症例は30歳女性,銃で撃たれ当院救命センターに搬送された。来院時意識は清明で循環動態は安定していた。左右上腕,左前胸部に貫通射創,右側胸部は盲管射創を認めた。CTで横行結腸近傍に銃弾と考えられる金属片が存在し,また右肺,肝臓,脾臓の損傷,消化管穿孔を疑う腹腔内遊離ガス像,腹腔内出血,右血気胸を認めた。緊急開腹術で横隔膜,肝,脾損傷を認め縫合止血,胃壁に挫創を認め穿孔部位と判断した。銃弾を検索したが不明で,術中腹部X線で確認できず,胸部X線を撮影すると下縦隔に銃弾を認めた。食道内の可能性を考慮し,内視鏡を施行すると下部食道に銃弾を確認でき,内視鏡で胃内に押し込み胃壁損傷部位より摘出した。今回われわれは,胸腹部銃創で術前には腹部に存在した銃弾が胸部食道内に移動した症例を経験した。消化管内の銃弾は手術操作などで容易に移動する可能性があり,X線の併用など工夫が必要と考えられた。

症例は85 歳,男性.糖尿病による慢性腎不全で血液透析導入となり,透析歴は2.7 年であった.高血圧,気管支喘息の合併あり.初発症状は発熱のみで,SARS-CoV-2 抗原検査の陽性が判明し入院となった.肺CT で両肺にすりガラス影が散在していた.第2 病日に酸素投与が必要となり,レムデシビル,デキサメサゾン,ファビピラビルの投与を開始したところ肺炎は改善した.第14 病日のCT ですりガラス影は改善するも左上区に新たな結節影が出現し,第27 病日には空洞を伴う陰影に変化した.第28 病日の血清ガラクトマンナン抗原(GM)が2.5(ODI)と陽性で,肺アスペルギルス症(PA)と診断し,第36 病日からボリコナゾールの投与を開始した.血清GM 抗原の低下とともに空洞病変は改善した.血液透析患者でCOVID-19 急性期に空洞を伴う非典型的な肺病変が出現した場合,PA を疑う必要がある.

【目的】基本動作能力を測定する新指標の開発およびその指標の反応性と信頼性の検証を目的とした。【方法】基本動作の動きそのものを判定尺度とし,動作の状態像をイラストで視覚化した基本動作能力のCapacityを測定する基本動作指標(以下,BMS)を作成した。対象は要支援1から要介護5の介護保険サービス利用者148名(男性49名,女性99名。平均年齢82.8±7.7歳)。理学療法士と作業療法士(セラピスト)の主観的評価に対するBMS,BI,FIM運動項目の感度と特異度を算出した。セラピストを検者とした検者内および検者間信頼性を求めた。【結果】BMSは感度89%(特異度34%)と各ツールの中で感度がもっとも高く,特異度がもっとも低かった。検者内と検者間ともに高い信頼性が得られた。【結論】BMSは,指標として高い信頼性があり,基本動作能力のCapacityを測定しセラピストの主観的評価に高い感度で反応する指標であることが示唆された。

第一に、紛争解決の理論・手法同士の関係性について「紛争を構成する要素」「要素に対応した解決理論」「解決を実践するための手法」という観点から整理し、多様な紛争事例に対応できる「戦略的紛争解決」という枠組みを創出した。第二に、欧米で発展した紛争解決の手法をアジアや日本で活用する場合の適用可能性に関して日中韓等の方に質問紙調査を行い、知見を得た。第三に、原発災害被災者の家族や地域内での人間関係の葛藤のケアと変容支援のためアクション・リサーチを行った。「福島の若手らの水俣訪問ツアー」等を実施し、環境災害からの人生と地域再生に向けた日本型の修復的正義プロジェクトの一つのモデルが形成された。

東京学芸大学では学生本位の学びの振り返りを促し,修得主義に基づく教員養成の実現を目指して「eポートフォリオ構築によるデジタル技術を活用した教育実習DX」を進め,本学附属学校園で行われてきた教育実習の教育実習日誌を電子的に扱い,実習の記録や指導・支援の記録を集約するシステム「教育実習日誌eポートフォリオ」(以下,実習ポートフォリオシステム)を開発した.そして実習ポートフォリオシステムを用いた附属学校園における教育実習の試行を実施した.本論文では,本学における教育実習日誌eポートフォリオの構築とその取組みの内容,試行に関する質問紙調査の結果について述べた.

Emojis are extensively used to convey emotional nuances non-verbally in computer-mediated communication. Previous studies have examined how emojis are used and how they change the expression of a sentence, as well as the subsequent impression of the reader. Regarding whether the writer’s emotion is understood correctly by readers, evidence shows that emojis can enhance specific emotions. However, the effects of variation in the kinds of sentence, emotion, and emoji on the shifting emotion conveyed have not been studied. In this study, we examined the various factors that affect the shift in emotions being communicated. Our results demonstrated that positive emotions can be communicated more correctly compared with negative emotions and that smiley emojis (a.k.a. emoticons) can communicate emotions more clearly compared with other kinds of emoji.

In this study, the agricultural utilization of coffee grounds was examined. The results were as follows. 1. The application of coffee grounds in soils inhibited the seedling growth of Chinese mustard. From experiments on nitrogen and toxic substance alleviation, it was suggested that this inhibition is rcsultant of nitrogen starvation and the existence of toxic substances. However, in the summer, when 2.5kg m^<-2> of coffee grounds was applied together with nitrogen fertilizer at 15 to 20g m<-2>, the coffee grounds didn't inhibit the seedling growth of Chinese mustaard with a maturing period of more than one month. 2. When more than 10 kg m<-20> of coffee grounds was applied into soil, the aggregated structure of soil was developed.

パリ協定における目標を達成するために脱炭素社会を実現し,今後も変化する気候に社会が適応するためには,多くの人々が気候変動のリスクに関して理解を深めることが重要な課題である。このため我々の研究グループは,これまでに気候変動リスク連鎖を包括的に分かりやすく可視化する手法の開発を行った。本論文では,我々が開発した手法によって得られた,水資源・食料・エネルギー・産業とインフラ・自然生態系・災害と安全保障・健康の7つの分野に関連するリスク連鎖の可視化結果(ネットワーク図・フローチャート)について議論することにより,気候リスク連鎖の全体像を明らかにする。また,可視化結果を利用して行った市民対話イベントの実例を紹介することにより,我々の開発したネットワーク図・フローチャートの有用性や,気候リスクに関する市民対話の重要性について論じる。さらに,日本における気候変動リスク評価の概要について紹介し,気候変動リスク連鎖を評価するための今後の課題について議論する。

2000 年代に入り、日本の企業による論文発表数が減っているといわれている。文部科学省科学技術政策研究所での研究でも、国内企業の論文発表件数は減少していると発表されている。しかしこれまで、日本国内で発表されている論文数の分析や、その結果と国内特許を合わせた分析は、ほとんど実施されていない。そのため JST と INFOSTA では、国内資料も含めた、電機系と化学系企業の論文発表および特許出願の状況調査を試みた。まず JDream II と PATOLIS を使用して、電機系と化学系企業の論文発表、特許出願状況を調査した。また、企業が投稿した論文誌等の科学技術資料の種類も調査した。科学技術資料は、分野、査読の有無、業界での注目度により分類できるため、企業が多く投稿する資料を分析した。さらに、今回調査した結果を基に、論文件数および特許出願数と社会情勢の比較等、調査した企業研究開発の状況を考察した。今後、他業種の企業の論文発表数調査や、企業内部の技術分野別の動向等、調査していきたい。

明治初年に計画された札幌は, 格子状の街区パターンをもち, その中に広幅員の大通 (火防線) を設定した。それが現在の大通公園である。この大通計画の目的は二つあった。市街地を大きく官地と民地に分け, 両者の空間的分離を図ること。同時に, 民地から頻発する火災による官地への延焼防止の役割を担わせることにあった。この大通火防線の計画は, 都市火災の予防という観点から, 近代的都市計画としての評価を受けてきたけれども, 支配, 被支配の構造が色濃くあらわれている近世城下町的な都市計画の論理を踏襲していることは, 角館や大野の事例からも明らかである。

PCのグラフィックボード(GPU)の果たす役割は年々加速しており,CPU中心の計算環境からGPU中心の計算環境へと徐々に移行している.また,GPUの持つ機能をプログラムで利用できる環境が整ったことにより,GPUをグラフィック処理専用のCPUの補助プロセッサとしてではなく,並列計算機として応用する研究が活発化している.本論文では,GPUコンピューティングで何が可能となり,今後どのような発展が期待できるかを,具体的な例を挙げながら述べる.

Mucoud otitis is known to be caused by mucoid typeStreptococcus pneumoniae, and the most of them are serotype 3. As the result of the development of antibiotics this bacteria has once been regarded as an unimportant pathogen. However recently, there are found severe cases in which this pathogen is responsible.During the period from January 1998 to July 1999, we have treated 25 cases in which the mucoid typeStreptococcus pneumoniaewas isolated.Based on these experiences, we summarized the current status of the mucoid otitis media as follows.1. The distribution of patients suffering from mucoid otitis medid ranged infants to elderly people. 2. The patients presented with symptomes, such as severe earache, headache, high fever, profuse otorrhea, aggravation of sensorineural hearing. 3. The relief of the symptomes is usually better and earlier when penicillin was selected as a first choice, while when cephem antibiotics were selected, healing of the otitis media tend to be delayed. 4. As 13 out of 14 strains isolated in 1999 were found to be penicillin-insensitiveStreptococcus pneumoniae (PISP) withpbp2xgene, it is conceivable that the resistance of this pathogen against cephem is under a rapid progress, probably due to abuse cephem in the past in this country. 5. The antibiotics of the first choice against mucoid otitis media is penicillin. If the pathogen would acquire penicillin resistance, treatment of mucoid otitis media may become as difficult as it used be in old days of preantibiotics. We should pay enough attention to this disease as an important reemerging infectious disease in the otolaryngologic field.

The main purpose of this paper is to develop an easy-to-use smartphone case with “fall prevention function”. It shows as a remarkable point that our developed technique of determining the grip form reflecting the traditional craftsmanship verifies for the made-to-order wooden products. It also provides the experimental result of wooden smartphone case using electromyography (EMG). The experiment consists of two cases, with using a smart phone case and not it. Then, it compared with these two cases, the result of measuring the amplitude of gripping forces. The experiment suggests that using the smartphone case relatively clearly that applies significantly less gripping force, so that prevents dropping a smartphone. The experiment suggests that using the smartphone case it applies significantly less gripping force. It will provide a new guideline of the universal design as a case study of Kansei/affective engineering regarding human grip.

To examine the possibility of electromagnetic precusor preceding an earthquake the vertical earth potential difference (EPD) has been observed continuously at an urban area. Iregularily continued pulses had been observed for several hours before and after the Off East Hokkaido earthquake. We have referred both a series of signals of the terrestrial magnetism and the lightning location (LLP) map. No significant signal is recognized on those of terrestrial magnetism, however, severe lightning flashes occured over the Japan Sea at this time. To clarify the cause of the EPD pulse signal, the LLP map and the radar echo map are shown in the case of (a) near lightning flashes (b) far lightning flashes (c) the Off East Hokkaido earthquake. The signals in the cases of (b) and (c) are similar, therefore, the pulses are detected by the far lightning electromagnetic change. The LLP map must be referred to examine electromagnetic precusors of an earthquake. The EPD signal generated by propagating seisamic wave is also recorded.

the taste of liver by physical/chemical measurements and sensory test of taste.Reduction in iron content was observed when the liver was soaked in water or milk. In short time boiling liver,45% of iron secretion was observed.No iron secretion was observed when the liver was broiled, but, significant iron secretion was observed when the liver was simmered.There was no significant difference in iron secretion by cooking between one-day and three-day-old liver after slaughtering.In sensory tests of broiled liver by three preparations, one-day old liver after slaughtering, there were no significant differences in overall evaluation.In broiled liver that soaked in milk, three-day old after slaughtering, significant positive result was observed in terms of tasting.In sensory test of liver simmered, preparation did not effect the taste.

In this study, a direct drive in-wheel system is proposed as a high power density system for reducing the intermediate parts, such as a speed reducer by increasing the torque. We propose a design strategy to show that multipolarization and the improvement of the gap magnetic flux density can effectively increase the power density. A Halbach array magnet rotor that combines the main electrode magnets and spoke magnets with the core is adopted as a technique for improving the gap magnetic flux density. We compare the Halbach array type rotor to the conventional SPM type rotor by using both of magnetic circuit calculation and FEM analysis. Moreover, the actual sized in-wheel motor prototype with the developed Halbach array magnet rotor is fabricated and measures the no-load induced back EMF. The measured no-load induced back EMF agrees well with the analyzed waveform.
