小細胞癌は肺に好発する腫瘍である。頭頸部に発生する小細胞癌は比較的まれであるが一般に悪性度が高く,高率に遠隔転移を生じ予後不良とされる。頭頸部原発小細胞癌の治療は未だ確立されたものはないが,肺小細胞癌の治療に準じて行われることが多い。限局性で早期の症例や手術を組み合わせた集学的治療の報告もあるが,放射線・化学療法による治療が行われる。今回われわれは,喉頭と副鼻腔が原発と考えられた小細胞癌の2症例について報告する。

本稿の目的は,グローバル社会における付加価値税の新しい潮流として,課税の効率性と公正性を備えたニューVATに焦点を当てることである。これまで付加価値税の標準モデルとしてEUモデルが考えられてきた。しかし,1993年のEU成立と同時に,EU域内取引が自由化されたことによって,加盟国の付加価値税制度の相違点(例えば,複数税率や非課税制度等)による問題がより顕在化するようになってきた。このため,EUモデルは, オールドVATとして制度改革が不可避となっている。これに対して,グローバル社会における付加価値税の新しい潮流として,効率性と公正性を備えたニューVATが注目されており,その代表が,ニュージランドモデルである。その基本的な考え方は,複数税率や非課税制度を廃止し,「単一の標準税率構造と広い課税ベース」とするシンプルなものである。このような考え方に基づくニューVATが,オールドVATの直面している問題の多くを改善することを明らかにする。

日本薬剤疫学会では,医薬品リスク管理(Risk Management Plan:RMP)を作成,実行する側の製薬企業と医療現場およびアカデミアからなるタスクフォースを設置し,2012年4月に厚生労働省より発出された医薬品リスク管理計画指針通知に明記されているICH E2E に準拠した安全性監視計画(Pharmacovigilance Plan:PVP)が立案可能となるようなガイダンスを作成した.内容は以下の 6つから構成されている.1.はじめに:市販後安全性監視に係るこれまでの当学会活動や,活動の目的2.安全性検討事項(Safety Specification:SS)の選択と特徴を記述するためのプロセス・SS をどう選択するか・SS をどう特徴付けるべきか・リサーチ・クエスチョン(Research Questions:RQ)にどうつなげるか3.RQ の決定と記述・RQ とは何か・各種ガイドラインではどう扱われているか・PVP へ RQ を記述する方法と具体的事例・PVP 全体としてみた最適とは4.RQ に最適化された PVP・通常の PVP で可能か,追加の PVP が必要か・追加の PVP のデザインの選択について(RQ と研究デザイン,PICO を用いた RQ の記述,評価の指標)・PVP の記載事項チェックリスト作成について5.結語:使用成績調査の位置づけ,背景発現率と比較群の必要性,今後の PVP の課題6.別添:PVP の記載事項チェックリスト以上をもって医薬品リスク管理計画指針に明記されている「ICH E2E ガイドラインに示されている安全性検討事項及びそれを踏まえた医薬品安全性監視計画」が作成,実行できることを期待したい.

Induction motor drive with inverter control, which can reduce the total weight of the system, has been introduced to the Shinkansen trains in order to achieve high speed transportation.The individual-motor-control type 953 train, which is a part of the STAR21 prototype Shinkansen for high speed experimental run, was used for measurement running, in which we observed wheel slip and adjusted re-adhesion control parameters at the speed of 200 to 300km/h. It was found that the slip detection within one percent speed difference from the reference, was possible by adopting sensitive detecting method. Quick torque reduction by sudden reduction of motor current was necessary for re-adhesion control, and one-second waiting time before re-applying current was also inevitable. For re-applying the current, an exponential curve with four-second time constant was proved to be effective.By these tests, high speed performance and re-adhesion characteristics improvement by adjusting control parameters were obtained. To the next step, for example creep control, which utilizes fine continuous slip, should be investigated in order to keep enough torque minimizing current reduction during re-adhesion control.

電気防食状態にあるコンクリート中の鋼材の腐食速度を管理指標とするためには,電気防食状態の腐食速度評価手法の確立が必要である.筆者らは電気防食状態にある湿潤環境におけるコンクリート中の鋼材の腐食速度評価手法として,RAP法(Reciprocating Anode Polarization Curve Measurement Method)を提案しているが,大気環境でのRAP法の評価精度は未検討であった.本研究ではモルタル試験体を用いた室内試験より,RAP法の大気環境における評価精度を検討した.RAP法は,自然腐食状態および電気防食状態ともに,既往研究で用いられるターフェル外挿法と比較して精度が向上することを確認した.

cDNA display is a robust and versatile in vitro protein selection tool developed on the basis of mRNA display. In this tool, a protein is covalently linked with its coding cDNA via a specially-designed puromycin-linker to improve the stability of the molecule. As a result of the stabilization, cDNA display succeeded in extending the variety of libraries (e.g., modified peptide library) and target molecules (e.g., cell surface molecules). Thus, this technology will enable to design functional proteins and peptides which could not be evolved by the previous in vitro protein selection tools.

In this study, the agricultural utilization of coffee grounds was examined. The results were as follows. 1. The application of coffee grounds in soils inhibited the seedling growth of Chinese mustard. From experiments on nitrogen and toxic substance alleviation, it was suggested that this inhibition is rcsultant of nitrogen starvation and the existence of toxic substances. However, in the summer, when 2.5kg m^<-2> of coffee grounds was applied together with nitrogen fertilizer at 15 to 20g m<-2>, the coffee grounds didn't inhibit the seedling growth of Chinese mustaard with a maturing period of more than one month. 2. When more than 10 kg m<-20> of coffee grounds was applied into soil, the aggregated structure of soil was developed.

Due to the increasing enrolment in universities and their role as a hub for lifelong participation in sports, the improvement of the sports life of university students is now positioned as an important issue. A review of previous studies on the sports life of university students identified the following issues: the existence of a quarter of Non-exercisers, half of those who are members of an athletic club exercise six or more days a week, and half of those who are members of a circle use off-campus facilities and bear the financial and time costs. However, no studies have examined these issues according to the characteristics of the university or department to which the students belong. Therefore, the purposes of this study were to examine the sports life of students according to the characteristics of their universities and faculties, and to clarify the details of their actual conditions and issues.A questionnaire survey was conducted at 41 universities across the country, asking questions about current exercise frequency, barriers to exercise, current exercise status, and venues and costs of activity. The participants were categorized by athletic environment into “Members of an athletic club”, “Members of a circle”, “Other exercisers” and “Non-exercisers”. Further analysis was conducted based on three characteristics: the “Establisher”, “University size”, and “Faculty”.As a result of the analysis, the following four points were identified as the actual conditions and issues in the sports life of university students.1: Non-exercisers are most common among students at large universities, public universities, and general faculties.2: There was a high frequency of physical education and sports department activities at medium and small universities.3: Circle students at large universities and private universities do not have access to on-campus facilities, and the cost of their activities is high.4: Students at small universities, public universities and general faculties feel that they do not have the opportunity to exercise.

社交不安の認知モデルによると、ネガティブで歪んだ自己イメージが不安の維持に重要な役割をもつと指摘されている。本研究の目的は、情動価の異なる自己イメージの保持が、社会的状況でのパフォーマンスへ与える影響を検討することであった。高社交不安詳と低社交不安詳の参加者を三つのイメージ条件に振り分け、スピーチ前にネガティブ、ポジティブ、統制のいずれかの自己イメージを生成させ、そのイメージを保持したまま、スピーチ課題を行った。その結果、ネガティブな自己イメージを保持した高社交不安者は、統制条件と比較して、不安症状が他者に見えていると評価した。評定者評価においても、ネガティブ条件のスピーチ場面のパフォーマンスの悪化が示された。ポジティブな自己イメージを保持した高社交不安者は、ネガティブ条件と同様に、状態不安が増加したが、不安症状が他者に見えているかどうかの評価は現実的であった。本研究の結果から、ネガティブな自己イメージは高社交不安者により否定的な影響を与え、ポジティブな自己イメージは、ネガティブな自己イメージとは異なる影響をもつことが示唆された。

本研究の目的は、特定行為を含む診療に関わる診療看護師が、職務上経験した倫理的問題を把握することである。日本NP教育大学院協議会が資格認定した診療看護師249名を対象とし、無記名自記式質問紙調査を行い54件の回答を得た。その結果、「患者の身体拘束・鎮静をすること、しないこと」看護の倫理原則である「患者の権利擁護」や「十分なケアを提供できない人員配置」の問題を高い頻度で経験していた。さらに、医師との信頼関係の構築や、看護師の上司や同僚との対人関係の問題に悩んでいた。診療看護師が抱える倫理的問題は、臨床看護師と同様の結果であり、看護師として倫理的感性が結果に反映されていると考えられる。加えて鎮静に関しては、診療看護師の経験として重要な問題となる。診療看護師は、医師や看護師との関係性に問題を抱え、教育の課程領域による問題の違いも示唆された。大学院修士課程での看護倫理教育をさらに発展させる必要がある。

A single specimen (153.7 mm total length) of large congrid leptocephalus, collected at a depth of 400 m off Kuro-shima Island, Osumi Islands, Kagoshima Prefecture, Japan, was subsequently identified as Congriscus maldivensis (Norman, 1939), having the following combination of characters: 46 lateral-line pores before anus; 149 total vertebrae, including 46 abdominal vertebrae; maximum body depth 27.2% of total length (TL); pectoral fin length 40.6% of head length; highest margin of head profile anterior to pectoralfin base; snout rounded, its tip anterior to tip of lower jaw; maxilla and mandibular teeth conical, uniserial; anterior nostril with short membranous tube, below snout margin; posterior nostril rounded, anterior to eye; posterior end of mouth posterior to vertical through middle of eye; cephalic sensory pores and lateral line developed; anus slightly anterior to middle of body; dorsal, anal, pectoral, and caudal fins present (dorsal-, anal-, and caudal-fin membranes fused); and dorsal fin origin slightly posterior to ventral through pectoral-fin base. A neighbor joining tree based on mtDNA cytochrome c oxidase subunit I (COI) gene sequences placed the specimen within a clade of C. maldivensis, based on previously published sequences, thereby supporting its conspecificity with C. maldivensis. The specimen was tentatively determined as a metamorphic larva, based on the following features: 153.7 mm TL, head length 12.7% of TL, and body opaque, with melanophores around the midline. An Indo-West Pacific species, C. maldivensis has previously been recorded from Tanzania to the Philippines, Wallis and Futuna Islands, and Australia, the present specimen therefore representing the first Japanese record and northernmost record of the species. The new standard Japanese name “Nan’you-okianago” is proposed for the species.

脳卒中の機能障害は多岐に渡るが,本稿では脳卒中の中核的機能障害である運動麻痺と痙縮に的を絞り,それらのとらえ方と評価法について検討する。運動麻痺についてはブルンストロームによる片麻痺の回復段階と脳卒中機能障害評価法(Stroke Impairment Assessment Set, SIAS)の麻痺側運動機能評価,痙縮についてはModified Ashworth ScaleとModified Tardieu Scaleを対比して検討する。また,脳卒中患者のバランス能力評価についても筆者らの研究結果も含めながら検討する。
