鼻腔(右鼻中隔)原発の Glomangiopericytoma の症例を経験した。症例は 80 歳、男性で、鼻閉感を主訴に前医より紹介受診となった。右鼻腔下鼻甲介前端よりやや後方から、後鼻孔を占拠する表面平滑で、血管豊富な腫瘍を認めた。画像検査および病理組織診の結果、鼻腔原発の Glomangiopericytoma と診断した。生検時易出血性であったものの、腫瘍は鼻腔に限局しており、周囲への浸潤も無かったため、経鼻的内視鏡下腫瘍切除術を施行した。術後合併症および、術後局所再発は認めていない。Glomangiopericytoma は近年まで Hemangiopericytoma と認識されていたものである。良性腫瘍であるが、報告により差はあるものの術後局所再発率が高く、外科的治療に際しては完全切除が求められる。本症例のように鼻腔に限局している場合、経鼻内視鏡下手術は有効であると思われた。

統合失調症の発症原因解明,予防や治療法の確立の手段の一つはヒトに類似した動物モデルを作製し,解析することである。ヒト疾患のモデル動物確立の際,3つの条件(妥当性)を満たす必要がある。①表面妥当性,②構成概念妥当性,③予測妥当性によって評価される。本研究では発症までの時間軸を取り入れた簡便な統合失調症モデルマウスの作製が急務であると考え,妊娠期のマウスに神経新生抑制薬であるメチルアゾキシメタノール酢酸(MAM)を投与し,出生した雄性マウスの行動解析および神経化学的検討を行った。胎生期MAM曝露マウスは,陽性症状様の運動亢進,陰性症状様の社会性低下,短期記憶・感覚情報処理の障害を惹起させた。これらの行動障害は,56日齢以降に顕著であり,統合失調症が思春期以降に発症することと一致していた。前頭前皮質におけるドパミン神経系の変化,海馬錐体細胞の形態的変化が認められた。胎生期MAM曝露マウスの行動障害は定型抗精神病薬では運動過多以外は改善されず,非定型抗精神病薬ではすべての障害が改善された。以上のように,胎生期MAM曝露マウスは,ヒト疾患のモデル動物確立の際,3つの条件(妥当性)を満たし,胎生期MAM曝露“ラット”よりも優れた統合失調症のモデル動物に成り得る可能性を提唱した。

国の分野横断型統合ポータル「ジャパンサーチ」は、我が国の幅広い分野のデジタルアーカイブのメタデータを集約し、多様なコンテンツをまとめて検索・閲覧・活用できるプラットフォームである。ジャパンサーチでは、コンテンツの二次利用を促進するため、分かりやすい利用条件の表示等に取り組んできた。本発表は、ジャパンサーチの活用事例と機能について紹介した上で、ジャパンサーチ上の二次利用条件整備の取組を解説するとともに、データのオープン化に向けた課題及び事例について報告するものである。

側頭葉の限局性萎縮により生じる意味痴呆と呼ばれる病態では,言語・相貌・物品など広範な対象についての意味理解が選択的に障害される。意味痴呆における葉性萎縮のパターンには,通常左右差が認められるが,萎縮の優位側に特異的な認知機能障害については,いまだ十分な合意的見解が得られていない。そこでまずわれわれは,左優位の萎縮例と右優位例の神経心理学的比較検討から,左右側頭葉の役割分化に関する手がかりを得ようと試みた。その結果,典型的な語義失語像を呈した左優位例では,呼称,語産生と,理解に関する項目で右優位例を下回り,知能検査についても言語性検査の成績低下が著明であった。一方右優位例では,総じて語義失語の程度はやや軽度で,代わって熟知相貌の認知障害,物品の認知ならびに使用障害を呈したが,言語の諸機能はまんがの理解を除き左優位例に比べ成績低下が軽度であった。また知能検査では言語性,動作性ともに低下し,何らかの視覚性知能の障害も併存している可能性が示唆された。さらに諺と物品という,それぞれ言語性・視覚性と異なる表象の保存状態を調べる補完課題を用いた比較では,どの意味痴呆患者も何らかの補完課題の障害を認めたが,諺の補完課題での成績低下が著明で物品の補完は比較的保たれる左優位例に対し,おもに物品の補完課題に著しい困難を呈し,諺では補完が比較的保たれる右優位例,という二重乖離が認められた。これらの結果は,左側頭葉前方部が言語性の,また右側頭葉前方部は視覚性の表象を司る神経基盤として重要であることを示唆している。

はじめに:腰部脊柱管狭窄症に対する監視下での理学療法(PT)は,非監視下での運動療法よりも短期的には有効であるが,その効果が持続するかについては明らかとなっていない.本研究の目的は,監視下でのPTと非監視下での運動療法の1年後の治療成績を比較することである.対象と方法:43例がPT群(週2回6週間),43例がホームエクササイズ(HE)群に無作為に割り付けられた.PT群は,徒手療法,個人に合わせたストレッチと筋力増強,自転車エルゴメーター,体重免荷トレッドミル歩行を実施した.主要評価項目は,チューリッヒ跛行質問票(ZCQ)の重症度とし,1年後の患者立脚型アウトカムと手術移行率を2群で比較した.結果:PT群はHE群に比べ,1年後のZCQ重症度と身体機能,日本整形外科学会腰痛質問票の腰椎機能障害,SF-36体の痛みと全体的健康感のMinimum clinically important difference達成率が高く,手術移行率は低かった(P < 0.05).結語:監視下での理学療法は,非監視下での運動療法に比べ,1年後でも有効で,手術移行率も低かった.

Okura tunnel is located in Route 4 of Kitakyushu Urban Expressway. The road in the up-line that has two lanes is restructured wider to have three lanes within 170m range from pithead. The construction is carried out while the two lanes are open by installing protectors as guardrail inside the tunnel.Road widening without traffic control by using protectors is conducted in other cases before but this construction is recognized to be the first case for widening of two lanes to three under urban expressway's heavy traffic.This report presents the construction's plan and design summary, and discusses the differences in the design and the actual construction of tunnel excavation.

The purpose of this study was to clarify the mechanism of rotator cuff tears seen in baseball players.Forty baseball players who injured their shoulders through repetitive throwing and underwent shoulder arthroscopy were studied retrospectively using a videotaped arthroscopic tour. They ranged in age from 13 to 48 years (average: 24 years) and were classified into 3 groups according to the rotator cuff findings: articular-side partial rotator cuff tear (APRCT)in Group 1 (29 patients), cuff inflammation only in Group 2 (5 patients), and intact cuff in Group 3 (6 patients). The existence and the site of rotator cuff tears, labral tears and notches of the greater tuberosity were examined in each group.All APRCTs were located around the posterior aspect of the supraspinatus tendon (SSP) and the anterior aspect of the infraspinatus tendon (ISP). Localized splits of the posterior rotator interval were recognized in 5 patients. In most of the other patients in Group 1, the deepest APRCT was also located in the posterior rotator interval. Posterior-superior labral fraying was seen in 22 patients (17/29 in Group 1,0/5 in Group 2, and 5/6 in Group 3). However, posterior labral lesions localized to the superior portion were only recognized in 4 patients in Group 1 and SLAP lesions were combined with this posterior lesion in 5 patients. The remaining 8 patients in Group 1 had labral fraying of the whole posterior portion. A notch of the greater tuberosity around the insertion of the ISP was recognized in 23 players, which showed it had a significant relationship to cuff lesions (21/29 in Group 1,2/5 in Group 2, and 0/6 in Group3).The most significant APRCTs were located around the posterior rotator interval between the SSP and ISP. APRCTs caused by repetitive throwing activity may be initiated by tearing between the SSP and ISP due to an eccentric or shearing force. They were not always associated with posterior-superior labral lesions as predicted by the internal impingement theory.

酵母Candida albicansは,日和見感染の原因菌としてごく普通に見られる代表的真菌である。近年,C.albicansを材料とする従来の多くの研究課題に,遺伝子を中心とする分子生物学的手法が取り入れられようとしている。その遂行のために不可欠となる基礎的な事項をここでまとめてみた。C.albicansをはじめとする数種のCandida属酵母は,核の遺伝暗号で通常ロイシンに翻訳されるCUGを,セリンとして読むことがわかってきた。現在までに多くの遺伝子が単離されているが,中でも自律複製配列が単離されてベクターに組み込まれることにより,形質転換の系も整備されつつある。パルスフィールドゲル電気泳動と稀切断制限酵素Sfi Iの組み合せにより,大まかなゲノムマップが作成され,単離された遺伝子が存在する染色体の特定と物理的マッピングが可能になってきた。以上のような情報を載せたCandida albicans serverがインターネット上で構築され,公開されている,等について紹介する。また,我々をはじめとする幾つかの研究室で取り上げられているC.albicansゲノム内の反復配列群についても言及する。

わが国での実施に適した新規認知行動療法プログラムである反芻焦点化認知行動療法(rumination-focused CBT: RFCBT)の実践を目的に、本研究ではRFCBT治療者用ハンドブックを作成し、パイロットケース(5例)としてRFCBTを行い、そのfeasibilityを確認した。さらに、原著者のEdward Watkins教授らのもと、面接音声ファイルを用いたスーパービジョンを受け、この経験を踏まえRFCBT治療者用ハンドブックの改良を図った。そして、うつ病に対するRFBCTの前後比較研究プロトコール(UMIN000037191)を作成し、実施した。

In this research, the dynamic characteristics of a small wheel bicycle are investigated. The bicycle model is formulated by using multibody dynamics. Then, the stability of bicycles is analyzed by parametric study using the bicycle model. The effects of the parameters of tire diameter and head angle are examined by focusing on a small wheel bicycle. The straight-ahead stability and upstanding stability are evaluated at each parameter. The results show the tendency of stability at each parameter and the influential parameter to the small wheel bicycle is found. The driving experiment using the small wheel bicycle is performed. In the experiment, the stability of the small wheel bicycles with variable head angle is evaluated by the subjects. It is confirmed that varying head angle increases the stability of the small wheel bicycle. The results are corresponding with the simulation results and it is shown that the simulation captures the tendency of the stability and expresses the characteristics of the small wheel bicycle.

Through the analysis of textbooks, this paper reveals the structure of research topics on the sociology of work in postwar Japan. The analysis revealed the following points. (1) The number of textbooks in the sociology of work has decreased compared to sociology textbooks in other fields. (2) Until 1970, the structure of topics was centered on organizational theory; since the 1990s, this structure has become fragmented. These findings correlate the view of previous studies that the sociology of work in Japan has stagnated. In addition, this demonstrates the need for a “sociology of the sociology of work.”

哺乳類とは異なり、鳥類の聴覚器官である基底乳頭では、有毛細胞再生が自発的に誘導され、聴覚機能も再生される。鳥類とは異なり、哺乳類では有効性が期待できるレベルの聴覚機能再生は報告されていない。近年、鶏に関する遺伝子情報が充実し、網羅的遺伝子解析手法を用いて、これまで困難であった鳥類における有毛細胞再生に関連する遺伝子およびシグナルの詳細な分子生物学的解析が可能となった。鳥類における旺盛な有毛細胞再生機構を哺乳類における有毛細胞再生活性化に応用し、哺乳類蝸牛における有毛細胞再生効率を向上させ、新しい感音難聴治療薬開発につなげる。

The purpose of this study was to make a proposition on the instruction in decision making in ball games from the theoretical standpoint. To put it concretely, the outline of contents and methods of the process-oriented instruction in it was given in this study.Firstly, a conceptual linear model of decision makig in ball games, which is consisted of four stagesselective attention to outer game situation (which means the external environment subsisting objectively), recognition of game situation (which consists of objective and subjective environmental elements), anticipation of game situation, decision on play-was presented, and the main points of instruction were discussed in each stage of the model.Secondly, the subjects of the method on field/ court, the method in room, and selecting game situations were discussed about the methodology of instruction.Resting on the basis of the proposition in this study, more improved practice and many corroborative studies on decision making in ball games should be proceeded in future.

本研究の目的は高齢者の社会参加と主観的幸福感の関連において,ソーシャル・キャピタルがどのような役割を果たしているのかを明らかにすることである.地域在住高齢者のデータ(74〜78歳,N=624)を分析した結果,社会参加と主観的幸福感の関連は認められないが,4種類の社会参加のなか,地縁組織への参加は主観的幸福感と統計的に有意な関連があること,また,ソーシャル・キャピタルへの認識と近所の人の数を介した間接効果があることが確認された.この結果から,社会参加の種類によって主観的幸福感との関連が異なり,地縁組織への参加はソーシャル・キャピタルを介して主観的幸福感に影響を与えることが明らかになった.

近年,スポーツにおけるITの活用が注目されており,試合分析や指導に用いられている.バレーボール競技においては,狭いコート上で選手が位置を移動しながらボールを追い,各ポジションの役割をこなす.各選手の動作の質や速さ,移動量などは今後の試合展開に大きく寄与する. 本研究では,試合動画からトスやレシーブ,スパイクなど選手の動作を検出し,指導や試合分析に活用する.バレーボール競技の試合動画を,選手の動作を学習させたモデルにかけ,検出する実験を行った.動画からの選手位置・動作の検出にはディープラーニングの物体検出アルゴリズムのひとつであるYOLOを用いた. 結果として,選手の位置と動作を検出することができた.

47歳,女性。ウガンダに渡航して7日目に右前腕屈側にそう痒を伴う紅色丘疹が出現した。帰国後,紅色丘疹が増加したため渡航20日目に近医皮膚科を受診した。副腎皮質ステロイド含有軟膏の外用を開始したが,右肩と腰部の紅色結節が増大し疼痛を伴うようになったため,渡航24日目に当院を受診した。数日後紅色結節の中心に虫体を認め,局所麻酔下に4匹の虫体を摘出した。虫体はクロバエ科のヒトクイバエの3齢幼虫と同定した。摘出1ヶ月後,潰瘍は上皮化し,その後症状の再燃は認めていない。ヒトクイバエは衣服や布団などに付着した虫卵を介して寄生する。寄生を予防するためにはヒトクイバエの生息地域への渡航後は衣服にアイロンをかけることが重要であり,ハエ症を疑った際には,身近な人の渡航歴も聴取する必要があると考えた。(皮膚の科学,13: 415-420, 2014)

厚生労働省の 「輸血療法の実施に関する指針」 が2005年9月に改定された後, AB型患者の大量出血に対して異型適合血輸血を考慮した2症例を経験した. 緊急輸血に携わる医療従事者は, 異型適合血輸血の適応に関する認識を新たにするとともに, 危機的出血に際して迅速に適合血の選択を確認できるよう小冊子を配置し, 定期的な勉強会を開催するなど, 速やかな緊急輸血実施のための院内体制の整備に努める必要があると考えられた.
