator, and the characters and their interconnection (the difference in sex of the reader could be included as well) is a fascinating topic, but one that is difficult to pursue. In an attempt to offer an approach this subject, this presentation will look at Dazai Osamu’s female monologue novels. Dazai, from the start of his creative career, devoted himself to meta-fiction and similar narrative styles, but from the 1930s to the 40s during the Asian-Pacific War, he focused on writing novels narrated by women. “Joseito” and the post-war work “Shayo” are supposed to be taking “quotes” from the already existing diaries of women, but these “quotes” are almost indistinguishable from “theft”. Rather than simply looking at similarities in expression as “theft”, I would like to focus on what kind of moral problems arise from a male author borrowing (stealing) the voice of a female narrator and the differences that arise due to differences in expression. The special period relevance and connectedness between the war and post-war periods, and if possible comparisons with Uno Chiyo and other female writers of narrative works at that time, will also be looked at.

Hume distinguishes two cases of personal identity in the Treatise, one with regard to ʻour thought or imaginationʼ and the other with regard to ʻour passions or the concern we take in ourselvesʼ. First, I consider Humeʼs concept of character from the perspectives of durability and revisability. I propose to interpret Humeʼs ʻcharacterʼ as the habit of particular ways of forming beliefs from these perspectives. Then I examine the reasoning from character to actions, and the relation between moral judgment and necessity in voluntary actions. Finally I present Humeʼs framework of the link between person and character evaluated in terms of reciprocal relationships in human life. Through this investigation, I show that Hume discusses, though implicitly, the second case of personal identity in terms of the concept of character in the three books of the Treatise.

地域連携薬局と専門医療機関連携薬局の知事認定制度は2021(令和3)年8月に開始した.そこで,薬局機能情報提供制度の情報を活用して,各薬局の機能を分析し,千葉県の各医療圏の薬局が地域連携薬局の認定基準をどの程度満たしているのかの実態を明らかにすることを目的とした.解析には千葉県内の薬局の薬局機能情報を用い,2019年は2,405件,2020年は2,430件の薬局を対象とした.解析項目は地域連携薬局の認定基準から一部を抜粋した.全項目のうち,最も達成率が低い認定基準は “地域における医療機関に勤務する薬剤師などに対して報告および連絡した実績” であり,達成していたのは,2020年で千葉県全体の2.3%(57薬局)だけであった.無菌製剤の調剤応需体制を有する薬局も少なく,自らの薬局の無菌調剤室を利用している薬局は,千葉県全体で2020年は87薬局(3.6%)しかなかった.在宅業務の実施に関連する要因を調べるために2項ロジスティック解析を行った.結果として “麻薬の調剤応需体制を有する”,“無菌製剤の調剤応需体制を有する”,“健康サポート薬局である”,“お薬手帳(電子版)に対応している” の4項目が在宅業務の実施に関連する要因であると判明した.

It is widely known that PTFE(polytetrafluoroethylene) produces harmful gases, for example, C3F6, C2F4, and HF by pyrolysis. In this study, CaCO3 or Ca(OH)2 were added to PTFE to prevent from producing harmful gases and to fixate F in the residue as CaF2 by acid-base reaction between PTFE and CaCO3 or Ca(OH)2. In a series of pyrolysis experiments, F in PTFE was fixated in the residue as CaF2. The yields of F as a CaF2 were about 20-40wt% in the case of CaCO3 addition and 50-60wt% in the case of Ca(OH)2 addition. To raise the yield of F as CaF2, Ca(OH)2, PE(polyethylene) and zeolite were added to PTFE. About 80wt% was obtained for the yield of F as CaF2. It is revealed that simultaneous pyrolysis of PTFE and PE promotes the formation of HF and is effective in the fixation F as a CaF2.

粘液水腫性昏睡は,甲状腺機能低下症が基礎にあり,直接あるいは何らかの誘因で中枢神経系の機能障害をきたす内分泌救急疾患である.稀であるが,1990年以後も死亡率が20~50%と高いと報告されている.粘液水腫顔貌や中枢神経症状に加え,低体温,循環不全,呼吸不全など,障害が多臓器に渡ることが多い.甲状腺ホルモンを直ちに開始する必要があるが,投与ホルモンの種類,経路,量には未だ確立されたものがない.現在,日本甲状腺学会が「粘液水腫性昏睡の診断基準と治療指針」を作成中であり,診断基準第三次案も合わせて報告する.

術中に低血圧を伴う発作性心房細動(Paroxysmal atrial fibrillation : Paf)を発症した2症例に超短時間作用型β-ブロッカーであるランジオロールを投与した. いずれの症例も投与開始後5分で心拍数の減少を認め, 10分後には洞調律に回復し, 血圧も安定し, 以後心房細動は出現しなかった. 2症例とも術中術後の循環動態や呼吸状態は良好に経過し, 血液検査上も異常を認めなかった. ランジオロールはPafに有効であり, 安全に使用できる可能性が示唆された.
